ABSTRACT

Objective To study the application of nurse-led management in the nursing safety quality of CCU transition patients. Methods The control group of 60 patients who were transferred to the general ward after CCU treatment in Wuhan University Renmin hospital from October to December 2017 was selected, and 60 patients from January to March 2018 were set as the experimental groups. The control group carried out routine nursing, and the experimental group adopted nursing leading management model to carry out nursing intervention. To observe and compare the quality of nursing safety, quality of life and satisfaction of patients before and after intervention. Results The nursing safety incidents, nursing gaps and CCU return rates of control group patients were 13.33% (8/60),23.33% (14/60) and 11.67% (7/60), respectively. For the experimental group patients, they were 3.33%(3/60) ,8.33%(8/60) and 3.33%(2/60), respectively. The difference was statistically significant ( χ2 = 4.415, 4.582, 4.312, P<0.05). After the intervention, the quality of life of the patients improved significantly. The conditions of physical function, emotional function, social function and overall quality of life of the control group were 56.07 ± 4.42, 66.62 ± 1.89, 56.34 ± 4.31, and 61.26 ± 4.15, respectively. For the experimental group, they were 71.88 ± 4.67, 73.52 ± 4.28, 63.94 ± 4.12, and 74.62±4.02, respectively. The difference was statistically significant(t=7.657-16.892, P<0.01). After the intervention, the patient's satisfaction also increased significantly. The control group′s psychological support dimension, cooperation dimension, and safety dimension were 7.96 ± 0.86, 7.39 ± 0.61, and 7.63 ± 0.16, respectively. For the experimental group, they were 9.56 ± 0.41, 9.08 ± 0.56, 9.21 ± 0.33, respectively. The difference between the two groups was statistically significant (t= 7.632, 9.116, 7.562, P<0.01). Conclusion Nurse-led management can effectively improve the quality of care safety, quality of life and satisfaction of CCU transition patients, reduce the return rate of CCU, and provide clinical reference for safe care of CCU transition patients.

ABSTRACT

Objective To explore the effect of nursing project management on the improvement of night nursing safety quality. Methods Totally 332 cases of inpatients from department of cardiovascular medicine, in accordance with the fore-and-aft sequence of implementing nursing project management, divided into experimental group (160 cases) and control group (172 cases). Through the nursing project improvement management, analyzing the reason of high incidence of adverse events related with the nursing safety at night in control group. Formulating standardized solutions to improve the quality of nursing safety at night. To compare the incidence of night nursing safety incidents, nursing defects and the prevention of nursing defects of the two groups before and after intervention. Results Experimental night nursing safety of nursing adverse events, gaps and prevent nursing defects were occurred in the experimental group 8.75% (14/160), 10.00% (16/160), 3.12% (5/160), the control group after the interventionwere 1.74% (3/172), 2.33% (4/172), 11.04% (19/172), the difference was statistically significant (χ2=6.904, 9.981, 10.626,P< 0.01). After the intervention, patients' self-management ability was enhanced, the experimental group patient compliance, psychological emotion, safety consciousness, rational drug use rate was 56.25%(90/160),52.50%(84/160), 62.50%(100/160), 67.88%(107/160), the control group 86.63%(149/172), 74.41%(128/172), 91.28%(157/172), 88.37%(152/172), the difference was statistically significant (χ2= 4.656- 10.756, P<0.01 or 0.05). Conclusion Nursing project management can effectively improve the quality of night nursing safety and self-management of patients, reduce the occurrence of nursing defects, and improve the safety of nightcare.

ABSTRACT

Objective To explore the method and effect of checklist tool implemented in the safety management of clinical blood transfusion in intensive care unit. Methods A checklist was specially designed according to the technical standard and check system of clinical blood transfusion. 379 patients prescribed a blood transfusion enrolled in ICU from January to May 2013 were conducted as the control group before the checklist was designed. After the application of checklist during the blood transfusion process, 846 patients prescribed a blood transfusion enrolled in ICU from June to December 2013 were conducted as the experimental group. The incidence of blood transfusion safety related adverse events between the two groups was compared. Results The incidence of adverse events was 1.32% (5 of 379 patients) in the control group, and there was no sign of adverse events occurred in the experimental group. The difference had statistical significance (χ2=11.21, P<0.05). Conclusion The application of checklist reduced the incidence of blood transfusion safety related adverse events,and effectively improved the quality of nursing safety and patient satisfaction.
