ABSTRACT

Objective: To establish work environments that prevent Japanese nursing assistants from leaving their jobs, we investigated the associations between their job satisfaction levels and intentions to remain on their jobs.Materials and Methods: We distributed anonymous self-administered questionnaires to Japanese female nursing assistants in 30 different hospitals. Regarding job satisfaction, we investigated various items relevant to both intrinsic facets (e.g., gaining various experiences) and extrinsic facets (e.g., salary), and conducted factor analyses of those items. Standardized partial regression coefficients were then calculated using multiple regression analyses. Intention to remain on the job was the dependent variable for this study. The factor scores calculated by the factor analyses of job satisfaction and respondents’ characteristics were the independent variables. Since multicollinearity occurred, we utilized two types of statistical models. After questionnaires with one or more missing values were excluded, 618 questionnaires were analyzed.Results: Using factor analyses, we discovered six factors related to job satisfaction among nursing assistants: “Intrinsic motivation”, “Salary”, “Relationships among nursing assistants”, “Registered nurses’ attitudes”, “Fatigue reduction”, and “Opportunities for vocational skills”. In statistical model 1, “Intrinsic motivation”, “Salary”, “Fatigue reduction”, “Age”, “Employment status”, and “Certified care worker status” were significantly associated with “Intention to remain on the job”. In statistical model 2, “Salary”, “Registered nurses’ attitudes”, “Fatigue reduction”, “Opportunities for vocational skills”, “Age”, and “Employment status” were significantly associated with “Intention to remain on the job”.Conclusion: “Intrinsic motivation” and “Opportunities for vocational skills” are included in intrinsic facets. To prevent nursing assistants from leaving their jobs, raising only their extrinsic job satisfaction levels is insufficient. Because nursing assistants have not received formal nursing education, they cannot take pride in having the same skills and expertise as registered nurses. However, hospital managers must respect their feelings and establish an appropriate working environment.

ABSTRACT

The huge shortage of nursing assistants in the industry, has made it imperative and feasible to build and develop a team of nursing assistants. This paper studied the present situation and problems of the nursing assistants in China, naming such setbacks as the personnel shortage, low competence, and poor professionalism. In comparison, overseas experiences in teambuilding were introduced with recommendations raised for China. These recommendations include higher social approval of elderly care industry, innovative development mechanism of nursing assistants, appraisal mechanism for nursing assistants performance, and better fund raising mechanisms for their incentives and training.

ABSTRACT

Objective:To investigate the supply status of dementia care services in pension agency in Guang zhou,thus to put forward suggestions for improving the service quality of the long-term care for dementia.Methods:Using questionnaire research and individual interview,an interview survey was conducted among managers,dementia and nursing assistant in ten pension agencies in Guangzhou.The data was analyzed by SPSS16.0 statistical method.Results:The dementia had been increasingly paid attention by the pension agencies,but there were still considerable defects in the aspect of the zone set up and service supply;the professional quality of nursing personnel in pension agencies had steadily increased,but shorthanded situation was more obvious.Conclusions:It suggests that we should provide specialized nursing level and the classification standard of care requirements for dementia elders;strengthen the training of mental health worker to enhance the level of medical care and supply ability of nursing assistant;allocate the mental health resources rationally;explore the “combination of medical treatment and endowment” service model which meets the need of dementia elders.

ABSTRACT

Objetivo: Identificar la presencia de estrés laboral en auxiliares de enfermería que trabajan en salas de hospitalización de adultos y las condiciones institucionales que lo configuran. Metodología: Estudio cualitativo de caso colectivo que parte de la determinación de características comunes o no entre los datos obtenidos de los casos individuales para su posterior análisis. Participaron seis casos: cinco mujeres y un hombre, auxiliares de enfermería con menos de 10 años de experiencia laboral en salas de hospitalización de adultos. Se utilizó la entrevista semiestructurada y se aplicó el consentimiento informado. Resultados: Las condiciones que provocaron el estrés fueron la Jornada laboral con tiempos reales de 13 ó 14 horas seguidas, sin espacios para la alimentación y el descanso, y actividades adicionales que no se encuentran dentro de las funciones normatizadas pero que son delegadas por otros profesionales de la salud y que se vuelven una carga más para ellas, tales como el transporte de pacientes, apoyo durante transfusiones sanguíneas, limpieza del carro de medicamentos y el estar ahí dando cuidado permanente a los pacientes. Como aminorante del estrés aparecen las relaciones entre el equipo de trabajo. Conclusión: Se identifica la presencia del estrés laboral en las auxiliares de enfermería que trabajan en salas de hospitalización de adultos. Las condiciones descritas limitan el tiempo para el cuidado de los pacientes y el cuidado de sí mismas y alteran su condición física, psicológica y emocional, factores que se relacionan con el estrés laboral. Objective: To identify the presence of stress in nursing assistants working in adult hospital rooms and institutional conditions that shape it. Methodology: Qualitative study of collective case, it parts on the determination of common or not characteristics between obtained dates of individual cases for later analysis. Six participant cases composed by five female and one male nursing assistants with less than 10 years of experience in adults hospital rooms. A semi-structured interview and a informed consent was applied. Results: The conditions that mediate stress were the workday with times of 13 or 14 hours continues without spaces for eating and resting and additional activities which aren’t in theirs functions but are delegated by other health professionals that increase their workload, like a moving patients, supporting blood’s transfusions, medication cart cleansing, and giving permanently care to patients. The relationships among the team appear as buffering of stress. Conclusion: The presence of occupational stress in nursing assistants that work in adults hospital rooms was identified. The conditions described limits the time for patient care and self care and affects their physical, psychological and emotional conditions, factors related to job stress. ABSTRACT

Objective To investigate the recognition of graded nursing and its influencing factors among nursing assistants in elder homes.Methods A questionnaire survey was conducted on 116 nursing assistants.Results The nursing assistants showed insufficient recognition of graded nursing,there were significant differences in role cognition,operation and practice training,salary,and the nursing workload,there were no significant differences in working years and educational levels.Conclusions The recognition of graded nursing was insufficient.It is suggested to increase the role cognition education,enhance the theory and practice training,provide higher salaries and allocate workload reasonably to ensure the implementation of graded nursing and increase the satisfaction level of the elderly.

ABSTRACT

Objective To describe nursing assistants' experience of caring for dementia patients with behavioral symptoms.Methods A descriptive qualitative design was used in this study.Twelve nursing assistants for dementia patients were recruited.A semi-structured interview was used to collect the data.The interview was related to nursing assistants' experience of caring for patients with behavioral symptoms.Content analysis was used to explicate the meaning of the interview data.Results Three themes were extracted:having multiple stressors; learning by practice; seeking mutual support from other nursing assistants.Conclusions Nursing assistants had multiple stressors from caring dementia patients with behavioral symptoms.Continuing supply of training on strategies of coping with behavioral symptoms as well as the emotional support are especially required.

ABSTRACT

While many studies have addressed the Korean nurse immigration to Germany in 1960s-70s in terms of the nurses' personal histories from a national perspective, few studies have pointed to its transnational impact on the medical field. Given this gap, the paper discusses its significance in the medical history through examining the nursing culture and policy changes made in both countries initiated by the immigration of the qualified Korean nurses. For this end, the paper first discusses differences between Korea and Germany in the nursing culture and professional standards at the time of the nurse immigration. The study then examines the transnational changes in the two nations in the nursing professional culture and related policies including nurse education and qualification process. More specifically, the paper argues that the Korean female nurses not only supplied labor in need but also contributed to the medical policy changes in Germany. The prevalent stereotype of nursing as a non-professional field in the German society begin to change while experiencing professionally qualified Asian immigrant nurses and practiced treatments that had been conduced by German doctors by then. This observation actually leads to the policy level reforms in nursing education and qualification process to meet the need of high-quality nurses when the labor immigration was stopped in mid-1970s. The paper also points out that the influence of the nurse immigration on the Korean society is not merely economic; it brought the policy level reforms in nurse education and qualification as well. The immigration, resulting in a lack of nurses in the Korean society, triggered two main responses: the expansion of the existing nurse education system and the establishment of a new system called "nursing assistant."

ABSTRACT

La razón enfermera-paciente es una medida que en ciertos países ha logrado establecer cuantitativamente algún tipo de relación con los indicadores de calidad de la atención de Enfermería. Objetivo: Establecer la relación de la razón Profesional de Enfermería-Paciente (REP) y Auxiliar de Enfermería-Paciente (RAUXP), con los indicadores de calidad de la atención de Enfermería en una unidad de cuidado intensivo de una IPS de la ciudad de Bogotá en el año 2012. Métodos: Estudio de tipo Descriptivo Correlacional desarrollado en una unidad de cuidado intensivo de Bogotá en un muestra de 537 turnos en los cuales se hicieron las observaciones. Este estudio fue parte de una investigación central sobre el tema realizado por un grupo de docentes de la Facultad de Enfermería de la Universidad Nacional de Colombia, cuya área temática es Gerencia en salud y enfermería. Resultados: No existe entre la REP y los indicadores estudiados una correlación significativamente distinta de cero. Con respecto a la RAUXP se identificó una correlación significativamente distinta de cero en salidas de catéter (p: 0,16) y extubaciones accidentales (p: 0,31) con valores marcadamente reducidos. A través de la prueba de Mann Withney se identificó una distribución similar de la REP y la RAUXP en los turnos donde se presentó los indicadores y en lo que no se presentó. Conclusiones: Los indicadores seguidos en este estudio no demostraron relación estadísticamente significativa con la REP o con RAUXP a través de la Coeficiente de correlación de Pearson excepto extubaciones y salidas de catéter. Se recomienda seguir estudiando esta relación con la inclusión de más variables como la severidad del paciente y ampliando el periodo de observación.
