ABSTRACT

Abstract The observation of the human retina in vivo began in 1851 after the invention of the first ophthalmoscope by the German physicist Hermann von Helmholtz. In the following decades, direct and indirect ophthalmoscopy, with the use of ophthalmoscopes and condensing lenses, respectively, became part of the clinical examination, especially in ophthalmology and neurology. Today, over 170 years later, many ophthalmoscopes and condensing lenses exist on the market. Nevertheless, ophthalmoscopy is still not widely adopted as part of the physical exam of general practitioners, and the teaching of ophthalmoscopy in medical school remains challenging. Studies have shown that students prefer using newer ophthalmoscope models or condensing lenses during training, but most do not feel confident in performing ophthalmoscopy afterwards, regardless of the models used. Also, few students acquire ophthalmoscopes for their future practice, and clinical trials have not clearly demonstrated superiority of newer ophthalmoscope models over the conventional ones in diagnostic accuracy. The technological improvement of smartphone cameras in recent years has made it feasible to photograph the fundus of the eye using ophthalmoscopes or condensing lenses, reducing the need for retinographs and similar equipment. Smartphone assisted indirect fundoscopy is becoming increasingly popular. This approach allows adequate identification of the structures of the fundus, is cost-efficient, easy to implement, and permits easy recording and sharing of the images obtained, which is useful for case discussions and medical teaching. However, controlled clinical trials validating this method in the evaluation of optic nerve pathologies are needed.


Resumo A observação da retina humana in vivo começou em 1851, após a invenção do primeiro oftalmoscópio pelo físico alemão Hermann von Helmholtz. Nas décadas seguintes, a oftalmoscopia direta e indireta, com o uso de oftalmoscópios e lentes condensadoras, respectivamente, tornou-se parte do exame clínico, especialmente em oftalmologia e neurologia. Hoje, mais de 170 anos depois, diversos oftalmoscópios e lentes condensadoras estão disponíveis no mercado. No entanto, a oftalmoscopia ainda não é amplamente realizada no exame físico de médicos generalistas e o ensino da oftalmoscopia na faculdade de medicina ainda é desafiador. Estudos mostram que estudantes preferem usar modelos novos de oftalmoscópio ou lentes condensadoras durante seu treinamento, porém a maioria dos estudantes não se sente suficientemente confiante em realizar oftalmoscopia mais tarde, mesmo quando utilizam os modelos mais novos. Além disso, poucos alunos adquirem oftalmoscópios para sua prática futura, e ensaios clínicos não demonstraram uma clara superioridade dos novos modelos de oftalmoscópio em relação ao modelo convencional em acurácia diagnóstica. O aperfeiçoamento tecnológico das câmeras de smartphones nos últimos anos tornou possível fotografar o fundo do olho utilizando oftalmoscópios ou lentes condensadoras, reduzindo a necessidade de retinógrafos e equipamentos similares. A fundoscopia indireta realizada com smartphones vem se tornando cada vez mais popular. Esta abordagem permite a adequada identificação das estruturas do fundo do olho, é custo-efetiva, fácil de implementar e permite o fácil registro e compartilhamento das imagens obtidas, o que é útil para discussão de casos clínicos e para o ensino médico. Entretanto, ensaios clínicos controlados para a validação deste método na avaliação de patologias do nervo óptico são necessários.

ABSTRACT

Background: In ophthalmology (OPD), cycloplegics or mydriatics were needed for doing correct refraction and evaluation of fundus on a daily basis. Aims and Objectives: The objectives of this study were to compare the clinical efficacy and systemic side effects of different dilutions of phenylephrine and tropicamide combinations drops on pupil dilatation and to measure and compare pulse rate and blood pressure before and after giving the drug. Materials and Methods: The present study is a hospital-based interventional study conducted among patients attending to OPD of Regional Eye hospital, Visakhapatnam during April 2012–August 2012. Eighty-one patients randomly allocated to two groups (41 patients in Group 1 and 40 patients in Group 2). Dilution of phenylephrine 5% and Tropicamide 0.8% mixture was applied to patients of Group A in both eyes and dilution of phenylephrine 2.5% and Tropicamide 0.4% mixture was applied to Group B patients in both eyes. Pupil diameter and pulse rate BP were measured and compared. Results: Mean age of Group 1 patients was 55.39 ± 11.47 and Group 2 was 49.95 ± 13.71. Among 36 males, 50% in Group 1 and 50% in Group 2 and among 45 females 51.1% in Group 1 and 48.9% in Group 2. Among both Group 1 and Group 2 patients, both left and right pupil size was significantly increased after application of dilutions, but there was more increase in the pupil size among Group 1 patients compared to Group 2 patients 15 min after and 30 min after application of drug. Conclusions: The combination of 0.8% tropicamide and 5% phenylephrine was effective compared to alternate application of 0.4% tropicamide and 2.5% phenylephrine for rapid and sustainable pupil dilatation for indirect ophthalmoscopy.

RESUMO Objetivo Avaliar os mapeamentos de retina realizados em interconsultas oftalmológicas, analisando as prevalências de alterações encontradas e comparando-as com o motivo da interconsulta. Métodos Estudo transversal, com dados encontrados durante exame de mapeamento de retina de pacientes internados ou em atendimento no pronto-socorro. Os dados analisados de cada paciente foram idade, sexo, especialidade médica solicitante, motivo da interconsulta e achados do exame fundoscópico. Resultados Foram avaliados 104 pacientes, com prevalência de alterações ao exame em 43,27%. Os motivos mais frequentes de solicitação de avaliação oftalmológica foram trauma (16,35%) e alterações neurológicas (15,38%). O achado com maior prevalência foi o edema de papila, presente em 17,3% das avaliações gerais. Outros achados muito prevalentes foram as retinopatias diabética e hipertensiva, ambas com 11,53% na avaliação geral, mas com 83,3% e 53,84% de prevalência nos pacientes cujos motivos da consulta foram diabetes mellitus descompensada e crise hipertensiva, respectivamente. Conclusão O edema de papila e as retinopatias diabética e hipertensiva foram as alterações mais prevalentes, o que demonstra a importância do atendimento multidisciplinar sempre que possível, com maior atenção aos pacientes hipertensos e diabéticos.


ABSTRACT Objective To evaluate the indirect ophthalmoscopy performed in ophthalmologic consultations, analyzing the prevalence of the study and comparing them with the reason for the consultation. Methods Cross-sectional study, with data found during the indirect ophthalmoscopy exam of inpatients or in emergency room care. The data analyzed for each patient was sex, the requested medical age, the reason for the consultation and the funduscopic examination findings. Results One hundred and four patients were evaluated and 43.27% of patients with a prevalence of changes in the exam. The most requested reasons for requesting ophthalmologic evaluation were trauma (16.35%) and neurological alterations (15.38%). The most prevalent finding was papillary edema, representing 17.3% of the general estimates. Other very prevalent findings were diabetic and hypertensive retinopathies, both with 11.53% in the general assessment, but with 83.3% and 53.84% prevalence in patients whose reasons for consultation were decompensated DM and hypertensive crisis, respectively. Conclusions Papillary edema and diabetic and hypertensive retinopathies were the most prevalent alterations, which demonstrates the importance of multidisciplinary care whenever possible, with greater attention to hypertensive and diabetic patients.

ABSTRACT

Purpose: To compare image characteristics of retinal nerve fiber layer (RNFL) between glaucoma patients and healthy controls using adaptive optics scanning laser ophthalmoscopy (AOSLO). Methods: This was a cross-sectional pilot study with two groups: a glaucoma group with patients with moderate or severe glaucoma as per the Hodapp–Parrish–Anderson classification system and a control group with healthy individuals. The optic nerve damage in moderate glaucoma was predominantly located in only one hemisphere; the other hemisphere was un- or minimally affected on optical coherence tomography and automated perimetry and is referred to as early glaucoma. The structure of RNFL bundles and gain (%) in RNFL images with mean pixel values between 15 and 35 were analyzed. Imaging was performed one degree away from the optic disc margin at two and four cardinal clock positions in the glaucoma and control groups, respectively. The field of view was 1.3° at 2.3 ? resolution. We studied one eye per participant. Results: There were 11 glaucoma patients and 7 healthy controls. Imaging was successful at 88% of the locations in controls and early glaucoma; the reflectivity differed significantly (0.51 and 0.56, respectively, P < 0.001) but not the structure of RNFL bundles (Cohen’s Kappa 0.11) between them. In patients with moderate and severe glaucoma, imaging was successful only at 46% of the locations; RNFL bundles were not discernible, and RNFL reflectivity did not differ from those with early glaucoma (P < 0.11). Conclusion: The recorded gain (%) of RNFL images obtained using AOSLO could be an objective indicator of early glaucoma

ABSTRACT

Purpose: Comparison of patient satisfaction with red?free (green) versus yellow light using binocular indirect ophthalmoscope for retinal examination. Methods: This is an observational questionnaire?based study of 100 myopes in the age group of 18–40 years coming for a routine check?up or for refractive surgery workup. The examination was done using an indirect ophthalmoscope and a 20D lens with green or yellow light and was assigned in two groups randomly using the coin toss method, following which, a questionnaire was used to assess the following parameters: a) level of comfort, b) any complaints of discomfort during examination, d) preference of the used light source in future, e) grading of discomfort on a linear scale, and f) patient cooperation and duration of examination. Results: Patients were randomized for observation with IDO using either green light (n = 55) or yellow light (n = 45) filter. In the study, 46 patients (83.6%) were very comfortable and only 9 patients (16.4%) experienced mild discomfort when using red?free (green) light, while only 3 patients (6.7%) were very comfortable and 31 (68.9%) had mild discomfort when using yellow light. The complaints of watering with yellow and green light were noted in 36 patients (80.0%) and 15 patients (27.3%), pain in 13 patients (28.9%) and 3 patients (5.5%), light sensitivity in 29 patients (64.4%) and 4 patients (7.3%), respectively, all being significantly more in yellow light category (P < 0.001). The time of examination was significantly more in yellow light category with 83 ± 10.75 seconds (P < 0.001). Conclusion: Retinal examination using the green filter of indirect ophthalmoscope is more comfortable in examining the patients as compared to routine yellow light with decreased number of complaints, lesser examination time, and better patient cooperation.

ABSTRACT

Purpose: To determine and validate retinal vascular caliber measurements by using the confocal scanning laser ophthalmoscopy system. Retinal vasculature changes are often regarded as clinical markers for systemic disease. Methods: It was a prospective observational study conducted on 600 eyes of 300 normal subjects with no systemic or ocular illness from January 1, 2016 to June 30, 2017 in a tertiary referral eye center. Non?mydriatic infrared reflectance, blue reflectance, and blue peak blue autofluorescence fundus imaging were done on the confocal scanning laser ophthalmoscopy system. The dimensions of the retinal vessels were measured using inbuilt calipers at 1800 ?m from the center of the optic disc. Internal and external dimensions were measured. Observer variation and its comparison using Image J software were assessed. Results: The median age was 29 years (18–50 years). Mean internal and external diameters for arterioles were 85.1 ± 12.4 ?m and 105.0 ± 12.0 ?m, and for venules were 133.8 ± 16.6 ?m and 145.4 ± 16.1 ?m, respectively. The mean internal and external wall thicknesses were 19.7 ± 8.0 ?m and 11.0 ± 5.6 ?m, and wall thickness?to?lumen ratios were 0.3 ± 0.1 and 0.1 ± 0.1, respectively. Arteriolar?to?venular ratio for lumen and vessel was 0.66 ± 0.1 and 0.74 ± 0.1, respectively. There was no statistically significant difference between age groups. Both inter? and intra?observer reproducibility was >95%. The Bland–Altman plot showed that the difference between measurements using both confocal scanning laser ophthalmoscopy and Image J software lies within the limits of agreement approximately 95% of the time. Conclusion: This is the first effort to develop a normative database by using a simple non?invasive confocal scanning laser ophthalmoscopy system with high observer reproducibility

Objetivo: avaliar o nível de conhecimento oftalmológico dos estudantes de medicina sobre achados do fundo de olho e alterações da retinopatia diabética, assim como o interesse destes alunos por um maior aprendizado sobre o assunto. Materiais e Métodos: A amostra foi constituída por alunos do curso de medicina do Centro Universitário Christus (Unichristus), na cidade de Fortaleza ­ Ceará ­ Brasil, durante os meses de novembro e dezembro de 2018. Eles responderam a um questionário composto por 20 questões de múltipla escolha, com quatro itens cada, contendo apenas um único item correto, com assuntos relacionados à fundoscopia e à retinopatia diabética. Foram escolhidos o quinto semestre (s5), por ter finalizado recentemente o módulo de oftalmologia, e o sétimo semestre (s7), para avaliação da retenção de conteúdo após um ano do término da disciplina. O questionário também abordou o interesse dos alunos em obter maior aprendizado sobre o assunto. Resultados: o questionário foi aplicado em 120 alunos (62 alunos do s5 e 58 alunos do s7). Houve um maior número de acertos no s5, com média de 57,2% de acertos, em relação ao s7, cuja média de acertos foi de 49,9%. Conclusão: Diante de uma média de acertos abaixo do desejável, e do interesse da maioria dos alunos em obter maior aprendizado, fica clara a demanda por uma maior inserção da disciplina de oftalmologia na graduação médica, por meio das instituições de ensino, dando a devida relevância ao tema, oferecendo aos seus alunos suporte para um aprendizado concreto e contínuo.


Objective: to evaluate the level of ophthalmological knowledge of medical students about fundus findings and changes in diabetic retinopathy, as well as the interest of these students in greater learning on the subject. Materials and Methods: The sample consisted of medical students from the Centro Universitário Christus (Unichristus), in the city of Fortaleza - Ceará - Brazil, during the months of November 2018 and December 2018. They answered a questionnaire composed of 20 multiple-choice questions, with four items each, containing only one correct item, with subjects related to fundus findings and changes in diabetic retinopathy. The fifth semester (s5) was chosen, for having recently completed the ophthalmology module, and the seventh semester (s7), to assess content retention one year after the end of the course. The questionnaire also addressed the students' interest in obtaining greater learning on the subject. Results: the questionnaire was applied to 120 students (62 students from s5 and 58 students from s7). There was a greater number of correct answers in s5, with an average of 57.2% of correct answers, compared to s7, whose average of correct answers was 49.9%. Conclusion: Given an average of hits below the desirable, and the interest of most students to obtain greater learning, it is clear the demand for a greater insertion of the discipline of ophthalmology in medical graduation, through educational institutions, giving due importance to the theme, offering its students support for learning concrete and continuous.

ABSTRACT

Background: Aim: Study fundal changes in different age groups & degrees of myopia.Measure ocular parameters (Corneal Curvature, Axial Length, Intraocular Pressure,Central Corneal Thickness) associated with myopia. Material And Methods: Study included 150 patients out of which 50 were low, 50 medium and 50 high myopia.Examination included best corrected visual acuity, slit lamp examination, keratometry, axial length, central corneal thickness and intraocular pressure, ultrasound B scan, dilated fundus with indirect ophthalmoscope. Result: All fundus findings had correlation with high myopia except choroidal neovascularization which did not differwith degree. All degree showed highest percentage of Keratometry K1 between 42 to 43.99 D and K2 between 44 to 45.99 D with lowest percentage K1 between 46 to 48 D and K2 between 48 to 50 D.Axial lengthincreased with severity. Highest percentage ofcentral corneal thickness in all degree was between 500 to 549 microns.100% of high myopic had tessalation,it did not differ with age. Temporal crescent was common in higher age group. Peripheral retinal degeneration was common between 20 to 29 years.Posterior staphyloma was associated between 30 to 39 years. Retinal detachment did not vary with age.Conclusion: Visual impairmentdue to myopia may be prevented by early detection of cause by appropriate investigations and treatment.

Resumo Objetivo: o ensino da oftalmoscopia direta vem recebendo menos atenção na graduação, o que resulta em médicos sem confiança para realizá-la. Diante disso, foi analisada a autopercepção dos acadêmicos dos cursos de medicina de Mato Grosso sobre a oftalmoscopia direta e os principais fatores responsáveis pela insegurança em executá-la. Métodos: foi realizada a revisão bibliográfica nas bases Pubmed, Scielo e Biblioteca Virtual em Saúde, sendo construído um questionário aplicado, pelo método bola de neve e após parecer favorável do Comitê de Ética, a acadêmicos de medicina do Mato Grosso que estavam cursando regularmente o quinto ou sexto ano do curso. A análise estatística foi realizada no software SPSS 20, considerando intervalo de confiança de 95%. Resultados: verificou-se que a proporção de acadêmicos que tiveram aulas sobre a oftalmoscopia direta foi superior em universidades públicas (p < 0,001); que a maioria dos alunos não acredita que suas escolas forneçam as condições adequadas para o aprendizado do exame (p = 0,001); que 10,5% se consideram conhecedores da teoria e prática da oftalmoscopia direta (p = 0,004); que a falta de treinamento durante a graduação é o principal fator que dificulta o aprendizado do exame; e que isso resulta em falta de confiança em realizar o mesmo. Conclusão: a oftalmoscopia direta não tem recebido a devida importância durante a graduação em medicina no estado de Mato Grosso. Com tais resultados, pode-se tomar medidas que melhorem o ensino-aprendizado da técnica, o que posteriormente trará benefício para a população.


Abstract Objective: direct ophthalmoscopy teaching is receiving less attention during graduation, which results in doctors without confidence to perform the exam. Therefore, the self-perception of students from medical schools of Mato Grosso about the direct ophthalmoscopy and the main factors responsible for the insecurity on performing it was analyzed. Methods: a literature review was done using the databases Pubmed, Scielo and Biblioteca Virtual em Saúde, then a questionnaire was applied, through the snowball method and after favorable assent of the Ethics Committee, to medical students from Mato Grosso who were regularly enrolled in the fifth or sixth year of the course. Statistical analysis was performed on the SPSS 20 software, considering a confidence interval of 95%. Results: it was verified that the proportion of students that have had classes about direct ophthalmoscopy was superior in public schools (p < 0,001); that the majority of students does not believe that their schools provide adequate conditions for learning the exam (p = 0,001); that 10,5% considerate themselves knowledgeable about theory and practice regarding direct ophthalmoscopy (p = 0,004); that few training during graduation is the main factor that hinders learning the exam; and that this results in underconfidence at performing it. Conclusion: direct ophthalmoscopy is not receiving the deserved importance during medical graduation in the state of Mato Grosso. With these results, measures can be taken to improve the teaching-learning process of the technique, which can bring benefit to the population.

@#AIM: To investigate the ability of Retcam3 in screening the retinopathy of prematurity(ROP)and its relationship with the change of intraocular pressure.<p>METHODS: A total of 80 premature infants who met the screening criteria admitted to our hospital from August 2017 to August 2019 were selected as subjects. They were screened by binocular indirect ophthalmoscope and Retcam3 system respectively. The former method was chosen as the gold standard, the sensitivity, specificity, positive and negative predictive values of Retcam3 system screening was calculated, the intraocular pressure changes before and after screening was compared. <p>RESULTS:A total of 37 children with ROP were examined by binocular indirect ophthalmoscopy. 35 infants with ROP were screened by Retcam3 system. The sensitivity of Retcam3 was 100%, the specificity was 95.8%, and the positive predictive value was 94.2%, the negative predictive value is 100%. The intraocular pressure of left eye and right eye after Retcam3 screening was higher than that before dilation, and the difference was statistically significant(<i>P</i><0.05).<p>CONCLUSION: The Retcam3 system can accurately screen patients with ROP, while it can cause an increase in intraocular pressure after screening.

RESUMEN El fondo de ojo es considerado por muchas especialidades médicas un proceder complementario. ¿Existe una correcta indicación del fondo de ojo de urgencia? Objetivo: Identificar los principales motivos de indicación del fondo de ojo urgente y su pertinencia. Métodos: Se realizó un estudio descriptivo transversal en el periodo comprendido desde el 29 de mayo hasta el 1ro. de diciembre del año 2018, en una muestra de 59 pacientes de un universo de 67 casos con indicación de fondo de ojo urgentes, a realizarse en la consulta del Cuerpo de Guardia de Oftalmología en la Unidad de Cuidados Intensivos Especiales y en salas intrahospitalarias del Hospital General Docente Provincial "Antonio Luaces Iraola", de la provincia de Ciego de Ávila, Cuba. Resultados: El 28,8 por ciento de los fondos de ojos fueron ordenados por Medicina Interna, seguido por Pediatría y Ginecobstetricia con 20,3 por ciento cada uno. El antecedente patológico personal ocular más frecuente fue la ametropía y el familar el glaucoma. El antecedente patológico personal sistémico más frecuente fue la hipertensión arterial. Las razones que motivaron su indicación urgente, en orden de frecuencia, fueron: el síndrome de hipertensión arterial en gestantes, el síndrome de cefalalgia, el traumatismo craneoencefálico en puérperas, el síndrome febril y el síndrome vertiginoso. Solo en 2 pacientes se visualizaron signos de sospecha de glaucoma, sin otras alteraciones a nivel de la papila óptica, y retina del polo posterior en los casos restantes. Conclusiones: Todos los fondos de ojos realizados fueron negativos para papiledema, signo que presumiblemente debe descartarse, en dependencia del motivo y la especialidad que lo indique con urgencia(AU)


ABSTRACT The fundus of the eye is considered, by many medical specialties, a complementary procedure. Is there a correct indication of the fundus of the eye as an emergency? Objective: To identify the main reasons for the indication of the fundus of the eye as an emergency and its relevance. Methods: A descriptive and cross-sectional study was carried out in the period from May 29 to December 1, 2018, with a sample of 59 patients from a study group of 67 cases with indication of urgent fundus of the eye to be carried out in the service of the Ophthalmology Emergency Room belonging to the Special Intensive Care Unit and in hospital wards of Professor Antonio Luaces Iraola General Hospital of Ciego de Ávila Province, Cuba. Results: 28.8 percent of the fundus of the eye were ordered by internal medicine, followed by pediatrics and gynecobstetrics, each accounting for 20.3 percent. The most frequent personal pathological ocular history was ametropia and the glaucoma family. The most frequent systemic personal pathological history was arterial hypertension. The reasons that motivated its urgent indication, in order of frequency, were the syndrome of hypertension in pregnant women, the headache syndrome, the traumatic brain injury in postpartum women, the febrile syndrome, and the vertiginous syndrome. Only two patients showed signs of suspected glaucoma, without other alterations at the level of the optic papilla, and retinal posterior pole in the remaining cases. Conclusions: All the fundus of eyes carried out were negative for papiledema, a sign that presumably must be discarded, depending on the reason and the specialty that indicates it with urgency(AU)

ABSTRACT

Introduction: The uvea plays an important role in ocularimmunological defense mechanism. Immunologicallyspeaking, the eye has a deviant and privileged position becauseof the blood retina barriers, absence of lymphatic drainageand a special feature called anterior chamber associateddeviation. This special defense mechanism contributed to thepreservation of vision. If these mechanism fail, intraocularinflammation will arise, which may lead to blindness. The aimof this research is to find out the incidence of blindness inpatients with uveitis.Material and methods: In this study, 50 cases of uveitisattending ophthalmic out patients and those who are admittedin department of ophthalmology, Regional Institute of MedicalSciences, Imphal were studied to find out clinical incidence ofblindness. All the patients were subjected to detailed ocularexamination including visual acuity, slit lamp biomicroscopy,direct/indirect ophthalmoscopy and intraocular pressure wasrecorded.Results: Out of 50 patients examined, 20 cases (40%)developed blindness or visual impairment. Bilateral loss ofvision developed in 5 cases (10%) and unilateral loss of visionoccurred in 11 cases (22%).Conclusion: Uveitis was found to be more common in the agegroup of 21 – 30 years with male preponderance. The maincause of visual loss were secondary glaucoma and cataract.Patients with panuveitis had the worst visual prognosis.

ABSTRACT

Purpose: To describe estimation dynamic distance direct ophthalmoscopy (eDDDO) and compare it with the monocular estimation method of dynamic retinoscopy (eDR) for the assessment of accommodation in children. Methods: In this prospective observational cohort study, an ophthalmologist performed eDDDO followed by eDR in children with normal eyes, and then under the partial effects of cyclopentolate and tropicamide to assess performance of eDDDO with eDR under the condition of pharmacologically induced accommodation failure. Only one eye of each child was recruited in the study. To study the inter-observer variation, two masked pediatric ophthalmology fellows performed eDDDO in the similar manner. Results: For the comparison of eDDDO with eDR, 60 eyes of 60 patients were recruited. The mean age of the patients was 10.4 years. The mean accommodation on eDDDO was 3.0D, 5.1D, 9.8D, and 11.3D at 40 cm, 25 cm, 10 cm, and 8 cm, respectively and 3.0D, 5.0D, 9.5D, and 11.0D on eDR. The eDDDO overestimated accommodation by a mean 0.17D (95% CL 0-0.48D, P = 0.5). The correlation of eDDDO with eDR was excellent (Pearson r 0.98, T value 76.0). The inter-observer difference with eDDDO was not significant (mean 1D, 95% CL 0-2.6D, P = 0.9) and the correlation between two observers was excellent (Pearson r 0.9, T value 12.7). The eDDDO and eDR were also performed on 12 eyes of 6 children with a mean age of 8.5 years (range 8-12 years) under the partial effect of cyclopentolate and tropicamide, where eDDDO overestimated the accommodation by a mean 0.3D (95% CL 0- 1.2D, P = 0.7) and the correlation was excellent (Pearson r 1.0, T value 45). Conclusion: eDDDO is a simple, reliable, quantitative, and objective technique of accommodation assessment for children. Further studies with larger sample are required to assess its performance in disorders of accommodation affecting younger children and in children with ocular comorbidities.

ABSTRACT

Diabetic retinopathy (DR) is one of the most common causes of visual impairment and blindness in diabetic patients.It is particularly important to set up simpler,safer,non-invasive and highly effective methods for diagnosis as well as monitoring DR.A variety of new fundus imaging techniques show great advantages in early diagnosis,treatment and monitoring of DR in recent years,The main characteristics of wide-field scanning laser imaging system is achieving a large range of retinal image in a single photograph and without mydriasis.It provides several options for color images,FFA and FAF,which satisfy to detect the retina,choroid and vascular structure.Multi spectral fundus imaging system is suitable for DR screening,because it is able to recognize the typical characteristics of DR,such as microaneurysms,hemorrhage and exudation,and is non-invasive and convenient.OCT angiography is a quantitative examination that provides foveal avascular zone area,macular blood flow density,which provides strong evidence for DR diagnosis.The improvement of these new techniques will help us to build up a personalized evaluation system of DR.

ABSTRACT

PURPOSE: To compare parameters of confocal scanning laser ophthalmoscopy (Heidelberg Retinal Tomograph [HRT] II) in high myopia with age- and sex-matched emmetropes. METHODS: A hospital-based cross-sectional study was conducted among Nepalese subjects aged 18 to 35 years at BP Koirala Lions Centre for Ophthalmic Studies, Institute of Medicine, Maharajgunj, Kathmandu from November 2015 to October 2016. Fifty consecutive subjects with high myopia (spherical equivalent ranging from −6.00 to −12.00 diopters) and age- and sex-matched emmetropic subjects were enrolled for comparison. Correlations between disc area and other HRT parameters, asymmetry between the right and left eyes, and comparisons between male and female subjects in both high myopic and emmetropic groups were evaluated. RESULTS: Disc area was not significantly (p = 0.11) larger in high myopic eyes than in emmetropic eyes. HRT parameters in highly myopic eyes involved smaller cup parameters and greater rim parameters compared with emmetropic eyes. Disc area was found to be significantly positively correlated with inter disc parameters and significantly negatively correlated with rim to disc area ratio in the high myopia group. Disc area and other intra-disc parameters showed significant correlations between right and left eyes in both high myopia and emmetropia, and no significant differences between males and females from a Nepalese population. CONCLUSIONS: Characteristics of HRT parameters in high myopic eyes involved smaller cup parameters and greater rim parameters compared with emmetropic eyes in a Nepalese population. The effect of disc area on HRT parameters differed significantly only in height variation contour by emmetropic eyes.

ABSTRACT

Purpose: To assess the variability of assessing the ocular torsion on fundus photographs among strabismus surgeons. Methods: This was a prospective, noninterventional, clinical trial involving 16 trained and experienced squint surgeons participated in the study. Two videos were prepared of a total of 10 fundus pictures with or without lines for disc foveal angle. The first video had a 4 s viewing time for each fundus image. The second video had the disc foveal lines drawn and a similar 4 s viewing time for each image. The participants were asked to grade the torsion. The primary outcome measure was to assess the agreement between the raters for ocular torsion measurement. Difference in the response of observers from the standard response was the secondary outcome measure. Results: A 4 s viewing time was given to mimic the exposure time in the clinic while assessing torsion in a patient. Large variability was found among the responses. The kappa test was done for comparing the agreement between various observers which ranged from slight to fair (<0.40). There was no difference in torsion grading in 30.6% and 26.3% responses in the first and second video from the standard response, respectively. When a limit of ±1 grade was taken as acceptable for the responses, 66.2% for the first and 68.7% for the second video respectively were similar to standard response. Conclusion: There is wide variability in assessing ocular torsion by fundus photography. The level of accuracy does increase with marking the line on photographs but still remains unreliable.

ABSTRACT

Objective:To study the foveal displacement during the closure of idiopathic macular holes (MHs).Methods:Thirty-seven idiopathic MH patients treated by pars plana vitrectomy and internal limiting membrane peeling were studied prospectively. Locations of MH center and foveal pit were measured by optic coherence tomography. Retinal displacement was observed using confocal scanning laser ophthalmoscopy.Results:A total of 40 eyes were included in this study and MHs were closed in 37 eyes (92.5%). The confocal scanning laser ophthalmoscopy showed that all of the retinal capillaries in the superior, inferior, nasal and temporal sides of the MHs moved toward the optic nerve head (ONH). The optic coherence tomography results showed that the mean nasal displacements of foveal pits were (102.9±61.2), (109.6±53.1), and (137.0±52.0) μm at 3, 6 and 12 months, respectively. And the mean vertical displacements were (55.9±49.4), (61.4±57.8) and (67.8±54.3) μm, respectively. Post-operative foveal pits were located in the nasal side of the MH centers. The extension of retina and nasal to the MH were in opposite directions: the nasal hole margin moved toward the MH, but the retina located closer to the ONH moved toward the ONH. The fellow eyes of three patients developed into idiopathic MH during the follow-up period and operations were performed for all of the three patients.Conclusion:Our results showed that center of macula does not move when an idiopathic MH develops, but it moves toward ONH during closure of hole; thus, new fovea is in nasal side of original fovea.

ABSTRACT

Objective To compare the imaging characteristics and detection of various types of lesions in diabetic retinopathy (DR) with colorful laser scanning fundus imaging (MSLI) and traditional color fundus photography (CFP).Methods Prospective case series observational study.A total of 38 eyes of 38 patients with DR diagnosed by clinical examination were included in the study.Among them,21 were male and 17 were female;the mean age was 62.6± 11.2 years;the average duration of diabetes was 14.3±7.5 years.All the patients were performed CFP,MSLI,frequency domain optical coherence tomography (SD-OCT),fluorescein angiography (FFA) examination.Using the Helielberg Spectralis HRA+OCT MSLI inspection,one scan simultaneously obtained 488 nm blue reflection (BR),515 nm green light reflection (GR),820 nm infrared light reflection (IR),and multicolor image (MC).The detection of traditional CFP and MC on microaneurysm (MA),hard exudation (HEX),cotton plaque (CWS),intraretinal hemorrhage (IRH),intraretinal microvascular abnormality (IRMA),venous bead (VB),venous ring (VL),macular edema (DME),macular anterior membrane (MEM) and laser photocoagulation (LB) were comparatively observed.The results of FFA examination were used as the diagnostic criteria for lesions.SD-OCT was used to determine the location and depth of lesions and the diagnostic reference for DME and MEM.Results The numbers of eyes with MA (x2=10.460),DME (x2=4.006),MEM (x2=4.444) was significantly higher in MC than that of traditional CFP.But the number of eyes with IRH (Z2=0.103),CWS (x2=1.515),HEX (x2=0.227),IRMA (x2=0.051),VB (x2=0.001),VL (x2=0.149),VH (x2=0.693) and LB (x2=0.720) were not statistically significant between two methods (P>0.05).The imaging quality of MSLI mode is obviously better than that of traditional CFP.Among them,GR imaging shows the best structural changes of superficial retina in MA,CWS,HEX,MEM,etc.IR imaging shows clear depth in deep retina such as LB.DME was green on MC and the weak low-reflection dark area was visible on the IR image,which were consistent with the DME range indicated by the SD-OCT examination.Conclusions Compared with the traditional CFP,the MSLI can clearly show the DR lesion.The number of checkouts is high on MA,DME and MEM by MC image.
