ABSTRACT

Purpose: The purpose of this study is to clarify the factors related to the content of consultation in the cancer nursing outpatient department of Gunma University Hospital. Method: A retrospective survey was conducted with 1084 cases, excluding the unknown cases, from 1308 consultations in FY2019. Survey items included age, gender, consulter, number of uses, treatment status, consultation content, etc. We conducted χ2 tests, and binomial logistic regression analysis between the content of the consultation and the attributes of the user. Results: The treatment-related content was associated with the following factors: 70s or older, family/relatives only, presence of recurrence/metastasis, first use, pre-treatment, urinary organs, uterine/ovary, and unknown primary. The body-related content was associated with the following factors: under treatment, post-treatment, no recurrence/metastasis, and digestive organs. The mental health-related content was associated with 30s or younger, 40s–60s, patient only, and second time or more. The social aspects-related content was associated with the following factors: patient only, family/relatives only, no recurrence/metastasis, and breast. Conclusion: The results reveal that associated factors differ by consultation content. These findings can be used to prepare for consultation based on the relevant associated factors.

ABSTRACT

Introduction: The purpose of this study was to clarify the issues for nurses in neurology departments of medical clinics in building a comprehensive community care system based on the implementation status of regional cooperation and individual support for hospitalized patients. We selected neurology as the subject of our study because patients who visit the neurology department are in the medical clinic for an extended period, and we believe that regional cooperation and individual support are implemented more than in other departments.Methods: A self-administered questionnaire survey was conducted by mail in February 2020 among nurses at 1,052 clinics, which were randomly selected from 2,104 clinics with neurology departments among medical institutions designated for intractable diseases.Results: We collected 174 responses (16.5% collection rate), 164 of which were valid (15.5% valid response rate). The regional cooperation rate was 71.3% and individual support by nurses was implemented at 29.9% of the clinics in the past year. Clinical physicians often sought "patient support in collaboration with related parties" and "patient guidance". Free descriptions included five categories such as [strengthening the awareness and abilities of nurses who are responsible for community healthcare] and [training nurses who can promote comprehensive community care].Conclusion: Regional cooperation and individual support are necessary as nursing activities linked to the functions of family physicians.

ABSTRACT

Introduction: The purpose of this study was to clarify the awareness and difficulties of home care support in outpatient nursing at an advanced acute care hospital.Methods: An anonymous questionnaire survey of outpatient nurses at a university hospital was conducted using a detention method.Results: In total, 195 questionnaires were analyzed (effective response rate: 89.9%). The number of years of outpatient nursing experience was within 1 to 3 years for 40%, being the highest. Regarding the importance of outpatient care in supporting home care, approximately 97% of the subjects answered that they 'always think' or 'sometimes think' that it is important. On the other hand, approximately 50% of subjects answered that they 'always' or 'sometimes' perform home care support. Eight difficulties in outpatient home care support were identified: [insufficient manpower], [shortage of time], [difficulties in understanding which patients need home care support], [lack of knowledge], [difficulties in in-hospital and community collaboration], [support not keeping up with current medical change], [number of outpatients], and [time-consuming support].Conclusion: It is necessary to improve the individual skills of outpatient nurses in order to provide seamless medical care and systematically improve the outpatient nursing system, including streamlining work, and facilitating in-hospital and community collaboration.

ABSTRACT

Objective: To clarify administrators' awareness of outpatient cancer nursing clinics.Methods: We conducted semi-structured interviews of six administrators overseeing outpatient cancer nursing clinics and performed a qualitative, inductive analysis.Results and discussion: In this study, interviews with the administrators led to the identification of ten categories for outpatient cancer nursing clinics such as [the nurses' role in easing the concerns and worries of patients with cancer and their families], [challenges in training and securing outpatient cancer nursing clinic personnel], and [the influence of the passion and confidence of nurses on practice in outpatient cancer nursing clinics]. These interviews revealed that, in practice, outpatient cancer nursing is a useful skill for elucidating the real needs of patients and supporting their decision-making. Coordinated efforts and collaboration among human resources/disciplines may affect day-to-day outpatient cancer nursing clinics.Conclusion: This study revealed that the development of assessment indices to substantiate the accomplishment of outpatient cancer nursing clinics and the training of personnel with a high level of practical skills are two challenges for the expansion of outpatient cancer nursing clinics.

The system consists of the triage queue module, fast track module for prioritized special outpatients and subsequent outpatients, vital signs data recording module, assessment module for high fall risk outpatients, and high risk outpatient transfer module. These functions enable outpatient triage nurses to focus on pre-examination, safety factor identification, and high risk outpatient transfer, thus shortening outpatient queue time and high risk outpatient transfer to emergency, and lower adverse events. With this system, nursing safety management becomes more scientific, standardized and systematic, enabling optimized nursing staff efficiency and fine management of nursing care.

ABSTRACT

Objective To investigate the effect of outpatient nursing intervention in patients with chronic rhinosinusitis(CRS)on the aspects of clinical efficacy, quality of life and nursing satisfaction after medical therapy. Methods The clinical efficacy according to the judgment standard for clinical effect was valued. Medical outcome study short-form 36-items health survey (SF-36) was used to assess the quali?ty of life of the subjects. Satisfaction Questionnaire was used to evaluate the satisfaction scale of patients. Eighty subjects with CRS were randomly divided into the control group (40 cases) and the trial group (40 cases). The trial group was given nursing intervention on basis of usual nursing. In three months after treat?ment, the data and statistics analysis was purely descriptive by SPSS 20.0. Results Clinical efficacy:in the group, nineteen subjects were cured,fourteen subjects were improved and inefficacy occurred in seven subjects. The effective rate was 77.5%(31/40). In the trial group, twenty-six subjects were cured,twelve subjects were improved and inefficacy occurred in two subjects. The effective rate was 95.0%(38/40). There were differences in general data between the two groups (χ2=5.165,P=0.023). Quality of life:the average score of the control group was 58.10±5.04, while the average score of the trial group was 72.22±5.63, the average score of quality of life in the trial group was significantly higher than that in the control group(t=3.08,P=0.025). Nursing satisfaction:there were significant differences in nursing satisfaction between the control group and the trial group (u=-7.048,P=0.000). Conclusions Outpatient nursing intervention can improve the effect of patients with chronic nasal sinusitis after medical therapy, improve the quality of life and improve nursing satisfaction.

ABSTRACT

For scientific and objective evaluation and promotion of the quality of nursing care in Out Patient Department(OPD),a nursing quality indicators system of OPD was established.This system is built based on the principles and methodology of nursing indicators,with reference to various appraisal standards and the hospital-wide nursing appraisal indicators,as well as the outpatient practice.It consisted of the nursing quality supervision indicators of hospital-wide practice,and those made for outpatient practice.It is recommended to determine the contents,baselines and assessment methods of evaluation criteria,to ensure continuous nursing quality improvement,and to increase the quality awareness for outpatient nursing staff standardization and management awareness,promoting the clinic nursing quality effectively.
