ABSTRACT

Introduction. L'étude visait évaluer les indicateurs de l'alimentation du nourrisson et du jeune enfant liés à l'état nutritionnel dans la zone de santé de Kapolowe, province du Haut Katanga avant les essais cliniques. Matériel et méthodes. Nous avons conduit une étude descriptive transversale dans la communauté auprès des 568 couples mère-enfant 6-23 mois évalués sur les indicateurs nutritionnels associés à la malnutrition. Résultats. L'allaitement maternel optimal était observé chez 10,7% des couples mère-enfant et l'alimentation complémentaire adéquate bénéficiée par 5,5% des enfants. Presque 25,6% des ménages utilisaient des toilettes améliorées, 98,8% des ménages buvaient l'eau des sources protégées et 12,1% des mères pratiquaient un minimum de lavage de mains. Le premier aliment complémentaire donné à la moitié des enfants (46%) était importé, mais 60,3 % des mères utilisaient le mélange farine de maïs + huile + sucre comme aliment de complément. La prévalence de la malnutrition aiguë, de la malnutrition chronique et de l'insuffisance pondérale était respectivement de 11,9%, 37% et 26,8%. La malnutrition aiguë était associée à l'occupation de la mère, au niveau socio-économique du ménage, à la Religion de la mère, à la consultation prénatale suivie par la mère, au Minimum de Diversité Alimentaire, à l'allaitement maternel non optimal, aux infections récurrentes et au faible poids de naissance. L'insuffisance pondérale était associée au sexe, à l'occupation de la mère, au niveau socio-économique, à la religion de la mère, au minimum de diversité alimentaire, à l'allaitement maternel non optimal, aux infections récurrentes et au faible poids de naissance. Introduction. The aim of the study was to assess infant and young child feeding indicators related to nutritional status in the Kapolowe health zone, Haut Katanga province prior to the clinical trials. Material and methods. We conducted a descriptive cross-sectional study in the community among568 mother-child pairs 6-23 months of age assessed on nutritional indicators associated with malnutrition. Results. Optimal breastfeeding was observed in 10.7% of the mother-child pairs and adequate complementary feeding in 5.5%. Almost 25.6% of households used improved toilets, 98.8% of households drank water from protected springs and 12.1% of mothers practiced minimal hand washing,12.1% of mothers practiced minimal handwashing. The first supplementary food given to half of the children (46%) was imported food, but 60.3% of mothers used maize flour + oil + sugar as a complementary food. The prevalence of acute malnutrition, chronic malnutrition and under weight were 11.9%, 37% and26.8% respectively. Acute malnutrition was associated with the mother's occupation, household socio-economic level, mother's religion, prenatal consultation attended by the mother, minimum dietary diversity, non-optimal breastfeeding, recurrent infections, and low birth weigh. Underweight was associated with gender, maternal occupation, socio-economic level, maternal religion, minimum dietary diversity, non-optimal breastfeeding, recurrent infections and low birth weight. Chronic malnutrition was associated with gender, socio-economic level, minimum meal frequency, type of toilet used, non-optimal breastfeeding, recurrent infections, and low birth weight. Conclusion. The prevalence of malnutrition, in all its forms, is still very high and worrying among children aged 6-23 months. The feeding practices are predictors of it.

ABSTRACT

Objective@#To investigate the effects of nutritional interventions based on nutritional risk assessment strategies on nutritional status and liver function in patients with cirrhosis.@*Methods@#Totally126 patients with cirrhosis admitted from July 2016 to October 2017 were enrolled. The prospective study was used and the patients were randomly divided into the treatment group and the control group with 63 cases in each group. The control group received routine nursing intervention, while the treatment group was combined nutritional intervention based on the nutritional risk assessment strategy. After 3 months of follow-up, serum nutrition indicators, liver function, and complications were compared between the two groups.@*Results@#The serum albumin, prealbumin and transferrin levels in the treatment group were (42.36±5.24) g/L, (0.37±0.08) g/L, and (3.01±0.42) g/L, which were significantly higher than those in the control group, which were (34.24±5.36) g/L, (0.30±0.07) g/L, (2.65±0.52) g/L (t=8.598, 5.227, 4.275, P<0.05 or 0.01); serum alanine aminotransferase and total bilirubin content were (95.32±12.45) μmol/L, (272.36±30.12) μmol/L, which were significantly lower than those of the control group [(114.21±13.20) μmol/L, (320.42±41.52) μmol/L]. The prothrombin time [(14.36±1.32) s] was significantly shorter than that of the control group [(16.12±1.20) s] (t=8.263, 7.437, 7.831, P<0.05); the complication rate of the treatment group was 7.94% (5/63), which was significantly lower than that of the control group (22.22%) (14/63)(χ2=5.020, P<0.05).@*Conclusions@#Nutrition intervention based on nutritional risk assessment strategy can improve nutritional status, relieve liver damage and reduce the incidence of complications such as peritonitis in liver cirrhosis patients.

ABSTRACT

Introduction: Malnourished children are about 20% in the developing world. Food insecurity is a key risk factor for child malnutrition. Food insuffi ciency, an extreme form of household food insecurity, can aff ect physiological mechanisms that are linked to an individual’s nutritional status. Food-insuffi cient children are also more likely to have poorer health status and to experience a range of negative academic and psychosocial outcomes. Methods: We administered a cross-sectional socioeconomic survey to 354 households in research site, including a validated food insuffi ciency measurement questionnaire, and obtained anthropometric measurements from children aged 12 to 24 months. We used chi-square tests to assess the relationship between household food insuffi ciency and nutritional status of children. Results: Average age of study children was 18 months and standard deviation was (± 3.2 months). Th e status of household food insuffi ciency was 56%. Th e prevalence of underweight, stunting and wasting was 24%, 36% and 8% respectively. Th e household food insuffi ciency was signifi cantly (p<0.05) associated with underweight and stunting but not with wasting (p>0.05). Discussion: Th e study results indicate that food insuffi ciency is associated with stunting and underweight but not with wasting in urban slum of Bangladesh. We also found that child malnutrition is associated with mother’s education, father’s education, monthly family income and people per room.

ABSTRACT

OBJETIVO: Analisar o desempenho de indicadores de obesidade central por correlação e concordância com índice de massa corporal para avaliação nutricional em adolescentes segundo sexo e idade. MÉTODOS: Estudo seccional do tipo censo incluindo 635 adolescentes, de 10 a 19 anos (342 do sexo masculino e 293 do feminino), realizado em escola pública da cidade do Rio de Janeiro. Variáveis do estudo: índice de massa corporal (IMC), circunferências de cintura e abdômen, razões cintura-quadril e cintura/altura, estado nutricional, sexo e idade. Desenvolveram-se análise univariada e bivariada controlada por estratificação. RESULTADOS: As circunferências de cintura e abdômen correlacionaram-se positiva e significativamente (p < 0,05) com o IMC em ambos os sexos, faixa etária e estado nutricional, enquanto a correlação de razão cintura/altura foi influenciada por sexo e estado nutricional. Para razão cintura/quadril, a variação do sentido da correlação foi influenciada ainda por idade. As circunferências de cintura e abdômen foram dependentes da variação dos valores de IMC em todos os grupos com elevados R² (0,66 a 0,80) e ² positivo, o que não ocorreu para razões cintura/quadril e cintura/altura. Somente a circunferência de cintura apresentou agregamento moderado com o IMC (Índice Kappa de 0,41 a 0,57) em todos os estratos, enquanto nos demais indicadores o agregamento variou de relativo a moderado. CONCLUSÕES: Dentre os indicadores de obesidade central analisados, as circunferências de cintura e abdômen apresentaram os melhores desempenhos, destacando-se a circunferência de cintura como mais apropriada para uso complementar ao IMC na avaliação nutricional de adolescentes.

ABSTRACT

This study aimed at learning about the nutritional profile of HIV-positive individuals undergoing antiretroviral therapy and at comparing the performance of nutritional-state indicators. A transversal study was performed on 94 patients attending the Tropical Diseases Outpatient Hospital of Botucatu Medical School (FMB) - UNESP. The body mass index (BMI) and the classification by Papini-Berto (PB) were used to evaluate nutritional state, aiming at detecting malnutrition and obesity. The waist-to-hips ratio (W/HR) and waist circumference (WC) were adopted for identification of abdominal obesity and lipodystrophy. According to BMI, most of the individuals were eutrophic, followed by 30.9 percent overweight and 6.4 percent malnourished. By using the PB classification, the frequency of malnourished increased (22.3 percent). The analysis of the PB classification in relation to BMI indicated that the former presented high sensitivity and good specificity for malnutrition diagnosis, namely 100 percent and 83 percent, respectively. The prevalence of abdominal obesity was 7.44 percent according to WC, and a higher prevalence (38.3 percent) was observed when taking W/HR into account. There was significant positive association between nutritional diagnosis according to PB and T CD4+ lymphocyte. The results support the use of PB classification for malnutrition detection as well as that of BMI and W/HR for overweight and fat redistribution.
