ABSTRACT

Objective: To determine the change in contrast sensitivity in patients operated on for myopia with surface techniques and its relationship with ocular aberrometry. Methods: An experimental study was carried out with 324 eyes of 162 myopic patients treated at the Refractive Surgery Clinic of the Cuban Institute of Ophthalmology "Ramón Pando Ferrer" between January 2019 and March 2020 who underwent refractive surgery with excimer laser and surface techniques and aspheric ablation profile. Contrast sensitivity was the main variable of comparison between both surgical techniques. Two groups of patients were sequentially assigned, the first group underwent PRK-MMC (81) and the second LASEK-MMC (81) with a 3-month follow-up. Results: Women with mild myopia and ages between 21 and 28 years predominated. After 3 months contrast sensitivity, uncorrected visual acuity, spherical equivalent and total average square value improved significantly, showing a positive relationship with contrast sensitivity in both groups, with no differences between them. Conclusions: Contrast sensitivity improves with surface techniques and has a positive and significant relationship with visual, refractive and aberrometric outcomes. The study of ocular aberrations provides a fundamental tool to assess the optical quality of the human eye, information that allows us to know the vision quality that can be expected in the patients evaluated(AU)

ABSTRACT

PURPOSE: We compared the ocular aberration and clinical outcome between different aspheric intraocular lenses (IOL) in both eyes. METHODS: This prospective randomized controlled study was comprised of patients with bilateral cataract who received two different aspheric IOLs implanted in both eyes: negatively aspheric Tecnis® ZCB00 and spherically neutral Akreos® MI60. Total and corneal aberrations computed by Wavescan® and Pentacam® were assessed at 6 months to investigate the effects of the IOL's spherical aberration on the eye and to analyze the incidence and degree of posterior capsule opacification. By using spherical aberration of the cornea and the IOLs, values calculated via Ray-tracing software and Wavescan® were compared. Total spherical aberration was analyzed by the MATLAB program and converting the pupil size to 6.0, 4.5, 3.0 mm. RESULTS: A total of 25 patients were included. Regarding pre-operative corneal aberration, ZCB00 group was 0.232 ± 0.119 µm while MI60 group was 0.240 ± 0.117 µm, and there was no difference between the two IOLs. At 6 months after total ocular spherical aberration, MI60 group (pupil size 6.0 mm; 0.296 ± 0.097 µm, 4.5 mm; 0.094 ± 0.032 µm, 3.0 mm; 0.019 ± 0.006 µm) had more positive values than ZCB00 group (pupil size 6.0 mm; 0.051 ± 0.105 µm, 4.5 mm; 0.009 ± 0.034 µm, 3.0 mm; 0.002 ± 0.007 µm) (p < 0.001). When calculated using the ray tracing method, based on the results after surgery, MI60 group's total spherical aberrations were higher than ZCB00 group. However, from 1 month to 6 months after surgery, the uncorrected distance visual acuity, spherical equivalent and posterior capsule opacification showed no differences between the two IOLs. CONCLUSIONS: In eyes with aspheric IOLs with negative spherical aberration, spherical aberration was lower than spherically neutral aspheric IOLs. Regarding postoperative visual acuity, spherical equivalent and posterior capsule opacification, there were no significant differences between the two groups.

ABSTRACT

PURPOSE: To report the distribution of ocular higher-order aberrations in candidates for refractive surgery measured by Zywave(R) II aberrometer. METHODS: The present study included 232 eyes of 116 subjects. Ocular aberration data were obtained by measurements per eye using Zywave(R) II aberrometer. The mean Zywave spherical equivalent (SE) and higher order aberrations (HOAs) calculated in the central 6-mm zone and expressed as root mean square (RMS) values were analyzed. Correlation analysis was performed to assess the association between ocular HOAs and gender, age, SE refractive error, or central corneal thickness (CCT) and investigate the aberration symmetry between right and left eyes. RESULTS: The average SE was-4.67 +/- 1.83 diopters (D). The mean RMS values of total HOA, 3rd, 4th or 5th summated HOAs, coma, trefoil and spherical aberration (SA) were 0.421 +/- 0.201 microm, 0.346 +/-0.206 microm, 0.202 +/- 0.105 microm, 0.087 +/- 0.048 microm, 0.241 +/- 0.172 microm, 0.225 +/- 0.154 microm and 0.136 +/- 0.102 microm, respectively. There was no significant differences of the mean total HOA, summated HOAs, coma, trefoil and SA between genders, age and refractive errors, but the 3rd order trefoil was strongly related with myopia (r = -0.900, p = 0.008). There was symmetry of ocular aberrations between both eyes and the ocular aberrations were not correlated with CCT. CONCLUSIONS: Herein, we report the normative distribution of ocular aberrations in the myopic Korean population measured by Zywave(R) II aberrometer. The 3rd order trefoil and myopia were correlated and mirror symmetry was observed between right and left eyes.
