ABSTRACT

Two hundred and eleven aged patients with type 2 diabetes were enrolled in the study.Blood glucose levels were monitored with continuing glucose monitoring system (CGMS) for 3 days and the cognitive function was evaluated with Mini-Mental State Examination(MMSE) scale.The correlation between hypoglycemia(or nocturnal hypoglycemia) and MMSE scores was evaluated.CGMS recorded hypoglycemia in 47% patients (100/211),82% of whom (82/100) had nocturnal hypoglycemia.Patients with hypoglycemia had lower MMSE scores than those without hypoglycemia (25.1 ± 5.1 vs.26.8 ± 3.0,P < 0.05).Furthermore,the incidence of hypoglycemia(or nocturnal hypoglycemia) was higher in patients with MMSE≤24 than that in patients with MMSE > 24(P <0.05).The median of hypoglycemia maximal time and nocturnal hypoglycemia maximal time were longer in patients with MMSE ≤ 24 than those with MMSE > 24 (P < 0.05).Stepwise regression analysis showed that the total time and maximal time of hypoglycemia(or nocturnal hypoglycemia),age,and education level were the main factors influencing MMSE scores in aged type 2 diabetic patients(all P <0.05).

La hipoglucemia es la complicación más frecuente en el tratamiento de la diabetes mellitus, y para algunos constituye el problema más grave asociado a la enfermedad. OBJETIVO: determinar la frecuencia y las características clínicas de la hipoglucemia en un grupo de niños y adolescentes diabéticos tipo 1. MÉTODOS: se realizó un estudio retrospectivo en 48 niños y adolescentes con diagnóstico de diabetes mellitus tipo 1, atendidos en el Servicio de Endocrinología del Hospital Pediátrico Docente William Soler, en el período comprendido entre Enero de 1987 y Enero de 2000. Se tomaron de las historias clínicas los datos generales del paciente, el régimen de insulinoterapia y la dosis total diaria de insulina, los resultados de la hemoglobina glucosilada, así como los episodios referidos de hipoglucemias, los que se clasificaron en ligeras y severas. RESULTADOS: se encontró una frecuencia elevada de episodios de hipoglucemias severas (56,25 por ciento). La primera hipoglucemia severa se registró con mayor frecuencia en el grupo de edad de menor de 5 años, mientras que las hipoglucemias ligeras fueron más frecuentes en el de mayor de 5 años. El primer episodio de hipoglucemia severa ocurrió antes del primer año de realizado el diagnóstico de la diabetes mellitus. Los pacientes con régimen intensivo sufrieron un mayor porcentaje de episodios de hipoglucemia severa, que los pacientes con régimen convencional. El control metabólico no influyó en la ocurrencia de los episodios de hipoglucemia severa. Los episodios de hipoglucemia ocurrieron con una elevada frecuencia, con predominio de las severas, con una mayor frecuencia en la madrugada, y con mayor afectación en el grupo de menores de 5 años de edad, lo que constituye un riesgo de afectación del neurodesarrollo en este grupo de edad(AU)


Hypoglycemia is the more frequent complication in the treatment of diabetes mellitus and for some it is a more serious problem associated with the disease. OBJETIVE: to determine the frequencies and the clinical features of hypoglycemia in a group of diabetic children and adolescents diagnosed with type 1 diabetes mellitus seen in the Endocrinology of the William Soler Children Hospital from January, 1987 and January, 2000. Medical records, general data of patient, insulin therapy , results of glycosylated hemoglobin were registered, as well as the recounted hypoglycemia episodes which were classified as slight and severe. RESULTS: there was a high frequency of hypoglycemia episodes (56,25 percent). The first severe hypoglycemia was registered as more frequent at the age group of 5 years. The first severe hypoglycemia episode occurred before the first year of diagnosis of diabetes mellitus. Patients under a intensive regime had a greater percentage of episodes of severe hypoglycemia than those under a conventional regime. The metabolic control not influenced in occurrence of severe hypoglycemia episodes. The above mentioned episodes had a high frequency with predominance of the severe ones during the dawn and with more involvement in the group aged under 5 being a neurodevelopmental affection of development in this age group(AU)
