ABSTRACT

El hidradenoma papilífero es una tumoración benigna, rara, generalmente localizada en la región vulvar o perineal siendo la región perianal una localización poco frecuente. Su asociación con enfermedades de transmisión sexual y el carcinoma ductal es controvertida; es importante realizar el estudio histopatológico de cualquier tumoración en esta zona. Se reporta el caso de una paciente de 49 años de edad, con un nódulo en la región perianal de 2 años de evolución; el estudio histopatológico confirmó el diagnóstico de hidradenoma papilífero. (AU)


The papilliferous hidradenoma is a benign, rare tumor, usually located in the vulvar or perineal region and the perianal region is a rare site. Its association with sexually transmitted diseases and ductal carcinoma is controversial. It is important to perform the histopathological study of any tumor in this area. The case of a 49-year-old patient with a nodule in the perianal region of 2 years of evolution is reported. The histopathological study confirmed the diagnosis of papilliferous hidradenoma.

ABSTRACT

Lipomas are ubiquitous lesions. Most of the lipomas have a fibrous capsule and are divided into lobules by delicate septae. On gross examination and histological analysis, lipomas resemble mature fatty tissue. They are more compact on the cut surface than normal adipose tissue and the lobules are smaller on histological analysis. There is undoubtedly some physiologic difference between fat of lipoma and normal fat. Perianal lipomas are rare and may have deeper or interspincteric extensions. Preoperative evaluation to rule it out is a necessary part of the work up for any perianal lipoma. The case report is of a soft swelling on left side in the perianal region, which was investigated to rule out nature of the lesion or deeper and interspincteric extension. Case was managed by complete enucleation of the lipomatous swelling. Gross and microscopic examination of the surgical specimen revealed a perianal pedunculated lipoma.

ABSTRACT

Tuberculosis remains a public health problem and its rare extrapulmonary forms persist with late diagnosis, which delays the diagnosis and favors the dissemination of the disease. The perianal manifestation occurs in approximately 0.7% of tuberculosis cases and usually manifests as fistulizing disease, but may also present with ulcerated lesions, with necrotic and quite painful clinical picture. Initial treatment should be carried out aimed at controlling perineal and perianal sepsis, with subsequent establishment of specific drug therapy provided by the Health Ministry. (AU)


A tuberculose ainda hoje se mantém como um problema de saúde pública e suas raras formas extrapulmonares persistem com diagnóstico tardio, o que retarda o diagnóstico e propicia a propagação do bacilo. A manifestação perianal ocorre em cerca de 0,7% dos casos de tuberculose e manifesta-se geralmente como doença fistulizante, mas que pode também apresentar-se com lesão ulcerada, de fundo necrótico e bastante dolorosa. O tratamento inicial deve ser realizado objetivando o controle da sepse perineal e perianal com posterior instituição de terapia medicamentosa específica fornecida pelo ministério da saúde. (AU)

ABSTRACT

Extra mammary Paget disease is rare and mostly affects perianal and vulvar zones, in patients of both sexes and from the sixth decade of life. We report a 66 years old woman with a vulvar Paget disease, subjected to a vulvectomy. Two years later, a pruriginous and erythematous plaque appeared in the perianal region. The pathological study informed an extra mammary Paget disease. The patient was subjected to a perianal excision and the correction of the surgical defect in a second intervention. After one year of follow up, there is no evidence of disease relapse.


La enfermedad de Paget de localización extramamaria (EPEM) es una patología infrecuente que afecta preferentemente a zonas perianal y vulvar en pacientes de ambos sexos y desde la sexta década de vida. Presentamos un caso clínico tratado en conjunto por los Servicios de Cirugía General, Cirugía Plástica, Ginecología y Anatomía Patológica del Hospital Universitario de Getafe.

ABSTRACT

Solitary fibrous tumors (SFT) are rare spindle-cell neoplasms that are found principally in the pleura, and extrathoracic SFTs are even less common. We report here on a case of malignant solitary fibrous tumor in the perianal region. A 47-year-old man was referred to our hospital for surgical treatment of an incidentally discovered perianal mass. Pelvic MRI showed a slightly lobulated solid mass with central necrosis in the ischiorectal fossa. We performed complete removal of tumor with using a postanal approach. The resected specimen was 6.5x5.0x5.0 cm in size, and it was an elastic solid tumor with invasion to the surrounding muscle structures. Histologically, the tumor was composed of a patternless proliferation of spindle cells with a collagenous matrix and the tumor cells showed increased cellularity, cellular pleomorphism and frequent mitotic figure. Immunohistochemical staining was positive for CD34 and negative for desmin, which indicated malignant SFT. We report here on a rare case of malignant SFT in the perianal region along with a review of the literature.

ABSTRACT

Since granular cell tumor was first described by Abrikossoff in 1926, it has been known as a rare disease. The histogenesis of this tumor is still controversial, but the origin is thought to be from a Schwann cell. About one third of the tumors occur in the tongue, and uncommonly in the perianal region. We report a case of granular cell tumor that developed in the perianal region. The tumor grew slowly for 5 years and was removed by a local excision. This tumor showed positive staining with neuron-specific enolase (NSE).
