ABSTRACT

SUMMARY INTRODUCTION: EUS-guided gastroenterostomy (EUS-GE) is a novel procedure for palliation of malignant gastric outlet obstruction (GOO). Our aim was to evaluate the outcomes of this technique in our initial experience. METHODS: Patients with GOO from our institute were included. Technical success was defined as the successful creation of a gastroenterostomy. Clinical success was defined as the ability to tolerate a soft diet after the procedure. We assessed adverse events and diet tolerance 1 month after the procedure. RESULTS: Three patients were included. Technical and clinical success was achieved in all cases. There were no adverse events and good diet tolerance was observed 1 month after the procedure in the included patients. CONCLUSION: EUS-GE is a promising treatment for patients with GOO.


RESUMO INTRODUÇÃO: A gastroenterostomia ecoguiada é um novo procedimento para paliação da obstrução maligna gastroduodenal. Nosso objetivo foi avaliar os resultados dessa técnica em nossa experiência inicial. MÉTODOS: Foram incluídos pacientes com obstrução maligna gastroduodenal de nossa instituição. O sucesso técnico foi definido como a realização adequada de uma gastroenterostomia. O sucesso clínico foi definido como boa aceitação de dieta pastosa durante a internação. Os eventos adversos e a aceitação alimentar foram avaliados um mês após o procedimento. RESULTADOS: Três pacientes foram incluídos. Os sucessos técnico e clínico foram alcançados em todos os casos. Não houve eventos adversos e a aceitação alimentar permaneceu adequada um mês após o procedimento nos pacientes incluídos. CONCLUSÃO: O EUS-GE é um tratamento promissor para pacientes com obstrução maligna gastroduodenal.

ABSTRACT

Introdução: A obstrução gástrica tem como principal fator etiológico a estenose por doença ulcerosa péptica, com alguns raros casos relacionados ao uso crônico de anti-inflamatórios não esteroides (AINEs). Métodos: Descreve-se um caso de um paciente com obstrução gástrica do estômago excluso, pós gastroplastia em Y de Roux. Relato: Paciente feminina, 51 anos, submetida a gastroplastia em Y de Roux em 2003, usuária crônica de AINEs devido ao quadro de fibromialgia. Evoluiu com quadro de obstrução gástrica por fibrose de piloro, sendo submetida a gastrectomia do estômago excluso. Conclusão: Complicações no estômago excluso são raras, fazendo-se necessário a investigação diagnóstica desses pacientes frente ao número crescente de operações bariátricas e suas complicações


Introduction: Gastric obstruction has as its main etiological factor stenosis due to peptic ulcer disease, with some rare cases related to chronic use of non-steroidal anti-inflammatory drugs (NSAIDs). Methods: A case report of a patient with gastric obstruction of the excluded stomach after Roux-en-Y gastroplasty. Report: A 51-year old female patient who underwent Roux-en-Y gastroplasty in 2003, a chronic NSAID user due to fibromyalgia, developed gastric obstruction for pyloric fibrosis and submitted to gastrectomy of excluded stomach. Conclusion: Complications in the excluded stomach are rare, requiring the diagnostic investigation of these patients in view of the growing number of bariatric operations and their complications

ABSTRACT

Os autores descrevem um caso de massa abdominal epigástrica palpável associada a sintomas dispépticos em uma paciente jovem sem antecedentes patológicos. Estudos por imagem evidenciaram corpo estranho gástrico de "aspecto enovelado". A paciente foi submetida a gastrotomia via laparotomia, com retirada de um tricobezoar de 1.950 gramas.


The authors describe the case of a palpable epigastric mass associated with dyspeptic symptoms in a young female patient with no previous pathologic history. Imaging studies demonstrated the presence of a gastric foreign body with an "entangled appearance". The patient underwent laparotomy and gastrotomy for removal of a trichobezoar weighing 1,950 grams.

ABSTRACT

Context: Trichobezoars (TB) are bezoars formed by ingested hair. TB are more common among girls and young women. The findings include a hard, mobile and painless abdominal mass in the epigastrium, anorexia, nausea and vomiting, weight loss and reports of thichophagia. Image abdominal studies can confirm the diagnosis. The treatment is based on the complete TB remotion.Case report: M.O.M., 16 years old, female, single, student, born and living in São Paulo. She reported anorexia, abdominal discomfort, halitosis and mild colicky abdominal pain associated with a lump three years ago. On examination: huge, non-tender, mobile, painful, palpable mass in the upper abdomen (13cm) without inflammatory sign. Plain films of abdomen: diffuse radiopaque mass in upper abdomen without gastric bubble. Abdominal ultrasound and computadorized tomography show enlarged stomach filled by heterogeneous material. Endoscopy demonstrates the stomach completely filled with black hair. Once the diagnosis was made, surgical treatment was indicated.
