ABSTRACT

Objective To evaluate the effect of FOCUS- PDCA model in hypoglycemia management during hemodialysis in patients with diabetic nephropathy. Methods Totally 107 patients with diabetic nephropathy were divided into two groups. Patients in the control group received traditional hypoglycemia management, while FOCUS- PDCA model was implemented in the experimental group in hypoglycemia management, including reducing insulin dosage before hemodialysis,injecting 50%glucose and taking hyperglycemia food during hemodialysis,enhancing the monitoring of blood glucose and health education. Compared the incidence of blood glucose levels and hypoglycemia in two groups of patients at different time of dialysis. Results The frequency of hypoglycemia in the experimental group at the beginning of dialysis [ (9.04±2.08) mmol/L ], 2 hours after dialysis[ (5.78±1.82) mmol/L ]and at the end of dialysis[ (6.94±2.04) mmol/L ] were significantly lower than that in the control group[ (8.60±2.02) mmol/L, (5.20±1.62) mmol/L, (4.97±1.05) mmol/L ], (P <0.05), and the total incidence rate of hypoglycemia was 1.6%(61/3 889), which significantly lower than that 4.8%(185/3 816) in the control group, the difference was statistically significant (χ2=5.288, P < 0.05). Conclusions The application of FOCUS- PDCA model in the hypog1ycemia management of patients with diabetic nephropathy can reduced the incidence of hypoglycemia during hemodialysis. It deserves to be widely used in clinical practice to improve the patients′quality of life.

ABSTRACT

OBJECTIVE To explore a management method for the prevention and control of infectious diseases in hospital. METHODS We applied the PDCA (plan,do,check and action) model to establish a management strategy for the prevention and control of infectious diseases for three years. The components of the management model included providing educational program for medical staff,designing and realizing direct network system for infectious disease reporting,monitoring diagnosed infectious diseases ,analyzing their epidemic tendency,guiding medical staff to prevent and control infectious diseases,and strengthening the surveillance,feedback,rewards and punishment. RESULTS The rate of delayed or missed report of infectious diseases was decreased from 5% to 1.1% by monthly check and from 6.8% to 0% by seasonally cross check. Although 46 cases of AIDS,5 cases of caesarean birth with HIV possitive and 1445 cases of hand-foot-mouth disease were admitted in our hospital,No medical staff and other patients was crossly infected. CONCLUSIONS Our way of management based on PDCA model can strengthen the prevention and control of infectious diseases and assure medical quality and patient safety.

ABSTRACT

Objective To explore a management strategy for disinfection supply in hospital. Methods A PDCA (plan, do, check, and action) model was applied to establish a manngement strategy for the disinfection supply, strengthen the education among all staff, promote the competence level of disin-fection supply center, organize joint inspection group for regular union supervision, give feedback promptly and implement strictly. Results By applying the PDCA model, the staff in the disinfection supply center strengthened the consciousness in standard cleanness, disinfection and sterilization, the number of pack-ages prepared by clinic room was reduced, and the supply of sterilized materials were more centralized. Conclusion The management strategy based on PDCA model of disinfection supply in hospital is effec-tive.

ABSTRACT

OBJECTIVE To investigate a management strategy for optimizing antimicrobial usage.METHODS We applied the PDCA(Plan,Do,Check,and Action) model to establish a management strategy for the appropriateness of antimicrobial usage.The components of the management model included providing educational program for medical staff,monitoring pathogens,analyzing the epidemic tendency of antimicrobial-resistant microorganisms,guiding physicians to proper use of antimicorbial treatment,and strengthening the surveillance,feedback and the(discipline).RESULTS These efforts resulted in a universal improvement in the awareness of appropriate(antimicrobial) therapy by medical staff,a significant decrease in the rate of antimicrobial use,and an increase in the numbers of specimens submitted for pathogen identification.Furthermore,the surgical prophylaxis was gradually standardized.CONCLUSIONS Our management strategy based on PDCA model is effective.
