ABSTRACT

RESUMEN Antecedentes: la falta de pene causa trastornos físicos y psicológicos graves. Su reconstrucción es compleja porque comprende restituir aspectos morfológicos y funcionales. El colgajo radial microquirúrgico (CRM) es una de las mejores opciones para la reconstrucción fálica (RF). Objetivo: describir los resultados de una serie de pacientes con RF mediante CRM. Material y métodos: trabajo observacional descriptivo a través de la revisión de los registros de pacientes a quienes se les realizó RF con CRM entre julio de 2014 y julio de 2017, no relacionados con disforia de género. Resultados: tres pacientes, de 27, 36 y 22 años, fueron reconstruidos, respectivamente, por amputación traumática, secuela de tratamiento oncológico y por malformación congénita. No se presentaron necrosis de los colgajos. La sensibilidad somática fue efectiva en los tres casos y la erógena en uno. Todos los pacientes refirieron satisfacción general con el procedimiento. Conclusión: el CRM, realizado por un equipo multidisciplinario, permitió una RF satisfactoria. La reconstrucción de la uretra debe ser decidida


ABSTRACT Background: The absence of the penis causes serious physical limitations and psychosocial distress. Phallic reconstruction (PR) is a complex procedure because it involves the restoration of morphological and functional aspects. The radial forearm flap (RFF) is one of the best options for PR. Objective: The aim of this study was to describe the results of a series of patients undergoing PR using RFF. Material and methods: We conducted a descriptive observational study by reviewing the records of patients who underwent PR with RFF not related to gender dysphoria between July 2014 and July 2017. Results: Three patients aged 27, 36, and 22 years underwent reconstruction for traumatic amputation, oncologic treatment sequelae, and congenital defect, respectively. None of the patients developed flap necrosis. Somatic sensation was effective in all three cases and erogenous sensation in one. All patients reported overall satisfaction with the procedure. Conclusion: The use of RFF for PR by a multidisciplinary team produced satisfactory and uncomplicated results. Urethral reconstruction should be decided depending on each patient.

ABSTRACT

Objective:To investigate the repair method and curative effect of iatrogenic penis skin necrosis of varying degrees.Methods:From June 2016 to December 2018, a total of 20 patients with varying degrees of iatrogenic penis skin necrosis were treated in our center. According to the area of the necrotic skin of penis, different treatment methods were selected: 2 patients with change dressing; 9 patients with relaxation suture; 7 patients with scrotal skin flap based on one side anterior scrotal artery; 2 patients with penile embedded in scrotum and secondary repair.Results:All patients were cured, there was no infection and flap necrosis. All of the patients were satisfied with the appearance of the penis, the flaps of anterior scrotal artery flap and scrotal pedicle embedding the penis survived well and the penis erectile function was not affected during the follow-up for 6 to 12 months.Conclusions:For patients of varying degrees of iatrogenic penis skin necrosis, individualized treatment should be adopted, which can effectively repair the penile defects and achieve satisfactory effect.

ABSTRACT

Objective To evaluate the efficacy of medial femoral tubed skin flap combined with penile elongation for repairing long-segment penile defects.Methods From June 2009 to January 2015,13 cases of long-segment penile defects were treated by medial femoral tubed skin flap with pe nile elongation.Results All cases were followed up for 0.5 to 5.0 years.All flaps survived and all patients had the minor scar in donor areas,with good appearance and feeling of penis.The length of pe nis in flaccid state was 6.3±1.6 cm.The length of penis in erection state was 8.5±1.2 cm;The circumference of penis was 5.3±1.9 cm.1.0 cm × 0.7 cm of necrosis at distal parts occurred in one case early after operation,and healed after dressing changes.Two cases had tumid appearance which influenced penile appearance and sexual intercourse.It was improved by local liposuction.No feeling and e rectile dysfunction developed during the following-up.Except 5 cases who did not married,the other married patients and their partners were satisfied or quite satisfied with sexual intercourse activity.Conclusions Medial femoral tubed skin flap combined with penile elongation is a good method for the treatment of long-segment penile defects.The prosthetic penis could have suitable length,good ap pearance,good feeling and erectile function.
