ABSTRACT

Nevo azul é uma lesão benigna originada de melanócitos dérmicos contendo grande quantidade de melanina. A variante agminada apresenta-se como um agrupamento de lesões de nevo azul com distribuição linear ou blaschkoide. Relatamos dois casos de pacientes com nevo azul agminado que surgiu sobre cicatrizes prévias de acne, sendo a dermatoscopia de grande auxílio para diagnóstico diferencial. Esta é a primeira ocorrência relatada de nevo azul agminado sobre cicatrizes prévias, o que pode ter ocorrido ao acaso ou em decorrência do processo de remodelamento do colágeno, próprio do processo cicatricial.


Blue nevus is a benign lesion arising from dermal melanocytes containing large amounts of melanin. The agminated variant presents a cluster of blue nevus lesions with linear or blaschkoid distribution. We report two cases of patients with agminated blue nevus that developed on previous acne scars, and dermoscopy helped a lot in the differential diagnosis. This is the first occurrence found in the literature of agminated blue nevus that appeared on a previous scar,

ABSTRACT

A discromia azul das unhas possui vários diagnósticos diferenciais. Crescimento da lesão, distrofia ungueal associada e extensão periungueal requerem avaliação para excisão cirúrgica. Mulher, 27 anos, apresentava mancha azulada, semicircular, ocupando cerca de 50% da lúnula, sem alteração da lâmina suprajacente, com pequena alteração da porção distal da unha, com camadas do tipo "onicosquizia localizada", sem história prévia de trauma ou sangramento. Realizada avulsão parcial da placa e biópsia excisional por saucerização da lesão fortemente pigmentada. O exame histopatológico foi compatível com nevo azul. Sugere-se que, neste caso, o nevo se situasse em posição submatricial, não interferindo, portanto, na coloração da lâmina ungueal


Blue nail dyschromia has several differential diagnoses. Lesion growth, associated nail dystrophy, and nail extension require evaluation for surgical excision. We report the case of a 27-year-old woman presenting a bluish, semicircular stain, occupying about 50% of the lunula. The patient presented no changes in the overlying lamina, small alteration of the distal nail portion, localized onychoschizia-type layers, and no previous trauma or bleeding history. We performed partial avulsion of the plaque and shave biopsy, evidencing an intensely pigmented lesion. Histopathological examination was compatible with blue nevus. In this case, the nevus should be located in the sub-matricial position, thus not interfering with the nail plate color

ABSTRACT

Abstract Secondary osteoma cutis is a phenomenon that may occur in several conditions. When it occurs in a melanocytic nevus it is named osteonevus of Nanta, an event considered uncommon and characterized by the presence of bone formation adjacent or interposed with melanocytic cells. There are reports of its occurrence in various melanocytic lesions, being more frequently associated with intradermal nevus. We report a case of osteonevus of Nanta in combined nevus, possibly the first description of this association.

ABSTRACT

Objective@#To analyze dermoscopic features of blue nevi.@*Methods@#Clinical and dermoscopic data were collected from patients with confirmed blue nevi in the Department of Dermatology and Venereology, Wuxi People′s Hospital from January 2008 to May 2018, and analyzed retrospectively. Chi-square test was used to analyze differences between groups.@*Results@#Totally, 65 patients with 66 skin lesions were enrolled into this study. A total of 23 skin lesions were subjected to dermoscopy, which showed homogeneous pattern in 20 (87.0%) lesions, pseudo-pigment network pattern in 2 (8.7%) lesions, and cerebriform pattern in 1 (4.3%) lesion, and the homogeneous pattern was more common than the other patterns (χ2 = 8.79, P < 0.05) . Under the dermoscope, 10 lesions were monochromatic (43.5%) , 11 were dichromatic (47.8%) , and 2 were multichromatic (8.7%) . Scar-like hypopigmentation was observed in 4 lesions (17.4%) , and vascular structures were observed in 5 (21.7%) , including punctate vessels in 4 (17.4%) and linear irregular vessels in 1 (4.3%) . Papillary hyperplasia was observed in 2 lesions (8.7%) . Additionally, pigmentation around lesions, white streaks, dots/globules, blotches, comedo-like openings were observed in 1 lesion (4.3%) each.@*Conclusions@#Blue homogeneous pattern is the most common dermoscopic feature of blue nevi. Pseudo-pigment network pattern, cerebriform pattern, scar-like hypopigmentation, multiple vascular patterns, papillary hyperplasia, white streaks, dots/globules, blotches, comedo-like openings can also be observed in blue nevus lesions.

ABSTRACT

Abstract: Blue nevus is a benign melanocytic lesion whose most frequent variants are dendritic (common) blue nevus and cellular blue nevus. Atypical cellular blue nevus presents an intermediate histopathology between the typical and a rare variant of malignant blue nevus/melanoma arising in a cellular blue nevus. An 8-year-old child presented a pigmented lesion in the buttock since birth, but with progressive growth in the last two years. After surgical excision, histopathological examination revealed atypical cellular blue nevus. Presence of mitoses, ulceration, infiltration, cytological atypia or necrosis may occur in atypical cellular blue nevus, making it difficult to differentiate it from melanoma. The growth of blue nevus is unusual and considered of high-risk for malignancy, being an indicator for complete resection and periodic follow-up of these patients.

ABSTRACT

Abstract: Blue nevus is a benign melanocytic lesion, typically asymptomatic and of unknown etiology. Many histological subtypes are recognized, the most commons being: common blue nevus, cellular blue nevus, and combined blue nevus. New rare variants have been described in the literature, with emphasis on eruptive blue nevus, plaque, agminate, linear, with satellitosis, disseminated, familial and targetoid. The diagnosis of blue nevus usually presents no difficulties, however, the presence of structures such as irregular edges or satellitosis, are highly suggestive of malignancy, and the differential diagnosis with malignant blue nevus and melanoma with peripheral spread should be considered. We report a case of blue nevus with satellitosis in a 15-year-old female patient.

ABSTRACT

Abstract: Blue nevi are benign melanocytic lesions located in the deeper reticular dermis, consequence of failure of melanocytic migration into the dermal-epidermal junction from the neural crest. Lesions are usually asymptomatic and solitary, but may present in a multiple or agminated (grouped) pattern. The agminated subtype is formed when bluish-pigmented lesions cluster together in a well-defined area. Lesions can be flat or raised. We report the case of a patient who presented multiple bluish macules (1-3 mm in diameter) grouped on the left upper back. Dermoscopy and anatomic pathological examination were consistent with blue nevus.

ABSTRACT

Argyria is a rare disease caused by prolonged skin contact with silver. Localized cases have been described regarding the use of topical medications, and trauma with objects containing this metal such as acupuncture needles and jewelry. Clinically, a macule or a patch, round or oval, appears in the infected area, with a characteristic bluey-gray color. To our knowledge this is the first time that this clinical condition has been described through the use of dermoscopy.


A argíria é uma doença rara, causada pelo contato prolongado da pele com prata. Foram descritos casos localizados relacionados ao uso de medicamentos tópicos e traumas com objetos que contem esse metal, como agulhas de acupuntura e jóias, por exemplo. Clinicamente, aparecem máculas ou manchas redondas ou ovais na área afetada, com uma característica cor azul-acinzentada. Até onde sabemos esta é a primeira vez que este quadro clínico foi descrito através da dermatoscopia.

There are elements in nature that may be compared to some dermatological lesions, such as the black pearl, which is very similar to a cellular blue nevus observed in the gluteus region of a 31-year-old male patient.


A natureza pode oferecer elementos passíveis de comparação com algumas lesões dermatológicas, como a semelhança entre a pérola negra e o nevo azul celular observado na região glútea de paciente do sexo masculino, 31 anos.

ABSTRACT

Objective To summarize and analyze the clinical characteristic of blue rubber bleb nevus syndrome (BRBNS).Methods The clinical data of four cases treated since 2001 and 30 BRBNS cases reported by domestic literature were retrospectively analyzed.The clinical manifestation,family history,endoscopy and imageology examination,site of lesions,treatment and follow up were analyzed.Results The male to female ratio was 1.8∶1 and median age was 19.5 years.A total of 33 cases (97.1%) presented with gastrointestinal bleeding,median age of gastrointestinal bleeding detected was 9.0 years.Among the 33 cases,anemiawas found as the primary symptom in nine cases (27.3%),and one case complicated with intussusception and intestinal necrosis accompanied with abdominal pain.Two cases have family history.Gastroscopy (85.3 %) and colonoscopy(73.5 %) were mainly examinations for detection.Lesions mainly involved skin (100.0%) and digestive tract (97.1%),and the locations of the lesion in digestive tract was stomach (64.7%),small intestine (64.7%),colon (58.8%),esophagus (29.4%).Treatment methods included symptomatic treatment,endoscopic therapy and surgery.Banding ligation and polypectomy resection were common endoscopic therapies.Gastrointestinal bleeding did not recur in six cases with endoscopic therapy and four cases receiving surgery during short-term follow up.Conclusions BRBNS lesions mainly involve skin and digestive tract,mostly complicated with gastrointestinal bleeding.For gastrointestinal bleeding,so far endoscopic therapy and surgery are the effective therapies.

ABSTRACT

El enfrentamiento a una lesión pigmentada cutánea es frecuente en la práctica médica diaria. Dado que el diagnóstico diferencial incluye al Melanoma Maligno (MM) y considerando que su pronóstico está claramente determinado por la precocidad del diagnóstico, es primordial conocer sus formas de presentación clínica. Además, es muy relevante conocer aquellas lesiones cutáneas que pueden simular clínicamente un MM. El objetivo de este trabajo es describir las características morfológicas del MM y entregar una descripción breve de sus simuladores.


Pigmented skin lesions are common in the daily practice of a physician. Since the differential diagnosis includes Malignant Melanoma (MM) and considering that its prognosis is determined by the precocity of the diagnosis, is essential to know its different clinical features. In addition, its critical to know how to identify MM simulators. Theaim of this work, is to briefly describe the clinical spectrum of MM and its most frequent simulators.

ABSTRACT

O nevo é uma má-formação congênita pigmentada, raramente encontrado na mucosa bucal. Cerca de 1/3 dos casos localizados nesta região anatômica são do tipo azul, uma variante histológica com considerável tendência à malignização. Este artigo relata o caso de um paciente do sexo masculino, tabagista crônico, portador de um nevo azul de 5 cm de diâmetro no palato duro. A excisão da lesão sem biópsia prévia foi a conduta terapêutica de eleição para o caso, uma vez que ainda existe controvérsia na literatura a respeito da realização de biópsia incisional em lesões pigmentadas malignas ou com potencial de malignização. O paciente foi acompanhado por um período de 2 anos, sem sinais de recorrência ou transformação maligna.


Nevus is a congenital pigmented malformation rarely found in the oral mucosa. Around one third of cases located in this anatomical region are of the blue type, a histological variant with considerable tendency to malignancy. This study reports the case of a male patient, chronic smoker, with a blue nevus measuring 5cm in diameter on the hard palate. Since controversy exists in the literature regarding the incisional biopsy of pigmented lesions with malignant or malignant potential, excision without previous biopsy of the lesion was the therapy of choice for this case. The patient was followed-up for two years with no sign of recurrence or malignant transformation.
