ABSTRACT

Resumen En este trabajo, reportamos y describimos evidencia de una anomalía en el trago de un individuo de Artibeus lituratus, capturado en los Andes Centrales de Colombia. En junio de 2019, durante un trabajo de campo en el Departamento de Caldas, municipio de Aranzazu, quedó atrapado un individuo de A. lituratus con inusual forma de trago. Este individuo tenía un trago de forma cilíndrica y asimetría. El individuo fue recogido y depositado en el Museo de Historia Natural de la Universidad de Caldas (MHN-UCa). Se realizó una revisión de la literatura para encontrar casos similares utilizando motores de búsqueda, pero no se encontraron informes previos de la anomalía. Además, revisamos el trago de los especímenes en el MHNUCa y ninguno de ellos presentaba malformaciones similares. Concluimos que esta anomalía no se había registrado previamente en Chiroptera.


Abstract In this work, we report and describe evidence of an anomaly in the tragus of an individual of Artibeus lituratus, captured in the Central Andes of Colombia. During field work in the Department of Caldas, municipality of Aranzazu, one individual of A. lituratus with unusual tragus form was trapped. This individual had a tragus with cylindrical shape and asymmetry. The individual was collected and deposited at the Museum of Natural History of the University of Caldas (MHN-UCa). A literature review was conducted to find similar cases using searching engines, but no previous reports of the anomaly were find. Furthermore, we reviewed the tragus of vouchers at the MHN-UCa and none of these had similar malformations. We concluded that this anomaly has not previously registered in Chiroptera.

Introdução: As orelhas proeminentes, popularmente chamadas de "orelhas em abano", representam a deformidade congênita mais comum da orelha externa, atingindo cerca de 5% da população. Métodos: Estudo primário, prospectivo e de intervenção comparando os resultados de pacientes submetidos ao procedimento cirúrgico de correção de orelhas proeminentes por meio da técnica de Converse e de Mustardé, realizado no Serviço de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE). Resultados: Foram avaliados 20 pacientes, 10 por meio da técnica de Converse e 10 por meio de Mustardé, no período de junho de 2016 a dezembro de 2017. Ambos os grupos mostraram diminuição das distâncias mastoideas auriculares ao final do período de observação, variando de 6.67 a 14.6 mm, a depender da técnica cirúrgica e do ponto de avaliação, mas sem significância estatística. Quanto às distâncias mastoideas auriculares médias ao final do período de observação, observou-se diferença de no máximo 6.3mm entre os grupos avaliados, mas sem significância estatística. Em relação a simetria das orelhas dentro do mesmo grupo, o nível máximo de assimetria média nos grupos Mustardé e Converse foi de respectivamente 0.9mm e 0.5mm. A porcentagem da perda de correção das medidas obtidas cirurgicamente ao longo do período de observação em ambos os grupos variaram de 15-19%, no entanto, sem significância estatística. No que se refere as complicações, houve 1 (10%) caso de hematoma no grupo Mustardé. Conclusão: As técnicas de Converse e Mustardé demostraram não ter diferença estatística nos resultados.


Introduction: prominent ears, popularly called "flappy ears," represent the most common congenital deformity of the external ear, affecting approximately 5% of the population. Methods: Primary, prospective and intervention study comparing the results of patients undergoing the surgical procedure to correct prominent ears using the Converse and the Mustardé techniques, performed at the Plastic Surgery Service of the Hospital das Clínicas, Federal University of Pernambuco (HC) -UFPE). Results: Twenty patients were evaluated, 10 with the Converse technique, and 10 with the Mustardé technique, from June 2016 to December 2017. Both groups showed a decrease in auricular mastoid distances at the end of the observation period, ranging from 6.67 to 14.6 mm, depending on the surgical technique and the evaluation point, but without statistical significance. Regarding the average auricular mastoid distances at the end of the observation period, a difference of a maximum of 6.3 mm was observed between the evaluated groups, but without statistical significance. Regarding the symmetry of the ears within the same group, the maximum mean level of asymmetry in the Mustardé and Converse groups was 0.9 mm and 0.5 mm, respectively. However, the percentage of loss of correction of the measures obtained surgically during the observation period in both groups ranged between 15 and 19%, without statistical significance. Regarding complications, there was 1 (10%) case of hematoma in the Mustardé group. Conclusion: Converse and Mustardé techniques did not show statistical differences in the results.

RESUMEN Introducción: Los objetivos de la reconstrucción auricular son mantener la permeabilidad del canal auditivo externo y restaurar la forma general y la proyección. Objetivo: caracterizar un caso clínico de reconstrucción postraumática de una pérdida parcial auricular, enfatizando en la importancia de la secuencia terapéutica empleada. Presentación del caso clínico: Acude a consulta externa un paciente masculino de 26 años. Refiere haber sido tratado en el cuerpo de guardia 10 días atrás, al haber perdido un fragmento de oreja por una mordida en una riña. Al examen físico se constata la existencia de una pérdida parcial irregular de la oreja derecha. Se decide esperar tres semanas, e indicar chequeo preoperatorio. Al cabo de este tiempo se realiza el primer tiempo reconstructivo quirúrgico, con injerto autólogo de cartílago auricular de la oreja izquierda, que fue colocado en forma de "bolsillo" en la región temporal. A las tres semanas del primer tiempo quirúrgico, se realiza, bajo anestesia local, el segundo tiempo reconstructivo con la liberación de la región auricular del "bolsillo" creado, el avance de un colgajo posterior y la colocación de un injerto libre de piel. El paciente refirió una gran satisfacción por los resultados estéticos obtenidos. Principales comentarios: se empleó una secuencia terapéutica ante un defecto traumático parcial del pabellón auricular en sus tercios superior y medio. Este tipo de secuela traumática requirió el empleo de procederes quirúrgicos en varios tiempos operatorios que garantizaran un buen aporte sanguíneo de los tejidos, ausencia de tensión y adecuados resultados estéticos finales(AU)


ABSTRACT Introduction: The aims of auricular reconstruction are to maintain the permeability of the external auditory canal and restore general shape and projection. Objective: Characterize a clinical case of posttraumatic reconstruction of a partial ear loss, highlighting the importance of the therapy sequence followed. Clinical case presentation: A male 26-year-old patient attends outpatient consultation. He reports that he was treated in the emergency service 10 days before upon having lost a section of his ear due to a bite in a fight. Physical examination reveals irregular partial loss of the right ear. It is decided to wait three weeks and indicate preoperative checkup. At the end of that period the first surgical reconstruction session is conducted, with autologous graft of auricular cartilage from the left ear, which was placed in the form of a "pocket" in the temporal region. Three weeks after the first surgical session, the second reconstruction is performed under local anesthesia, with release of the "pocket" from the auricular region, advancement of a posterior flap and placement of a free skin graft. The patient reported great satisfaction with the esthetic results obtained. Main remarks: A therapy sequence was followed in response to a partial traumatic defect of the outer ear in its upper and middle thirds. This type of traumatic sequel required the performance of surgical procedures at several operative moments, ensuring good blood supply to tissues, absence of tension and appropriate final esthetic results(AU)

ABSTRACT

La tos prolongada puede ser un desafío diagnóstico y terapéutico. Presentamos el caso de un enfermo de 62 años con tos seca y quebrantamiento del estado general durante cuatro semanas, seguido de un síndrome febril prolongado cuyo diagnóstico final fue arteritis de la temporal, con confirmación por anatomía patológica. El dolor en los pabellones auriculares al adoptar el decúbito lateral fue otro síntoma interesante. La tos prolongada puede ser una manifestación inicial de la arteritis temporal en adultos mayores.


Prolonged cough could be a diagnostic and therapeutic dilemma. We present here the case of an elder man who suffered from a dry cough and malaise during four weeks, followed by a fever of unknown origin. After many studies and review of symptoms, a biopsy of the temporal artery confirmed a giant cell arteritis. Cough and malaise could be an early symptom of temporal arteritis in elderly patients. Pain in the outer ear on the lateral decubitus could be another interesting symptom in this disease.
