ABSTRACT

Objective@#To study on the effect of clinical nursing in preventing adverse drug reaction of injection preparation of traditional Chinese medicine, explore nursing strategy of adverse drug reaction,promote the rational use of traditional Chinese medicine injection preparations in clinical practice by analyzing the adverse drug reactions of traditional Chinese medicine injection in our hospital.@*Methods@#A retrospective analysis of 362 cases of adverse drug reactions reported by the National ADR Surveillance Center from January 2017 to November 2018 was carried out in our hospital. In accordance with the guidelines of the Ministry of Health Manual on reporting and monitoring adverse drug reactions. By using Microsoft office Excel 2007, the ADR reports were summarized and analyzed according to age, sex, type and frequency, time of adverse reactions, and involvement system of adverse reactions.@*Results@#In 362 cases of ADR, the ratio of male to female patients was about 1.1∶1.There were 65 cases of children under 12 years old, accounting for 18%, and 117 cases of people over 61 years old, accounting for 32.3%,the proportion of other age groups was lower than those of the elderly and children. Most of the drugs with ADR appeared in blood circulation (208 cases), followed by heat-clearing and detoxifying drugs (102 cases) and anti-tumor drugs (52 cases). The common ADR involvement system of traditional Chinese medicine injection is nervous system reaction, skin allergic reaction, cardiovascular system reaction, local injury of injection, gastrointestinal tract system.@*Conclusions@#Attention should be paid to the role of nurses in monitoring adverse reactions of traditional Chinese medicine injections, strengthening the propaganda and education of nurses' ADR monitoring work, strengthening nurses' cognition of ADR, enhancing nurses' service and consciousness to patients with static injection of traditional Chinese medicine, and standardizing the process of drug use, thus reducing possible injury to patients due to adverse reactions caused by the use of traditional Chinese medicine injection.

ABSTRACT

Objective@#To summarize the nursing cooperation of axillobifemoral bypass surgery treating stent graft infection after endovascular aortic repair of abdominal aortic aneurysm.@*Methods@#To review a case of stent graft infection after endovascular aortic repair of abdominal aortic aneurysm and summarize the nursing cooperation of this patient retrospectively.@*Results@#After the medical care tacit cooperation and the infection protection, the procedure was successfully completed. The artificial blood vessels of the patient were unobstructed after the operation, and the pulse of both dorsalis pedis arteries was strong.@*Conclusions@#Axillobifemoral bypass surgery poses the characteristicsof long duration and complex. Designation of appropriate nursing measures, such as multi-incision management, thrombosis preventing and prevention of ischemia based on potential complications, are essentials of successful operation.

ABSTRACT

Objective To explore the effect of improving the process of intravenous therapy using six Sigma methodology among hospitalized patients. Methods Select patients of a certain ward in our hospital as the objects. Used the steps of standard six Sigma methodology as define, measure, analyze, improve, and control. Analyze the relevant causes and formulate improvement measures after identifying and defining the adverse events. Quality control team ensured the implementation of the improvement measures, and finally compared the incidences of adverse events before and after the implementation of the measures. Results Before implementation of the improvement measures, intravenous therapy was performed within 1056 persons in a month. There were 132 adverse events, and the percentage was 12.5%. After the implementation of improvement measures, 1102 patients were performed intravenous therapy. There were 81 adverse events, and the percentage was 7.4%. The incidence of adverse events was significantly reduced (χ2=16.08, P<0.01). Conclusion The application of six Sigma management method can effectively improve the effect of intravenous treatment of inpatients with reducing the incidence of adverse events.

ABSTRACT

The patients in the intensive care unit (ICU) often suffer from complicated and serious diseases, they are frequently necessary to have multiple tubes indwelling into the body and many practical measures or surgical operations performed by doctors or nurses, and generally they have no family members accompanying them in the unit. In order to avoid various accidents and ensure reaching the aim of treatment, according to the patient's individual situation, physical restraint straps are regularly applied. But the straps used for ICU patients may cause many problems in the course of restraint. In this paper, the existing problems are analyzed to reduce the adverse effects of this protective restraint on the physical and mental health of the patients.

ABSTRACT

Objective To investigate the nursing of replantation methods of sea-water soaked rotated avulsion amputated fingers.Methods There were 8 cases (16 fingers) with sea-water soaked rotated avulsion amputated fingers.5 cases were replanted by synchronic replantation method,the other 3 cases were replanted by traditional replantation method.We adopted complete debridement and high quality vascular anastomosis during the operation,and give hyperbaric oxygen therapy and anticoagulant treatment post-operation.Results There were 15 survival fingers in 16 replanted fingers.The survival rate was 93.75%.After follow-up for three months to two years,the total good rate was 87.5%.No wound infection and nonunion of fracture occurred.The outline of the fingers was satisfying and patients felt well.The twopoint discrimination was 3~6 mm after 3~24 months follow-up.Conclusions The injury of sea-water soaked rotated avulsion amputated fingers is complicated and the replantation operation is difficult.but if we can adopt individual measure according to the actual circumstance,it is possible that the sea-water soaked rotated avulsion amputated fingers can survive.

ABSTRACT

Objective To analyze the clinical characteristics and nursing measures of infection in hospital ICU,and to provide the refer-ence for clinical nursing. Methods From January 2010 to June 2013,116 patients with nosocomial infection in surgical ICU were collected as observation group,and 72 patients with nosocomial infection in the surgery were as the control group. The risk factors of nosocomial infec-tion were retrospectively analyzed,and the problems related to nursing were discussed. Results The nosocomial infection rate was 8. 44% in ICU,which was significantly higher than the surgical infection rate(4. 43%) in general ward (P<0. 05). Whether patients in ICU or surgery ward,respiratory and urinary tract infections were the most common site of nosocomial infection. The mortality rate in ICU was 9. 48%,which was higher than that of nosocomial infection in surgical ward (4. 17%),P<0. 05. The nosocomial infections were obviously related to the age of patients,duration of hospitalization,the invasive operation,antibiotic and immunosuppressive agents and disturbance of consciousness in ICU and general wards (P<0. 01). Conclusion The ICU nosocomial infection rate was significantly higher than that in general ward,the main infection sites are respiratory tract and urinary tract,with a variety of factors,the key is to establish the mechanism of prevention and ef-fective nursing strategy.

ABSTRACT

ObjectiveTo study the nursing strategy and high risk factors of critically ill patients complicated with lower respiratory tract infection in department of neurosurgery. Methods672 critically ill patients in department of neurosurgery in our hospital from January 2007 to September 2011 were selected as research object,and the rate of lower respiratory tract infection and high risk factors were studied and analyzed,and they were taken care of with special nursing. Results54 cases of 672 patients were with lower respiratory tract infection,the rate was 8.04%.Infection rate of elderly patients,patients of poor nutrition state,patients with underlying diseases was higher than those of other patients.Infection rate of patients with invasive operation,sputum suction,application of respirator,longer operative time and oxygen inhalation time was higher than those of other patients.Infection rate of patients with longer hospitalization time,emergency operation,application of many antibiotics and complication of preoperative infection of other site was higher than those of other patients,there were significant differences.The infection was well controlled with special nursing and treatment,four patients died of primary diseases. ConclusionsThe high risk factors of critically ill patients complicated with lower respiratory tract infection in department of neurosurgery include personal factors,treatment operation factors and other factors,and it can be controlled by special nursing and treatment.

ABSTRACT

O toque terapêutico (TT) é uma das mais antigas terapias vibracionais ainda em uso. Dolores Krieger e Dora Kunz, na década de 1970, foram precursoras da aplicação do método na enfermagem. Desde então, as pesquisas envolvendo o uso do toque terapêutico têm sido desenvolvidas com o intuito de demonstrar a efetividade da intervenção na redução de sinais e sintomas relacionados a diversas doenças. Muitos estudos avaliam o uso do TT de forma isolada, porém, grande parte das pesquisas compara esta estratégia com outras terapias complementares, tais como: música, imagem guiada e relaxamento. Os resultados desses estudos apontam melhora ou redução dos sinais e sintomas investigados nos pacientes submetidos ao TT. Entretanto, vieses metodológicos impossibilitam a generalização dos resultados, tornando indispensável a realização de estudos clínicos bem delineados capazes de comprovar a efetividade da intervenção, além de estudar populações que por ora não são abordadas, como os idosos e recém-nascidos.


Therapeutic touch (TT) is one of the oldest vibrational therapies still in use. Dolores Krieger and Dora Kunz, in the 1970s, pioneered the application of the method in nursing. Since then, research involving the use of therapeutic touch has been developed in order to demonstrate the effectiveness of the intervention to reduce the signs and symptoms related to various diseases. Many studies have evaluated the use of TT in isolation, however, a large part of these research studies compare this strategy with other complementary therapies, such as: music, guided imagery and relaxation. The results of these studies indicate improvement or reduction of signs and symptoms investigated in patients undergoing TT. However, methodological biases make it impossible to generalize the results, making it essential to conduct well-designed clinical trials capable of determining the effectiveness of the intervention, in addition to studying populations that have not yet been addressed, such as the elderly and newborns.


El toque terapéutico (TT) es una de las más antiguas terapias vibracionales aun en uso. Dolores Krieger e Dora Kunz, en la década de 1970, fueron precursoras de la aplicación del método en la enfermería. Desde entonces, las investigaciones que involucran el uso del toque terapéutico ha sido desarrollada con la intención de demostrar la efectividad de la intervención en la reducción de signos y síntomas relacionados a diversas enfermedades. Muchos estudios evalúan el uso del TT de forma aislada, sin embargo, gran parte de las investigaciones compara esta estrategia con otras terapias complementarias, tales como: música, imagen guiada y relajación. Los resultados de esos estudios aseguran la mejora o reducción de signos y síntomas investigados en los pacientes sometidos al TT. Entre tanto, los sesgos metodológicos imposibilitan la generalización de los resultados, volviendo indispensable la realización de estudios clínicos bien delineados capaces de comprobar la efectividad de la intervención, además de estudiar poblaciones que por ahora no son abordadas, como los ancianos y recién nacidos.

ABSTRACT

Objective To know the quality of life-related factors in patients with asthma, and then carry out certain effective counter nursing measures. Methods Investigated 137 patients with severe asthma, to know their nursing requirement and factors which can influence their quality of life, and then counert nursing measures were carried out for patients according to the results of investigation. Compared the quality of life of patients with asthma before and after the research. Results There was a negative correlation between the quality of life for patients with asthma and their smoking habit, sleeping station, disease course and complica-tions respectively, while a positive correlation between the quality of life for patients with asthma and their edu-caiton background, asthma-related knowledge and incomes. The nursing requirement about health education, psychological interview, treatment services were the main requirements. After the nursing intervention, the quali-ty of life for patients with asthma was significant better than before. Conchlsions There were several factors which can influence the quality of life for patients with asthma, analysis,carry out certain strategy and eval-uation a kind of proper mode for adult patients with asthma.

ABSTRACT

ObjectiveTo analyze the causes of discomfort occurring in the interventional treatment of diabetic foot, to discuss the individualized nursing measures for improving the patient's comfort and cooperation,and for increasing the successful rate of the interventional procedure.MethodsThe control group included 9 patients who received conventional nursing care.The study group had 13 patients who accepted individualized nursing care and nursing intervention, which was designed according to every patient's individual conditions. ResultsIn the control group, one patient could not endure the surgery to the end because of the long operating time. Another two patients had to take examinations repeatedly because the imaging quality was very poor,which was caused by the body movement due to uncomfortable mechanical stimulation.Urinary retention occurred in one patient.The mean operative time of the control group was 2.8 hour per surgery.The average dosage of contrast medium used was 150 -300 ml per procedure.All the subjects in the study group completed the surgery successfully,the mean operative time was 2.2 hour per surgery and the average dosage of contrast medium used was 100 -200 ml per procedure.Conclusion During the perioperative period of interventional treatment for diabetic foot, the effective individualized nursing care and nursing intervention,the measures to improve patient's comfort and the cooperation,etc. can certainly increase the successful rate,saving the operating time and reducing the contrast dosage.

ABSTRACT

Objective To study the related factors in diabetic patients with lOwer extremital deep vein thrombosis(DVT) and the nursing strategy. Methods 58 diabetic cases underwent lower limbs 99mTc-MAA veins imaging and/or ultrasunography. The related laboratory data such as fasting plasma glucose(FPG),blood cholesterol, blood triglycerides and clinic informations such as age, disease courses, lower limbs symptoms (swelling, erooted veins and diabetic foot) and acute complication (diabetic ketoacidosis and hyperosmolar non ketotic diabetic coma) were collected simultaneously. The relationship of each factors with DVT incidence were analyzed. Results 28 patients (48.3%) complicated with lower limbs DVT. The DVT incidence was signifi-cantly different among patients with different age, disease courses, FPG, blood cholesterol, blood triglycerides, lower limbs symptoms and acute complication, respectively. No significant difference was seen in patients with different blood pressure, body weight index, diabetes type. Conclusions Various dangerous factors accumu-lating in DM such as old age, long disease courses, high FPG, high blood cholesterol and high blood triglyc-erides contribute to the high incidence of lower limbs DVT. Early intervention, comprehensive treatment and in-tensive nursing should be applied to reduce the incidence of DVT.
