ABSTRACT

Malignant pleural effusion (MPE) is one of the frequent and distressing conditions identified at the advanced stage of different malignant conditions. This problem seems to affect patients with lung and breast malignancies which contribute to the lower life quality. Less commonly MPE presents as a loculated fluid collection. We present an unusual case of cystic fluid collection in contact with the heart appearing similar to a pericardial cyst, but on cytological examination turned out to be loculated MPE. Furthermore, there was the presence of malignant fissural effusion. This text highlights the unusual location of this loculated MPE along with a few characteristic features to differentiate it from the benign pericardial cyst

ABSTRACT

Resumen El diagnóstico de los quistes de pericardio ha aumentado debido a los avances de las ayudas diagnósticas imagenológicas, como la tomografía computarizada, la resonancia magnética y la videotoracoscopia. Sin embargo, para la comunidad médica actual aún continúa siendo poco común su hallazgo, debido a que en la mayoría de los pacientes el diagnóstico es incidental y están asintomáticos. Este artículo presenta dos casos clínicos de pacientes de sexo femenino, de 47 y 63 años de edad, quienes consultaron inicialmente por palpitaciones y disnea, y a quienes se les realizaron imágenes que mostraron un quiste de pericardio en el ángulo cardiofrénico derecho, por lo cual fueron remitidas a la institución donde las atendían regularmente para manejo por cirugía cardiovascular. Ambas se intervinieron con ayuda de videotoracoscopia y mediante cirugía mínimamente invasiva, y no presentaron complicaciones durante el procedimiento ni posteriores a este.


Abstract The diagnosis of pericardial cysts has been increasing due to advances in imaging diagnostic aids, such as computed tomography, magnetic resonance imaging and video-thoracoscopy. However, for the current medical community, its finding is still uncommon because in most patients the diagnosis is incidental and they are asymptomatic. This article presents two clinical cases of female patients of 47 and 63 years of age, who initially consulted for palpitations and dyspnea, and to those who underwent images that showed a pericardial cyst in the right cardio-phrenic angle, so they were subsequently referred to the institution where they were regularly attended for management by cardiovascular surgery. Both were operated with the help of video-thoracoscopy and minimally invasive surgery, without complications at the time of procedure or subsequent to it.

ABSTRACT

This case discusses a patient who presented with symptoms of dyspnea and cough. Prior to being seen and admitted, the patient was seen multiple times over a one month period with complaints of intermittent chest pain, most notable in the retrosternal region, and frequent heart palpitations. Cardiac monitoring of the patient revealed multifocal atrial tachycardia over multiple ECG, the cause of which was later revealed to be a pericardial cyst located in the right atrium on CT scan and transesophageal echocardiogram.

ABSTRACT

Paciente femenino, de 24 años que concurre por dolor torácico y disnea. Al examen por TC se observa masa homogénea, en seno cardiofrenico derecho, que no realza tras la administración EV de contraste iodado, y teniendo como característica, contenido líquido. Provoca desviación mediastinica hacia la izquierda. Luego de su exeresis, se concluye en el diagnóstico de quiste pericardio-celómico.


Female patient, 24 years that concurs with chest pain and dyspnea. To review by TC is observed homogeneous mass, in the cardiofrenico angle, which does not enhance after contrast administration EV iodide, and taking as a feature, liquid contents. Causes mediastinal deviation to the left. After excision, it is concluded in the diagnosis of pericardial cyst-coelomic.

El quiste pleuro pericárdico es una lesión muy poco común que se produce por una alteración en la embriogénesis de la cavidad celómica primordialmente, aunque también puede ser adquirido. Su incidencia es infrecuente y su localización es esencialmente a nivel del ángulo cardiofrénico derecho. Presentación de casos. Se trata de tres pacientes de sexo femenino de la quinta y sexta década de la vida, con historia, tos, disnea y hallazgo incidental en la radiografía de tórax de lesión quística de localización mediastínica. Conclusiones. El quiste pleuro pericárdico es una entidad de diagnóstico generalmente incidental y su tratamiento es en principio conservador. La cirugía ya sea por toracotomía o videotoracoscopía se reserva para pacientes con indicaciones específicas


Pleuro pericardial cyst is a very rare injury that is caused by an alteraton in the embryogenesis of the coelomic cavity primarily, but can also be acquired. Its incidence is rare and its locaton is essentally level cardiophrenic right angle. Presentaton of cases. There are three female patents of the ffh and sixth decade of life, with history, cough, dyspnea and incidental fnding on chest radiograph of a mediastnal cystc lesion localizaton. Conclusions. Pleuro pericardial cyst is an entty generally incidental, diagnosis and treatment is conservatve. Surgery either by thoracotomy or VATS is reserved for patents with specifc indicatons

ABSTRACT

Left atrial space occupying lesions pose diagnostic challenge on echocardiography. We report a 79-year-old man with history of pheochromocytoma with left adrenal mass presented with heart failure symptoms after adrenalectomy. Echocardiogram revealed a large echo lucent structure (6.9 x5.9cm) compressing the posterior wall of the left atrium. Further investigation with computer tomography and barium swallow confirmed the diagnosis of hiatal hernia. In this report we explain the differential diagnosis and simple steps to assist the diagnosis of left atrial space occupying lesions noninvasively.

ABSTRACT

Pericardial cysts are reported by some authors, but epicardial cysts are extremely rare. We report one case of epicardial cyst that was detected incidentally and was removed successfully. Furthermore, unusually, pathological examinations confirmed that the cyst wall was looked like a vessel wall.

ABSTRACT

Pericardial cysts occur rarely, with an incidence rate of 1 per 100,000. They are usually detected by chance and clinically silent in most cases. Pericardial cysts are the most common benign tumours of the pericardium and presents by the third or fourth decade of life, and equally common in males and females. In principle, they only require follow-up, however, an enlarging or symptomatic cyst requires surgical removal. We report a case of a 32 year-old Malay lady, who presented with history of recurrent pericardial effusion followed by right pleural effusion. Computed tomography (CT) thorax identified a large mediastinal cyst as the cause of her problem, requiring exploratory thoracotomy.

ABSTRACT

El quiste pericárdico es una entidad rara, de origen congénito, que acontece en el 7% de los casos de masas mediastínicas, localizado habitualmente en el lado derecho del pericardio y el mediastino anterior. En general, los pacientes evolucionan en forma asintomática, a excepción de los casos en los que el quiste presenta una localización fuera de los sitios mencionados o en los que por su tamaño condiciona síntomas. En los sintomáticos, las manifestaciones dependen del sitio, la localización del quiste y los órganos involucrados. En esta presentación se describe el caso clínico de un paciente a quien se le realizó un trasplante cardíaco y posteriormente desarrolló un quiste pericárdico.


Pericardial cysts are an uncommon congenital anomaly which represents 7% of mediastinal masses. Cysts frequently occur in the right cardiophrenic angle and in the anterior mediastinum. Although most pericardial cysts are asymptomatic, symptoms may develop in unusually located or large cysts. The clinical manifestations depend on the site of location and on the organs involved. We describe the case of a heart transplant patient who developed a pericardial cyst after transplantation.

ABSTRACT

Pericardial cyst is an uncommon congenital mediastinal tumor. The majority of pericardial cysts are located in the right cardiophrenic angle, but rarely they can be located intrapericardially. We now present a case of a huge intrapericardial pericardial cyst excised with video-assisted thoracoscopic surgery.

ABSTRACT

Pericardial cyst may be either congenital or acquired abnormality of the pericardium. It originates from the weak point of pericardium as a defect in the fusion of the parietal recesses of the pericardial coelom. In acute pericarditis, this weak point is aggravated by pressure of the pericardial fluid in pericardial cavity. It is an uncommon pathology that rarely causes symptom. The prevalence is 1:100,000. To the authors’ knowledge there was 1 reported case in Thai Journal. We present a case from Chantrarubeksa Hospital. The patient was we repeated chest X-ray and found abnormal density at the fight border of the heart. Pericardial cyst was diagnosed and confirmed by ultrasonography. Excision was done without complication.
