ABSTRACT

Objective To study the effect of sitz bath with 1.6%concentrated brine solution and metronidazole on perianal infection in acute leukemia patients. Methods Seventy-six acute leukemia patients admitted from June 2010 to December 2014 with perianal infections were randomly divided into the control group and the observation group in equal number. The two groups were given routine nursing care and symptomatic supportive treatment . Apart from the basic nursing , the control group was treated with sitz bath with 10 min 1 ∶ 5000 potassium permanganate. The observation group was treated with sitz bath with 1.6% brine solution and 0.1%metronidazole two times every day and another one if defecation was done. The perianal infections of two groups were compared in 7 days. Result The total effectiveness in the observation group was significantly better than that of the control group (P < 0.05). Conclusions Sits bath with 1.6% brine solution and metronidazole two times a day is more effective than with 1 ∶ 5000 potassium permanganate. It is economic and convenient, safe and effective for the treatment of perianal infections and therefore worth popularizing in clinical application.

ABSTRACT

Objective To investigate the clinical features and therapy of fournier's gangrene.Methods The clinical data of 12 patients with fournier's gangrene who were treated at the Third Affiliated Hospital of Nanjing University of Chinese Medicine from January 2010 to February 2012 were retrospectively analyzed.Eleven patients had perianal abscess and 1 patient had perianal foreign body,most of the patients were presented with perianal pain,fever (> 38.5 ℃) and tachycardia.The mean interval between the onset of symptoms and admission to the hospital was 8 days (range,3-20 dyas).Seven patients were complicated with diabetes.Only 2 patients were administered hypoglycemic agents,but the effects were poor.Six patients were complicated with hypertension.One patient had the history of resection of sigmoid colon cancer 2 months before operation.Debridement and drainage were applied to all the patients after examination.Antimicrobial therapy applied to all the patients according to the results of drug sensitivity test.The incisions were washed by hydrogen peroxide solution postoperatively.Redebridement was applied if the disease was progressed.All patients were followed up via out-patient examination and phone call till June 2012.Results Results of clinical features:Perianal tissues necrotized,with the color of black.The scrotum or labia majora of 8 patients were invaded by the necrotic tissues.Results of laboratory test:the white blood cell count was (3.8-27.6) × 109/L,the white blood cell count of 6 patients was above 10 × 109/L,3 was above 20 × 109/L.The ratio of neutrophil granulocytes was 0.61-0.93.Four patients received single debridement,5 received redebridement,and 3 patients received debridement for more than 2 times.Two patients had sapremia and infectious shock,and they received treatment for 3 days and 4 days at the intensive care unit.Results of bacterial culture:6 patients were infected by the escherichia coli,4 by klebsiella pnenmoniae,and 1 by pseudomonas aeruginosa.The mean duration of hospital stay was (25± 14)days (range,1-49 days).All patients were followed up for 4-29 months,1 patient was discharged at postoperative day 2,and the prognosis of the 11 patients was good,no mortality was observed.Conclusions The clinical features of fournier's gangrene include severe pain and rapid spread of infections caused by necrosis of perianal subcutaneous tissues.Diabetes might be the risk factor of fournier's gangrene.Early diagnosis and prompt and aggressive surgical debridement are critical for improving survival.

ABSTRACT

Objective To investigate the clinical efficacy of nursing interventions on leukemia patients with fever caused by perianal infection during bone marrow suppression stage.Methods 78 leukemia patients during bone marrow suppression stage were divided into the observation group and the control group.Pertinent integrated care intervention was taken in the observation group,while routine nursing care was taken in the control group.The infection rate was compared between the two groups.Results The rate of perianal infection in the control group was 30.77%,higher than 5.13% of the intervention group.Conclusions Leukemia patients during bone marrow suppression stage have low immunity and are easily to occur perianal infection.Pertinent integrated care intervention can obviously decrease infection rate and improve their life quality.

Describir las características de la infección por virus de papiloma humano anal y perianal asociada a la infección por genital por el mismo virus. Se seleccionaron 65 pacientes con virus de papiloma humano genital, a quienes se les realizó citología, identificación viral por reacción en cadena de polimerasa y colposcopia de región genital, ano y periano. En el Servicio de Ginecología de la Maternidad "Concepción Palacios". Se observó una frecuencia de detección del virus en ano y periano de 31,3 por ciento. En 19,67 por ciento de las pacientes hubo concordancia entre la infección en genitales y la de ano y región perianal (P > 0,397). Entre los genotipos virales hubo concordancia del 33,74 por ciento (P= 0,0053), esta correlación fue mayor para el virus 6. Fueron evaluables 38 citologías anales y perianales (59,4 por ciento) y ninguna diagnosticó anormalidades. Entre estas citologías, al hacer la reacción en cadena de polimerasa, 15 resultaron positivas para virus de papiloma humano, 21 negativas y 2 insatisfactorias. Las lesiones más frecuentes fueron subclínicas. La distribución del resultado de la anoscopia, refleja que la normalidad es lo más frecuente (67,2 por ciento). El riesgo de infección por virus de papiloma humano en ano y periano se incrementa en pacientes con infección genital. Consideramos que es importante extender la evaluación ginecológica a la región anal y perianal a pesar de las limitaciones del uso de la citología y la colposcopia


To describe the characteristics of the anal and perianal human papillomavirus infection associated with genital infection by the same virus. We selected 65 patients, with genital infection by human papillomavirus, who was made them cytology, polymerase chain reaction and colposcopy in genital region, anus and periano. Servicio de Ginecologia de la Maternidad "Concepción Palacios" The frequency of detection of papillomavirus in anus and periano was 31.3 percent. There was consistency between the genital papillomavirus infection and the anus and region parianal in 19.67 percent (P > 0,397) and between viral genotypes in 33.74 percent (P = 0,0053), this correlation was increased to virus 6. They were 38 evaluable Papanicolaou test anal and perianal (59.4 percent) and none diagnosed abnormalities. Among these, 15 were polymerase chain reaction positive for papillomavirus, 21 negative and 2 unsatisfactory. The most common lesions were subclinical. The distribution of the anoscopia result reflects normal is most often (67.2 percent). The risk of HPV infection in anus and periano is increased in patients with genital infection. We believe it is important to extend the gynecologic evaluation to the anal and perianal region despite the limitations of the use of Papanicolaou test and the colposcopy
