ABSTRACT

@#Abstract: In recent years, the HIV prevention intervention strategy has had a positive effect on reducing the HIV infection prevalence in China. HIV non-occupational post-exposure prophylaxis (nPEP) has become an effective measure to prevent HIV for men who have sex with men (MSM) exposed to possible risks. From the perspective of the "knowledge, attitude, and practice" theory, domestic and overseas scholars used outpatient data and questionnaires to investigate the awareness, attitude, usage, medication, follow-up, and drug prescription of HIV nPEP among MSM in China. This article reviewed the above-mentioned and found that the overall awareness rate of HIV nPEP among MSM in China is high with a positive attitude, but the actual utilization rate is very low. On a regional level, their follow-up after the medication is not ideal despite good medication. Drug prescription varies between different regions domestically. The prescription rate of tenofovir + emtricitabine + raltegravir is the highest, followed by the prescription rate of tenofovir + emtricitabine + dolutegravir, and tenofovir + lamivudine + dolutegravir. In the future, researchers should expand the number of study objects and the scope of the research sites in China, analyze the "knowledge, attitude, and practice" of HIV nPEP, medication follow-up, and medication prescription among MSM, and increase the research on HIV nPEP service areas to provide the reference for further formulating and refining the comprehensive HIV prevention intervention strategy among MSM in China.

@#Abstract: Objective To investigate the drug demand and related influencing factors of AIDS non-occupational post-exposure prophylaxis (nPEP) among men who have sex with men (MSM) in Wuhan, and to provide a scientific basis for the development of subsequent intervention policies for MSM. Methods With the assistance of social organizations in Wuhan, MSM was recruited by the snowball method to carry out an online questionnaire survey to collect information on demographics, AIDS-related knowledge, high-risk behaviors, and the need for nPEP medication. The χ2 test and unconditional Logistic regression were used to analyze the related factors of the demand for nPEP medication. Results A total of 308 valid subjects were included in this study, with predominantly 18-29 years old (78.57%, 242/308). The self-reported sexual orientation was mainly homosexuality (82.47%, 254/308), and the awareness rate of AIDS knowledge was high (89.29%, 275/308). Among the survey respondents, 35.06% (108/308) did not know the situation of HIV infection among MSM population in Wuhan; 55.19% (170/308) had two or more same-sex sexual partners in the last six months; 90.91% (280/308) had heard of nPEP before participating in this survey. After passing nPEP and informing the protective effect of nPEP, 59.42% (183/308) of them needed nPEP. After HIV exposure, 73.38% (226/308) were willing to spend money to buy nPEP drugs, and 88.64% (273/308) were willing to take nPEP drugs because of the known side effects. Logistic regression analysis showed that in the last six months, the needs for taking nPEP medication in those who had 2 or more same-sex sexual partners (OR=2.121, 95%CI: 1.329-3.386) and who had received peer education (OR=1.740, 95%CI: 1.088-2.781) were higher than those of those who had a same-sex sexual partner in the last six months and who had not received peer education. Conclusions The MSM population in Wuhan has a great demand for nPEP drugs, and peer education is an important way to carry out nPEP publicity and promotion. At the same time, we should continue to strengthen warning publicity and education and behavioral intervention to reduce MSM risky sexual behaviors and reduce new HIV infections.

ABSTRACT

Objective To evaluate nPEP willingness and uptake among MSM in Shanghai. Methods MSM were recruited through a NGO-based HIV voluntary counseling testing clinic and several virtual communities using snowball sampling. Results A total of 606 MSM were successfully recruited in which 82.8% (502/606) had heard of nPEP and 81.2% (492/606) were willing to use nPEP. MSM who had been diagnosed with sexually transmitted diseases (STD) were more likely to hold willingness in using nPEP. High cost was the most common reason for being unwilling to use nPEP. 9.9% (60/606) had used nPEP in which 51.7% (31/60) had used twice or more, and 50%(30/60) had missed medication in the last nPEP. MSM who were heterosexual or with other sex orientations except for being bisexuality (OR=2.943, 95%CI: 1.057‒8.191), being receptor in anal intercourse (OR=3.361, 95%CI: 1.552‒7.282), having 2 or more sexual partners in the last 30 days (OR=2.242, 95%CI: 1.225‒4.105), having been diagnosed with STI (OR=2.317, 95%CI: 1.223‒4.389), and using drugs (OR=2.177,95%CI: 1.052‒4.503) were more likely to use nPEP. Conclusion MSM in Shanghai have a relatively high willingness to use and uptake of nPEP. Comprehensive strategies need to be developed on improving adherence and reducing HIV-risk behaviors.

ABSTRACT

Background Non-occupational carbon monoxide (CO) poisoning is a public health problem that seriously affect people’s health and lives. Objective To describe the prevalence of non-occupational CO poisoning during 2007—2018 in Shanghai, analyze its epidemiological characteristics and potential influencing factors, and explore effective prevention and control measures. Methods Daily reported non-occupational CO poisoning cases and meteorological factors from 2007 to 2018 were collected in Shanghai, epidemiological characteristics were analyzed by descriptive epidemiology methods, and a distributed lag nonlinear model was used to assess the association between temperature and non-occupational CO poisoning. Results A total of 2264 non-occupational CO poisoning events and 3866 cases from 2007 to 2018 were reported in Shanghai, including 59 death cases. More than half of the poisoning cases were female (56.3%), and young adults accounted for more cases than any other age group (54.8%). The poisoning events mainly occurred in winter (from December to next February); however, cases reported in summer increased in recent years. The peak period of the events was from 20:00 to 24:00. Households (85.2%) and restaurants (8.0%) were the common places of non-occupational CO poisoning events, and the main cause was improper use of gas water heater (36.9%). A nonlinear curve was found between daily average temperature of current day and the occurrence of non-occupational CO poisoning. Temperature was negatively associated with the risk of non-occupational CO poisoning when the temperature was lower than 9.6 ℃, while a positive association was found during 9.7-26.0 ℃. Conclusion Winter is a high season for non-occupational CO poisoning in Shanghai, rising cases reported in summer is also worthy of attention. Supervision should be strengthened to ban sales of unqualified gas water heaters, and health education on CO poisoning prevention and control should be conducted through multiple channels, in order to reduce the incidence of CO poisoning.

ABSTRACT

The prevention of occupational exposure to HIV has been carried out in China for many years, and achieved remarkable results. However, the practice of preventive drugs among people with non occupational exposure has many difficulties. Although the high-risk groups show high willingness and demand, their awareness of the preventive drugs is low due to the fear of increasing unprotected sex after the use of the drug. Other factors negatively affecting the application of the drugs are the cost and the side effects. In order to provide preventive treatment on time for the high-risk groups, the awareness of non occupational exposure prophylaxis should be raised.

ABSTRACT

The occupation post exposure prophylaxis for AIDS has been carried out for many years and achieved remarkable result,but local trials of non-occupational exposure prophylaxis encountered many ethical problems and difficulties.In the context of high willingness in the AIDS high-risk population to accept non occupational exposure prophylaxis and increasing demand for drugs,starting from the value concept of life dignity,based on the ethical principles of good without hurt,fairness and justice,it is recommended to strengthen the propaganda of non occupational exposure prophylaxis and improve the availability of prophylaxis drug for non occupational exposure,in order to enable the high-risk groups possibly infected with HIV received timely preventive treatment.

ABSTRACT

Objective To analyze the cases of brucellosis in non-occupational population in Tongxiang City of Zhejiang Province,and to provide a scientific basis for making strategies in brucellosis prevention and control.Methods Rose bengal plate agglutination test (RBPT) and serum tube agglutination test (SAT) were used to detect brucellosis in the general population who were closely contacted with cases in non-occupational groups and who had brucellosis-related symptoms.If the pathogen result was positive,Brandt strain type identification should be done to analyze the reasons for non-occupational population who infected with brucellosis.Results A total of 7 cases of brucellosis were detected in the non-occupational groups,which accounted for 14.58% of the total number of cases (7/48).Misdiagnosis was common,seasonal incidence was not obvious,transmission route and population infected were diverse.Conclusion The incidence rate of brucellosis is an upward trend,public brucellosis prevention knowledge should be strengthened,and training in diagnostic and treating skills for medical professionals should be provided.

ABSTRACT

OBJECTIVE: This study investigated case clusters of pneumoconiosis among residents near cement factories in Young_Wol, Korea. METHODS: There were 1,396 residents of a total 3,418 residents surveyed for this study. Informations on demographics, health-related behaviors (smoking and drinking), past medical history, and clinical symptoms were collected using a self-administered questionnaire. All study participants underwent a physical examination, including chest X-ray, and physicians' interview. Chest CT (computerized tomography) was taken if there was abnormal finding on chest X-ray and physicians' interview. RESULTS: We detected 5 cases of pneumoconiosis. 2 cases were related to occupational exposure to dust, such as underground mining and the breaking up of concrete. However, 3 cases were not related to occupational or environmental exposure, except that persons were living near cement factories and limestone mines. CONCLUSION: This study suggests that the long-term exposure of cement dust among residents near cement factories and mines may lead to the development of pneumoconiosis.

ABSTRACT

This study was conducted to analyse medical facilities utilization based on predisposing factors, enabling factors and need factors, and effect of these factors on medieal facilities utilization, by applying Anderson's medical care service behavioral model as the most well known medical demand model to 163 available for interview among 246 workers who, as the result of health examination in 1994, were evaluated as the non occupational diseases. The results of this study were summarized as follows. Among variables of predisposing factors, knowledge for disease showed significant relations with the medical utilization, other variables of predisposing factors, such as sex, age, marital status, educational level, number of family members, type of occupation, attitude toward the disease, were not related with the medical utilization. The medical utilization was not related with variables of enabling factors. Among variables of need factors, medical advice of health personnel was significant variable for medical utilization. With multiple logistic regression analysis, the significant variables haying an effect on the medical facllities utilization were the type of occupation and knowledge for the disease among predisposing factors, ana seriousness for the disease and medical advice of health personnel among need factors. In case of the production job, the medical facilitles utilization was higher for the persons with more knowledge for disease and experience of medical treatment guidance. The number of non-occupational diseases detected, but untreated person was 76 (46.6%) of total subjects, mainly because the non-occupational diseases seemed to be a slight disease (55.3%). On consideration of above findings, health education for workers and adequate medical advice by health personnel would be needed for health management of workers with non occupational diseases.
