ABSTRACT

Objetivo: compreender o processo de alta hospitalar de crianças dependentes de tecnologias por profissionais de unidades pediátricas e neonatais. Método: estudo descritivo com os profissionais da assistência à criança dependente de tecnologias de um Hospital Universitário entre julho e setembro de 2020. Os dados foram obtidos foram exportados para o software Iramuteq e interpretados à luz da teoria de análise de conteúdo. O estudo possui aprovação pelo Comitê de Ética e Pesquisa. Resultados: a nuvem de palavras contextualizou e forneceu subsídios para a identificação de duas categorias: Compreensão da necessidade de trabalho em equipe para planejamento de alta hospitalar e percepção da necessidade de implementação de ferramentas como protocolos e fluxos para o direcionamento do preparo para alta e continuidade do cuidado. Conclusão: os profissionais referem que a alta hospitalar é realizada conforme a rotina assistencial e ainda segue um modelo medicalocêntrico.


Objective: to understand the process of hospital discharge of technology-dependent children by professionals from pediatric and neonatal units. Method: descriptive study with technology-dependent child care professionals at a University Hospital between July and September 2020. The data obtained was exported to the Iramuteq software and interpreted in light of content analysis theory. The study is approved by the Research Ethics Committee. Results: the word cloud contextualized and provided support for the identification of two categories: Understanding the need for teamwork to plan hospital discharge and perception of the need to implement tools such as protocols and flows to guide preparation for discharge and continuity of care. Conclusion: professionals report that hospital discharge is carried out according to the care routine and still follows a medical-centric model.


Objetivo: comprender el proceso de alta hospitalaria de niños dependientes de tecnología por parte de profesionales de unidades de pediatría y neonatología. Método: estudio descriptivo con los profesionales de cuidado al niño que depende de tecnologías en un Hospital Universitario, entre julio y septiembre de 2020. Los datos obtenidos se exportaron al software Iramuteq y se interpretaron a la luz de la teoría del análisis de contenido. El estudio es aprobado por el Comité de Ética en Investigación. Resultados: la nube de palabras contextualizó y trajo subsidios para la identificación de dos categorías: comprensión de la necesidad de trabajo en equipo para planificar el alta hospitalaria y percepción de la necesidad de implementar herramientas como protocolos y flujos para orientar la preparación para el alta y la continuidad de la atención. Conclusión: los profesionales refieren que el alta hospitalaria se realiza según la rutina asistencial y sigue un modelo médico centrado.

ABSTRACT

Objective:To develop a training program for specialty nurses of traditional Chinese medicine (TCM) technology, in order to provide a basis for the training of traditional Chinese medicine nursing techniques for traditional Chinese medicine speciallty nurses.Methods:Literature review and semi-structured interviews were conducted to preliminarily develop training program outline. From February to April 2022, a total of 18 experts were invited to finish the questionnaire survey. And the items were modified according to the experts′ advice.Results:The response rates of 2 rounds of consultation were both 100%, the authority coefficients of experts were 0.894 and 0.903 respectively, and the kendall harmony coefficients were 0.201 and 0.272 respectively. Finally, the training program was formed, which consists of 6 first-level indexes, 25 second-level indexes and 81 third-level indexes.Conclusions:The training program for specialty nurses of TCM technology constructed in this study is scientific, reliable and practical, which can provide a basis for the training of TCM nursing technology.

ABSTRACT

Introduction: pronation is a procedure used at the Intensive Care Unit with patients presenting Acute Respiratory Distress Syndrome (ARDS) with severe hypoxemia. Bibliographical references clearly support the application of this procedure due to the improvement it produces in oxygenation and the increased survival rate in patients. The main foundation behind this technology is that Ventral Decubitus (VD) promotes an improved redistribution of ventilation toward the dorsal regions of the lung, which are mainly collapsed during Dorsal Decubitus. Nursing staff have used this procedure because VD has proven to be a strategy with a useful and accessible impact over respiratory physiology. Objective: to analyze Pronation as a nursing technology and its promotion of COVID-19 patients' wellbeing at the General Hospital Dr. Manuel Gea González, in Mexico City. Material and methods: a nonexperimental, quantitative, cross-sectional, prolective, descriptive study to analyze pronation, as a nursing technology that promotes wellbeing in the care of COVID-19 patients at General Hospital Dr. Manuel Gea González, in Mexico City. Pronation as a nursing technology was the variable being measured. The universe was 530 nursing professionals working at the Hospital. The population and sample were 52 nursing specialists working with COVID-19 patients, representing 9.81% of the Universe. Results: as for training and experience with Pronation, 50% of the staff consider they have received sufficient training in regards to the SARS-CoV-2 virus, but they are still under training. Additionally, 84.62 % consider that Pronation promotes a remarkable recovery in the patients, with oxygen saturations between 80 to 100 %; 71.16 % believe that patients do show hemodynamic instability due to the position shift, but not in every case; and 80.77 % consider they make sure their patients do not develop pressure ulcers, given that 65.39 % practice patient hygiene and movement to prevent such ulcers. Discussion: from the interviewed staff, 50.01 % consider they have received sufficient training regarding COVID and the pronation of patients. This is consistent with Hernandez et al., who describe the need of a team trained specifically on the procedure, preferably comprised of Nurses specialized in Intensive Care. Likewise, 84.62% of staff members believe Pronation improves oxygen saturation in patients from 80% to 100%, this is contrasted with Barrantes and Vargas, who indicate that if Pronation does not result in 5% improvement in oxygen saturation, the patient must be returned to dorsal decubitus. Conclusion: ventilation in prone position is a pulmonary protective action used for over 30 years that produces an evident and continuous improvement in blood oxygenation and the respiratory physiology. Therefore, nursing specialists caring for COVID-19 patients recommend its use.


Introducción: la pronación es una maniobra utilizada en las unidades de cuidados intensivos, en pacientes que presentan síndrome de distrés respiratorio agudo (SDRA), cuando la hipoxemia es severa. Apoyada por una sólida evidencia científica, tiene impacto en la forma en que se ventila a los pacientes como parámetro de seguridad, esto aumenta la supervivencia. Objetivo: analizar la pronación, como tecnología de enfermería, que produce bienestar en el cuidado de los pacientes COVID-19, en el Hospital General Dr. Manuel Gea González, en la Ciudad de México. Material y métodos: estudio de carácter cuantitativo, no experimental, transversal, prospectivo, descriptivo. Variable medida: la pronación, como tecnología de enfermería. El universo 530 profesionales de enfermería que trabajan en el hospital, muestra: 52 especialistas de enfermería que trabajan con pacientes COVID-19. Resultados: en relación a la capacitación y experiencia en pronación, el 50% del personal manifiestan que sí recibieron suficiente capacitación sobre el virus SARS-CoV-2, aunque siguen capacitándose en ésta área; el 84.62 % consideran que en la pronación, los pacientes mejoran bastante, saturando entre el 80 y 100 % de oxígeno y el 80.77 % siempre cuidan que los pacientes no tengan úlceras por presión.Discusión: el 50.01 % del personal entrevistado manifiesta que sí recibió suficiente capacitación sobre la COVID-19 para pronar a los pacientes, lo que es semejante a la investigación de Hernández GD., et al.¹ Conclusión: la ventilación en posición prono es una medida de protección pulmonar utilizada hace más de 30 años que produce una mejoría evidente y sostenida de la oxigenación de la sangre y en la fisiología respiratoria, por lo que los especialistas de enfermería que atiende pacientes COVID-19 recomiendan su aplicación, pues está asociada al aumento de probabilidad de supervivencia.

Objective@#By a statistical analysis and summary of the literature on Traditional Chinese Medicine (TCM) nursing technology published in the core nursing periodical, research the currect status of TCM nursing technology.@*Methods@#Retrieved CNKI database (CNKI) and Wanfang medical journal database,according to Chinese Journal Citation Report (2017 edition) of China Institute of science and Technology Information Research, Choose 10 core nursing periodicals and retrieve the experimental articles related to TCM nursing technology from January 1,2013 to January 1, 2019.this paper analyze time, resources, authors, fund-sponsored researches, statistical methods by bibliometrics analysis.@*Results@#A total of 624 papers were included,of which 132 were in 2013 and 124 in 2014, 103 in 2015, accounting for 57.5%. "Nursing Research" contains 227 articles, 100 "Chinese Journal of Modern Nursing, 98 "Journal of Nursing", accounting for 68.1%. The literature has been published in 28 provinces and cities, 117 in Jiangsu Province, 100 in Guangdong Province, and 89 in Shanghai, accounting for 62.6%. The source of funds for fund papers is mainly provinces and cities, accounting for 38.4%. Involving 22 TCM nursing technology operations, 762 applications, 174 acupoint massage, 113 auricular point application, 109 acupoint points, and 106 moxibustion methods, accounting for 66.3%. There are more than 120 types of diseases involved, mainly cancer, stroke, hypertension, and diabetes. There are more than 180 kinds of symptoms, mainly about pain, constipation, quality of life and so on.@*Conclusion@#TCM nursing research has made great progress, TCM nursing technology is currently widely used in clinical practice. Scientific research has also confirmed its effectiveness, but it lacks high-quality core papers. At the same time, due to the diversity of research content, there is a lack of evidence-based nursing research literature.

A prematuridade é a principal causa de morte em recém-nascidos, sendo um problema de saúde global, pois, no mundo, a cada 15 bebês nascidos prematuramente, um vai a óbitos. Os avanços tecnológicos na saúde contribuem para o aumento da sobrevida de bebês cada vez mais imaturos e, associado a este avanço há o aumento de dependência de tecnologias permanentes ou temporárias por estes bebês para auxílio da sobrevida ou qualidade de vida. Visto isso, sabe-se da importância da presença precoce dos pais nas unidades neonatais e da educação destes para os cuidados domiciliares com seus filhos. Objetivou-se assim, desenvolver um protótipo de um aplicativo móvel sobre o cuidado com o bebê pré-termo, com necessidades especiais e dependente de tecnologia, para a família. Trata-se de uma pesquisa aplicada que segue os preceitos do método do Design Instrucional Construtivista para o desenvolvimento do aplicativo móvel, de forma dinâmica, a partir da identificação na literatura das demandas de conhecimento dos pais de bebês com necessidades especiais e dos tópicos a serem abordados, desenvolvendo design visualmente ergonômico, intuitivo em relação à usabilidade e agrupamento de saberes, multimídia e que representasse a clientela, a fim de garantir significado à aprendizagem. Como produto final, o protótipo do app Baby Care Tech está organizado em conteúdos sobre colostomia, gastrostomia, oxigenoterapia, sonda enteral e traqueostomia, subdivididos em 22 telas, além do ícone do app, implementadas de multimeios (textos e hipertextos, vídeos, animações, figuras, áudios e games) que abordam desde cuidados diários como manutenção, higiene e monitorização, até sinais de alerta e possíveis problemas que o bebê possa apresentar, favorecendo a interatividade, motivação, autonomia e integração com a realidade na busca da aprendizagem significativa em que haja apreensão e modificação dos saberes. Para tanto, foi utilizada uma linguagem simples e adequada, associada a ícones com objetivo de favorecer intuição e entendimento, além da definição de cores claras que destaquem as ilustrações e escritas, a fim de gerar contraste para destacar os dispositivos temas do material, gerar maior legibilidade, torna-lo visualmente aprazível, assim como retratar a população alvo. Por fim, o protótipo do app desenvolvido é parte de um novo e relevante nicho, uma vez que há a demanda educacional dos cuidadores dessas crianças e a quase inexistência de tecnologias desenvolvidas para esta clientela, bem como, o fato de vivenciarem a era digital. Os usuários do app, muitos nativos e outros imigrantes digitais, conectados às tecnologias nos diferentes setores da vida e experiências têm demonstrado que as ferramentas contribuem com a aprendizagem e devem estar cada vez mais presentes no processo educacional. Em estudos futuros, em continuidade, pretende-se findar o desenvolvimento deste aplicativo, validar com especialistas e usuários e integrá-lo aos serviços de saúde


Prematurity is the leading cause of newborns death, being a global health problem because in the world, every 15 babies born prematurely, one dies. The technological advances in health contribute to the increase of the survival of babies increasingly immature and, associated to this advance, there is a raise in the dependence of permanent or temporary technologies by these babies to help their survival or quality of life. Given this, we know the importance of early parental presence in neonatal units and their education to home care their children. The aim of this project was to develop a mobile application prototype for the family about preterm baby cares, with special needs and dependent on technology. This is an applied research based on the precepts of the Constructivist Instructional Design method for the development of the mobile application, dynamically, starting by the identification in the literature of the knowledge demands of the parents of babies with special needs and the topics to be approached, developing visually ergonomic design, intuitive in relation to the usability and grouping of knowledge, multimedia and that represents the clients, in order to guarantee meaning to the learning. As a final product, the prototype of the Baby Care Tech app is organized into contents about colostomy, gastrostomy, oxygen therapy, enteral catheter and tracheostomy, subdivided into 22 screens, as well as the app icon, implemented in multimedia (texts and hypertexts, videos, animations, figures, audios and games) that range from daily care such as maintenance, hygiene and monitoring, to warning signs and possible problems that the baby may present, favoring interactivity, motivation, autonomy and integration with reality in the search for meaningful learning in which there is apprehension and modification of the knowledge. For that, a simple and suitable language was used, associated with icons with the purpose of favoring intuition and understanding, as well as the definition of clear colors that highlight the illustrations and writings, in order to contrast the material themes of the material, generate greater legibility, visually pleasing, as well as represent the target population. Finally, the developed app prototype is part of a new and relevant niche, since there is the educational demand of the caregivers of these children and the almost nonexistence of technologies developed for this clientele, as well as the fact of experiencing the digital era. App users, the most natives and other digital immigrants, connected to technologies in different walks of life and experiences have demonstrated that the tools contribute to learning and should be increasingly present in the educational process. In future studies, in continuity, it intends to finish the development of this application, to validate with specialists and users and to integrate it to the health services

Transitó por la institución desde sus estudios técnicos, cuando era Politécnico de la Salud, en el cual asumió diversas responsabilidades; actualmente se mantiene como profesora del Departamento de Informática Médica donde desempeña una meritoria labor


The pedagogic activity, scientific work, recognitions and work carried out in the Internationalist Mission by the university graduate Vivian Benito Valenciano are exalted, who with 33 years of working experience constitutes a personality of obligatory consultation when it is sought to deepen in the history of the Nursing Technology Faculty in Santiago de Cuba, of which she is founder. She was in the the institution since her technical studies, when it was Health Polytechnic, in which she assumed diverse responsibilities; at the moment she stays as a professor of the Medical Computer Science Department where she performes a meritorious work

Introdução: Esta pesquisa toma como objeto o desenvolvimento de uma tecnologia de comunicação e informação, o app, como instrumento educativo para abordar as necessidades em saúde de jovens. A partir da saúde coletiva, toma como referência teórica, a concepção de juventude como categoria social, conforme determinada pelo modo de produção vigente e por suas necessidades de reprodução, que impõem aos jovens, das diferentes classes sociais, padrões de socialização recortados pelas necessidades de preparação para o mercado de trabalho. Tomou-se também como referência o conceito de necessidades em saúde como necessidades sociais, ou seja, como necessidades referidas às classes sociais. Objetivo: construir um app, de caráter educativo, relacionado às necessidades em saúde de jovens, considerando-se a perspectiva desses jovens. Método: Trata-se de investigação de natureza qualitativa, que utilizou a metodologia da pesquisa-ação emancipatória desenvolvida por vertente teórico-metodológica materialista histórica e dialética. O processo empregou 24 oficinas com 12 jovens do segundo grau de uma escola pública, do município de São Paulo, durante período de 5 meses, em 2017. Os jovens participantes envolveram-se porque estavam motivados pela temática; são provenientes de famílias da classe trabalhadora, cujos pais trabalham em ocupações que exigem pouca ou nenhuma qualificação profissional. O app desenvolvido com a participação dos jovens foi analisado por um painel de especialistas, a partir de instrumento padronizado. Resultados: O aplicativo foi denominado "fala sério" e apresenta como abas os seguintes temas eleitos pelos jovens: família, saúde mental (dividida em pressões e drogas), corpo e sexualidade. As oficinas foram analisadas de acordo com as categorias de análise: necessidades de reprodução social; necessidades de presença do Estado; e necessidades de participação social. Os jovens trouxeram à tona as seguintes categorias empíricas: as formas de enfrentamento das famílias das precárias formas de trabalhar e viver podem ser individualizadas ou coletivas; os jovens têm medo de não dar conta das pressões sociais, que supostamente viabilizariam seu futuro; o consumo de drogas é visto socialmente como a causa dos problemas dos jovens e a educação sobre drogas está desvinculada da realidade; educação sexual na escola é importante e a relação com a própria sexualidade é barrada por valores dominantes, principalmente machismo; as dificuldades sociais enfrentadas pelos jovens limitam suas possibilidades. A análise dos especialistas foi positiva para a maior parte dos conteúdos, destacando-se os conteúdos de família, pressões e drogas. Houve vários apontamentos para aperfeiçoamento dos conteúdos principalmente relativos às temáticas redução de danos, corpo e sexualidade, e ainda à dimensão técnico-operacional do aplicativo, como funções e interatividade. Conclusão: o aplicativo foi desenvolvido a partir das necessidades em saúde que mais preocupam o grupo de jovens participantes da pesquisa, o que indica serem as maiores dificuldades do grupo. O resultado deve refletir as dificuldades de outros grupos que vivem condições semelhantes de reprodução social; eles se referem principalmente a configurações familiares, que sofrem com empregos precários e desemprego, ausência ou inconstância de apoio paterno, ressocialização através da escola e processos de sociabilidade precários, expressos em desgastes como ansiedade, depressão, consumo problemático de drogas. Certas condições contemporâneas universalizantes e geracionais, que perpassam as classes sociais, permitirão também o uso do App por grupos de jovens em diferentes condições sociais. Nota-se com frequência a preocupação com a gravidez na adolescência, como caraterística de sociabilidade e socialização desse grupo. O processo educativo gerado pela pesquisa-ação emancipatória mostrou-se importante instrumento de desalienação. Os jovens se envolveram profundamente no processo de desenvolvimento do app e reivindicaram a continuidade das discussões.


Introduction: The objective of this research was to develop a technology of communication and information, an app, as an educational instrument to address the health needs of young people. Our theoretical reference considers the concepts of the collective health field and of youth as a social category influenced by the prevailing production mode and its needs of reproduction. This mode imposes, for young people of the different social classes, social standards permeated by the need to prepare for the labor market. We also used the concept of health needs as social needs, i.e., as the needs of social classes. Objective: to develop an educational app aimed at the health needs of young people, considering their perspective. Method: This is a qualitative research in which we used the emancipatory methodology of action research that was developed using the theoretical-methodological principles of dialectical and historical materialism. We performed 24 workshops with 12 young people at high school from a public school in the city of São Paulo, Brazil, for five months in 2017. They participated in the study because they were motivated by the topic; they are from working-class families whose parents work in occupations that require little or no professional training. The app developed by the young participants was analyzed by a panel of experts using a standardized instrument. Results: The application was named Fala sério and features the following subjects as tabs: family, mental health (divided into pressures and drugs), body and sexuality. The workshops were analyzed according to the categories: needs of social reproduction; needs of the state protection; and needs of social participation. The participants pointed the following empirical categories: the coping strategies of the families to handle the precarious work and living conditions can be individualized or collective; they are afraid of being unable to handle the social pressures that would supposedly support their future; drug use is seen by society as the cause of their problems and the education about drugs is detached from their reality; sex education at school is important and their own relation with sexuality is difficult due to dominant values, mainly machismo; the social difficulties they face limit their possibilities. The analysis of specialists was positive for most contents, especially for family, pressures and drugs. Several notes were made to improve the contents, especially those related to the themes of harm reduction, body and sexuality and to the technical and operational dimension of the application, such as functions and interactivity. Conclusion: the application was developed from the health needs that most concern the youth group who participated in the research, this indicates that these are the major difficulties of this group. The result must reflect the difficulties of other groups living in similar conditions of social reproduction; they refer mainly to family settings that suffer with precarious jobs and unemployment, the absence or inconstancy of paternal support, resocialization through school and precarious processes of sociability expressed by stresses such as anxiety, depression, and problematic drug use. Certain contemporary conditions are more inclusive, generational and capable of perpass social classes, these conditions will allow the use of the app for young people from different social conditions. The concern with adolescent pregnancy frequently appears as a characteristic of sociability and socialization of this group. The use of the emancipatory action research served as an important instrument of de-alienation through the educational process. The youth group became deeply involved with the development of the app and claimed the continuity of the discussions.

Objective To evaluate the clinical application of TCM nursing therapies and identify the problems during the therapy implementation, in order to explore the countermeasures to promote its clinical improvement. Methods A descriptive cross-sectional study was conducted,and a self-designed questionnaire was employed to investigate 218 nurses from 148 grade A class three TCM hospitals in China. Results 25 TCM nursing therapies were implemented in these hospitals. Ear acupressure therapy was the most frequently implemented,accounting for 83.5%(182/218);while wax therapy was the least implemented, accounting for 7.8%(17/218). The implementation frequency of these technologies varied significantly, 0~860 times a week. The top three of them were acupoint application therapy, hot compression of Chinese medicine and ear acupoints therapy.TCM nursing therapies were considered to be the most frequently used in insomnia, headache, edema, arthralgia, cold, epigastric pain, hypochondriac pain and fever.Storage of TCM nurses,lacking of TCM knowledge and inapposite therapy implementation were the main problems in the application. Conclusions Scientific and rational allocation of human resources, cultivation of the TCM-specific nursing professional, enhancement of nurses'awareness of Chinese medicine, establishment of a reasonable charging standard, and strengthening the risk management in implementation will promote its clinical development.

Objective To discuss the teaching method and effect of teaching-learn-do integration mode in the Basic Nursing Technology.Methods Students of two classes of grade 2010 and two classes of grade 2011 in our nursing college were randomly divided into the experimental group and the control group,they respectively adopted teaching-learn-do integration teaching mode and first-theory-after-practice traditional teaching method.The teaching effect of the two groups by theory and skills testing was compared.Results The experimental groups were better than the control groups in two grades in skill examination results.For the theoretical examination results,the experimental group of grade 2011 was better than the control group,while the two groups of grade 2010 were undifferentiated in theoretical examination.Conclusions Teaching-learn-do integration mode can not only improve students' operating skills,but also promote the theoretical learning,and improve the students' learning ability of independent innovation,suitable for practical and technical-strong course teaching in Basic Nursing Technology.

ABSTRACT

Objective To improve the nursing skills of low age and service seniority nurses and meet the needs of clinical nurses,in order to serve patients better and improve the quality of cave.Methods According to knowledge tests 50 points for nurse job-related skill issued by Chinese Ministry of Health,our hospital selected 12 commonly used basic operation skills and designed comprehensive cases through integration and reformation.We trained and examined nurses with undergraduate degree came to our hospistal in 2008.Results After training,the score of clinical nursing skill reached 91.81,86 nurses reached the standard and the effective compliance rate was 95%.Conclusions It proved to be an effective method to improve the nursing skills of low-age and service seniority nurses by carrying out the comprehensive casestyle training methods.
