ABSTRACT

OBJETIVO: identificar na literatura científica aplicações para o uso da ultrassonografia por enfermeiros no bloco operatório. MÉTODO: revisão de escopo realizada de acordo com a metodologia proposta pelo Instituto Joanna Briggs para responder à pergunta norteadora: Quais os possíveis usos da ultrassonografia por enfermeiros do bloco operatório? Os artigos foram buscados nas principais bases internacionais de literatura científica e cinzenta. Os descritores utilizados serão ultrassonografia e centros cirúrgicos. A pesquisa se orientou pelo acrônimo P-C-C: P ­ Participantes: Equipe de Enfermagem do Centro Cirúrgico; C ­ Conceito: Ultrassonografia; C - Contexto: Centro Cirúrgico. Registro do protocolo: osf.io/p8zne. RESULTADOS: os estudos encontrados versavam sobre três usos principais da ultrassonografia para enfermeiros perioperatórios no cenário do bloco operatório: avaliação gástrica (N=7), avaliação de conteúdo vesical (N=10) e auxiliar para punção periférica venosa e arterial (N=3). CONCLUSÃO: os enfermeiros perioperatórios devem buscar formação para práticas avançadas incluindo o uso de ultrassonografia como auxiliar em sua prática.


OBJECTIVE: To identify applications for using ultrasonography by nurses in the operating room in the scientific literature. METHOD: Scope review carried out according to the methodology proposed by the Joanna Briggs Institute to answer the guiding question: What are the possible uses of ultrasound by nurses in the operating room? The articles were sought in the main international scientific and gray literature bases. The descriptors used will be ultrasonography and surgical centers. The research was guided by the acronym P-C-C: P ­ Participants: Surgical Center Nursing Team; C - Concept: Ultrasound; C - Context: Surgical Center. Protocol Registration: osf.io/p8zne. RESULTS: The studies found covered three main uses of ultrasound for perioperative nurses in the operating room scenario: gastric evaluation (N=7), evaluation of vesical content (N=10), and auxiliary for venous and arterial peripheral puncture (N=3). CONCLUSION: Perioperative nurses should seek training for advanced practices, including using ultrasound to aid in their practice.

ABSTRACT

Objective:To explore the effect of cluster nursing in robot-assisted surgery for the treatment of reducible atlantoaxial dislocation.Methods:A retrospective cohort study was conducted to analyze the clinical data of 41 patients with reducible atlantoaxial dislocation treated by robot-assisted surgery in Honghui Hospital affiliated to Xi′an Jiaotong University from January 2019 to December 2021, including 28 males and 13 females; aged 18-79 years [(45.2±10.3)years]. Ninteen patients received cluster nursing (cluster nursing group), with operating room nursing team set up on the basis of routine nursing and performed cluster nursing in line with evidence-based medicine. Twenty-two patients received routine nursing (routine nursing group). The operation time, intraoperative blood loss, frequency of intraoperative C-arm fluoroscopy, time of drainage tube placement and chief surgeon′s satisfaction for nursing were compared between the two groups. The degree of pain was evaluated by pain numerical score (NRS) at 12 hours, 24 hours, 48 hours, 72 hours, 1 month and 3 months after operation and at the last follow-up. The neck disability index (NDI) was assessed at 1 day before operation, 1 month after operation, 3 months after operation and at the last follow-up. The complications were observed.Results:All patients were followed up for 12-18 months [(16.7±3.7)months]. The operation time, intraoperative blood loss, frequency of C-arm fluoroscopy and time of drainage tube placement in cluster nursing group were (82.9±10.4)minutes, (105.9±11.8)ml, (3.8±0.6)times and (1.5±0.4)days, while those in routine nursing group were (125.7±12.8)minutes, (208.4±13.8)ml, (9.7±2.3)times and (3.6±0.6)days, respectively (all P<0.01). The chief surgeon′s satisfaction for nursing was 94.7% (18/19) in cluster nursing group and was 68.2% (15/22) in routine nursing group ( P<0.05). The NRS in cluster nursing group was (6.2±0.4)points, (6.0±0.7)points, (4.9±1.1)points, (2.7±0.5)points, (1.9±0.4)points, (1.8±0.4)points and (1.5±0.3)points at 12 hours, 24 hours, 48 hours, 72 hours, 1 month and 3 months after operation and at the last follow-up, while it was (7.6±0.6)points, (6.8±1.2)points, (5.8±1.5)points, (4.2±0.8)points, (3.4±0.7)points, (2.6±0.5)points and (2.2±0.5)points in routine nursing group ( P<0.05 or 0.01). There was no significant difference in the NDI between the two groups at 1 day before operation, but the NDI in cluster nursing group was 20.6±4.5, 14.6±2.8 and 10.7±2.5 at 1 month and 3 months after operation and at the last follow-up, while it was 26.9±4.1, 18.7±3.3 and 13.7±1.7 in routine nursing group (all P<0.01). There was no hematoma, infection or implant-related complications in both groups .Conclusion:For robot-assisted surgery in the treatment of reducible atlantoaxial dislocation, cluster nursing is associated with shortened operation time and time of drainage tube placement, decreased intraoperative blood loss and frequency of intraoperative fluoroscopy, increased chief surgeon′s satisfaction for nursing, reduced pain and accelerated functional recovery.

ABSTRACT

Objective:To explore the effect of emergency rescue ability training of operating room nurses based on video tracking method, and provide reference for improving emergency ability of operating room nurses.Methods:This study was a quasi-experimental study. In March 2021, 85 nurses working in the Department of Anesthesiology and Surgery of the Provincial Hospital Affiliated to Shandong First Medical University were selected by cluster sampling method. We implemented a training program of rescue emergency ability based on video tracking and adopted the inspection list of rescue emergency ability assessment and doctors′ satisfaction questionnaire of operating room nurses to evaluate the scores of each item in the nurses' rescue emergency ability list and doctors' satisfaction of rescue cooperation of operating room nurses.Results:The scores of the dimensions of recognition of fatal arrhythmias, fatal arrhythmia, several situations that can directly call help, writing of emergency state nursing records and handover of medical records, use of defibrillator, correct use and maintenance of negative pressure attraction, use and management of rescue vehicle and situation disposal in the inspection list of rescue ability after training were 9.80 ± 1.61, 26.06 ± 2.20, 17.34 ± 1.29, 13.00 ± 1.57, 7.35 ± 0.74, 10.24 ± 1.14, 33.89 ± 2.73, which were higher than before training 9.24 ± 1.18, 24.92 ± 2.15, 15.69 ± 1.92, 12.21 ± 1.66, 6.55 ± 0.92, 8.94 ± 1.32, 32.94 ± 2.20. The differences were statistically significant ( t values were -6.83 to -2.51, all P<0.05); after the training, in the questionnaire of doctors' satisfaction with nurses, surgical materials and instruments preparation, first aid skill operating level, attention to surgical progress, active and correct delivery, orderly and busy, coordination and communication ability, professional knowledge, evaluation ability and foresight, ability to deal with emergencies, clear division of labor and good cooperation, and responsibility scores were 4.22 ± 0.58, 4.52 ± 0.54, 4.53 ± 0.47, 4.43 ± 0.58, 4.44 ± 0.44, 4.37 ± 0. 59, 4.45 ± 0.51, 4.51 ± 0.53, 4.51 ± 0.57, 4.17 ± 0.63, which were higher than the pre-training 4.05 ± 0.58, 4.38 ± 0.56, 4.26 ± 0.76, 4.04 ± 0.67, 4.25 ± 0.62, 4.19 ± 0.74, 4.25 ± 0.74, 4.34 ± 0.67, 4.21 ± 0.84 and 3.56 ± 0.58. All differences were statistically significant ( t values were -8.22 to -2.10, all P<0.05). Conclusions:The training method based on video tracking method improved the emergency rescue ability of operating room nurses and the doctor's satisfaction with rescue cooperation, and provided a reference for the training of operating room nurses.

ABSTRACT

Objective:Use linear programming model to predict the allocation of surgical nursing human resources, and optimize the allocation of nursing staff.Methods:This study was a controlled clinical trial. A total of 91 nurses from 5 surgical departments in Affiliated Hospital of Qingdao University were selected by convenience sampling method. The nurses who participated in the scheduling in August and September 2020 were the routine scheduling groups, and in March and April 2021 were the linear scheduling groups. The linear programming model of surgical nursing human resources was established. The LINGO 11.0 software was used to calculate the minimum number of nurses required for the next day operation. According to the predicted results and the requirements of the operation specialty and rank level, the surgical staff was arranged for the next day. The overtime hours of nurses in the routine scheduling groups and the linear scheduling groups were compared and analyzed.Results:The number of on-duty nurses was the same in 4 groups, the overtime hours of the conventional scheduling groups in August and September 2020 and the linear scheduling groups in March and April 2021 were 865 (505, 1 435), 780 (475, 1 355), 650 (460, 910) and 720 (350, 915) min, the difference of overtime hours was statistically significant ( H=13.66, P<0.05). The overtime hours of the routine scheduling group in August 2020 were significantly different from those of the linear scheduling group in March 2021 and April 2021 respectively ( Z=-2.69, -2.55, both P<0.05). The overtime hours of the routine scheduling group in September 2020 were significantly different from those of the linear scheduling group in March 2021 and April 2021 respectively ( Z=-2.62, -2.58, both P<0.05). Conclusions:The linear programming model was used to predict the human resource allocation of surgical nursing staff, optimized the allocation of operating room human resources. It reduced the overtime hours of surgical nursing staff effectively. Indirectly, it accelerated operations, improved operation efficiency and ensured the safety of patients.

Resumo Objetivo Mapear as principais atividades do enfermeiro de Centro Cirúrgico no cenário brasileiro, explicitadas na literatura vigente. Métodos Trata-se de uma scoping review elaborada de acordo com as diretrizes vigentes do Joanna Briggs Institute, utilizando o checklist para condução e relatório do estudo. A busca foi realizada por duas pesquisadoras independentes, no período entre agosto e dezembro de 2021, em seis bases de dados da área da saúde, utilizando os descritores: nurses, nurse role, intraoperative period, perioperative nursing, surgical centers . Fizeram parte da amostra artigos publicados entre janeiro de 2011 e outubro de 2022, nos idiomas inglês, português ou espanhol. As informações foram extraídas, classificadas, sintetizadas e apresentadas descritivamente. Resultados Foram incluídos 18 na síntese da pesquisa. A análise dos resultados possibilitou evidenciar como as atividades prevalentes na assistência a implementação de medidas de segurança cirúrgica (50%); na gestão, o gerenciamento de materiais, insumos e equipamentos (72%) e preparo das salas (39%); no ensino, educação permanente da enfermagem (50%). Não foram identificados estudos relacionados à atividade de pesquisador. Conclusão No cenário brasileiro, as atividades do enfermeiro de Centro Cirúrgico circundam-se entre gerenciais, assistenciais com menor proporção no ensino, destacando-se sua posição estratégica na mobilização das ações que promovem segurança e qualidade nos processos perioperatórios.


Resumen Objetivo Mapear las principales actividades de los enfermeros de quirófano en el escenario brasileño mencionadas en la literatura vigente. Métodos Se trata de una scoping review elaborada de acuerdo con las directrices vigentes del Joanna Briggs Institute, utilizando la lista de chequeo para la conducción e informe del estudio. La búsqueda fue realizada por dos investigadoras independientes, entre agosto y diciembre de 2021, en seis bases de datos del área de la salud, utilizando los descriptores: nurses, nurse role, intraoperative period, perioperative nursing, surgical centers . Artículos publicados entre enero de 2011 y octubre de 2022, en inglés, portugués y español formaron parte de la muestra. La información se extrajo, se clasificó, se sintetizó y se presentó descriptivamente. Resultados Se incluyeron 18 artículos en la síntesis del estudio. El análisis de los resultados permitió evidenciar las siguientes actividades predominantes: en la atención, la implementación de medidas de seguridad quirúrgica (50 %); en la gestión, la administración de material, insumos y equipos (72 %) y la preparación de las salas (39 %); en la enseñanza, la educación permanente de la enfermería (50 %). No se identificaron estudios relacionados con la actividad del investigador. Conclusión En el escenario brasileño, las actividades de los enfermeros de quirófano giran en torno de la gestión, la atención con menor proporción la enseñanza, y se destaca su posición estratégica en la movilización de las acciones que promueven seguridad y calidad en los procesos perioperatorios. Abstract Objective To map perioperative nurses' main activities in the Brazilian scenario, explained in the current literature. Methods This is a scoping review prepared in accordance with current JBI guidelines, using the checklist for conducting and reporting the study. The search was carried out by two independent researchers, between August and December 2021, in six databases in the health area, using the descriptors nurses, nurse role, intraoperative period, perioperative nursing, surgical centers. Articles published between January 2011 and October 2022, in English, Portuguese or Spanish, were part of the sample. The information was extracted, classified, synthesized and presented descriptively. Results Eighteen were included in the research synthesis. The analysis of results made it possible to highlight the implementation of surgical safety measures as prevalent activities in care (50%); in management, material, input and equipment (72%) and room preparation (39%) management; in teaching, permanent nursing education (50%). No studies related to researcher activity were identified. Conclusion In the Brazilian scenario, perioperative nurses' activities range from management to assistance with a smaller proportion in teaching, highlighting their strategic position in mobilizing actions that promote safety and quality in perioperative processes. ABSTRACT

Objetivo: compreender as percepções dos acompanhantes que aguardam notícias de seus entes queridos que estão em cirurgia, sobre a utilização da música como estratégia para promover saúde no hospital. Método: estudo descritivo, exploratório, qualitativo, fundamentado nos pressupostos teóricos da Promoção da Saúde, com a participação de 15 acompanhantes que aguardavam notícias de seus entes queridos em uma sala de espera no ambiente hospital. Realizaram-se entrevistas semiestruturadas e os dados foram organizados e analisados conforme análise de conteúdo. Resultados: os participantes destacaram algumas possibilidades para promover saúde na sala de espera de cirurgia do hospital, como a música, a qual proporciona distração da realidade preocupante, felicidade, vida, alegria, esperança, sensações boas, ânimo e tranquilidade. Considerações finais: a música destaca-se como tecnologia para promover saúde, despertando sentimentos positivos durante as intervenções musicais no hospital. Portanto, faz-se necessário ampliar a discussão e utilização da música como estratégia para promover saúde no âmbito hospitalar


Objective: to understand the perceptions of companions awaiting news from their loved ones who are undergoing surgery, about the use of music as a strategy to promote health in the hospital. Method: a descriptive, exploratory, qualitative study, based on the theoretical assumptions of Health Promotion, with the participation of 15 companions who were waiting for news from their loved ones in a waiting room in the hospital environment. Semi-structured interviews were carried out and the data were organized and analyzed according to content analysis. Results: the participants highlighted some possibilities to promote health in the hospital surgery waiting room, such as music, which provides distraction from the worrying reality, happiness, life, joy, hope, good feelings, cheer and tranquility. Final considerations: music stands out as a technology to promote health, arousing positive feelings during musical interventions in the hospital. Therefore, it is necessary to expand the discussion and use of music as a strategy to promote health in the hospital environment


Objetivo: comprender las percepciones de acompañantes en espera de noticias de sus seres queridos que están siendo operados, sobre el uso de la música como estrategia de promoción de la salud en el hospital. Método: estudio descriptivo, exploratorio, cualitativo, basado en los presupuestos teóricos de la Promoción de la Salud, con la participación de 15 acompañantes que esperaban noticias de sus seres queridos en una sala de espera del ambiente hospitalario. Se realizaron entrevistas semiestructuradas y los datos fueron organizados y analizados según el análisis de contenido. Resultados: los participantes destacaron algunas posibilidades para promover la salud en la sala de espera de cirugía hospitalaria, como la música, que proporciona distracción de la realidad preocupante, alegría, vida, alegría, esperanza, buenos sentimientos, alegría y tranquilidad. Consideraciones finales: la música se destaca como tecnología para promover la salud, despertando sentimientos positivos durante las intervenciones musicales en el hospital. Por lo tanto, es necesario ampliar la discusión y el uso de la música como estrategia de promoción de la salud en el ámbito hospitalario

ABSTRACT

Objetivo: analisar as principais características dos aplicativos móveis, disponíveis em lojas virtuais, que tratam sobre a temática da realização de procedimentos cirúrgicos. Método: prospecção tecnológica, de abordagem qualitativa do tipo exploratória, realizada por busca de aplicativos nas lojas virtuais Apple Store ® e Google Play ®, utilizando as palavras-chave: Centro Cirúrgico, Cirurgia, Simulador de cirurgia e Jogos de cirurgia, em português, inglês e espanhol. Realizou-se análise temática categorial dos achados. Resultados: 67 ocorrências foram classificadas em quatro categorias: 'Aplicativos destinados à orientação para procedimento cirúrgico', 'Aplicativos sobre o trabalho em centro cirúrgico', 'Aplicativos voltados à equipe de saúde', e 'Aplicativos relacionados a jogos de entretenimento'. Conclusões: os aplicativos voltam-se tanto para indivíduos que buscam orientações sobre procedimentos, quanto a profissionais da área da saúde. No entanto, há uma lacuna referente à orientações pré-operatórias específicas para pediatria, onde nenhuma ocorrência foi registrada


Objective: to analyze the main characteristics of mobile applications available in virtual stores, which deal with performing surgical procedures. Method: technological prospection, with a qualitative exploratory approach, carried out by searching for applications in the Apple Store® and Google Play® virtual stores, using the keywords: Surgical Center, Surgery, Surgery Simulator, and Surgery Games, in Portuguese, English, and Spanish. We performed a categorical thematic analysis of the findings. Results: 67 occurrences were classified into four categories: 'Applications intended to guide the surgical procedure,' 'Applications about working in the operating room,' 'Applications aimed at the health team,' and 'Applications related to entertainment games. Conclusions: the applications are aimed both at individuals seeking procedure guidance and healthcare professionals. However, there is a gap regarding specific preoperative guidelines for pediatrics, where no occurrence was recorded

Método: prospección tecnológica, con enfoque exploratorio cualitativo, realizada mediante la búsqueda de aplicaciones en las tiendas virtuales Apple Store® y Google Play®, utilizando las palabras clave: Centro Quirúrgico, Cirugía, Simulador de Cirugía y Juegos de Cirugía, en portugués, inglés y español. Se realizó un análisis temático categórico de los hallazgos. Resultados: 67 ocurrencias fueron clasificadas en cuatro categorías: 'Aplicaciones destinadas a orientar el procedimiento quirúrgico', 'Aplicaciones sobre el trabajo en quirófano', 'Aplicaciones dirigidas al equipo de salud' y 'Aplicaciones relacionadas con juegos de entretenimiento'. Conclusiones: las aplicaciones están dirigidas tanto a personas que buscan orientación sobre procedimientos como a profesionales de la salud. Sin embargo, existe un vacío con respecto a las pautas preoperatorias específicas para pediatría, donde no se registró ninguna ocurrencia

ABSTRACT

ABSTRACT Objective: to identify the state-of-the-art developed about surgical Nursing care in the operating room. Method: an integrative review with search and selection of primary studies conducted in April and May 2022 in four major data sources in the health field, namely: LILACS, SciELO, BDENF and PubMed, from 2018 to 2022. The sample consisted of 13 studies, 8 selected from a universe of 293 articles and 5 obtained through the manual search. Results: three categories were generated: Associating surgical care with care, people and material resources management for patient safety; Surgical care and ethical sensitivity in the search for patients' well-being; and Operating room nurses' manifestation and omission experiences and surgical care. Conclusions: surgical Nursing care in the operating room is varied and is manifested through meeting the administrative demands, ethical sensitivity, body temperature maintenance, preservation of individuality and meeting all the needs in the intraoperative period when patients are more vulnerable, including being duly informed as a way to provide safe surgical care. On the other hand, nurses acknowledge the need to change their world view and direct surgical care towards a more human, social and cultural perspective.


RESUMO Objetivo: identificar o estado da arte que foi desenvolvido sobre os cuidados cirúrgicos do enfermeiro da sala de cirurgia. Método: revisão integrativa com pesquisa e seleção de estudos primários realizada entre abril e maio de 2022 em quatro fontes de dados destacadas no campo da saúde: LILACS, SciELO, BDENF e PubMed de 2018 a 2022. A amostra consistiu-se de 13 estudos, 8 selecionados de um total de 293 artigos e 5 pesquisados manualmente. Resultados: foram geradas três categorias: associação do cuidado cirúrgico com a gestão dos cuidados, pessoas e recursos materiais para a segurança do paciente; cuidado cirúrgico e sensibilidade ética na busca do bem-estar do paciente e as experiências de manifestação e omissão do enfermeiro da sala de cirurgia e o cuidado cirúrgico. Conclusões: o cuidado cirúrgico na sala de cirurgia é variado e se manifesta através da atenção às exigências administrativas, sensibilidade ética, manutenção da temperatura corporal, preservação da individualidade e atenção a todas as necessidades no ambiente intra-operatório onde o paciente é mais vulnerável, incluindo o dever de estar informado como um meio de proporcionar um cuidado cirúrgico seguro. Por outro lado, reconhece a necessidade de mudar sua cosmovisão e direcionar os cuidados cirúrgicos para uma perspectiva mais humana, social e cultural.


RESUMEN Objetivo: identificar el estado de arte que se ha desarrollado sobre el cuidado quirúrgico de la enfermera de sala de operaciones. Método: revisión integrativa con indagación y selección de estudios primarios efectuada en abril y mayo de 2022 en cuatro fuentes de datos destacadas en el campo de la salud: Lilacs, SciELO, BDENF y PubMed de 2018 a 2022. La muestra se conformó por 13 estudios, 8 seleccionados de un universo de 293 artículos y 5 de búsqueda manual. Resultados: se generaron tres categorías: asociando cuidado quirúrgico con gestión de cuidado, de personas y de recursos materiales para la seguridad del paciente; el cuidado quirúrgico y la sensibilidad ética en la búsqueda de bienestar para el paciente y las experiencias de manifestación y omisión del enfermero de quirófano y el cuidado quirúrgico. Conclusiones: el cuidado quirúrgico en la enfermera de sala de operaciones es variado y se manifiesta a través de la atención a las demandas administrativas, la sensibilidad ética, el mantenimiento de la temperatura corporal, conservación de la individualidad y atención a todas las necesidades en el intraoperatorio donde el paciente es más vulnerable, incluyendo el estar informada como una forma de brindar un cuidado quirúrgico seguro. Por otro lado, reconoce la necesidad de cambiar su visión de mundo y dirigir el cuidado quirúrgico hacia una mirada más humana, social y cultural.

ABSTRACT Objective: To implement, on health management software, electronic records of the perioperative nursing process and the stages of transoperative and immediate postoperative nursing diagnoses, based on the NANDA International taxonomy. Method: Experience report conducted from the completion of the Plan-Do-Study-Act cycle, which allows improvement planning with a clearer purpose, directing each stage. This study was carried out in a hospital complex in southern Brazil, using the software Tasy/Philips Healthcare. Results: For the inclusion of nursing diagnoses, three cycles were completed, predictions of expected results were established, and tasks were assigned, defining "who, what, when, and where". The structured model covered seven possibilities of aspects, 92 symptoms and signs to be evaluated, and 15 nursing diagnoses to be used in the transoperative and immediate postoperative periods. Conclusion: The study allowed implementing electronic records of the perioperative nursing process on health management software, including transoperative and immediate postoperative nursing diagnoses, as well as nursing care.


RESUMEN Objetivo: Implementar, en un software de manejo de la salud, registros electrónicos del proceso de enfermería perioperatorio y la etapa de diagnósticos de enfermería transoperatorios y postoperatorios inmediatos, con base en la taxonomía NANDA internacional. Método: Informe de experiencia realizado a partir de la consecución del ciclo Plan-Do-Study-Act), que permite planificar la mejora con un propósito más claro, dirigiendo cada etapa. Este estudio fue realizado en un complejo hospitalario en el sur de Brasil, utilizando el Software Tasy/Philips Healthcare. Resultados: Para la inclusión de diagnósticos de enfermería, se concluíran tres ciclos, se estableceran predicciones de los resultados esperados, y se asignaron tareas, definiendo "quién, qué, cuándo y dónde". El modelo estructurado contempló siete posibles aspectos, 92 signos y síntomas para ser evaluados y 15 diagnósticos de enfermería para ser utilizados en el transperatorio y postoperatorio inmediato. Conclusión: El estudio permitió implementar, en un software de manejo de la salud, registros electrónicos del proceso de enfermería perioperatorio, que comprende diagnósticos de enfermería transoperatoria y postoperatoria inmediata, además de los cuidados de enfermería.


RESUMO Objetivo: Implementar, em um software de gestão em saúde, os registros eletrônicos do processo de enfermagem perioperatório e a etapa de diagnósticos de enfermagem transoperatório e pós-operatório imediato, fundamentados na taxonomia NANDA International. Método: Relato de experiência conduzido a partir da realização do ciclo PDSA (Plan-Do-Study-Act), o qual permite o planejamento de melhoria com um propósito mais claro, direcionando cada etapa. Este estudo foi realizado em um complexo hospitalar da região sul do Brasil, utilizando o software Tasy/Philips Healthcare. Resultados: Para a inclusão dos diagnósticos de enfermagem, rodaram-se três ciclos, estabeleceram-se previsões de resultados esperados, e as tarefas foram atribuídas, definindo "quem, o quê, quando e onde". O modelo estruturado contemplou sete possibilidades de aspectos, 92 sinais e sintomas a serem avaliados e 15 diagnósticos de enfermagem para serem utilizados no transoperatório e pós-operatório imediato. Conclusão: O estudo possibilitou implementar, em um software de gestão em saúde, os registros eletrônicos do processo de enfermagem perioperatório, compreendendo diagnósticos de enfermagem do transoperatório e pós-operatório imediato, além de cuidados de enfermagem.

ABSTRACT

O objetivo deste estudo foi investigar as atribuições do enfermeiro auditor no Centro Cirúrgico voltadas à qualidade da assistência. Trata-se de uma revisão integrativa, realizada nas bases de dados Scientific Eletronic Library Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde, Biblioteca Virtual em Saúde e Base de Dados de Enfermagem através dos descritores Auditoria de Enfermagem, Enfermagem de Centro Cirúrgico e Centro Cirúrgico. A amostra foi composta por onze artigos. Observou-se que a auditoria de enfermagem em centro cirúrgico tem papel fundamental na gestão de riscos e adequação de inconformidades. É responsável também por observar falhas e propor educação permanente afim de assegurar qualidade e continuidade na assistência ao paciente cirúrgico. Conclui-se que, o enfermeiro auditor assume um papel de liderança frente à equipe cirúrgica estando atento às demandas e realizando ações educativas sempre que necessário para promoção de melhorias dos serviços prestados aos pacientes cirúrgicos.


The aim of this study was to investigate the duties of the auditor nurse in the Operating Room focused on the quality of care. This is an integrative review, carried out in the databases Scientific Electronic Library Online, Latin American and Caribbean Literature on Health Sciences, Virtual Health Library, and Nursing Database using the descriptors Nursing Audit, Operating Room Nursing, and Operating Room. The sample consisted of eleven articles. It was observed that the nursing audit in the operating room has a fundamental role in risk management and adequacy of nonconformities. It is also responsible for observing failures and proposing continuing education to ensure quality and continuity of care for surgical patients. It is concluded that the nurse auditor assumes a leadership role in the surgical team, being attentive to the demands and carrying out educational actions whenever necessary to promote improvements in the services provided to surgical patients.


El objetivo de este estudio fue investigar las funciones del enfermero auditor en el Centro Quirúrgico enfocadas a la calidad de la atención. Se trata de una revisión integradora, realizada en las bases de datos Scientific Electronic Library Online, Latin American and Caribbean Literature on Health Sciences, Virtual Health Library y Nursing Database a través de los descriptores Auditoría de Enfermería, Centro Quirúrgico de Enfermería y Centro Quirúrgico. La muestra estuvo compuesta por once artículos. Se observó que la auditoría de enfermería en quirófano tiene un papel fundamental en la gestión de riesgos. También es responsable de observar las fallas y proponer la educación continua para garantizar la calidad. Se concluye que el enfermero auditor asume un rol de liderazgo frente al equipo quirúrgico, estando atento a las demandas y realizando acciones educativas siempre que sea necesario.

ABSTRACT

Objetivo: Avaliar os níveis de risco decorrentes do posicionamento cirúrgico em pacientes submetidos a cirurgias cardíacas. Método: Estudo observacional, transversal, documental, descritivo, realizado em um hospital filantrópico em Salvador, Bahia. A amostra foi composta de 258 pacientes que se submeteram a cirurgias cardíacas entre janeiro de 2018 e janeiro de 2019. Utilizou-se um formulário com variáveis sociodemográficas, clínicas e a Escala de Avaliação de Risco para Desenvolvimento de Lesões Decorrentes do Posicionamento Cirúrgico (ELPO). Os dados foram analisados quantitativamente, utilizando-se tabelas com frequências absolutas e relativas. Resultados: Dos pacientes, 59,7% eram do sexo masculino; 53,1% foram submetidos à revascularização do miocárdio; 69,8% foram classificados como grau 3 segundo a American Society of Anesthesiologists (ASA III); 68,2% tinham índice de massa corporal alterado para mais; todos os pacientes foram submetidos a anestesia geral e posicionados em supina, com os membros em posição anatômica; 63,2% tiveram tempo de cirurgia acima de 4 horas; todos utilizaram colchão de visco elástico e coxins; 60,5% exibiam doença vascular. A pontuação da ELPO revelou 90,7% dos pacientes com baixo risco de desenvolver lesões por posicionamento. Conclusão: Segundo avaliação por meio da ELPO, os pacientes apresentaram baixo risco para lesão de pele durante as cirurgias cardíacas, resultado atribuído ao uso de colchão de polímero viscoelástico nas mesas cirúrgicas.


Objective: To assess the risk levels resulting from surgical positioning in patients undergoing cardiac surgery. Method: This is an observational, cross-sectional, documentary, descriptive study carried out in a philanthropic hospital in Salvador, state of Bahia, Brazil. The sample consisted of 258 patients who underwent heart surgery between January 2018 and January 2019. A form with sociodemographic and clinical variables and the Risk Assessment Scale for the Development of Injuries Due to Surgical Positioning (ELPO) were used. Data were analyzed quantitatively, using tables with absolute and relative frequencies. Results: Among the patients, 59.7% were men; 53.1% were submitted to coronary artery bypass grafting; 69.8% were classified as stage 3 according to the American Society of Anesthesiologists (ASA III); 68.2% had higher body mass index; all patients were submitted to general anesthesia and positioned in the supine position, with the limbs in anatomical position; 63.2% had over 4 hours of duration of surgery; all used viscoelastic mattress and pads; and 60.5% had vascular disease. The ELPO score showed 90.7% of patients at low risk of developing positioning injuries. Conclusion: According to the ELPO evaluation, patients were at low risk of developing skin injury during cardiac surgeries, a result attributed to the use of a viscoelastic polymer mattress on the operating tables.


Objetivo: Evaluar los niveles de riesgo derivados del posicionamiento quirúrgico en pacientes sometidos a cirugía cardiaca. Método: Estudio observacional, transversal, documental, descriptivo, realizado en un hospital filantrópico de Salvador, Bahía. La muestra estuvo conformada por 258 pacientes que se sometieron a cirugía cardiaca entre enero de 2018 y enero de 2019. Se utilizó un formulario con variables sociodemográficas, clínicas y la Escala de Evaluación de Riesgo para el Desarrollo de Lesiones Procedentes del Posicionamiento Quirúrgico (ELPO). Los datos fueron analizados cuantitativamente, utilizando tablas con frecuencias absolutas y relativas. Resultados: 59,7% de pacientes del sexo masculino; 53,1% fueron sometidos a revascularización miocárdica; 69,8% ASA III; 68,2% con índice de masa corporal cambiado a más; todos los pacientes sometidos a anestesia general y posicionados en decúbito supino, con los miembros en posición anatómica; 63,2% con tiempo de cirugía mayor a 4 horas; todos usaron colchones y cojines viscoelásticos; 60,5% tenían enfermedad vascular. La puntuación ELPO mostró un 90,7% de pacientes con bajo riesgo de desarrollar lesiones de posicionamiento. Conclusión: De acuerdo con una evaluación utilizando la ELPO, los pacientes tenían un bajo riesgo de lesiones en la piel durante las cirugías cardíacas, resultado atribuido al uso de un colchón de polímero viscoelástico en las mesas quirúrgicas.

ABSTRACT

Objective:To explore the application effect of 6S and PDCA cycle lean management in nursing management of orthopaedic trauma surgery.Methods:A retrospective cohort study was conducted to analyze the clinical data of 80 patients with orthopedic trauma admitted to University-Town Hospital of Chongqing Medical University from January 2020 to November 2021, including 51 males and 29 females; aged 23-68 years [(44.5±9.2)years]. According to the order of operation date, 40 patients from January 2021 to November 2021 received routine management plus 6S and PDCA cycle lean management (lean management group), and 40 patients from January 2020 to December 2020 received routine surgical nursing management (routine management group). The nursing quality score in operating room (instrument and equipment management, surgical instrument preparation, surgical coordination, disinfection and isolation), qualified rate of infection monitoring in operating room (air in operating room, hand hygiene of medical staffs and sterile articles in operating room), surgeons′ satisfaction with operating nurses (circulating nurses and scrub nurses), self- rating anxiety scale (SAS) of patients before operation, incidence of complications at postoperative 12 days and patients′ satisfaction with nurses in operating room at postoperative 2-3 days.Results:The four dimensions of the nursing quality score in operating room in lean management group were (27.8±1.5)points, (18.1±1.1)points, (26.6±1.5)points and (18.1±0.8)points, significantly higher than (23.7±2.3)points, (14.6±1.1)points, (22.4±1.8)points and (13.7±1.1)points in routine management group (all P<0.01). The three dimensions of qualified rate of infection monitoring in operating room in lean management group were all 100%, showing no significant differences in routine management group (75%, 100%, 100%) (all P>0.05). Surgeons′ satisfaction with circulating nurses and scrub nurses in lean management group was 90% and 85%, higher than 73% and 65% in routine management group (all P<0.05). Preoperative SAS in lean management group was (32.5±8.2)points, significantly lower than (54.6±10.7)points in routine management group ( P<0.01). The incidence of postoperative complications in lean management group was 0, significantly lower than 15%(6/40) in routine management group ( P<0.05). Patients′ satisfaction with nurses in operating room in lean management group was 95% (38/40), significantly higher than 78%(31/40) in routine management group ( P<0.05). Conclusion:6S and PDCA cycle lean management can effectively improve the nursing quality of orthopedic trauma surgery, reduce patients′ anxiety and postoperative complications, and improve surgeons′ and patients′ satisfaction, which is worthy of further clinical application.

ABSTRACT

Abstract Objective: to assess the effectiveness of supplemental oxygenation with high FiO2 when compared to conventional FiO2 in the prevention of surgical site infection. Method: an effectiveness systematic review with meta-analysis conducted in five international databases and portals. The research was guided by the following question: Which is the effectiveness of supplemental oxygenation with high FiO2 (greater than 80%) when compared to conventional FiO2 (from 30% to 35%) in the prevention of surgical site infections in adults? Results: fifteen randomized clinical trials were included. Although all the subgroups presented a general effect in favor of the intervention, colorectal surgeries had this relationship evidenced with statistical significance (I2=10%;X2=4.42; p=0.352). Conclusion: inspired oxygen fractions greater than 80% during the perioperative period in colorectal surgeries have proved to be effective to prevent surgical site infections, reducing their incidence by up to 27% (p=0.006). It is suggested to conduct new studies in groups of patients subjected to surgeries from other specialties, such as cardiac and vascular. PROSPERO registration No.: 178,453.


Resumo Objetivo: avaliar a efetividade da oxigenação suplementar com FiO2 elevada comparada com FiO2 convencional na prevenção de infecção de sítio cirúrgico. Método: revisão sistemática de efetividade com metanálise em cinco bases de dados e portais internacionais. A pesquisa foi norteada pela pergunta: Qual a efetividade da oxigenação suplementar com FiO2 elevada (maior que 80%) comparada com FiO2 convencional (de 30 a 35%) na prevenção de infecção de sítio cirúrgico em adultos? Resultados: foram incluídos quinze ensaios clínicos randomizados. Embora todos os subgrupos tenham apresentado um efeito geral a favor da intervenção, as cirurgias colorretais tiveram essa relação evidenciada com significância estatística (I2=10%; X2=4,42; p=0,352). Conclusão: uma fração inspirada de oxigênio maior que 80% durante o período perioperatório em cirurgias colorretais tem se mostrado efetiva para prevenir a infecção de sítio cirúrgico, reduzindo sua incidência em até 27% (p=0,006). Sugere-se a realização de novos estudos em grupos de pacientes submetidos a cirurgias de outras especialidades, tais como cardíacas e vasculares. Registro PROSPERO: 178453.

ABSTRACT Objective: To know the conceptions regarding patient safety during their surgical positioning from the perspective of nurses in a Surgical Center. Method: Qualitative descriptive exploratory study, with seven nurses from private hospital in southern Brazil in August 2018. Data generation using the Focus Group technique, with content analysis by Bardin. Results: From the data analysis, six categories emerged: "patient safety", "surgical patient risks", "nurses in the operating room", "training", "involvement of the interprofessional" team and application of the risk assessment scale. The development of injuries resulting from positioning was identified as the greatest risk for the surgical patient, highlighting the presence of the nurse in the operating room as essential for the prevention of this risk, through patient assessment and staff training. Final considerations: Nurses identified risk to patient safety in relation to surgical positioning and considered they presence in the operating room to be indispensable.


RESUMEN Objetivo: Conocer los conceptos sobre seguridad en el posicionamiento del paciente quirúrgico desde la perspectiva del enfermero en un Centro Quirúrgico. Método: Estudio exploratorio descriptivo cualitativo, con siete enfermeras de un hospital privado del sur de Brasil, em agosto de 2018. Generación de datos mediante la técnica de Focus Group, con análisis de contenido de Bardin. Resultados: Del análisis de los datos surgieron seis categorías: "seguridad del paciente", "riesgos del paciente quirúrgico", "enfermeras en quirófano", "formación", "implicación del equipo interprofesional" e "implementación de la escala de valoración de riesgos". El desarrollo de las lesiones derivadas del posicionamiento fue identificado como el mayor riesgo para los pacientes quirúrgicos, destacándose la presencia de enfermeras en el quirófano como fundamental para la prevención de este riesgo, a través de la valoración del paciente y la formación del equipo. Consideraciones finales: Los enfermeros identificaron riesgos para la seguridade del paciente en relación con el posicionamiento quirúrgico, considerando indispensable su presencia en quirófano.


RESUMO Objetivo: Conhecer as concepções em relação à segurança do paciente durante seu posicionamento cirúrgico sob a ótica das enfermeiras de um Centro Cirúrgico. Método: Estudo exploratório descritivo qualitativo, com sete enfermeiras de hospital privado da região sul do Brasil, no mês de agosto de 2018. Geração de dados pela técnica de Grupo Focal, com análise de conteúdo de Bardin. Resultados: Da análise dos dados emergiram seis categorias: "segurança do paciente", "riscos do paciente cirúrgico", "enfermeiro em sala cirúrgica", "capacitação", "envolvimento da equipe interprofissional" e "implementação da escala de avaliação de risco". Identificou-se como maior risco o desenvolvimento de lesões decorrentes do posicionamento, sendo a presença do enfermeiro em sala cirúrgica destacada como fundamental para prevenção deste risco, através da avaliação do paciente e capacitação da equipe. Considerações finais: As enfermeiras identificaram riscos à segurança do paciente em relação ao posicionamento cirúrgico, considerando indispensável a presença dos mesmos em sala cirúrgica.

Abstract Objectives: To assess the actions performed by the operating room nurse during anesthesia and their behavior for patient safety regarding the reporting on adverse events, and to analyze their knowledge about anesthetic practices. Method: This is a cross-sectional study carried out using an electronic questionnaire consisting of socio-demographic, professional practice, knowledge in anesthesia, patient safety, and professional practice questions, conducted from January to March 2019 with operating room nurses. Results: One hundred nurses participated, 89 (89%) being women, with a mean age of 41.09 years (SD = 9.36), time of undergraduate completion of 14.33 years (SD = 8.34). The average attendance was 4.69 operating rooms (SD = 2.07) per nurse, with an emphasis on action before induction (49; 49%). Professionals reported performance of simultaneous activities (72; 72%) and insufficient number of employees (57; 57%) as difficulties of their daily practice. Among the participants, 77 (77%) correctly cited the periods of general anesthesia and 80.4% always reported the occurrence of an adverse event. Conclusion: Nurses identified their role in anesthesia, with limitations for assistance from multiple activities and lack of professionals.


RESUMEN Objetivos: Evaluar las acciones realizadas por los enfermeros en el quirófano durante la anestesia y sus conductas para la seguridad del paciente en la notificación de eventos adversos, analizando sus conocimientos sobre prácticas anestésicas. Método: Estudio transversal realizado con un cuestionario electrónico compuesto por preguntas sociodemográficas, práctica profesional, conocimientos en anestesia, seguridad del paciente y cuestiones de ejercicio profesional, realizado de enero a marzo de 2019 con enfermeros de quirófano. Resultados: Participaron 100 enfermeros, 89 (89%) mujeres, con una edad media de 41,09 años (DE = 9,36), tiempo de formación de 14,33 años (DE = 8,34). La asistencia media fue de 4.69 quirófanos (DE = 2.07) por enfermero, con énfasis en la actuación antes de la inducción (49; 49%). Los profesionales informaron como limitación de su práctica diaria la ejecución de actividades simultáneas (72; 72%) y la insuficiencia de empleados (57; 57%). Entre los participantes, 77 (77%) citaron correctamente los períodos de anestesia general y el 80,4% siempre reportaron la ocurrencia de un evento adverso. Conclusión: Los enfermeros identificaron su papel en la anestesia, con limitaciones en la atención por múltiples actividades y falta de profesionales.

Método: Estudo transversal realizado com questionário eletrônico composto por perguntas sócio-demográficas, prática profissional, conhecimento em anestesia, segurança do paciente e questões de exercício profissional, conduzido de janeiro a março de 2019 com enfermeiros de centro cirúrgico. Resultados: Participaram 100 enfermeiros, sendo 89 (89%) do sexo feminino, com média de idade de 41,09 anos (DP = 9,36), tempo de formação de 14,33 anos (DP = 8,34). A média de atendimento foi de 4,69 salas operatórias (DP = 2,07) por enfermeiro, com destaque para a atuação antes da indução (49; 49%). Os profissionais relataram como limitação de sua prática diária a execução de atividades simultâneas (72; 72%) e funcionários insuficientes (57; 57%). Entre os participantes, 77 (77%) citaram corretamente os períodos da anestesia geral e 80,4% sempre notificavam a ocorrência de um evento adverso. Conclusão: Os enfermeiros identificaram seu papel na anestesia, com limitações para assistência devido a múltiplas atividades e carência de profissionais.

ABSTRACT

Objetivo: Testar uma ação educativa entre enfermeiros de centro cirúrgico para discutir as limitações na assistência durante a anestesia. Método: Estudo descritivo de uma situação problema fundamentada na aprendizagem baseada em problemas. Amostra composta de 19 enfermeiros, divididos em três grupos, com a realização de dois encontros para levantamento, resolução de problemas e síntese de conceitos. Resultados: No primeiro encontro, identificaram-se problemas relacionados a operacionalização da assistência, conhecimento e treinamento dos profissionais para execução de cuidados e problemas no processo de trabalho, que impactam as ações dos enfermeiros. No segundo encontro, responderam-se às questões elaboradas com base na revisão de literatura sobre anestesia, diretrizes de cuidados e uniformidade de condutas. Mais da metade dos enfermeiros avaliou que a atividade contribuiu para refletir sobre as ações do enfermeiro na anestesia, as atividades diárias e a postura profissional, a necessidade de envolvimento com a assistência e a revisão de conceitos. Conclusão: O uso da situação problema como metodologia para educação em enfermagem favoreceu a discussão sobre conceitos de anestesia entre os enfermeiros e a reflexão sobre a atuação em sala cirúrgica.


Objective: To test an educational procedure among operating room nurses in order to discuss care limitations during anesthesia. Method: A descriptive study about a problem situation based on problem-based learning. A sample composed of 19 nurses that were divided into three groups which resulted in two meetings for a survey, problem resolution and concept synthesis. Results: In the first meeting, problem limitations were identified in relation to care organization, knowledge, professionals' training for care practice and problems in the work process, which influenced the nurses' actions. In the second meeting, the formulated questions were answered according to the literature review about anesthesia, care protocols and uniformity of actions. More than half of the nurses investigated considered that the activity contributed as a means to reflect on the nurses' actions in anesthesia, daily activities and professional attitudes as well as on the need for involvement with care and review of concepts. Conclusion: The use of problem situation as a methodology for education in nursing supported the conceptual discussions among nurses about anesthesia and the reflection on performance in the operating room


Objetivo: Probar una acción educativa entre enfermeras de un centro quirúrgico para discutir las limitaciones en la atención durante la anestesia. Método: Estudio descriptivo de una situación problemática, basado en el aprendizaje basado en problemas. Muestra compuesta por diecinueve enfermeros, divididos en tres grupos, con dos reuniones de encuesta, resolución de problemas y síntesis de conceptos. Resultados: En la primera reunión se identificaron problemas relacionados con dificultades en la operacionalización de la atención; conocimiento y formación de profesionales para realizar cuidados; y problemas en el proceso de trabajo, que impactan la acción de enfermeros. En el segundo encuentro se respondieron las preguntas elaboradas a partir de la revisión de la literatura sobre anestesia, pautas de atención y estandarización de conducta. Más de la mitad de los enfermeros evaluaron que la actividad contribuyó a reflexionar sobre las acciones de los enfermeros en anestesia, acciones cotidianas y postura profesional, la necesidad de involucramiento con el cuidado y revisión de conceptos. Conclusión: El uso de la situación problema como metodología para la formación en enfermería favoreció la discusión sobre conceptos de anestesia entre enfermeros y la reflexión sobre el trabajo en quirófano.

Objetivo: Elencar os materiais de maior impacto financeiro no centro cirúrgico e estimar a perda de receita que representaram para a instituição. Método: Pesquisa retrospectiva e quantitativa, com coleta em banco de dados e planilhas do centro cirúrgico e garantia da receita dos anos de 2015 e 2018 de uma instituição privada de extra porte de São Paulo. Resultados: Os materiais que causaram maior impacto financeiro foram: Sevorane®, Plasma Lyte®, manta térmica, sensor de monitor bispectral, máscara de anestesia, caneta para bisturi, perneira Sequel® e placa para bisturi. Representaram perda financeira de R$ 342.473,00 em 2015, quando ainda não havia a prática de auditoria in loco, comparada ao valor de R$ 114.157,00, em 2018, após ação de conferências em sala operatória. Conclusão: Houve benefício financeiro alcançado com a prática da auditoria in loco, com declínio de 66,6% na quantidade de materiais e medicamentos excluídos pela auditoria em razão da elevação da qualidade das anotações em prontuário.


Objective: To list the materials of greatest financial impact in the operating room and to estimate the loss of revenue they represented for the institution. Method: Retrospective and quantitative research, with collection in the database and spreadsheets of the surgical center and revenue guarantee for the years 2015 and 2018 of a private and large institution in São Paulo. Results: The materials that had the greatest financial impact were: Sevorane®, Plasma Lyte®, thermal blanket, bispectral monitor sensor, anesthesia mask, eletric scalpel pen, Sequel® leg protector, and electrosurgical plate. They represented a financial loss of BRL 342,473.00 in 2015, when there was still no on-site audit practice, compared with the amount of BRL 114,157.00, in 2018, after implementing checking initiatives in the operating room. Conclusion: There was a financial benefit achieved with the practice of on-site audit, with a 66.6% decline in the amount of materials and medicines excluded by the audit due to the enhancement in the quality of the notes found in medical records.


Objetivo: Enumerar los materiales con mayor impacto financiero en el Centro Quirúrgico y estimar la pérdida de ingresos que representaron para la institución. Método: Investigación retrospectiva y cuantitativa, con recolección en la base de datos y hojas de cálculo del Centro Quirúrgico y garantía de los ingresos para los años 2015 y 2018, de una institución privada extra grande de São Paulo. Resultados: Los materiales que causaron mayor impacto económico fueron: Sevorane®, Plasma Lyte®, Manta térmica, Sensor monitor biespectral, Máscara de anestesia, Pluma para bisturí, Perneira Sequel® y Placa para bisturí. Representaron una pérdida económica de R$ 342.473,00 en 2015, cuando aún no existía la práctica de auditoría in situ, frente al valor de R$ 114.157, en 2018, tras la acción de conferencias en el quirófano. Conclusión: Se logró un beneficio económico con la práctica de la auditoría in situ, con una disminución del 66,6% en la cantidad de materiales y medicamentos excluidos por la auditoría, debido al aumento en la calidad de las notas en las historias clínicas.

Objetivo: Conhecer a opinião de profissionais de enfermagem sobre os fatores que interferem no tempo de intervalo entre as cirurgias. Método: Estudo exploratório, descritivo, prospectivo, com análise qualitativa. A coleta de dados foi realizada em um complexo hospitalar no sul do Brasil, com aplicação de questionário sobre a percepção de profissionais de enfermagem que atuam no centro cirúrgico. Para o tratamento dos dados, utilizou-se análise de conteúdo de Bardin. Resultados: Participaram do estudo 25 profissionais, sendo quatro enfermeiros e 21 técnicos de enfermagem. Foram elencadas duas categorias, fatores relacionados à equipe e fatores relacionados aos processos do centro cirúrgico, e sete subcategorias. Conclusão: Diversos fatores interferem no tempo de intervalo entre cirurgias, destacando-se: capacitação da equipe, dimensionamento de pessoal, colaboração entre as equipes, porte cirúrgico e processos burocráticos.


To know the opinion of nursing professionals about the factors that interfere with the turnover time. Method: This is an exploratory, descriptive, prospective study, with qualitative data analysis. Data collection was carried out in a hospital complex in southern Brazil, with the application of a questionnaire on the perception of nursing professionals who work in the surgical center (SC). For data analysis, Bardin content analysis was used. Results: A total of 25 professionals participated in the study, four nurses and 21 licensed practical nurses. Two categories were listed, factors related to the team and factors related to the processes of the SC, and seven subcategories. Conclusion: Several factors interfere in the turnover time, highlighting team training, adequate staff, collaboration among teams, surgery size, and bureaucratic processes.


Objetivo: Conocer la opinión de los profesionales de enfermería sobre los factores que interfieren en el intervalo de tiempo entre cirugías. Método: Estudio exploratorio, descriptivo, prospectivo, con análisis cualitativo. La recolección de datos se realizó en un complejo hospitalario en el sur de Brasil, con la aplicación de un cuestionario sobre la percepción de los profesionales de enfermería que laboran en el quirófano. Para el tratamiento de los datos se utilizó el análisis de contenido de Bardin. Resultados: Participaron del estudio 25 profesionales, cuatro enfermeros y 21 técnicos de enfermería. Se enumeraron dos categorías, factores relacionados con el equipo y con los procesos del quirófano, y siete subcategorías. Conclusión: Varios factores interfieren en el intervalo de tiempo entre cirugías, destacando: formación de equipos, dimensionamiento del personal, colaboración entre equipos, tamaño quirúrgico y procesos burocráticos.

ABSTRACT

Objective:To compare the effect of systematic nursing in operating room and routine nursing in reducing intraoperative stress injury in patients with spinal fracture and spinal cord injury.Methods:A retrospective case-control study was conducted to analyze the clinical data of 285 patients with cervical or thoracolumbar fracture associated with spinal cord injury admitted to Honghui Hospital Affiliated to Xi'an Jiaotong University from January 2018 to December 2019, including 168 males and 127 females, with the age of 38-59 years [(47.8±8.5)years]. All patients underwent posterior decompression and fusion with internal fixation. Of all, 138 patients received systematic nursing in operating room including systematic evaluation and management before, during and after operation (observation group), and 147 patients received routine nursing including only intraoperative preventive care of pressure ulcer (control group). The incidence of pressure injury on the day after operation, degree of injury and location of injury at postoperative 3 days, and area of injury on the day after operation and at postoperative 3 days were compared between the two groups. The degree of injury was evaluated using the new stress injury staging assessment published by the American National Pressure Ulcer Advisory Panel (NPUAP).Results:The incidence of pressure injury in observation group [5.1% (7/138)] was lower than that in control group [12.2% (18/147)] on the day after operation ( P<0.05). The incidence of stage I, stage II, and stage III pressure injury in observation group [2.9% (4/138), 2.2% (3/138), 0.0%] was also lower than that in control group [8.2% (12/147), 3.4% (5/147), 0.6% (1/147)] at postoperative 3 days ( P<0.05). There was no significant difference in the incidence of pressure injury in the knee, chest, face and anterior superior iliac spine between the two groups ( P>0.05). The total proportion of pressure injury in the face and anterior superior iliac spine was 71% (5/7) in observation group, showing no significant difference from that in control group [83% (15/18)] ( P>0.05). The area of injury was (3.2±1.2)cm 2 and (3.2±1.1)cm 2 in observation group on the day after operation and at postoperative 3 days, lower than that in in control group [(5.1±1.5)cm 2 and (5.1±1.4)cm 2] ( P<0.01). Conclusion:Compared with the routine nursing, systematic nursing in operating room can significantly reduce the incidence, degree and area of intraoperative pressure injury in patients with spinal fracture accompanied by spinal cord injury, and deserves clinical promotion.

ABSTRACT

Objective:To study the influence of objective structured clinical examination (OSCE) assessment mode on learning interest and critical thinking of nursing students in operating room.Methods:There were 49 operating room nursing students from April 2019 to April 2020 who were selected as the observation group, receiving the teaching and assessment mode of OSCE. In addition, 49 operating room nursing students from March 2018 to March 2019 were included as the control group, receiving the conventional teaching and assessment mode. The nursing students' learning attitude scale was used to compare the learning interest of the two groups, and the critical thinking level of nursing students was recorded by Chinese critical thinking disposition inventory-Chinese version (CTDI-CV). SPSS 24.0 software was used to analyze the data with t test and chi-square test. Results:There were statistically significant differences in the scores of learning interest, learning experience, learning habits, professional cognition and the total scores of the learning attitude of nursing students after the assessment between the observation group and the control group ( t=4.861, t=4.885, t=3.466, t=4.206, t=8.292; all P<0.05). There were statistically significance of curiosity, truth seeking, open thinking, analytical ability, systematic ability, self-confidence in thinking, cognitive maturity and the CTDI-CV total scores after the assessment between the observation group and the control group ( t=4.262, t=4.311, t=2.754, t=4.854, t=3.984, t=2.864, t=4.267, t=2.729; P<0.05). Conclusion:The OSCE assessment mode used in operating room for nursing students is helpful to improve the learning interest and the cultivation of students' critical thinking.
