ABSTRACT

Objective: To compare the effectiveness of conservative medical treatment versus minimal invasive surgical techniques like percutaneous catheter drainage (PCD) and percutaneous needle aspiration (PNA) in the management of liver abscess. Material and methods: All patients with liver abscess who were admitted in Surgery from October 2013 to June 2014 were included in this study and were exposed to four different treatment modalities. The patients were first treated with combination of medicine (Option-A). If they failed to respond to this treatment then they were subjected to ultrasound guided aspiration (Option -B). If Option - B failed they were exposed to pig tail catheter placement (Option -C). Final option was surgical drainage (Option - D) if it was rupture liver abscess and if the patient presented with co morbid conditions like septicaemia and peritonitis.PNA was repeated every third day if the cavity size had not declined to 50% of the original for up to three times. Persistence of cavity or of clinical symptoms was considered failure of treatment. Results: Out of 51 patients, 14 patients responded to drug therapy alone. 34 patients required ultrasound guided aspiration and Pig tail catheter placement and 3 patients required open surgical drainage. A combination of drug therapy and ultrasound guided needle aspiration was effective for majority of 83% patients Duration of hospital stay was similar in the two groups. Conclusion: Minimal surgical interventions like PCD and PNA are better than conservative treatment for the management of liver abscesses of size >5 cm, in terms of duration to attain clinical relief and duration for which parenteral antibiotics are needed. Pyogenic liver abscess are less common than amoebic liver abscess. Right lobe of the liver is most commonly involved in both types of abscesses. Radio-imaging techniques like ultrasonography (US) and computerized tomography (CT) are the modalities of choice for investigation purposes. Treatment modalities of these abscesses, first emphasizes on medical treatment, but if it is unsuccessful then only the surgical intervention should be taken up. Laparotomy and Drainage or Laparoscopic Drainage remains the standard of care for ruptured liver abscess.

ABSTRACT

BACKGROUND: Isocitrate dehydrogenase (IDH) catalyzes the oxidative decarboxylation of isocitrate to yield alpha-ketoglutarate (alpha-KG) with production of reduced nicotinamide adenine dinucleotide (NADH). Dysfunctional IDH leads to reduced production of alpha-KG and NADH and increased production of 2-hydroxyglutarate, an oncometabolite. This results in increased oxidative damage and stabilization of hypoxia-inducible factor alpha, causing cells to be prone to tumorigenesis. METHODS: This study investigated IDH mutations in 61 Ewing sarcoma family tumors (ESFTs), using a pentose nucleic acid clamping method and direct sequencing. RESULTS: We identified four cases of ESFTs harboring IDH mutations. The number of IDH1 and IDH2 mutations was equal and the subtype of IDH mutations was variable. Clinicopathologic analysis according to IDH mutation status did not reveal significant results. CONCLUSIONS: This study is the first to report IDH mutations in ESFTs. The results indicate that ESFTs can harbor IDH mutations in previously known hot-spot regions, although their incidence is rare. Further validation with a larger case-based study would establish more reliable and significant data on prevalence rate and the biological significance of IDH mutations in ESFTs.

ABSTRACT

BACKGROUND: Pulmonary carcinoid tumors consisting of typical carcinoid tumors (TC) and atypical carcinoid tumors (AC) are rare, accounting for 2-5% of all lung tumors. TC is considered a low-grade tumor with a rate of distant metastasis up to 12%. In contrast, ACs are more aggressive tumors, displaying a metastatic rate up to 70%. Surgery is the treatment of choice; however, the current treatment outcomes of metastatic lung carcinoids are discouraging. This study aimed to investigate the EGFR mutation using the PNA-mediated clamping method and to provide basic data for using EGFR-TK1 and its clinical implications. MATERIALS AND METHODS: A total of 14 cases that underwent surgery were diagnosed as carcinoid tumors and pathologically classified as TC and AC. The paraffin-embedded tissues were analyzed for EGFR mutations using the PNA-mediated PCR clamping technique. The mutant type was noted in the cases with a DeltaCt greater than 2.0. RESULT: Of 14 cases, eight were AC and six cases were TC. No known EGFR mutation was detected with a DeltaCt less than 2.0. CONCLUSION: The EGFR genotype determined using the PNA-mediated PCR clamping method was wild-type in all pulmonary carcinoid tumors. Therefore, the application of EGFR-TK1 is limited in pulmonary carcinoid tumors.

ABSTRACT

OBJECTIVES: Clinical natural orifice surgery has been applied for abdominal surgery in recent years. Despite potential advantages of transcolonic NOTES for colorectal diseases, it was since now not yet clinically applied. The study describes the first successful human application of transcolonic NOTES in the literature, in a new transrectal TME procedure for rectal cancer. METHODS: IRB approval was obtained at the institution for the study, and the patient signed informed consent. In a 54yrs-old male patient with a rectal adenocarcinoma, total mesorectal resection and rectosigmoidectomy with lymphadenectomy was performed using a posterior transcolonic access 3cm from the anal verge. Mesorectal dissection was achieved using a flexible colonoscope and endoscopic instrumentation and laparoscopic assistance. The specimen was extracted transanally, and transorificial anastomosis was performed, with proximal stoma. RESULTS: Operative time was 350 min, no intraoperative complications occurred. The postoperative course was uneventful, patient was discharged after 6 days. CONCLUSION: Successful first human report on Transcolonic NOTES potentially brings new fronteers and applications for minimally invasive surgery. The treatment of colorectal diseases through a flexible Perirectal NOTES Access (PNA) is a promising new approach besides existing laparoscopic and open surgery to improve patient care.

ABSTRACT

Introduction: The assessment of food consumption, particularly of protein intake, constitutes an important tool in the prevention, treatment, and monitoringof malnutrition in hemodialysis patients. Food record and protein equivalent of nitrogen appearance (PNA) are the most commonly used methods for suchpurpose in the clinical practice. Aim: To evaluate the agreement between food record and PNA for the assessment of protein intake in hemodialysis patients.Subjects and Methods: Thirty-nine hemodialysis patients (67% male, 46.8 ± 14.7 years old, BMI 23.5 ± 4.6 kg/m2) were studied. The exclusion criteriawere catabolic illnesses, residual renal function and use of glucocorticoids. Food intake was recorded 3 days and PNA was calculated as proposed by theNKF-DOQI. Results: The protein intake assessed by food record was lower than that obtained by PNA (56.3 ± 23.4 vs 67.9 ± 23.9 g/day, respectively;P≤0.001). The methods were well correlated (r=0.54; P<0.001), however, a large individual variability was observed (-51.7 to +28.5 g/day). The protein intakeobtained by both methods correlated positively with BMI (food record: r=0.41 and PNA: r=0.80; P<0.05). The difference between the methods (food record,PNA) correlated negatively with BMI (r=-0.49; P<0.001). The analysis by subgroups of BMI showed that the food record method underestimated proteinintake in relation to PNA only in patients with BMI ≥25kg/m2. The multiple linear regression analysis pointed to BMI ≥25kg/m2 as a factor independentlyassociated with variations between the methods. Conclusion: The food record method showed lower values of protein in comparison with the PNA methodin hemodialysis patients, and overweight/obesity status was an important determinant of these variations.

ABSTRACT

OBJECTIVE: In this study, we evaluated whether the hypo-osmotic swelling test (HOST) can be used as a vital marker in combination with peanut agglutinin (PNA) - labeling in fresh and cryopreserved spermatozoa. MATERIALS AND METHODS: Human sperm populations were exposed to a hypo-osmotic medium for 60 minutes, and then incubated in a 1 µg/mL solution of the fluorescent dye Hoescht 33258 (H33258) for 10 minutes. Excess stain was removed by washing in phosphate-buffered saline (PBS) solution, and the pellet was resuspended in 100 µL of culture medium. Twenty microliters of this solution were subsequently smeared on a microscope slide, and fixed in ice-cold methanol to permeabilize the sperm membranes. The fixed smears were finally incubated in a 40-µg/mL FITC-PNA solution for 20 minutes. Simultaneous assessment of acrosome and viability scores was done in a fluorescent microscope equipped with appropriate filters and phase contrast illumination. The same slide was examined for FITC-PNA labeling, tail swelling, and for Hoechst-33258 staining by interchanging the filters and phase contrast optics. RESULTS: In fresh specimens, HOST was found to provide viability assessments comparable to those obtained using the H33258 method (r = 0.95). However, the results of HOST and H33258 were not correlated in cryopreserved specimens (r = 0.22). There was no alteration of PNA-labeling due to the HOST or H33258. CONCLUSIONS: FITC-PNA labeling in conjunction with the visualization of the morphological change induced by exposure to hypo-osmotic solution provides a simple but effective method for establishing the state of acrosomal membrane and viability in fresh human spermatozoa, but this technique is not reliable for cryopreserved ones.

ABSTRACT

The management of children on PD requires constant monitoring of the dose of dialysis and nutritional support in order to maximise growth and development in the uraemic child. The measurement of a series of dialytic and nutritional variables allow the adjustment of treatment to the childs needs. Objectives: 1) to measure the different variables in child undergoing PD, 2) calculate the dialysis dose (Kt/V), the protein catabolic rate (PCR), the urea equivalent of nitrogen appearence (PNA), the nitrogen balance, the peritoneal equilibrium test (PET) and, 3) to determine the correlation between them. Patients and Methods: A prospective study of 15 children undergoing chronic PD, with 59 serial measurements in a 6 month period. Monthly measurements of weight/height, body surface area, urine volume, dialysis volume in 24 hrs, and the values of creatine, protein, albumin and urea in plasma, urine and dialysate, and in plasma the acid-base and electrolytes. A diet containing proteins according to RDA was given. Kt/V urea peritoneal and residual, PCR, PNA and nitrogen balance were measured monthly, PET every 6 months. Results: The mean Kt/V residual and total was 1.49 ± 1 and 3.41 ± 0.86 respectively. A positive correlation between Kt/v and residual creatine clearence was found (p < 0.05). There were no significant differences between the PET for creatinin (4th hour) or glucose D4/D0 at the beginning and end of the study, 0.78 ± 0.01 and 0.74 ± 0.13 for creatinine and 0.35 ± 0.11 and 0.34 ± 0.08 respectively. Dietary protein intake was 3.32 ± 1.05, protein catabolism 1.32 ± 0.47 gm/kg/day, with a nitrogen balance of 2 gm/kg/day and a PNAn of 0.94 ± 0.33. Total and residual Kt/V was correlated with the PNAn, and a negative correlation between protein intake and plama bicarbonate was found. Conclusions: The variables studied show a positive protein balance and a Kt/V greater than the recommended for adults, suggesting that bigger dialysis doses results in a better nutritional status. The negative correlation between plasma bicarbonate and protein intake suggests a relation between protein intake and acidosis, of which needs to be carefully monitored.

ABSTRACT

Lectins are glycoproteins that specifically bind carbohydrate structures and may participate in the biodefense mechanisms of fish. In this study, the binding of three lectins, Dolichos biflorus agglutinin (DBA), soybean agglutinin (SBA), Bandeiraea simplicifolia BS-1 (isolectin B4), Triticum vulgaris (WGA), Arachis hypogaea (PNA) and Ulex europaeus (UEA-I) were studied in the gill, liver, intestine, kidney, heart, and spleen of the flat fish Paralichthys olivaceus. DBA was detected in intestinal mucous cells, as well as in gill epithelial and mucous cells. It was weakly detected in renal tubule epithelial cells and in bile duct epithelial cells. The strong SBA staining was seen in the intestinal club cells, in bile duct epithelial cells and renal tubule epithelial cells. There were intense positive reactions for isolectin B4 in gill epithelial and mucous cells, and the strong isolectin B4 staining was seen in epithelial cells of the bile duct and intestine. The strong WGA staining was seen in the gill mucosal cells, sinusoid, renal tubule epithelial cells and mucosal cells of the intestine. UEA-I was detected in the gill epithelial and mucosal cells, bile duct epithelial cells and renal tubular epithelial cells. These results suggest that the six lectins examined were localized in the covering epithelia of the various organs of the flat fish and they may participate in the biodefense mechanism of the intra body surface in which is exposed to various antigens.

ABSTRACT

In order to explore expression of the PNA receptor in early myocardial infarction, immunohistochemical technique was performed (S-P method). The expression of the PNA receptor occured in the early acute cardiac ischemic area induced experimentally by ligation of left coronary arteries of 32 SD rats. The following changes on myocardial cell membranes at the ischemical areas were found. Positive staining of PNA could be observed in ischemic area at 15min after ischemia, and it increased along with the prolongation of ischemic period. It became the strongest after is chemic 2h and then decreased. This may be of some value in forensic medicine practice.

ABSTRACT

Molecular technologies are sensitive, fast, and cheap in the study on microbial ecology. However, these methods do not provide information about morphology, number, and spatial distribution of the microorganisms. In contrast, Fluorescence in situ hybridization (FISH) combines the precision of molecular biology with the visual information from microscope, and permits visualization and identification of individual miciobial cells within their natural habitat. FISH not only allows the detection of slow growing microorganisms, but also of yet-to-be cultured. It is useful for many applications in diagnosis and assessment of the population structure of complex microbial communities, and is a powerful tool for molecular ecology studies in microbiology, In this paper, major techniques and progresses of FTSH were described. Its application in microbial ecology, as well as problems , pitfalls, and perspectives of FISH are discussed.

ABSTRACT

Histiocytosis X is characterized by aggregates of Langerhans cells with other inflammatory cells. These Langerhans cells are antigen-presenting cells to T lymphocytes and identified by characteristic morphology, ultrastructural demonstration of Birbeck granules and immunologic reactivity with OKT-6 and HLA-DR antibodies. In this report, the tumor arising in a 2-years-old baby was examined byimmunostaining with several macrophage/dendritic cell markers. The main tumor cells showed cytoplasmic and nuclear staining with S-100 protein and ring-like surface and paranuclear staining with PNA. However, they were negative for follicular dendritic cell marker CD21, macrophage markers lysozyme, Mac 387, alpha-1 antitrypsin and CD68, and interdigitating reticulum cell marker ID4 and ID5. These observations demonstrate the usefulness of S-100 protein and PNA for the identification of Langerhans cells in paraffin-embedded tissue.

ABSTRACT

Recently, extensive uses of lectins as cytochemical markers have made of studies for various epithelial and nonepithelial neoplasia, however, investigations of epithelial cell surface of transitional cell carcinomas of the urinary bladder have been few. Thus, the atuhors performed a study of PNA binding in the authors performed a study of PNA binding in transitional cell carcinomas with comparision with that in normal mucosa of the urinary bladder to allow more accurate diagnosis and histological grade or degree of differentiation. The results of this study are as follows: 1) PNA shows negative reactions on all ten normal mucosae of the urinary bladder but positive staining at the glycocalyx of umbrellar cells in two cases. 2) PNA shows negative reactions on all four cases of von Brun'n nests and cystitis cystica. 3) PNA shows positive reactions on thirty (50%) of total sixty-one cases of transitional cell carcinomas and reveals two (20%), nine (41%), eleven (55%) and eight (88%) cases in grade I, II, III and IV, respectively. 4) PNA shows positive reactions on the intracytoplasm and/or degree of PNA binding activity in grade I to IV transitional cell carcinomas is not statistically significantly different (p>0.05). In summary, PNA did not react with normal nucosa and metaplastic lesions such as von Brunn's nests and cystitis cystica, however, it reacted with 50% (30/61 cases) of transitional cell carcinoma and its positivity is significantly increased with gradings of transitional cell carcinomas (p<0.05).

ABSTRACT

It was found that the suppressive activity of PNA~+ cells was mediated by asoluble factor which can be abrogated in the supernatant by pretreating with anti-Thy-1 serum plus complement. Thus, the suppressor factor here mentioned must becontributed to being a product of T cell line, putatively being defined as T suppres-sor factor(TsF).Physicochemical analysis revealed that this factor was heat-stableat 56℃ and not affected by acidification to pH 2 or alkalization to pH 11,but inac-tiviated at 100℃ or after 2-Mercaptotoethanol reduction. As indicated in enzymaticdigestion test, this suppressor factor was sensitive to trypsin but resistant to RNase. Therefore,it may be considered to be one kind of protein(or polypeptide)at least inpart. The approximate molecular weight of which was 50 KD,as deduced by Sepha-dex G 100 gel filtration.Specific oligosaccharide blocking test confirmed the factthat the suppressive effect on LxGvHR was able to be blocked by N-acetyl-glucosa-mine.And also, the suppression on PFC response was able to be eliminated by L-rha-mnose.It was still found that both of the cell synthesis and/or the release of the suppre-ssive factor was able to be blocked by adding Sodium Azide or Cytochalasin B tothe media initially. The in vitro induction experiment indicated that highly activesuppressive factor can be harvested from the supernatant in normal mice derivedPNA~+ cells briefly exposed to cell-free tumor cultures. After the factor was absor-bed onto relevant viable tumor cells, the residual suppressive activity was found tobe diminished. The suppressor factor-mediated suppression was MHC(H-2)restricted,for it cannot exert its action on target cells bearing incompatible H-2 products. As faras the immunological features concerned above, there must exist tumor-specific TsFwith antigen binding site as well as H-2 determinants.

ABSTRACT

Analysis of ribosomes and the post ribosomal supernatant fraction of actively growing cells of Thermomyces lanuginosus showed the presence of free 5 S RNA in the supernatant fraction. This 5 S RNA was identical to the ribosomal 5 S RNA in its electrophoretic mobility on 10% Polyacrylamide gel and in its base composition. 5 S RNA from both the sources gave evidence for the presence of diphosphate at the 5’ end. Most of the 5 S RNA that appeared in the cytoplasm was that transported from the nucleus during the isolation. This could be prevented by the use of a hexylene glycol-HEPES buffer.

ABSTRACT

The change of PNA receptors on the surface of thymocytes in protein deficient mice was investigated. After the mice were fed with protein deficient diet, the body weight continuously declined and the thymus gradually a-trophied. Qualitative test of FITC-PNA indicated the number of PNA positive cells was reduced and the fluorescent antibody reaction on the surface of PNA positive cells weakened. Quantitative test of FITC-PNA showed the amount of FITC-PNA coupled on the surface of PNA positive cells was mark- edly decreased as compared with the cotrol (P
