ABSTRACT

Objective@#To observe the distribution of occupational activity disorders of extremely severe mass burn patients in recovery period after injury.@*Methods@#From December 2014 to December 2015, 65 extremely severe burn patients conforming to the inclusion criteria involved in August 2 Kunshan factory aluminum dust explosion accident were admitted to Kunshan Rehabilitation Hospital. They received comprehensive rehabilitation treatment after burns, including wearing pressure clothes, ultrasound treatment, semiconductor laser and red light irradiation, motor function training, and so on. Over 2 years after injury, a cross-sectional survey was conducted on the patients′ occupational activity disorders. Modified Barthel index (MBI) was used to assess the degree of activities of daily living (ADL) disorder of patients and to grade the independent level of completing each item of MBI, and then the independent level of patients completing self-care MBI items (bathing, dressing, grooming, eating, going to the toilet, urine control, and stool control) was compared with that of mobility items (going up and down stairs, bed and chair transfer, and walking). The Canadian Occupational Performance Measure (COPM) was used to assess the distribution of occupational activity disorders of patients. The distribution of the five most serious occupational activity disorders was counted, then the frequency and probability of which with frequency greater than or equal to 16 times were calculated. Data were processed with Pearson Chi-square test.@*Results@#Over 2 years after injury, the MBI score of patients was (76±22) points, and the ADL of 83.08% (54/65) patients reached completely self-care or light ADL disorder level. The MBI items arranged according to the completing independent level of patients from high to low were urine control/stool control, walking, bed and chair transfer, going up and down stairs, going to the toilet, eating, grooming, dressing, and bathing. The independent level of patients completing self-care MBI items was lower than that of mobility items (χ2=62.298, P<0.001). Over 2 years after injury, the five most serious occupational activity disorders in COPM dimension were mainly concentrated in the self-care dimension, accounting for 55.38% (180/325), followed by 22.46% (73/325) of production activities and 22.15% (72/325) of recreational activities, and the centrally distributed item was the personal self-care item under self-care dimension, accounting for 42.46% (138/325). Over 2 years after injury, the five most serious occupational activity disorders with frequency greater than or equal to 16 times were dressing and undressing, bathing, perineal cleaning, wearing pressure clothes, caring for children, visiting relatives and friends, 31, 25, 16, 17, 18, and 22 times respectively, with a probability of 47.69%, 38.46%, 24.62%, 26.15%, 27.69%, and 33.85% respectively.@*Conclusions@#Over 2 years after injury, most of the patients with extremely severe burns caused by the aluminum dust explosion were completely or basically self-care in their daily life. The disorder of self-care ADL was more serious than that of mobility, and the disorder of individual self-care activity was still the most serious occupational activity disorder of patients in this stage.@*Clinical trial registration@#Chinese clinical trial registry, ChiCTR-OOC-16009188.

ABSTRACT

Abstract Aim To conduct an exploratory study about cancer incidence and mortality in rural workers in the state of Acre, Brazil. Methods The Proportional Cancer Incidence Ratio (PCIR) was calculated for rural male workers who live in Rio Branco, Acre, with histological diagnosis of cancer, and the Cancer Mortality Odds Ratio (CMOR) comparing cancer deaths occurred in rural workers in the state of Acre with those occurred in people with other occupations in the period from 2007 to 2012. Results High and statistically significant PCIR were observed for tumors of larynx, PCIR = 7.55 (95% CI 4.23-12.46); melanoma, PCIR = 11.44 (95% CI 6.55-18.54); and non – Hodgkin lymphoma, PCIR = 10.00 (95% CI 5.17-17.50) compared to the population of Rio Branco. Regarding the mortality compared analysis, we can observe CMOR = 6.94 (95% CI 1.73-27.89) for pancreatic cancer and CMOR = 13.89 (95% CI 1.25-153.60) for cancer of the urinary tract. Conclusion The results suggest an increase in the incidence estimates of mortality for selected tumor sites among rural workers that are consistent with the literature.


Resumo Objetivo Realizar um estudo exploratório sobre a incidência e mortalidade por câncer em trabalhadores rurais do estado do Acre, Brasil. Metodologia Foram calculadas a Razão Proporcional de Incidência de Câncer (PCIR) para os trabalhadores rurais residentes em Rio Branco, Acre, com diagnóstico de neoplasia e a Razão de Chances de Mortalidade por Câncer (CMOR), comparando-se os óbitos por câncer de trabalhadores rurais no estado do Acre com aqueles ocorridos com indivíduos de outras ocupações no período de 2007 a 2012. Resultados PCIR elevadas e estatisticamente significativas foram observadas para os tumores de laringe, PCIR = 7,55 (95% IC - 4,23-12,46), melanoma, PCIR = 11,44 (95% IC - 6,55-18,54) e linfoma não Hodgkin, PCIR = 10 (95% IC - 5,17-17,50) em relação à população de Rio Branco. Quanto à análise comparada da mortalidade, foram encontradas CMOR = 6,94 (95% IC - 1,73-27,89) para câncer de pâncreas e CMOR = 13,89 (95% IC - 1,25-153,60) para neoplasias do trato urinário. Conclusão Os resultados observados sugerem uma elevação nas estimativas de incidência e mortalidade de localizações tumorais selecionadas entre os trabalhadores rurais, consistentes com a literatura.

STUDY DESIGN: Prospective cohort study. PURPOSE: To determine whether there was any change in the quality of life of patients in sedentary/non sedentary occupations treated with epidural steroid injection for lumbar disc herniations using the 8 components of the SF 36 questionnaire.Overview of Literature: No previously done similar study published. OVERVIEW OF LITERATURE: No previously done similar study published. METHODS: Ninety patients comprising sedentary and non sedentary occupations with lumbar disc herniations on magnetic resonance imaging who were treated with epidural steroid injection at St. John's Hospital Bangalore who met the Spinal Outcomes Research Trial eligibility criteria from April 2009 to May 2010. RESULTS: Of the 90 patients evaluated 44 were of Sedentary and 46 were of non sedentary activity levels, At 6 months primary outcomes physical functioning (p = 0.573, in difference between sedentary and non sedentary, improvement p = 0.001) energy/fatigue (difference between the two p = 0.917, improvement p = 0.001), emotional well being (difference p = 0.912, improvement, p = 0.001), social functioning (difference p = 0.523, improvement p = 0.232), pain (difference p = 0.535, improvement p = 0.001), general health (difference p = 0.738, improvement p < 0.001). CONCLUSIONS: There was a statistically significant improvement in patients of both the sedentary and non sedentary groups p < 0.001 in all components of the SF36 in both sedentary and non sedentary patients except social functioning where the improvement was not statistically significant, and there was no significant difference between non sedentary and sedentary populations over time.

ABSTRACT

@#Objective To apply Canadian Occupational Performance Measure (COPM) on the patients injured in Sichuan Earthquake. Methods COPM was applied to evaluate 51 patients in hospital before and 1 months after treatment. The first evaluation confirmed the problems on occupational activities for them. Then the plan was made to solve the problems. The second evaluation was to assess the effect of the treatment.Results The problem of self-care activity is more than that of productive activities and leisure activities(P<0.01). Total scores of performance of occupational activity and satisfaction improved(P<0.01, P<0.01)Conclusion COPM is helpful to confirm the problems of occupational activities and contribute to develope the primary goals for rehabilitation and treatment programs.
