ABSTRACT

ABSTRACT The CONVINCE study, recently published in the New England Journal of Medicine, reveals a groundbreaking 23% reduction in the relative risk of all-cause mortality among end-stage kidney patients undergoing high convective volume hemodiafiltration. This significant finding challenges the conventional use of high-flux hemodialysis and offers hope for improving outcomes in chronic kidney disease patients. While some controversies surround the study's findings, including concerns about generalizability and the causes of death, it is essential to acknowledge the study's design and its main outcomes. The CONVINCE study, part of the HORIZON 2020 project, enrolled 1360 patients and demonstrated the superiority of hemodiafiltration in reducing all-cause mortality overall, as well as in specific patient subgroups (elderly, short vintage, non-diabetic, and those without cardiac issues). Interestingly, it was shown that hemodiafiltration had a protective effect against infection, including COVID-19. Future research will address sustainability, dose scaling effects, identification of subgroups especially likely to benefit and cost-effectiveness. However, for now, the findings strongly support a broader adoption of hemodiafiltration in renal replacement therapy, marking a significant advancement in the field.


RESUMO O estudo CONVINCE, publicado recentemente no New England Journal of Medicine, revela uma redução inovadora de 23% no risco relativo de mortalidade por todas as causas entre pacientes renais em estágio terminal submetidos à hemodiafiltração de alto volume de convecção. Esse achado significativo desafia o uso convencional da hemodiálise de alto fluxo e oferece esperança de melhoria dos desfechos em pacientes com doença renal crônica. Embora algumas controvérsias cerquem os achados do estudo, incluindo preocupações sobre a generalização e as causas de óbito, é essencial reconhecer o desenho do estudo e seus principais desfechos. O estudo CONVINCE, parte do projeto HORIZON 2020, inscreveu 1.360 pacientes e demonstrou a superioridade da hemodiafiltração na redução da mortalidade por todas as causas em geral, bem como em subgrupos específicos de pacientes (idosos, HD de curta duração, não diabéticos e aqueles sem problemas cardíacos). Curiosamente, demonstrou-se que a hemodiafiltração teve um efeito protetor contra infecções, incluindo a COVID-19. Pesquisas futuras abordarão sustentabilidade, efeitos de escalonamento da dose, identificação de subgrupos especialmente propensos a se beneficiar e a relação custo-benefício. No entanto, por ora, os achados apoiam fortemente uma adoção mais ampla da hemodiafiltração na terapia renal substitutiva, marcando um avanço significativo na área.

ABSTRACT

La albúmina sérica humana es la proteína más abundante en el plasma, su estructura molecular le confiere estabilidad, pero también flexibilidad para ligar y transportar un amplio rango de moléculas. Su función oncótica es la propiedad más reconocida que la lleva a introducirse en la terapéutica médica como un expansor de volumen. Sin embargo, en los últimos años se le han adicionado funciones con carácter antioxidante, inmunomodulador y de estabilización endotelial, que hacen presumir que su impacto terapéutico está más allá de sus funciones volumétricas. En los últimos años, específicamente en la cirrosis y la falla hepática aguda sobre crónica, se ha tenido un cambio en el paradigma fisiológico, desde una perspectiva netamente hemodinámica hacia una perspectiva inflamatoria, en donde las funciones oncóticas y no oncóticas de la albúmina están alteradas y tienen un carácter pronóstico en estas entidades. Este conocimiento creciente, desde una perspectiva inflamatoria, hace que se fortalezca el uso terapéutico de la albúmina sérica humana desde las indicaciones tradicionales como prevención de la disfunción circulatoria posparacentesis, prevención y tratamiento de lesión renal aguda, hasta las discusiones para administración a largo plazo en pacientes cirróticos con ascitis.


Human serum albumin is the most abundant protein in plasma, with a molecular structure that provides stability while also allowing flexibility to bind and transport a wide range of molecules. Its oncotic function is the most recognized property, leading to its introduction in medical therapy as a volume expander. However, in recent years, additional functions with antioxidant, immunomodulatory, and endothelial stabilization properties have been identified, suggesting that its therapeutic impact extends beyond its volumetric functions. Specifically, in cirrhosis and acute-on-chronic liver failure, there has been a shift in the pathophysiological paradigm from a purely hemodynamic perspective to an inflammatory perspective, where both oncotic and non-oncotic functions of albumin are altered and have prognostic significance in these conditions. This growing understanding from an inflammatory perspective strengthens the therapeutic use of human serum albumin, not only for traditional indications such as the prevention of post-paracentesis circulatory disfunction, prevention and treatment of acute kidney injury, but also for discussions regarding long-term administration in cirrhotic patients with ascites.

Resumo Introdução: Atualmente o Brasil mostra um expressivo aumento da sobrevida e consequente elevação do número de idosos na sua população. Do ponto de vista de propostas para a melhoria da qualidade de vida deles, a sexualidade deve ser compreendida a partir do princípio holístico, não somente do fator biológico. Nesse cenário, novas reflexões de profissionais da saúde tornam-se indispensáveis para o planejamento de ações específicas, objetivando a atenção integral à saúde do idoso. Relato de experiência: O presente relato de experiência traz o início de um diálogo com acadêmicos de Medicina de uma universidade pública sobre a complexidade da sexualidade e infecções sexualmente transmissíveis (IST) em idosos, buscando identificar o conhecimento desses discentes sobre o assunto e idealizar propostas de ação ao público-alvo. Para isso, estruturou-se uma estratégia de ensino-aprendizagem cuja abordagem consistiu na adaptação e aplicação da metodologia do Arco de Maguerez para o debate sobre IST no contexto da saúde pública brasileira, o qual foi realizado com base nas diretrizes do ensino remoto emergencial. Nossos resultados vão de encontro à literatura que visa compreender as representações sociais acerca da sexualidade dos idosos. Neles, verificou-se que as representações sociais acerca da sexualidade na terceira idade assemelham-se à descrição científica, apresentado similitude entre o senso comum e o conhecimento erudito. Discussão: Diante da discrepância descrita na literatura quanto aos conceitos errôneos por parte dos idosos e dos familiares e pelos profissionais de saúde acerca da sexualidade na terceira idade, são imprescindíveis a realização de campanhas e o desenvolvimento de medidas e estratégias preventivas voltadas para idosos, a fim reduzir a incidência de IST nessa comunidade. Para isso, são fundamentais estratégias de educação em saúde no ensino acadêmico e de educação profissional continuada para sedimentar os conhecimentos necessários na idealização de propostas eficazes. Conclusão: A partir da formação médica adequada e por meio da discussão dos aspectos complexos sobre o assunto, serão possíveis a construção, a implementação e a avaliação de políticas públicas de saúde para o enfrentamento das IST, de modo a diminuir as barreiras relacionadas à prevenção de doenças e à promoção de saúde sexual na população idosa.


Abstract Introduction: Brazil is currently reporting a significant increase in survival and consequent rise in its elderly population. From the point of view of proposals to improve their quality of life, sexuality should be understood from a holistic, and not just a biological, perspective. In this scenario, new reflections by health professionals become indispensable for the planning of specific actions aimed at providing comprehensive health care for the elderly. Experience report: This experience report describes the beginning of a dialogue with medical students from a public university about the complexity of sexuality and sexually transmitted infections among the elderly, seeking to identify their knowledge of the subject and devise proposals for action with the target audience. To this end a teaching-learning strategy was structured based on an approach that adapted and applied the Arc of Maguerez methodology to the debate on sexually transmitted infections (STIs) in the context of Brazilian public health, which was conducted out within the guidelines of emergency remote learning. Our results are in line with the literature that seeks to understand the social representations about sexuality among the elderly. It was found that the social representations about sexuality in the elderly are similar to the scientific description, with similarities between common sense and scholarly knowledge. Discussion: Given the discrepancy described in the literature regarding the misconceptions on the part of the elderly, family members and health professionals about sexuality in old age, it is essential that campaigns be conducted, and preventive measures and strategies developed for the elderly to reduce the incidence of STIs in this community. Hence, health education strategies are needed in academic teaching and continuing professional education to consolidate the knowledge required to devise effective proposals. Conclusions: Based on fit-for-purpose medical education, the discussion of complex aspects about this subject will support the construction, implementation, and evaluation of public health policies to tackle STIs, reducing barriers related to the prevention and promotion of sexual health in the elderly population.

ABSTRACT

BACKGROUND@#The thoracic small biopsy sampling procedure including transbronchial forceps lung biopsy (TBLB) and endobronchial ultrasound transbronchial needle aspiration (EBUS-TBNA) can be accompanied by rapid on-site evaluation (ROSE) of sample material to provide immediate feedback for the proceduralist. The present study aims to investigate the supplemental effect of ROSE smear samples for lung cancer molecular test.@*METHODS@#In a retrospective study, 308 patients admitted to our hospital from August 2020 to December 2022 undergoing diagnostic TBLB and EBUS-TBNA with ROSE and subsequently diagnosed as non-small cell lung cancer (NSCLC) were analyzed. The matched formalin-fixed paraffin-embedding (FFPE) tissue section and ROSE smears for tumor cellularity were compared. DNA yields of smears were determined. Real-time polymerase chain reaction (PCR) and next-generation sequencing (NGS) were performed on adequate smear samples.@*RESULTS@#ROSE smear samples were enriched in tumor cells. Among 308 biopsy samples, 78 cases (25.3%) exhibited inadequate FFPE tissue sections, whereas 44 cases (14.3%) yielded adequate smear samples. Somatic mutations detected in the FFPE tissue section samples were also detected in the matching adequate smear sample.@*CONCLUSIONS@#ROSE smear samples of the thoracic small biopsies are beneficial supplemental materials for ancillary testing of lung cancer. Combined use of cytology smear samples with traditional FFPE section samples can enhance the detection rate of informative mutations in patients with advanced NSCLC. We recommend that the laboratory could further evaluate the ROSE cell smears of the patient when FFPE tissue sections are inadequate, and that adequate cell smears can be used as a supplemental source for the molecular testing of NSCLC.

ABSTRACT

The neuroimmune system is crucial for the development, aging, and damage of the central nervous system, and has gradually become a research hotspot. Triggeringreceptor expressed on myeloid cells-2 (TREM2) is a transmembrane receptor of the immunoglobulin superfamily and is mainly expressed in the microglia in the central nervous system. An increasing number of studies indicate that TREM2 has great potential to improve cognitive dysfunction related to Alzheimer's disease, vascular dementia, Parkinson's disease, postoperative cognitive impairment, obesity, etc. However, there is a lack of a systematic summary of the specific role of TREM2 in cognitive dysfunction. This paper reviews the progress in the latest research on the related mechanisms of TREM2 in cognitive dysfunction, in order to provide new strategies for the treatment of cognitive dysfunction.

ABSTRACT

ObjectiveTo investigate the clinical value of serum creatinine-to-cystatin C ratio (CCR) in evaluating the prognosis of hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). MethodsA retrospective analysis was performed for the clinical data of 130 patients with HBV-ACLF (treatment group) who were hospitalized in Department of Infectious Diseases, The First Affiliated Hospital of Soochow University, from January 2021 to November 2022. According to the treatment outcome, they were divided into survival group with 87 patients and death group with 43 patients; according to the presence or absence of infection, they were divided into infection group with 37 patients and non-infection group with 93 patients. A total of 30 individuals who underwent physical examination during the same period of time were enrolled as control group. Routine blood test results were collected on the day of admission, including white blood cell count, platelet count, neutrophil count, and lymphocyte count; serum creatinine, cystatin C, serum albumin (Alb), and prothrombin time (PT) were observed on the day of admission and on days 5, 10, and 15 of hospitalization, and related indicators were calculated, including CCR, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), prognostic nutritional index (PNI), CCR5 (CCR on day 5 after admission), ΔCCR5 (CCR on day 5 after admission minus CCR on the day of admission), CCR10 (CCR on day 10 after admission), ΔCCR10 (CCR on day 10 after admission minus CCR on day 5 after admission), CCR15 (CCR on day 15 after admission), and ΔCCR15 (CCR on day 15 after admission minus CCR on day 10 after admission). The above indicators were compared between the survival group and the death group and between the infection group and the non-infection group. The Mann-Whitney U test was used for comparison of continuous data between two groups, and the Kruskal-Wallis H test was used for comparison between multiple groups. The univariate and multivariate logistic regression analyses were used to investigate the influencing factors for disease prognosis; the receiver operating characteristic (ROC) curve was used to assess the value of CCR in predicting HBV-ACLF death events, and the DeLong test was used for comparison of the area under the ROC curve (AUC). ResultsThere were significant differences in CCR, NLR, PNI, PT, and Alb at baseline between the treatment group and the healthy control group (all P<0.001), and there were significant differences in CCR, NLR, and PT between the survival group and the death group on the day of admission (all P<0.05). Among the 130 patients with HBV-ACLF, there were 25 in the precancerous stage, 48 in the early stage, 32 in the intermediate stage, and 25 in the advanced stage, and there were significant differences in baseline CCR, PLR, and PT between the patients in different stages of HBV-ACLF (all P<0.05). There were significant differences in ΔCCR5 and NLR between the infection group and the non-infection group (P<0.05), and there were significant differences in ΔCCR5, CCR10, and CCR15 between the survival group and the death group (all P<0.05). The multivariate logistic regression analysis showed that ΔCCR5 (odds ratio [OR]=1.175, 95% confidence interval [CI]: 1.098‍ — ‍1.256, P<0.001), NLR (OR=0.921, 95%CI: 0.880‍ — ‍0.964, P<0.001), and PT (OR=0.921, 95%CI: 0.873‍ — ‍0.973, P=0.003) were independent influencing factors for the prognosis of HBV-ACLF patients. ΔCCR5 had an AUC of 0.774, a sensitivity of 0.687, and a specificity of 0.757, and the AUC of ΔCCR5+PT+NLR was 0.824, which was significantly higher than the AUC of ΔCCR5, NLR, or PT alone (all P<0.05). ConclusionΔCCR5, NLR, and PT can reflect the condition and prognosis of patients with HBV-ACLF and are independent predictive indicators for death events in patients with HBV-ACLF. The combination ofΔCCR5, PT, and NLR has the best predictive efficiency.

ABSTRACT

ObjectiveTo establish the clinical comprehensive evaluation index system for Chinese patent medicine(CPM) based on Evidence and Value:Impact on DEcisionMaking(EVIDEM) framework, so as to promote the scientific, systematic and standardized implementation of clinical comprehensive evaluation of CPM. MethodThe clinical comprehensive evaluation index system was determined through literature review, semi-structured interview and Delphi method, and the weights of each index were clarified by analytic hierarchy process(AHP). ResultThe recovery rates of both rounds of expert consultation were 100%, and the authority coefficient of experts was 0.90 and 0.905, respectively. Kendall's coordination coefficients(W) of the second- and third-level indicators in the first-round consultation were 0.320 and 0.283(P<0.001), and in the second round were 0.411 and 0.351, respectively(P<0.001). The finally constructed clinical comprehensive evaluation index system for CPM included 6 first-level indicators, 13 second-level indicators and 28 third-level indicators. Among the first-level indicators, the weights of effectiveness, safety, economy, innovation, suitability and accessibility were 37.34%, 32.68%, 11.85%, 5.87%, 5.79% and 6.47%, respectively. ConclusionThis study has constructed a universal clinical comprehensive evaluation index system for CPM, and the domain and criteria are introduced and interpreted in detail, which can provide reference and information for carrying out the clinical comprehensive evaluation of CPM, but it needs to be refined and improved in combination with the clinical practice of CPM for specialized diseases.

ABSTRACT

Acute-on-chronic liver failure has complex conditions, rapid progression, and a high mortality rate, and further studies are still needed to clarify its pathogenesis and etiology. The establishment of animal models for acute-on-chronic liver failure can not only provide a good basis for exploring the pathogenesis of acute-on-chronic liver failure, but also provide an experimental basis for clinical treatment. Through a literature review, this article summarizes the methods commonly used to establish the animal models of acute-on-chronic liver failure, including carbon tetrachloride combined with LPS/GaIN, thioacetamide combined with LPS, serum albumin, and bile duct ligation. This article analyzes the characteristics of various animal models, so as to provide documentary and experimental bases for further exploration of more ideal animal models.

ABSTRACT

ObjectiveTo sort out the historical evolution, prescription evolution and modern clinical application of Huagaisan. MethodHuagaisan and its synonym Huagaitang are used as keywords to search the databases of Traditional Chinese Medicine Think Tank, Chinese Medical Dictionary, Airusheng Chinese Medical Database and China National Knowledge Infrastructure(CNKI). According to the inclusion and exclusion criteria, we obtained the information of ancient books and modern clinical research literature related to Huagaisan, and systematically reviewed and analyzed the historical origin, prescription composition, preparation method, dosage, efficacy, medicinal material origin, processing method and modern clinical application of Huagaisan. ResultA total of 198 pieces of ancient book information were included, involving 93 ancient Chinese medicine books. Huagaisan was composed of fried Perillae Fructus, red Poria, fried Mori Cortex, Citri Eoxcarpium Rubrum, stir-fried Armeniacae Semen Amarum, Ephedrae Herba and fried Glycyrrhizae Radix et Rhizoma, which had the efficacy of promoting the lungs and relieving epidemiological symptoms, expelling phlegm and relieving cough, and treating cough with wind-cold bundled epidemiological symptoms and stagnation of phlegm and Qi. The preparation method was suggested as boiling powder, crushing the seven herbs into coarse particles, the dosage of each drug was fried Perillae Fructus of 1.27 g, red Poria of 1.27 g, fried Mori Cortex of 1.27 g, Citri Eoxcarpium Rubrum of 1.27 g, stir-fried Armeniacae Semen Amarum of 1.27 g, Ephedrae Herba of 1.27 g and fried Glycyrrhizae Radix et Rhizoma of 0.64 g, taking 8.26 g when decocting, adding 300 mL of water, decocting to 210 mL, removing the dregs, and taking it warmly after meals. Twenty-one clinical research papers were included to analyze the modern clinical application of Huagaisan, which was mainly used in the treatment of respiratory diseases such as pneumonia, asthma, bronchitis and so on. ConclusionThis paper has verified and summarized the key information of the famous classical formula Huagaisan, which can provide a detailed reference basis for the development and clinical application of its compound preparation.

ABSTRACT

Data analysis models may assist the transmission of traditional Chinese medicine (TCM) experience and clinical diagnosis and treatment, and the possibility of constructing a “data-knowledge” dual-drive model was explored by taking gastric precancerous state as an example. Data-driven is to make clinical decisions around data analysis, and its syndrome-differentiation decision-making research relies on hidden structural models and partially observable Markov decision-making processes to identify the etiology of diseases, syndrome elements, evolution of pathogenesis, and syndrome differentiation protocols; knowledge-driven is to make use of data and information to promote decision-making and action processes, and its syndrome-differentiation decision-making research relies on convolutional neural networks to improve the accuracy of local disease identification and syndrome differentiation. The “data-knowledge” dual-driven model can make up for the shortcomings of single-drive numerical simulation accuracy, and achieve a balance between local disease identification and macroscopic syndrome differentiation. On the basis of previous research, we explored the construction method of diagnostic assisted decision-making platform for gastric precancerous state, and believed that the diagnostic and decision-making ability of doctors can be extended through the assistance of machines and algorithms. Meanwhile, the related research methods were integrated and the core features of gastric precancerous state based on TCM syndrome differentiation and endoscopic pathology diagnosis and prediction were obtained, and the elements of endoscopic pathology recognition based on TCM syndrome differentiation were explored, so as to provide ideas for the in-depth research and innovative application of cutting-edge data analysis technology in the field of intelligent TCM syndrome differentiation.

On-site supervision is a risk-based regulatory system that requires the scientific development of supervision plans for quality risks and hidden dangers in pharmaceutical enterprises, the rational allocation of supervision resources based on their risk levels, and the implementation of classified supervision measures. In this study, the quality risk monitoring business support system is set up for pharmaceutical enterprises by establishing the quality risk expert database and quality risk monitoring index system for pharmaceutical enterprises based on the difficulty analysis of on-site drug supervision. Based on this support system, the quality risk classification method, the differentiated spot check strategy and business auxiliary visualization system are established. This support system is used to learn the risk level of pharmaceutical enterprises, so as to innovate supervision methods and optimize monitoring strategies. Taking Jiangxi Province as an example, it is verified that the support system can guide the risk assessment of sample enterprises, can improve the targeting of on-site drug supervision in the process of technical review, scheme editing, on-site implementation and comprehensive evaluation, and can effectively improve the quality and efficiency of supervision.

ABSTRACT

Immune checkpoint inhibitors (ICIs) have shown good efficacy in tumor treatment and have changed the landscape of tumor treatment. However, some patients treated with ICIs have not only failed to achieve the desired therapeutic effect, but also developed an atypical response pattern of abnormally accelerated tumor growth, namely hyperprogressive disease (HPD). The pathogenesis of HPD is still unclear and it is difficult to diagnose, which poses a challenge for clinical identification and treatment decisions. Exploring the underlying mechanism of HPD is important to improve the effect of immunotherapy. Based on the theory of "Yang deficiency and toxic knot", this paper discussed the mechanism of HPD in immunotherapy from the perspective of "spleen and kidney Yang deficiency and hefty toxic pathogens". It was concluded that the inactivation of p53 oncogene and immunosuppressive microenvironment were the manifestations of the deficiency of healthy qi in the body and declined yang in the spleen and kidney, serving as an important basis for the occurrence of HPD. Adverse reactions caused by ICIs belong to the category of "drug toxicity". The occurrence and development of murine double minute 2 (MDM2)/murine double minute 4 (MDM4) activation, epidermal growth factor receptor (EGFR) mutation, and tumor inflammatory microenvironment are the manifestations of the hyperactivity of pathogenic Qi, conflict of cancer toxicity and drug toxicity, and being hefty by virtue of deficiency, which can promote the abnormal proliferation of tumor cells, and they are the core pathogenic elements of HPD and are closely related to disease prognosis. In terms of treatment, under the guidance of the theory of "five views on differentiation and treatment" (time-space view, core view, symptom view, precision view, and disease-before-onset view), which was summarized according to the clinical practice of this research team, this paper, taking the prevention and treatment of HPD as the entry point, formulated traditional Chinese medicine (TCM) compounds to reinforce healthy Qi and warm Yang and realize the dynamic management of the whole spatiotemporal cycle, and removed toxins and resisted cancer to realize the all-round systemic intervention of the specimen. ABSTRACT

ObjectiveTo investigate the clinical features of patients with acute-on-chronic liver failure (ACLF) and bacterial infection and early warning indicators associated with multidrug-resistant infections. MethodsA retrospective analysis was performed for 130 patients with ACLF and bacterial infection who attended The Second Affiliated Hospital of Air Force Medical University from January 1, 2010 to December 31, 2021, and according to the drug susceptibility results, the patients were divided into multidrug-resistant (MDR) bacterial infection group with 80 patients and non-MDR bacterial infection group with 50 patients. General information and laboratory examination results were compared between the two groups to screen for the early warning indicators associated with MDR bacterial infection. The Student’s t-test was used for comparison of normally distributed continuous data with homogeneity of variance between two groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data or continuous data with heterogeneity of variance between two groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. The binary logistic regression analysis and the receiver operating characteristic (ROC) curve were used to assess the predictive value of early warning indicators. ResultsAmong the 130 patients with ACLF and bacterial infection, sputum (27.7%) was the most common specimen for detection, followed by blood (24.6%), urine (18.5%), and ascites (17.7%). Bacterial infections were dominated by Gram-negative bacteria (58.5%). Of all bacteria, Escherichia coli (18.5%), Klebsiella pneumoniae (14.6%), and Enterococcus faecium (13.8%) were the most common pathogens. Gram-positive bacteria had a high resistance rate to the antibacterial drugs such as erythromycin (72.2%), penicillin (57.4%), ampicillin (55.6%), and ciprofloxacin (53.7%), while Gram-negative bacteria had a high resistance rate to the antibacterial drugs such as ampicillin (73.3%), cefazolin (50.0%), and cefepime (47.4%). The patients with ACLF and bacterial infection had a relatively high rate of MDR bacterial infection (61.5%). Comparison of clinical data between the two groups showed that compared with the patients with non-MDR bacterial infection, the patients with MDR bacterial infection had significantly higher levels of alanine aminotransferase (Z=2.089, P=0.037), aspartate aminotransferase (Z=2.063, P=0.039), white blood cell count (Z=2.207, P=0.027), and monocyte count (Z=4.413, P<0.001). The binary logistic regression analysis showed that monocyte count was an independent risk factor for MDR bacterial infection (odds ratio=7.120, 95% confidence interval [CI]: 2.478‍ ‍—‍ ‍20.456,P<0.001) and had an area under the ROC curve of 0.686 (95%CI: 0.597‍ ‍—‍ ‍0.776) in predicting ACLF with MDR bacterial infection(P<0.001), with the optimal cut-off value of 0.50×109/L, a sensitivity of 0.725, and a specificity of 0.400. ConclusionACLF combined with bacterial infections is mainly caused by Gram-negative bacteria, with the common pathogens of Escherichia coli and Klebsiella pneumoniae and a relatively high MDR rate in clinical practice. An increase in monocyte count can be used as an early warning indicator to distinguish MDR bacterial infection from non-MDR bacterial infection.

ABSTRACT

Objective The volume and cortical thickness of gray matter in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO) were compared and analyzed by voxel⁃based morphometry (VBM) and surface⁃based morphometry (SBM), and the differences in the structural changes of gray matter in the two diseases were discussed. Methods A total of 21 MS patients, 16 NMO patients and 19 healthy controls were scanned by routine MRI sequence. The data were processed and analyzed by VBM and SBM method based on the statistical parameter tool SPM12 of Matlab2014a platform and the small tool CAT12 under SPM12. Results Compared with the normal control group (NC), after Gaussian random field (GRF) correction, the gray matter volume in MS group was significantly reduced in left superior occipital, left cuneus, left calcarine, left precuneus, left postcentral, left central paracentral lobule, right cuneus, left middle frontal, left superior frontal and left superior medial frontal (P<0. 05). After family wise error (FWE) correction, the thickness of left paracentral, left superiorfrontal and left precuneus cortex in MS group was significantly reduced (P<0. 05). Compared with the NC group, after GRF correction, the gray matter volume in the left postcentral, left precentral, left inferior parietal, right precentral and right middle frontal in NMO group was significantly increased (P<0. 05). In NMO group, the volume of gray matter in left middle occipital, left superior occipital, left inferior temporal, right middle occipital, left superior frontal orbital, right middle cingulum, left anterior cingulum, right angular and left precuneus were significantly decreased (P<0. 05). Brain regions showed no significant differences in cortical thickness between NMO groups after FWE correction. Compared with the NMO group, after GRF correction, the gray matter volume in the right fusiform and right middle frontal in MS group was increased significantly(P<0. 05). In MS group, the gray matter volume of left thalamus, left pallidum, left precentral, left middle frontal, left middle temporal, right pallidum, left inferior parietal and right superior parietal were significantly decreased (P<0. 05). After FWE correction, the thickness of left inferiorparietal, left superiorparietal, left supramarginal, left paracentral, left superiorfrontal and left precuneus cortex in MS group decreased significantly (P<0. 05). Conclusion The atrophy of brain gray matter structure in MS patients mainly involves the left parietal region, while NMO patients are not sensitive to the change of brain gray matter structure. The significant difference in brain gray matter volume between MS patients and NMO patients is mainly located in the deep cerebral nucleus mass.

Microglia are the central nervous system's resident myeloid-derived immune cells, which play a major role in the innate and acquired immunological responses of brain. In the maintenance of brain tissue function under both healthy and pathological conditions, microglia take a protective or damaging role, depending on cell phenotypes and functions. The traditional microglia classification of pro- or anti-inflammatory phenotypes refers to the profile of macrophages, hence the term “brain macrophages:has been drawn. More microglia phenotypes are being discovered as new technologies and research methods are developed, and the newly discovered microglia phenotypes are often disease-, brain region-, and function-specific, providing an important foundation for studying the pathological processes underlying the development of specific diseases and developing appropriate interventions. ABSTRACT

ObjectiveTo investigate the clinical efficacy of double plasma molecular adsorption system (DPMAS) and sequential plasma exchange (PE) combined with continuous renal replacement therapy (CRRT) in the treatment of patients with acute-on-chronic liver failure (ACLF) and acute kidney injury (AKI). MethodsA retrospective analysis was performed for the clinical data of 90 patients with ACLF and AKI who were hospitalized in The Affiliated Hospital of Guizhou Medical University from January 2019 to December 2022, and according to the method for blood purification, they were divided into DPMAS sequential PE+CRRT group (observation group with 31 patients) and DPMAS sequential PE group (control group with 59 patients). General data on admission and laboratory markers before and after blood purification were collected from all patients, including hepatic and renal function, coagulation function, and inflammation markers, and estimated glomerular filtration rate (eGFR) and MELD combined with serum sodium concentration (MELD-Na) score were calculated. The independent-samples t test was used for comparison of normally distributed continuous data between two groups; the Wilcoxon rank sum test was used for comparison of non-normally distributed continuous data within each group before and after treatment, and the Mann-Whitney U test was used for comparison between two groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. ResultsThe observation group had a significantly higher response rate than the control group [48.4% (15/31) vs 27.1% (16/59), χ2=4.071, P=0.044]. The methods for blood purification in both groups could effectively improve total bilirubin, alanine aminotransferase, aspartate aminotransferase (AST), prothrombin time activity, serum creatinine (Scr), procalcitonin (PCT), C-reactive protein, eGFR, and MELD-Na score (all P<0.05), and both groups had significant reductions in platelet count (PLT) and hemoglobin (Hb) after treatment (all P<0.05), while there were no significant changes in blood urea nitrogen, albumin, and international normalized ratio after treatment (all P>0.05). There were significant differences between the two groups in the changes in AST, Scr, PCT, eGFR, MELD-Na score, Hb, and PLT after treatment (all P<0.05). ConclusionDPMAS sequential PE combined with CRRT can effectively remove inflammatory mediators, improve renal function, stabilize the internal environment of human body, and achieve a relatively good clinical efficacy.

Yigongsan is derived from Xiaoer Yaozheng Zhijue written by QIAN Yi in the Northern Song dynasty, which is the No. 3 formula in the Catalogue of Ancient Famous Classical Formulas(The Second Batch of Pediatrics) released by the National Administration of Traditional Chinese Medicine(TCM) in September 2022, and it can be developed as a class 3.1 new TCM drug. By referring to ancient medical books and modern literature, this study conducted herbal textual research on Yigongsan from five aspects, including historical evolution, origin and processing, dosage conversion, usage and preparation methods, and functional application, then formed the key information table of this formula, in order to provide reference for the development of reference samples and preparations of Yigongsan. Based on the results of the study, it is recommended that Panax ginseng should be removed the basal part of stem(rhizoma), Poria cocos should be removed the peel, Citrus reticulata should be cut into shreds and Glycyrrhiza uralensis should be used. According to 4.13 g/Qian(钱), 1 g/slice for ginger, 3 g for each jujube and 300 mL/Zhan(盏), the doses of Ginseng Radix, Poria, Atractylodis Macrocephalae Rhizoma, Glycyrrhizae Radix et Rhizoma, Citri Reticulatae Pericarpium, Zingiberis Rhizoma Recens, Jujubae Fructus were 1.652, 1.652, 1.652, 1.652, 1.652, 5, 6 g, and the total amount was 19.26 g. The decocting method was to crush the medicinal materials into fine powder with 50-80 mesh, add 300 mL of water and decoct to 210 mL for each dose, then remove the dregs and take it warmly. ABSTRACT

Traditional Chinese medicines (TCMs) possess a rich historical background, unique theoretical framework, remarkable therapeutic efficacy, and abundant resources. However, the modernization and internationalization of TCMs have faced significant obstacles due to their diverse ingredients and unknown mechanisms. To gain deeper insights into the phytochemicals and ensure the quality control of TCMs, there is an urgent need to enhance analytical techniques. Currently, two-dimensional (2D) chromatography, which incorporates two independent separation mechanisms, demonstrates superior separation capabilities compared to the traditional one-dimensional (1D) separation system when analyzing TCMs samples. Over the past decade, new techniques have been continuously developed to gain actionable insights from complex samples. This review presents the recent advancements in the application of multidimensional chromatography for the quality evaluation of TCMs, encompassing 2D-gas chromatography (GC), 2D-liquid chromatography (LC), as well as emerging three-dimensional (3D)-GC, 3D-LC, and their associated data-processing approaches. These studies highlight the promising potential of multidimensional chromatographic separation for future phytochemical analysis. Nevertheless, the increased separation capability has resulted in higher-order data sets and greater demands for data-processing tools. Considering that multidimensional chromatography is still a relatively nascent research field, further hardware enhancements and the implementation of chemometric methods are necessary to foster its robust development.
