ABSTRACT

Objective To investigate the effect of laparoscopic salpingotomy and salpingectomy on ovarian reserve function in patients with tubal pregnancy??Methods From October 2014 to March 2017,201 cases of tubal pregnancy in Jizhong energy Fengfeng Group Hospital were selected and divided into observation group (112 cases) and control group (89 cases) according to different treatment methods??The observation group was treated with laparoscopic salpingectomy, while the control group was treated with laparoscopic salpingectomy??The number of sinus follicles and ovarian volume were measured by color Doppler ultrasound at third days of menstruation on first,third,sixth months after the operation respectively and the ovulation was detected in 6 months after the operation??The intrauterine pregnancy of the two groups was observed one year after operation??Results There was no significant difference in the number and volume of healthy ovarian antral follicles between the two groups in 1,3 and 6 months after operation respectively (all P>0??05)??The number of sinus follicles in the 1,3 and 6 months after the operation were (7??71 ± 3??12), (7??86 ± 2??03) and (7??65 ± 1??89) in observation group respectively,while that in the control group were (4??06 ± 1??05),(3??91 ± 1??14) and (4??89±2??03)respectively??There was no significant difference in the number of sinus follicles in different time groups ( Fintra?group=0??573,P>0??05)??The difference between the groups was statistically significant (F inter?grouP=634??306,P<0??01,Finteraction=463??257,P<0??01)??The ovarian volume of the patients in the 1,3 and 6 months after the operation were (8??49 ± 1??67),(9??01 ± 3??07) and ( 8??51 ± 2??67) cm3 in observation group respectively, and were ( 5??70 ± 2??58), ( 4??81 ± 2??10) and ( 6??03 ± 1??96) cm3 in control group respectively??There was no significant difference in the ovarian volume of the affected side at different time points after operation( Fintra?group=0??671,P>0??05)??The difference between the groups was statistically significant ( Finter?group= 449??106, P< 0??01 ), F interaction= 261??017, P< 0??01 )??The ovulation rate at 6 months ( 82??1%, 92/112 ) and intrauterine pregnancy rate at 1 year ( 71??8%,79/110) in the observation group were higher than those in the control group (56??2%,50/89) and (31??0%,27/87)??There were significant differences between the two groups(χ2=12??601,28??753,all P<0??05)??Conclusion Laparoscopic salpingotomy is better than salpingectomy in the treatment of tubal pregnancy??The ovulation rate and intrauterine pregnancy rate are also superior to salpingectomy??It shows that laparoscopic fenestration can protect the ovarian reserve reserve function and reproductive function of patients??

ABSTRACT

Objective To investigate the impact of different kinds of laparoscopic surgery including conventional blunt elimination and modified acute elimination on sex hormone, antral follicle count and ovarian volume of patients with endometriosis (EMs). Methods 100 patients with EMs were chosen from January 2013 to April 2016 and randomly divided into control group (50 patients) with conventional blunt elimination and observation group (50 patients) with modified acute elimination; and the thickness of elimination lesion, the removal rate of ovary cortex, the thickness of ovarian cortex, the level of serum sex hormones, the AFC number of affected side and the volume of ovary before and after operation of the two groups were compared. Results There was no significant difference in the thickness of lesion elimination, the removal rate and removed thickness of ovarian cortical between the two groups (P > 0.05). The thickness of lesion elimination and the thickness of ovarian cortex in middle position of observation group were significantly lower than that in control group (P 0.05). There was no significant difference in the levels of AFC number of affected side before and after treatment (P > 0.05). The levels of AFC number of affected side in hilus ovarii of control group after treatment were significantly lower than that before treatment (P < 0.05). The volume of ovary of both groups after treatment were significantly lower than that before treatment (P < 0.05). The volume of ovary of observation group after treatment were significantly higher than that in control group (P < 0.05). Conclusion Compared with conventional blunt elimination, modified acute elimination in the treatment of patients with EMs can efficiently shorten the operation time, reduce the surgical trauma degree, speed up the recovery process after operation, regulate the level of FSH and AMH and be helpful to protect the ovarian reserve function.

ABSTRACT

OBJECTIVE: The aim of the present study was to investigate the relationship between ovarian follicle count and volume on ultrasonography and serum hormone levels including the levels of the anti-Mullerian hormone (AMH) and gonadotropin in women with the polycystic ovary syndrome (PCOS). METHODS: A total of 118 Korean women aged 18-35 years who were newly diagnosed with PCOS at a university hospital were included in this study. Serum LH, FSH, and AMH levels were measured in the early follicular phase, and the total antral follicle count (TFC) and the total ovarian volume (TOV) were assessed by ultrasonography. The correlations between serum hormonal parameters and ultrasonography characteristics in women with PCOS were evaluated using Pearson's correlation coefficients and a linear regression analysis. RESULTS: Serum AMH levels were significantly correlated with serum LH levels and LH/FSH ratios, and TFC and TOV were significantly correlated with each other on ultrasonography. Serum AMH and LH levels and the LH/FSH ratio were significantly correlated with TFC. Statistically significant correlations between TOV and the LH level (r=0.208, p=0.024) and the LH/FSH ratio (r=0.237, p=0.010) were observed. However, the serum AMH level was not significantly correlated with the ovarian volume, and this result did not change after adjusting for age and body mass index. CONCLUSION: Serum AMH is not related to the ovarian volume in women with PCOS. My results suggest that serum LH level and the LH/FSH ratio may be more useful than the serum AMH level for representing the status of the ovarian volume in women with PCOS.

ABSTRACT

This study was conducted to establish the normal values of ovarian volumes in this locality. A convenience sample of 141 apparently and gynecologically healthy and non pregnant females was studied using transabdominal sonography. The length; width and anteroposterior dimensions of both the right and left ovaries were measured. Volumes of the ovaries were determined using the prolate ellipsoid formula. The mean ovarian volumes were found to be a 9.5cm3 and 10.cm3 for the right and left ovaries respectively. A mean ovarian volume (Right and left) was 9.9cm3. A mean (right and left) range of 5.3cm3 to 13.9cm3 was also established. ABSTRACT

OBJECTIVE: To evaluate the sensitivity and specificity of ovarian volume using 3D VOCAL and 2D ultrasound, antral follicle count, and age as predictors of menopausal status in pre- and postmenopausal women. METHODS: The subjects of this study were premenopausal (n=38) and postmenopausal (n=23) healthy women aged between 35 and 57 years of age who were interviewed about social, demographic, and medical conditions. These women underwent transvaginal ultrasound using 3D VOCAL and 2D probe to determine ovarian volume. Chi-square test was applied to evaluate the correlation between ovarian volume, antral follicle count, age and menopausal status. Receiver operating Characteristic (ROC) curves were elaborated to evaluate the sensitivity, specificity and positive and negative predictive values. RESULTS: Premenopausal women presented larger ovaries than postmenopausal women (p<0.01). Premenopausal women had a higher number of antral follicles than postmenopausal women (p<0.01). ROC curves showed that antral follicle count and ovarian volume using 3D VOCAL transvaginal probe were more sensitive and specific markers of menopausal status than age or ovarian volume using 2D transvaginal probe. CONCLUSIONS: Ovarian volume and antral follicle count differ according to age in pre- and postmenopausal women. The data suggest that antral follicle count and ovarian volume using 3D VOCAL transvaginal probe are quite useful in predicting menopausal status.

ABSTRACT

OBJECTIVE: To find out whether the determination of the baseline ovarian volume would be a suitable predictor for ovarian response to gonadotropins stimulation and the outcome of IVF-ET. As the ovarian reserve is closely related with the clinical success of IVF-ET, its prediction prior to stimulation is useful in counselling and in adjusting the stimulation protocol for individual patients in ART program. METHODS: A total of 68 infertile women undergoing their first cycle of IVF-ET using a standard regimen of COH (controlled ovarian hyperstimulation) from January, 2000 to June, 2001 were included. Basal levels (Day 2-3) of serum FSH, LH, prolactin and estradiol (E2) were measured. Both ovarian volumes of each woman were measured by TVUS (transvaginal ultrasonography) before stimulation. Based on both ovarian volumes of each woman, the patients were divided into three groups of less than 7 ml, between 7 ml and 11 ml, and more than 11 ml, and their results were analysed for the comparison. RESULTS: Women with larger ovarian volumes have more basal antral follicles (0.0077), more oocytes retrieved (p<0.0007), and higher peak E2 concentration (p<0.0001). No correlations were found between ovarian volumes and age, basal FSH levels, E2 concentrations, and hMG doses and durations. Considering oocytes number as dependent variable, the significant correlations were found in ovarian volumes (p<0.05), basal antral follicle numbers (p<0.0001), and peak E2 concentrations (p<0.0002), but no correlation was found in ages, basal FSH, basal E2, and hMG doses and duration. Ovarian volume was not significantly related with clinical pregnancy rate (p=0.3014). CONCLUSION: It is suggested that the baseline ovarian volumes could be considered as a good marker of ovarian reserve, and it also could be used for the prediction of ovarian response to gonadotropin therapy.
