ABSTRACT

Objective: To investigate the pschological status of the patients with oral lichen planus disease (OLP) . Methods: 100 patients with OLP and 100 healthy controls were included. The self-rating anxiety scale (SAS), self-rating depression scale (SDS), neuroticism extraversion openness (NEO) and WHOQOL-BREF were used to analyze the life quality and pschology of the subjects.Andrenocor-cicotropin (ACTH) and cortisol (CORT) in serum were measured. The relationship between OLP and mental illness, stress, personality traits and life quality was analyzed. Results: There were no significant differences of gender and age between the case group and the control group (P> 0. 05) . There were significant differences of the SAS score, SDS score, NEO score, WHOQOLBREF score and the level of ACTH and CORT (P < 0. 05) in serum between the 2 groups. Logistic regression analysis showed that anxiety, depression, personality and life quality were the influencing factors of OLP prevalence. Conclusion: The development of OLP may be related to psychological status, depression, stress, anxiety and so on.

ABSTRACT

Objective:To investigate the significance of periodontal treatment on the levels of MMP-3 and IFN-γin the serum of patients with oral lichen planus(OLP) and chronic periodontitis(CP) before and after treatment.Methods:60 patients with erosion OLP and CP were divided into 2 groups randomly (n =30).The patients in experimental group received basic periodontal treatment combined with drug therapy,those in control group received drug therapy only.Before and after treatment the periodontal indexes of PD,AL,BI and PLI were measured in the experimental group,the levels of MMP-3 and IFN-γ in serum were tested by ELISA in the 2 groups,the differences of the data between 2 groups were statistically analysed.Results:After treatment the periodontol indexes were decreased in the experimental group(P < 0.05),the serum MMP-3 and IFN-γ levels were lower than those before treatment in 2 groups(P<0.05),and the decline of MMP-3 and IFN-γ in experimental group was more obvious than those in the control group (P<0.05).Conclusion:Periodontal initial therapy has positive influence on the treatment of OLP and can reduce the level of inflammation.

ABSTRACT

60 patients with pathologically diagnosed erosive lichen planus of bilateral buccal mucosa were included.The lesion on one side was treated by local application of triamcinolone acetonide dental paste(test group),the lesion on the other side without treatment was used in the control group.7,1 4,21 and 28 d after treatment the pain level and the lesion situation scores in the test group decreased significantly (P <0.001 ),and lower than those in the control group at the same time point(P <0.05).No adverse reaction was found in the 2 groups. Triamcinolone Acetonide Dental Paste is effective in the treatment of erosive oral lichen planus.

ABSTRACT

Objective:To explore the expression of PD-1 mRNA and PD-L1 mRNA in peripheral blood mononuclear cells(PBMCs)of OLP patients.Methods:The peripheral blood lymphocyte subsets (CD3 +,CD4 +,CD8 +,CD1 9 +,CD1 6 ++56 +)and humoral immunity indexes (lgG,lgA,lgM,C3,C4)were detected in 36 patients with OLP by flow cytometry and nephelometry respectively.The expres-sions of PD-1 mRNA and PD-L1 mRNA in PBMCs of 36 patients with OLP and 1 8 healthy individuals(the controls)were detected by fluorescence quantitative PCR,the correlation of PD-1 mRNA or PD-L1 mRNA with the immune function of OLP patients was analysed. Results:In OLP patients the percentages of peripheral blood lymphocyte subsets (CD3 +,CD4 +,CD8 +,CD1 9 +,CD1 6 + +56 +)as well as the complement of C3 and C4 were lower(P <0.05),the percentages of CD1 9 +(B)and the level of lgMwere higher(P <0.05),the expression levels of PD-1 mRNA and PD-L1 mRNA in rPBMCs were higher(P <0.05)than in the controls.There was positive correla-tion between PD-1 mRNA and PD-L1 mRNA expression level.The expression level of PD-1 mRNA was negatively correlated with CD4 +,but positively with lgG,the expression level of PD-L1 mRNA was positively correlated with CD1 9 +.Conclusion:The up-regu-lation of PD-1 mRNA and PD-L1 mRNA in PBMCs is correlated with the immunity of OLP patients.PD-1 /PD-L1 pathway may play an important role in immune pathogenesis of OLP.

ABSTRACT

Objective:To observe the efficacy of different forms of triamcinolone acetonide( TA) in the treatment of buccal erosive oral lichen planus (EOLP). Methods:120 EOLP patients were randomly divided into 4 groups(n=30) and treated with Kangfuxin mouth-wash followed by topical ointment of( TA) , TA injection, TA mouth-wash and only Kangfuxin mouth-wash only, respectively. Before treatment, 1, 2 and 4 weeks after treatment, the pain (VAS score) and erosion area were measured, the effects were compared among groups. Results:The VAS scores of A, B and C groups were significantly lower than those of the control group after 1, 2 and 4 week treatment, and the total effective rate was significantly higher than that of the control group, respectively(P0. 05). The average erosion area of group A and B was significantly smaller than that of the control group after 1, 2 and 4 week treatment(P0. 05), but 4 weeks after treatment, the average area of erosion of group C was significantly smaller than that of the control group(P<0. 05). Conclusion:Triamcinolone acetonide is effective in the treatment of OLP and the oral oint-ment form is safe and convenient for OLP treatment.

ABSTRACT

Objective:To identify differentially expressed saliva proteins of oral lichen planus(OLP)patients by two-dimensional fluo-rescence difference electrophoresis(2-D DIGE)and mass spectrometry(MS).Methods:3 pairs of saliva samples from OLP patients and matched healthy adults were collected.Saliva proteins were separated by 2-D DIGE and identified by liquid chromatography-mass spectrometry(LC-MS).Results:SDS-PAGE examination showed that the electrophoresis bands were clear and protein loss was rare. Protein dots were highly reproducible by 2-D DIGE.In average,the abundance of (31 7 ±71 )saliva protein spots were found in OLP pa-tients.4 highly reproducible spots were identified to be secretory IgA1 ,zincα-2-glycoprotein,salivary amylase and serum albumin by LC-MS and they were at higher level in OLP patients than those in the healthy controls.Conclusion:Secretory IgA1 ,zincα-2-glyco-protein,salivary amylase and serum albumin are highly expressed in the saliva of OLP patients,and may be related to the occurrence and development of oral lichen planus.

ABSTRACT

Objective:To evaluate of effects of Qingxianyin in the treatment of oral lichen planus(OLP),and to explore its regula-tion of T lymphocytes cellular immune mechanism.Methods:25 cases of OLP were treated by Qingxianyin for 8 weeks and 25 heathy subjects were served as the blank controls.Before and after treatment,peripheral blood CD4 +,CD8 + lymphocyte counts and percentages and CD4 +/CD8 + ratio were measured by flow cytometry.The clinical effects of Qingxinayin were evaluated.Results:After 8 weeks of treatment,significant effect,improvement and ineffecacy were observed in 1 8,5 and 2 cases respectively.The total effective ratio was 92.0%.The CD4 + lymphocyte counts and percentage increased(P <0.05),CD8 + counts and percentage de-creased(P <0.05),CD4 +/CD8 + ratio increased(P <0.05).After treatment the indexes changed towards those of the healthy con-trols.Conclusion:Qingxianyin is effective in the treatment of OLP by promoting mucosal healing,microcirculation and improving im-mune function.

ABSTRACT

Objective:To investigate the expression of LAIR-1 on Treg cells and TGF-βin the peripheral blood of patients with oral lichen planus(OLP).Methods:The peripheral blood was obtained from 22 OLP patients and 22 healthy controls.Flow cytometry was used to evaluate the proportion of LAIR-1 on CD4 +Foxp3 +Treg cells,and the level of TGF-βwas measured by ELISA.Results:In the peripheral blood of OLP patients,the proportion of LAIR-1 on CD4 +Foxp3 +Treg cells and TGF-βcontent were significantly higher than those of the healthy controls(P <0.05),The proportion of LAIR-1 on CD4 +Foxp3 +Treg cells was positively correlated with the level of TGF-β(r =0.43,P <0.05).Conclusion:The high expression of LAIR-1 on CD4 +Foxp3 +Treg cells and TGF-βin the pe-ripheral blood of OLP patients may be responsible for the pathogenesis of OLP.

ABSTRACT

BACKGROUND: Several different kinds of drugs have been used to treat chronic oral lichen planus (OLP). During the last decade, there have been several reports demonstrating success with levamisole and low dose prednisolone therapy for treating OLP. However, some OLP patients who have underlying diseases such as diabetes, hypertension and malignancy are unable to take steroids. OBJECTIVE: The aim of this study was to evaluate levamisole monotherapy for treating OLP. METHODS: Eleven patients who had OLP were treated with levamisole between 2005 and 2007. The levamisole was administered at a dose 50 mg thrice daily for three consecutive days, but then it was not administered on the following four days. RESULTS: After 2 weeks of treatment, 8 patients reported a partial response, 3 patients reported no response and no patients reported clearance of lesion. After 4 weeks of treatment, 6 patients reported a partial response, 3 patients reported no response and 2 patients reported clearance of lesion. Furthermore, after 3 months of treatment, 3 patients reported a partial response, 3 patients reported no response and 5 patients reported complete clearance of lesion. Clinical improvement was shown in 2 weeks, whilst the mean duration to achieve clearance of lesion was 6.2 weeks. Although 1 patient had mild itching, there were no significant adverse effects. CONCLUSION: Levamisole monotherapy could be a successful and safe treatment option for patients with chronic OLP and who cannot take steroids.
