ABSTRACT

Objective To explore the adaptive modulation of brain in structure and function when moving to high altitude areas based on voxel-based morphometry (VBM),amplitude of low frequency fluctuation (ALFF)and regional homogeneity (ReHo) method. Methods Resting-state functional magnetic resonance imaging (fMRI)of brain was respectively performed in 1 9 normal adults who moved to high altitude areas within a week, and they have matched age and education level. The subj ects were performed the same scanning after one year. The VBM,ALFF and ReHo methods were adopted to analyze the functional MRI data. Results There were no significant differences in brain structure between the two groups. There were significant differences in brain function in resting state.Subj ects staying in high altitude areas for one year had increased ALFF in left middle frontal gyrus, decreased ALFF in right lingual gyrus(P<0.01),increased ReHo in the left orbital frontal gyrus, and decreased ReHo in right cuneus and right superior parietal gyrus(P<0.05).Conclusion There are significant changes in brain function of normal adult after stayed in high altitude for one year, which might be associated with long-term chronic hypoxia.

ABSTRACT

PURPOSE: By achieving informations about sagittal shape and motion of each lumbar segment in normal subjects, we tried to get the standards useful in management of spinal disorders and fractures. MATERIALS AND METHODS: Thirty normal adults without any disorder on their back were adopted. L3 centered thoracolumbar (T-L) X-ray's including standing lateral, AP's in righ-tleft bending, and flexion- extension lateral were checked for each person. Cobb angles of each segment were measured. Statistical analysis was done for comparison by gender and correlation of each factors. RESULTS: Body shape of upper lumbar vertebrae was kyphotic but caudal bodies are changed into lordotic gradually. Disc angle was lordotic in all the discs. Segmetal motion in sagittal plane was greatest at L4-L5 (18.6degrees). However, segmental motion at coronal plane was largest at L3-L4 (12.6degrees). CONCLUSION: We achieved Korean standard of shape and motion of each lumbar segment. Proportion of each factor contributing to total lumbar lordosis was evaluated also.

ABSTRACT

OBJECTIVE: To compare the foot pressure of the insole with medial longitudinal arch support and metatarsal pad and insole with medial longitudinal arch support only. METHOD: Thirty three normal subjects participated in this study. Insole were fabricated by Hana-meditec company after shaping the subjects' foot. The left insole had only medial longitudinal arch support and right insole had medial longitudinal arch support with metatarsal pad. Bare foot pressure were measured by EMED system(r) during comfortable gait. Shoes were manufactured by Osan University. Foot pressure in the shoes was measured by PEDAR system(R) during comfortable gait. Plantar pressure were analyzed by PEDAR C-expert program at T0 (whole foot), M1 (heel), M2 (midfoot), M3 (1st and 2nd metatarsal area), M4 (3rd, 4th and 5th metatarsal area), M5 (great toe), M6 (2nd and 3rd toes area), and M7 (4th and 5th toes area) zone. RESULTS: Right bare foot plantar peak pressure was identical with left side. Right plantar peak pressure with metatarsal pad in shoes was lower than left side with medial longitudinal arch support only at M1, M2 and M4 zone. CONCLUSION: For reducing the plantar peak pressure, the insole with medial longitudinal arch support and metatarsal pad were better than the insole with medial longitudinal arch support only.

ABSTRACT

Objective:To evaluate magnetic resonance imaging(MRI) and 1 H magnetic resonance spectroscopy( 1HMRS)in the study of normal biochemical process of the brain as well as differentiation of normal senile brain from cerebral diseases related to senility.Methods:180 cases of healthy adults were selected to perform MR examination,in which, none of the subjects had the history of neurological and psychotic diseases according to MRI and clinical results.Meanwhile,60 healthy subjects were selected to perform 1 HMRS examination. The ages ranged from 18 to 80 years. They were divided into six age groups purposely.Point resolved spectroscopy sequence was required for 1 HMRS.The metabolites in the spectra included:N-acetylaspartate(NAA),choline compounds(CHO),creatine compounds(CR),myo-inosito(MI),glutamate and glutamine(Glu-n).Results:(1)In 180 cases of MRI,T 2 relation time was lowest in the deep gray matter in the same age group;T 1relation time was in low-high order, while T 2 relation time was decreased with the increase of age in the different age group.(2)The amplitudes in high-to-low order were as follows in 60 cases of 1 HMRS:NAA、CR、CHO、MI、Glu-n.No prominent difference of shape and peak arrangement was seen at the different ipsilateral site in the same age groups;while slight difference at the same site in the different age groups was present. The ratio of NAA/Cr and Glu-n CR was higher in senile age group;while that of MI/Cr was lower. The ratio of CHO/CR was in low-to high order with the difference of age.The ratio of NAA/CR and MI/CR was gradually lower from anterior to posterior part of the brain;the ratio of CHO/CR was highest in occipital cortex however,no definite changing rule was observed in the ration of Glu-n/CR.Correlation of T 1 relation time and partial metabolite ratio with age was present in gray matter. Conclusion:Quantitative study of MRI and 1 HMRS is essential for determination of normal myelinization and neuronal integrity and age-related biochemical changes in the brain.

ABSTRACT

PURPOSE: The pressure-flow study is only objective study that can determine the presence or absence of bladder outlet obstruction and impaired detrusor contractility. Although many results of pressure-flow study are reported in diseased state such as benign prostatic hyperplasia, but are rarely evaluated in adult male patients without voiding symptoms as control group. The purpose of this study was to evaluate the findings of pressure-flow study in asymptomatic male patients. MATERIALS AND METHODS: Twenty-three male patients without voiding symptom were recruited for this study. Mean age of these patients was 49.8 years(range 21-70). Mean AUA symptom score was 1.1(range 0-4) and mean prostatic volume was 27.3gm(range 20-35) on digital rectal examination. The study was done by using 7 Fr. urethral catheter on sitting or standing position. RESULTS: Mean values were followed; PdetQmax was 48.1cmH2O(range 25-94), Qmax was 15.7ml/sec(range 3-23), and postvoid residual urine was 14.5ml(range 0-80). According to the Abrams-Griffiths nomogram, 8 were unobstructed and 4 were obstructed, the remaining 11 falling in the equivocal zone. In each types, mean group specific urethral resistance factor (URA) was 12.8cmH2O in unobstructive type, 51.5 in obstructive type, and 22.1 in equivocal type. CONCLUSIONS: The results were maybe guessed as following; First, the values of defining obstrucion were set too low. Second, obstruction was less relevance in the development of symptoms than had been suggested previosly by some observers. Therefore, we thought that interpretations of pressure-flow finding were carefully considered to method of test, and/or uncomfortable voiding.

ABSTRACT

OBJECTIVES: The respiratory compensation threshold(RCT) is thought to be one of parameters by some for the evaluation of physical performance despite its controversial status. The purpose of this investigation is to establish the reference values of the RCT in healthy Korean adults, and to examine the usefulness of this parameter in evaluating physical performance. METHODS: Symptom-limited maximal exercise test by Bruce protocol was carried out, and RCT and AT were determined by the aid of the computerized V-slope method, along with the VO2 max, in 441 healthy Korean adults consisting of 284 men and 157 women aged from 20`s to 60`s. In addition, correlation between RCT and AT was observed, and regression equations of the RCT were also derived. RESULTS: The RCT VO2 was tended to decrease with age in men, but not in women and the RCT VO2 of women was 26-30% less than that of men. The ratio of RCT VO2 to VO2 max (RCT VO2/VO2 max) was tended to increase with age and the RCT V was significantly correlated with AT VC4 and VO2 max in both sexes. CONCLUSION: Our study reveals that the RCT is another useful submaximal index along with the AT in evaluating physical performance. The regression equation of the RCT was RCT VO2(L/min)=1.4232-0.0102 A(age in year)+0.0204 W(weight in kg)-0.4889 S (sex, coded O for males and 1 for females).

ABSTRACT

Nocturnal penile tumescence (NPT) evaluation is the single most important step and widely used technique for the differential diagnosis between psychogenic and organic impotence. It is based on findings that male subjects have erection in association with rapid eye movement sleep. A normal pattern of NPT indicates that neural supply, vascular supply and penile structures are intact. Nocturnal penile tumescence minitoring (NPTM) in the abscence of rigidity evaluation in the assessment of the impotent male is of questionable value, but we conducted a study of NPT in 41 healthy male subjects aged 20 to 69 by means of portable tumistore NPT monitor to provide normative data for clinical utility in diagnosis and prognosis of impotence. The results indicate that NPT occurs consistently in a healthy male population and its expression is significantly affected by age.
