ABSTRACT

Objective To explore the clinical application value of minimally invasive endoscopic visual artificial abortion operation system in Painless abortion. Methods 250 cases early pregnant women who underwent minimally invasive endoscopic visual artificial abortion operation system implementation of Painless abortion from June 2015 to December 2015 were selected as visual painless abortion group, while Two hundred and fifty cases early pregnant women who demanded routine painless abortion during the same period were selected as routine painless abortion group. The operation time, the amount bleeding, the villus integrity, the number of cases of once again clear palace operation , the vaginal bleeding time after operation, and the first menstruation recovery time after operation among the pregnant women in the two groups were analyzed. Results There was statistically significant difference in the operation time, amount bleeding, villus integrity, the number of cases of once again clear palace operation , vaginal bleeding time after operation, and the first menstruation recovery time after operation between the two groups ( P<0. 05) . The Observation index of the pregnant women in visual painless abortion group was Obviously superior of that in routine induced abortion group . Conclusion Minimally invasive endoscopic visual artificial abortion operation system implementation of Painless abortion is worthy to be popularized and applied in clinic.

ABSTRACT

Objective To investigate the operation effect of cardiopulmonary suppository before painless artificial abortion. Methods 300 patients with painless artificial abortion were enrolled in our hospital from August 2013 to October 2016. They were divided into control group and observation group according to randomized and double-blind way. The patients in the control group were not given any drugs before operation. The patients in the observation group were placed preoperatively in the posterior vagina. The cervical relaxation, expansion time, intraoperative blood loss, analgesic effect and induced abortion syndrome Happening were observed in the two groups. Results The cervical relaxation rate in the observation group was 132/150 (88.00%),which was significantly better than that of the control group 83/150 (55.33%), the difference was statistically significant (P<0.05). The effective rate of analgesia in the observation group was 94.67%, significantly higher than that in the control group 70% . In the observation group, the time of dilatation was (31.6±3.59) s, intraoperative blood loss was (8.9±3.12) mL, and the incidence of induced abortion syndrome was 2.67%, which were significantly better than those in the control group, the time of dilatation (53.2±4.21) s, intraoperative blood loss (14.9±3.96) mL, abortion syndrome occurred 25.33% , the difference was statistically significant (P<0.05). Conclusion The painless artificial abortion before the use of preoperative suppository, can effectively expand the patient's cervix, reduce the operation time, and effectively reduce the amount of intraoperative blood loss, painless abortion is an effective relief of patient suffering drug.

ABSTRACT

OBJECTIVE:To study the adhibition of misoprostol combined with palace cavity attraction tube in the treatment of painless abortion after cesarean section. METHODS:214 patients who received painless abortion after cesarean section were ran-domly divided into group A(71 case),group B(71 case)and group C(72 case). After all patients were given conventional intrave-nous anesthesia,Group A was given misoprostol 0.6 mg for oral administration in preoperative 2 h. Group B was given Misoprostol tablet 0.2 mg abrasive powder put in posterior fornix in preoperative 3 h. Group C did not give any drugs. All the groups used dis-posable suction curettage tube. The efficacies of sedation and cervical dilatation, operation time, intraoperative blood loss, recovery time, dosage of propofol and incidences of complications and adverse reactions in the 3 groups were observed. RESULTS:The op-eration time, intraoperative blood loss in group A were lower than group B and lower than group C,the differences were statistical-ly significant(P0.05). The total effective rates of sedation and cervical dilatation in group A and B were significantly higher than group C,and the difference was statistical-ly significant(P0.05). The incidence of compli-cations in group A was lower than group B and lower than group C,and the difference was statistically significant (P0.05). CONCLUSIONS:The efficien-cy and safety of misoprostol for oral administration or medicated vaginal suppository combined with palace cavity attraction tube in the treatment of painless abortion after cesarean section is similar, but oral administration can reduce the dosage of anesthetics,op-eration time and smaller intraoperative blood loss volume.

ABSTRACT

OBJECTIVE:To compare mental state and drug safety of puerperant underwent propofol and etomidate painless abortion after regain of consciousness. METHODS:102 puerperant underwent painless abortion were selected and randomly divided into control group and trial groups with 51 cases in each group. The control group received Propofol injection 2 mg/kg intravenous-ly,and the trial group received Etomidate fat emulsion injection 0.2 mg/kg intravenously. The changes of patient’s mental state were evaluated with Positive and Negative Emotions Scale (PANAS) before anesthesia and after regain of consciousness. The changes of HR,BP,MAP and SpO2 were observed and compared between 2 groups before anesthesia and 1,2,3 and 5 min after anesthesia. The anesthetic effect and ADR were compared between 2 groups. RESULTS:Compared to before anesthesia, positive mood scores of the trial group were increased slightly after regain of consciousness, while that of the control group were increased significantly;there was statistical significance before anesthesia and after regain of consciousness (t=9.023,P0.05). The total effective rate of anesthesia in control group was 80.4%,while that of trial group was 78.4%;there was no statistical signifi-cance between 2 groups (χ2=1.273,P>0.05). The incidence of nausea, vomiting, dizziness and other ADR were similar in 2 groups, but the incidence of fantasy(39.2%)in control group was significantly higher in trial group(11.8%);there was statistical significance between 2 groups(χ2=10.120,P<0.05). CONCLUSIONS:Propofol and etomidate have good stability on the respira-tory and circulatory system, anesthetic effects are correspond to the incidence of ADR. Etomidate has a better safety. But the psy-chological status of patients in the propofol group is preceding than etomidate group.

ABSTRACT

To explore the analgesic effect of propofol combine with dezocine after painless abortion and the influence on stress hormone and hemodynamics of the patients. Methods:Totally 126 cases of patients with painless abortion in our hospital from March 2013 to March 2014 were randomly divided into the observation group and the control group with 63 ones in each. The observa-tion group was anesthetized with propofol combine with dezocine, while the control group was anesthetized only by propofol. The dosage of propofol, anesthesia effect and postoperative recovery of the patients in the two groups were compared, and the level of stress hor-mones and hemodynamic parameters before and after the surgery were also compared. Results: Compared with that in the control group, the dosage of dezocine and the score of VAS after the surgery was significantly reduced in the observation group with obviously better anesthesia effect, and the differences had statistical significance (P<0. 05). The level of Cor, NE and ACTH was significantly improved, and the level of A-I and A-II was reduced significantly in the observation group, and compared with that in the control group, the differences showed statistical significance(P<0. 05). Compared with those that in the control group, the levels of SBP, DBP and SpO2 were improved significantly and the level of HR was reduced significantly in the observation group during and after the surgery, and the differences showed statistical significance (P<0. 05). The incidence of adverse drug reactions in the observation group was significantly lower than that in the control group, and the difference had statistical significance (P<0. 05). Conclusion:Propofol combine with dezocine for the surgery of painless abortion can reduce the dosage of dezocine, improve the anesthesia effect and reduce the harem pain with lower stress hormone fluctuation, hemodynamic effect and incidence of adverse reactions.
