ABSTRACT

Objective:To observe the clinical characteristics and optical coherence tomography (OCT) features of pseudopapilledema (PPE) combined with peripapillary hyper-reflective ovoid mass-like structures (PHOMS) in children.Methods:A retrospective observational study. From October 2019 to May 2021, total 22 eyes from 12 children diagnosed as PPE combined with PHOMS in the Neuro-ophthalmology Department of The First Hospital of Xi’an (Affiliated of The First Hospital of Northwest University) were recruited. Among the children, 6 were male and 6 were female. The average age was (10.6±2.7) years. The average course from disease onset to diagnosis of PPE combined with PHOMS was (8.0±7.5) months. All patients underwent best corrected visual acuity (BCVA), relative afferent papillary defect (RAPD), Ishihara's test, fundus photography, OCT, fundus autofluorescence (FAF), ocular B-mode ultrasound, visual field and patternvisual evoked potential (P-VEP). The clinical and OCT characteristics of the patients were observed.Results:The anterior segments of the patients were normal. The intraocular pressures and Ishihara's test were all normal. All RAPD were negative. Total 22 eyes, BCVA was 1.0 in 21 eyes and one eye was 0.12. The fundus photography revealed blurred optic discs margin, showing mild to moderate edema-like elevation with more prominent in the nasal parts, presenting as a "C" shape halo. No obvious abnormal fluorescence was observed in FAF. The OCT scan of involvement eyes showed an elevated appearance in vary degrees, and the sharply marginated ovoid hyper-reflective mass-like structures which laterally herniated into the peripapillary region under retinal nerve fiber layer and above the Bruch membrane were detected with consecutive nasal enlargement scanning, corresponding to the nasal parts in the fundus photography. The higher degree of elevation, the larger the volume. Macular retina pigment epithelium layer and ganglion cell thickness were normal. Ocular B-mode ultrasound showed that the head of the optic nerve in the posterior wall of the eyeball (in front of the optic disc) was elevated in all affected eyes, and there was no strong signal echo in it. Visual field examination showed physical blind spot enlargement in 3 eyes and visual field defect in 2 eyes. P-VEP examination showed that the peak was slightly delayed in 3 eyes and the amplitude was slightly reduced in 3 eyes.Conclusions:Enlarged nasal optic disc OCT scan can improve the detection rate of PHOMS. PHOMS were detected bilaterally in the cases with binocular PPE while only in the effected eye in the cases of monocular PPE; the higher degree of PPE, the lager volume of PHOMS. PHOMS were could contribute to the diagnosis of PPE in children.

ABSTRACT

Objective:To analyze the thickness of peripapillary retinal nerve fiber layer (pRNFL) and photoreceptor (PR) sublayer in Leber hereditary optic neuropathy (LHON) and G11778A mutation carriers. Methods:A cross sectional study. From September 2020 to October 2021, 68 LHON patients (136 eyes) (patient group) and 40 G11778A mutation carriers (80 eyes) of LHON patients' families (carrier group) were included in the study. All patients were found to have G11778A mutation by Genetic testing. Forty healthy volunteers with 80 eyes matched to the age and gender of the patient group were recruited as a normal control group. All eyes were examined by optical coherence tomography (OCT). The pRNFL thickness was automatically measured by the built-in software of the OCT device. The total retinal thickness (MT) and the thickness of the outer bundle layer (OPL), outer nuclear layer (ONL), external limiting membrane to retinal pigment epithelium (ELM-RPE) in macular OCT images were measured by Image J software. Linear mixed model was used to analyze and compare the thickness of pRNFL, macular fovea and four layers above the nasal and temporal paracentral retina in patients, carriers and normal controls. The correlation between pRNFL and macular retinal sublayer thickness and the course of disease was also analyzed. Results:The thickness of the upper and lower pRNFL, temporal pRNFL and average pRNFL of the patients were smaller than those of the carriers and the normal control group ( P<0.01), and the nasal pRNFL thickness of the patients was smaller than that of the carriers ( P<0.01). Fovea: compared with the normal control group, the thickness of MT and ONT in the patient group was decreased, ONL thickness decreased in carrier group, with the significant different ( P<0.05). Parafovea: compared with normal control group, the thickness of MT and temporal ONL decreased and temporal OPL increased in the patients group, with the significant different ( P<0.05). In the carrier group, the thickness of MT and temporal, nasal ONL decreased, and the thickness of nasal OPL increased, with the significant different ( P<0.05). Compared with the carrier group, the MT thickness of the patient group was decreased, and the nasal ONL and nasal ELM-RPE thickness were increased, with the significant different ( P<0.05). Correlation analysis results showed that the thinning of pRNFL in the superior, nasal, temporal and average ( r=-0.22, -0.21, -0.25, -0.22), and the thickening of ELM-RPE in foveo-temporal ( r=0.19) were correlated with the course of disease ( P<0.05). Conclusions:The pRNFL of LHON patients with G11778A mutation becomes thinner and is related to the course of the disease. There were significant differences in the thickness of MT and PR sublayers between patients and carriers compared to the normal control group.

ABSTRACT

【Objective】 To investigate the clinical effect of optical coherence tomography angiograph (OCTA) applied in retinal microvascular screening in patients with non-proliferative diabetic retinopathy (NPDR). 【Methods】 Thirty patients with NPDR (NPDR group) diagnosed at The Third Affiliated Hospital of Soochow University and 30 healthy volunteers (control group) were selected to receive OCTA examination. The area, perimeter, and circularity of the fovea avascular zone (FAZ) were measured and blood flow density in the superior, inferior, nasal, temporal quadrants of the macular superficial retinal capillary layer (SCP) and the peripapillary radial capillary layer (RPC) of the optic disc were quantified. 【Results】 In NPDR group, blood flow density in the four quadrants of macular SCP and RPC were decreased significantly compared with that in control group (43.68±3.03 vs. 46.98±3.04, 42.79±3.17 vs. 50.45±2.25, 43.21±2.67 vs. 47.44±2.42, 44.21±3.22 vs. 51.72±5.32, 46.43±3.54 vs. 53.02±2.62, 45.97±3.67 vs. 52.53±2.82, 44.63±2.73 vs. 48.19±3.67, 41.73±3.15 vs. 45.12±3.31) (all P<0.01). The area and perimeter of FAZ in NPDR group were significantly higher than those in control group [(0.50±0.06 vs. 0.43±0.47) mm2, (3.10±0.21 vs. 2.87±0.22) mm]. The circularity of FAZ was significantly lower in NPDR group than in control group [(0.63±0.05 vs. 0.67±0.05)%, P<0.01]. 【Conclusion】 OCTA can detect early retinal microstructure changes in NPDR, and thus can be used as an auxiliary screening of NPDR to provide information for early diagnosis and treatment.

ABSTRACT

AIM: To investigate the changes in peripapillary choroidal thickness(pCT)within 1a of the first episode of acute primary angle closure glaucoma(APACG).METHODS: A prospective cohort study. A total of 31 patients with APACG who admitted to the ophthalmology department of Shijiazhuang People's Hospital from October 2015 to September 2019 were selected, with 31 eyes in attack group, 31 fellow eyes in preclinical group and 30 cases(30 eyes)in control group. pCT of the three groups was measured respectively at the attack period, 1wk, 1, 3, 6mo and 1a after surgery.RESULTS: The pCT of the attack group was thicker than that in the attack period when at 1wk after surgery, and continued to become thinner within the following 1a(P&#x0026;#x003C;0.05). The pCT of the attack group was thicker than that of the other two groups during the attack period and at 1wk after surgery, while it became thinner at 1a(P&#x0026;#x003C;0.05). In the attack group, the average pCT was positively correlated with the duration of intraocular hypertension and negatively correlated with the anterior chamber depth(P&#x0026;#x003C;0.05).CONCLUSION: For patients with first episode of APACG, pCT was diffusely thickened during attack and at 1wk after surgery. The pCT returned to normal at 1mo, while it became thinner at 1a. Furthermore, the average pCT was positively correlated with the duration of intraocular hypertension, and the choroidal thickness may play an important role in the attack of APACG.

ABSTRACT

Purpose: The present study aimed to assess the changes in optic disc and peripapillary retinal nerve fiber layer (RNFL) parameters in myopic patients and its correlation with axial length (AL) and spherical equivalent (SE) using optical coherence tomography (OCT). Methods: This was a cross?sectional study carried out from August 2019 to September 2021 in the ophthalmology department of a tertiary care hospital in eastern India. Myopic patients in the age group of 20–40 years and SE between ? 0.5 to ? 10 Diopters (D) were included in the study. Patients were divided into two groups on the basis of degree of myopia and AL. Appropriate statistical analysis was done at the end of the study period. Results: The study included 307 eyes of 307 myopic patients. There were 181 females (58.96%) and 126 males (41.04%). The mean age of the patients enrolled for the study was 28.78 ± 5.76 years. Statistically significant difference (P < 0.001) was found between SE and AL in between the subgroups of A and B. With every 1 D increase in SE, the average peripapillary RNFL thickness decreased by 0.61 ? while with every 1 mm increase in AL, the average peripapillary RNFL thickness was found to reduce by 1.03 ?. Conclusion: Analysis of optic nerve head parameters and RNFL thickness by OCT for the diagnosis should be compared with a normative control group that has been matched for refractive error and AL instead of comparison with a normative database that has only been age matched.

ABSTRACT

Purpose: To observe the effects of caffeine intake on vessel densities in macular and peripapillary regions and to analyze the correlations of vascular properties of these two interlinked regions in healthy volunteers using optical coherence tomography angiography (OCTA). Methods: In this prospective randomized, double?masked, placebo?controlled study, 120 participants were grouped into two groups: sixty subjects as a study group, which received caffeine and sixty subjects as a placebo group, which received a placebo. OCTA imaging was performed at baseline and 1 h after drug intake. Changes in macular and radial peripapillary capillary (RPC) vessel densities (VD) and foveal avascular zone (FAZ) parameters and correlation of these parameters were analyzed. Results: Baseline macular and RPC VD and FAZ parameters showed no significant difference between study and placebo groups (P > 0.05 for all). VD parameters in all segments of superficial and deep macular regions and peripapillary region showed a statistically significant decrease after caffeine intake (P < 0.001 for all). All macular superficial VD parameters showed a positive correlation with deep perifovea VD (P < 0.05 for all). Also, superficial perifovea VD was positively correlated with all deep macular VD parameters except deep fovea VD (P < 0.05 for all). Conclusion: We show that caffeine causes a decrease in macular and peripapillary vessel densities in healthy subjects, and correlations were observed between macular and peripapillary VD parameters. We believe that future studies are needed to support the effect of caffeine on peripapillary VD and the idea of using deep perifovea VD as the main parameter to estimate superficial macular parameters.

ABSTRACT

AIM:To analyze the diagnostic value of optical coherence tomography(OCT)for early primary open angle glaucoma(POAG)by measuring the thickness of macular ganglion cell complex(mGCC), peripapillary retinal nerve fiber layer(pRNFL), macular retinal nerve fiber layer(mRNFL)and retinal ganglion cell layer+inner plexiform layer(GCIP)in the macular region.METHODS:A case-control study design was used to collect 82 patients(82 eyes)with early POAG who admitted to our hospital from January 2019 to January 2020. There were 40 healthy volunteers(40 eyes)in the same period selected as the control group(20 eyes for left and right eyes, respectively). All subjects underwent tests of uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA)and equivalent spherical power. The upper, lower, nasal, temporal and average pRNFL thickness, the upper, lower and average mGCC, mRNFL and GCIP thickness in the macular region were analyzed. Spearman was used to analyze the correlation among the average thickness of eachindices, and ROC curve was used to analyze the value of mGCC, pRNFL, mRNF and GCIP thickness parameters in diagnosing early POAG.RESULTS:In the early POAG group, the thickness of upper, nasal, lower, temporal, average pRNFL and the thickness of upper, lower and average mGCC, mRNFL and GCIP were lower than those in the control group(all P&#x003C;0.05). Average thickness of pRNFL was positively correlated with average thickness of mGCC, average mRNFL and average GCIP(rs=0.582, 0.632, 0.456, all P&#x003C;0.05); average thickness of mGCC was positively correlated with average thickness of mRNFL and average GCIP(rs=0.583, 0.851, all P&#x003C;0.05); Average thickness of mRNFL was positively correlated with average thickness of GCIP(rs=0.528, all P&#x003C;0.01). ROC curve analysis shows that the AUC value of mGCC thickness and average value in early diagnosis of POAG were all above 0.8, and the diagnostic efficiency was the highest.CONCLUSION: OCT measurement of mGCC has certain value in diagnosing early POAG.

ABSTRACT

Objective@#This study compared the peripapillary retinal nerve fiber layer (pRNFL) microangiographic properties [vessel area density (VAD) and blood flux index (BFI)] of unilateral open-angle glaucomatous eyes to contralateral eyes-at-risk, and to eyes of healthy age- and sex-matched subjects.@*Methods@#This was a single-center, case-control study of Filipinos diagnosed with unilateral primary openangle glaucoma (POAG) or normal-tension glaucoma (NTG). Mean overall and quadrantal VAD and BFI of the three groups were measured with optical coherence tomography- angiography (OCT-A). Area under the receiver operating characteristic (AROC) was used to measure diagnostic ability.@*Results@#Twenty-two (22) glaucomatous subjects (15 POAG and 7 NTG eyes), 22 contralateral eyes-at-risk, and 22 normal eyes from age- and sex-matched control subjects completed the study. Eyes with glaucoma showed lower mean overall VAD (40%) and BFI (0.37) compared to eyes-at-risk (44.4% and 0.42, respectively; p <0.001) and control eyes (45.6% and 0.44, respectively; p <0.001). Mean VAD and BFI values of eyes-at-risk and control groups did not significantly differ from each other. Overall pRNFL thickness showed highest diagnostic accuracy for glaucoma (AROC = 0.97), followed by VAD (0.94), and BFI (0.88) (p=0.46).@*Conclusion@#VAD and BFI were significantly diminished in unilateral open-angle glaucoma, suggesting that the utility of OCT-A in the detection of glaucoma is comparable to pRNFL thickness.

OBJECTIVE@#We wanted to investigate the radial peripapillary capillary (RPC) network in patients with Bietti crystalline dystrophy (BCD).@*METHODS@#We compared RPC densities in the disk and different peripapillary regions, obtained using optical coherence tomography angiography in 22 patients with BCD (37 eyes) and 22 healthy subjects (37 eyes). The BCD group was then divided into Stage 2 and Stage 3 subgroups based on Yuzawa staging, comparing the RPC densities of the two.@*RESULTS@#The disk area RPC density was 38.8% ± 6.3% in the BCD group and 49.2% ± 6.1% in the control group ( P < 0.001), and peripapillary region RPC density was significantly lower in the BCD group than in the control group (49.1% ± 4.7% and 54.1% ± 3.0%, respectively, P < 0.001). There were no significant RPC density differences between the tempo quadrant and inside disk of Stages 2 and 3 subgroups; the other areas showed a significantly lower RPC density in Stage 3 than in Stage 2 BCD.@*CONCLUSION@#The BCD group RPC density was significantly lower than the control group. The reduction of RPC density in the tempo quadrant occurred mainly in the Stage 1 BCD. In contrast, the reduction of RPC density in superior, inferior, and nasal quadrants occurred mainly in Stage 2.

ABSTRACT

@#AIM: To quantitatively analyze the microcircular and structural changes of the macular and peripapillary area in patients with diabetic kidney disease(DKD), exploring non-invasive effective monitoring indexes that can predict the occurrence and development of diabetic kidney disease used optical coherence tomography angiography(OCTA).<p>METHODS: A cross-sectional study. The diabetic patients(52 cases)and healthy subjects(20 cases)who came to Shanxi Provincial People's Hospital from January 2020 to January 2021 were collected, and the diabetes mellitus(DM)patients were further divided into DKD group(23 cases)(complicated with diabetic kidney disease)and non-DKD group(29 cases)(not complicated with diabetic kidney disease). All patients underwent slit-lamp microscopy, best corrected visual acuity(BCVA), OCTA and fundus color photography. OCTA was used to scan the macula and peripapillary area of all participants, measuring the shallow and deep blood flow density, the area and circumference of the fovea(FAZ), vascular density in the 300μm area around FAZ(FD300), retinal thickness, peripapillary-VD, retinal nerve fiber layer(RNFL)thickness, and ganglion cell complex(GCC)thickness. The correlation between the changes of these parameters in OCTA, DR staging and DKD staging was analyzed. <p>RESULTS: Parafoveal SCP-VD, perifoveal DCP-VD, peripapillary-VD, and FD300 showed a decreasing trend among the healthy group, non-DKD group, and DKD group(<i>P</i><0.05). The foveal DCP-VD was decreased only in the DKD group(<i>P</i><0.05), and there was no significant difference in the SCP-VD of fovea among the three groups(<i>P></i>0.05). FAZ area and GCC thickness were gradually increased among healthy group, non-DKD group and DKD group(<i>P</i><0.05). DR staging was positively correlated with DKD staging(<i>rs</i>=0.648, <i>P</i><0.05). Parafoveal SCP-VD, parafoveal DCP-VD, and perifoveal DCP-VD were negatively correlated with DKD staging(<i>rs</i>= -0.535, <i>P</i>=0.009; <i>rs</i>= -0.712, <i>P</i><0.001; <i>rs</i>= -0.641, <i>P</i>=0.001). Compared with non-DKD group, DKD group had higher SBP, lower eGFR, and higher UACR(<i>P</i><0.05). <p>CONCLUSION: OCTA can detect retinal microcirculation and structural changes in patients with DKD, including decreased vessel density in the macular and peripapillary area, increased FAZ area and GCC thickness. More importantly, we found that retinal vessel density decreased with renal function impairment, suggesting that retinal vessel density may be a noninvasive and effective predictor in the severity of DKD.

ABSTRACT

@#<p style="text-align: justify;">Aicardi Syndrome (AS) is a rare X-linked congenital disorder traditionally characterized by a triad of dysgenesis of corpus callosum, seizures, and chorioretinal abnormalities. Patients often have severe psychomotor delay and shortened life expectancy. However, Aicardi syndrome is a clinically heterogeneous disorder. We present a case of a 14-year-old with the traditional triad of history of infantile spasm, complete agenesis of the corpus callosum, and chorioretinal abnormality but with peripapillary staphyloma and with no psychomotor delays. Based on the review of literature, this is the first reported case of AS in the Philippines, the first reported case of AS with peripapillary staphyloma, and is one of the 3 reported cases of AS with normal psychomotor development. There remains no factor that can prognosticate cognitive function in AS at present including genetic testing.</p>

ABSTRACT

@#AIM:To measure the macular and peripapillary choroidal thickness(CT)in primary angle-closure disease(PACD)with enhanced depth imaging optical coherence tomography(EDI-OCT). To explore the characteristics of CT in each subtypes of PACD and to evaluate its role in the pathogenesis of PACD.<p>METHODS: This was a prospective clinical study. A total of 155 PACD eyes(82 patients)were enrolled in the study, including 24 PACS eyes(24 patients), 35 APAC eyes(28 patients), 38 CPAC eyes(30 patients), 58 eyes PACG(38 patients). 87 normal eyes(87 patients)were set up as control. The EDI-OCT was used to measure the macular and peripapillary choroidal thickness in all study patients. <p>RESULTS: PACD eyes exhibited thicker choroid than the control eyes at all macular locations(<i>P</i><0.05). Choroidal thickness of PACG was thinner than other PACD eyes in area except for 3mm nasal from the fovea(<i>P</i><0.05). Subfoveal choroidal thickness(SFCT)of APAC was thickest(357.17±61.49μm), followed by PACS group(318.04±56.52μm). PACG group presented the thinnest SFCT(263.55±67.87μm). The average macular CT at 1mm centered at the fovea was thinner than SFCT(<i>P</i><0.05)in all subgroups except for CPAC. The average macular CT at 3mm as well as 1mm centered at the fovea was thinner than SFCT in all subgroups(<i>P</i><0.05). There was no statistical differences in CT at peripapillary locations between PACD and controls groups(<i>P</i>>0.05).<p>CONCLUSION: In PACD and controls groups, the CT of subfoveal location was the thickest with decreasing thickness when moving eccentrically from the fovea. The thicker CT might be another anatomic characteristic of PACD. Increased CT in macular location might be a contributing factor of acute attacks. There was no characteristic distinction in the peripapillary CT of PACD when compared with normal controls.

Un paciente de 3 meses con sospecha de estrabismo presenta al examen una gran anisometropía con -9.50 esfera en su ojo izquierdo y al fondo de ojo destaca una papila alterada similar a la papila de Morning-Glory. Las anomalías congénitas de nervio óptico (ACNO) son un grupo de condiciones que se presentan clínicamente como baja agudeza visual, estrabismo y/o nistagmus. El diagnóstico dife-rencial es clínico y complejo por la sobre posición morfológica entre estas. Los principales diagnósticos diferenciales son la papila deMorning-Glory, el coloboma y la hipoplasia de nervio óptico. Se recomienda manejo multidisciplinario por sus asociaciones sistémicas.


A 3-month-old patient with a suspicion of strabismus on examination has a great anisometropia with -9.50 sphere in his left eye and at the fundus examination an altered papilla similar to the Morning-Glory papilla. Congenital optic nerve abnormalities (ANOC) are a group of conditions that present clinically as low visual acuity, strabismus, and nystagmus. The differential diagnosis is clinical and complex due to the morphological position between them. The main differential diagnoses are the Morning-Glory papilla, the coloboma, and optic nerve hypoplasia. Multidisciplinary management is recommended for its systemic associations.

ABSTRACT

RESUMEN Los drusen de nervio óptico fueron descritos por primera vez por Liebreich en el año 1868. Otros términos para designar esta entidad incluyen cuerpos hialinos y cuerpos coloides del disco óptico. Tienen una prevalencia de 1 por 500 y el 60 por ciento de los casos se encuentran profundos en la cabeza del nervio óptico. La patogenia primaria de los drusen puede ser una displasia hereditaria del canal óptico del disco óptico y su vasculatura, lo que predispone a la formación de estos. La evolución natural de los drusen es un proceso dinámico que transcurre durante toda la vida. Entre las complicaciones asociadas se presentan defectos de campo visual, pérdida de visión central (rara pero bien documentada), neuropatía óptica isquémica, oclusiones vasculares retinales, pérdidas transitorias de la visión, neovascularización subretinal peripapilar, corioretinopatia serosa central peripapilar y hemorragias pre y peripapilares. Se presenta una paciente de 64 años de edad con antecedente de haber sido operada de desprendimiento de retina del ojo izquierdo, y en el ojo derecho presentaba una hemorragia peripapilar subretinal profunda asociada a drusen(AU)


ABSTRACT Optic nerve drusens were first described by Liebreich in the year 1868. Other terms to designate this condition are optic disc hyaline bodies and colloid bodies. They have a prevalence of 1 per 500 and 60 percent of the cases occur deep in the optic nerve head. The primary pathogenesis of drusens may be an inherited dysplasia of the optic canal of the disc and its vasculature, which leads to their formation. The natural evolution of drusens is a lifelong dynamic process. Associated complications include visual field defects, central vision loss (rare but well documented), ischemic optic neuropathy, retinal vascular occlusion, transient sight loss, peripapillary subretinal neovascularization, central serous peripapillary chorioretinopathy, and pre- and peripapillary bleeding. A case is reported of a 64-year-old female patient with a history of surgery for retinal detachment of the left eye. In the right eye the patient presented deep peripapillary subretinal bleeding associated to drusen(AU)

ABSTRACT

Objective:To investigate the effect of Tiluo Yangxue Mingmu prescription on the improvement of visual function,and to analyze the effect on the vessel density and the thickness of retinal nerve fiber layer(RNFL) of optic nerve in glaucoma patients. Method:The clinical data of 234 eyes of 117 patients of primary glaucoma treated with Tiluo Yangxue Mingmu prescription from January 2017 to July 2020 were retrospectively analyzed. Fifty-seven eyes treated with the Tiluo Yangxue Mingmu prescription were reviewed as the treatment group,and 57 eyes that were not treated with the prescription matched in gender,age and course of treatment were selected as the control group. The changes of BCVA,mean sensitivity(MS) and mean deviation(MD) of visiual field before and after treatment were analyzed. Optical coherence tomography(OCTA) was used to analyze the changes of VD and thickness of RNFL of potic nerve before and after treatment. Result:After treatment,compared with that before treatment,BCVA was increased (P=0),MS was increased (P=0),MD was decreased(P=0.001),VD of the whole image(P=0),VD of the inside disc(P=0.004) and all subdivisions of peripapillary(P<0.05) was increased,the RNFL thickness(except inferior nasal area) was not significantly changed. After treatment,MS was increased(P=0.003) and MD was decreased(P=0.024) in treatment group compared with those before treatment. In the control group,there was no statistical difference in the changes of visual field indicators after treatment compared with that before treatment. Conclusion:For glaucoma patients whose intraocular pressure is stable within the range of target intraocular pressure,Tiluo Yangxue Mingmu prescription can improve the visual function and VD of optic nerve,but has no significant effect on RNFL thickness.

@#AIM: To explore the clinical application value of optical coherence tomography angiography(OCTA)in patients with retinal vein occlusion(RVO). <p>METHODS:Retrospective case study. From January 2018 to December 2019, 40 cases with 40 eyes of RVO patients diagnosed in the Eye Center of the Affiliated Hospital of Weifang Medical University were selected, and 40 healthy subjects with 42 eyes matching the age and gender of RVO patients were selected as normal control group. OCTA was used to measure the papillary vessel density of the two groups, differences in parameters were evaluated. <p>RESULTS: The blood vessel density in the areas of the whole image blood vessels(<i>t</i>=-2.953, <i>P</i><0.001), the peripapillary blood vessels(<i>t</i>=-3.533, <i>P</i>=0.001), the whole image capillaries(<i>t</i>=-3.192, <i>P</i>=0.003), the peripapillary capillaries(<i>t</i>=-3.930, <i>P</i><0.001), inferior nasal(<i>t</i>=-2.854, <i>P</i>=0.007), inferior tempo(<i>t</i>=-3.696, <i>P</i>=0.001), tempo inferior(<i>t</i>=-3.418, <i>P</i>=0.002), tempo superior(<i>t</i>=-3.170, <i>P</i>=0.003), superior tempo(<i>t</i>=-3.082, <i>P</i>=0.004)and superior nasal(<i>t</i>=-2.912, <i>P</i>=0.006)in the eyes of patients with RVO was significantly lower than that of the fellow eyes of patients with RVO. The blood vessel flow density in the all areas of in the eyes of patients with RVO was significantly lower than that of the normal control group. The blood vessel density in the areas of the whole image blood vessels(<i>t</i>=-2.213, <i>P</i>=0.032), the blood vessels inside disc(<i>t</i>=-2.270, <i>P</i>=0.028), the whole image capillaries(<i>t</i>=-2.192, <i>P</i>=0.033), capillaries inside disc(<i>t</i>=-2.449, <i>P</i>=0.018)and tempo superior(<i>t</i>=-2.147, <i>P</i>=0.037)in the fellow eyes of patients with RVO was significantly lower than that of the normal control group.<p>CONCLUSION:Quantitative OCTA reveals a decrease in the vessel density of papillary area of patients with retinal vein occlusion, suggesting that papillary area with quantified OCTA has clinicalapplication value for the assessment of the severity and prevention of the fellow eyes with retinal vein occlusion disease.

ABSTRACT

Objective: To measure the parameters of optic disc and macula by spectral domain-optical coherence tomography (SD-OCT) in healthy controls and patients with mild or moderate primary open-angle glaucoma (POAG), and to analyze the related influencing factors. Methods: Forty patients (40 eyes) with mild or moderate POAG who were admitted to our department from Sep. 2015 to Aug. 2018 were enrolled, including 20 patients aged 20-39 years old and 20 patients aged 60-79 years old. Contemporaneous 40 healthy controls (40 eyes) were also included (20 aged 20-39 years old and 20 aged 60-79 years old). All subjects received comprehensive ophthalmic examination. SD-OCT was used to measure the thickness of peripapillary retinal nerve fiber layer (pRNFL), the thickness of macular average ganglion cell layer combined with the inner plexiform layer (GCL-IPL), the macular minimum GCL-IPL thickness, and the average thickness of the macular cube. Results: The average and minimum GCL-IPL thickness decreased significantly with age in both healthy control and POAG groups (P0.05). The mean, superior, inferior and temporal thicknesses of pRNFL in the POAG group (both 20-39 years old and 60-79 years old subgroups) were significantly thinner than those in the healthy control group (P0.05). And no significant difference was found in the nasal thickness of pRNFL between POAG group and healthy control group of the same age (P>0.05). Conclusion: The pRNFL thickness measured by SD-OCT is related to POAG, and it can be used as a detection index for early diagnosis of POAG.

Introducción: La miopía es uno de los errores refractivos más prevalentes, a nivel global 28,3%. La apariencia morfológica del disco óptico en la miopía con las variaciones propias de la condición hacen del diagnóstico clínico y el monitoreo de la progresión del glaucoma muy difícil. Recientemente, nuevas modalidades de imagen de alta resolución se desarrollaron para darnos más información y ayudar en la detección de los cambios micro anatómicos de la cabeza del nervio óptico. Objetivo: Determinar la variabilidad del espesor de la capa de fibras nerviosas peripapilares (CFNR) medido con tomografía de coherencia óptica (OCT) entre los diferentes grupos según severidad de miopía y grupo control. Metodología: Se trata de un estudio observacional, descriptivo, de corte transversal. Muestreo no probabilístico de casos consecutivos. Con componente analítico. Resultados: Se analizaron un total de 144 ojos, de los sujetos estudiados aproximadamente dos tercios eran del sexo femenino (n: 97 ojos). La edad media de los sujetos estudiados fue de 30,68 +/- 11,58. Se estudiaron 50 (34,7%) ojos emétropes como grupo control, entre los diferentes grupos según severidad de la miopía se estudiaron 27 (18,8 %) ojos con miopía leve, 33 (22,9 %) con miopía moderada y 34 (23,6 %) ojos con miopía severa. La media de longitud axial encontrada fue de 23,12 +/- 0,81mm para el grupo control (emétropes); 23,72 +/- 0,76 para el grupo con miopía leve; 25,11+/-0.79mm para el grupo con miopía moderada y de 26,42 +/- 1,45mm para el grupo con miopía severa, encontrándose diferencias estadísticamente significativas entre estos grupos con una longitud axial mayor para el grupo con miopía severa. Se encontraron diferencias estadísticamente significativas entre los grupos estudiados, para diferencias encontradas en el espesor promedio y en el espesor de los cuadrantes superior, inferior y temporal. Las diferencias encontradas en el espesor del cuadrante nasal fueron no significativas. Conclusión: A pesar de las limitaciones, este estudio claramente demuestra que los ojos con miopía severa tienen un espesor menor en la CFNR peripapilares que los ojos emétropes en los cuadrantes superior e inferior. En el cuadrante temporal y nasal se encontró un aumento del espesor, siendo estadísticamente significativo en el cuadrante temporal. Por un lado este adelgazamiento podría ser un factor de riesgo para el desarrollo del glaucoma ya que las variaciones en la disposición de las fibras nerviosas de la cabeza del nervio óptico se postulan que hacen del ojo miope más susceptible al daño glaucomatoso. Para la relevancia clínica, debemos tener en cuenta las diferencias propias de la miopía para realizar el diagnóstico correcto en los casos de sospecha de glaucoma y evaluar progresión con OCT. Así también sería ideal agrupar según rango etario por las diferencias propias de esta variable que podría haber influenciado en la interpretación de los resultados.


Introduction: Myopia is one of the most prevalent refractive errors, globally 28.3%. The morphological appearance of the optic disc in myopia with the variations of the condition make clinical diagnosis and monitoring of glaucoma progression very difficult. Recently, new high resolution imaging modalities were developed to give us more information and help in the detection of micro anatomic changes of the optic nerve head. Objective: To determine the thickness variability of the peripapillary nerve fiber (RNFL) layer measured with optical coherence tomography (OCT) between the different groups according to myopia severity and control group. Methodology: This is an observational, descriptive, cross-sectional study. Non-probabilistic sampling of consecutive cases. With analytical component. Results: A total of 144 eyes were analyzed, approximately two thirds of the subjects studied were female (n: 97 eyes). The average age of the subjects studied was 30.68 +/- 11.58. 50 (34.7%) emmetropic eyes were studied as a control group, among the different groups according to severity of myopia 27 (18.8%) eyes with mild myopia were studied, 33 (22.9%) with moderate myopia and 34 (23.6%) eyes with severe myopia. The mean axial length found was 23.12 +/- 0.81mm for the control group (emétropes); 23.72 +/- 0.76mm for the group with mild myopia; 25.11 +/- 0.79mm for the group with moderate myopia and 26.42 +/- 1.45mm for the group with severe myopia, with statistically significant differences between these groups with a greater axial length for the group with severe myopia. Statistically significant differences were found between the groups studied, for differences found in the average thickness and in the thickness of the upper, lower and temporal quadrants. The differences found in the thickness of the nasal quadrant were not significant. Conclusion: Despite the limitations, this study clearly demonstrates that eyes with severe myopia have a smaller thickness in the peripapillary RNFL than the emmetropic eyes in the upper and lower quadrants. An increase in thickness was found in the temporal and nasal quadrant, being statistically significant in the temporal quadrant. On the one hand this thinning could be a risk factor for the development of glaucoma since variations in the arrangement of the nerve fibers of the optic nerve head are postulated that make the myopic eye more susceptible to glaucomatous damage. For clinical relevance, we must take into account the differences of myopia to make the correct diagnosis in cases of suspected glaucoma and evaluate progression with OCT. Thus it would also be ideal to group according to age range due to the differences of this variable that could have influenced the interpretation of the results.

Purpose: To compare the anatomic success of pars plana vitrectomy (PPV) after internal limiting membrane (ILM) peeling at macular area and macular plus peripapillary area versus no peeling in rhegmatogenous retinal detachments (RRD). Methods: A prospective observational study between July 2014 and March 2017 conducted on 289 eyes of 287 patients with RRD were randomly assigned to three treatment procedures, viz., PPV with no ILM peeling, PPV with macular peeling, and PPV with macular plus peripapillary peeling. Recurrent RD (ReRD) was treated as an event and accordingly the overall primary (PS) and final success (FS) rates were obtained. The risk of ReRD associated with peeling procedures after adjusting for risk factors were obtained using Cox-proportional hazard analysis. Results: The PS percentage for no peel, macular, and macular plus peripapillary procedures were 77.78% (70/90), 82.18% (83/101), and 94.89% (93/98; maximum), respectively, which was statistically significant with a P value of 0.003. The FS percentage for no peel, macular, and macular plus peripapillary were 93.33%, 95.04%, and 100%, respectively, which was significantly different with a P value of 0.048. With reference to no peeling, the adjusted hazard ratio for macular peeling was 0.841 [95% CI: 0.44–1.60] while 0.235 [95% CI: 0.088–0.626] for macular plus peripapillary peeling. Conclusion: The anatomic success rate of PPV with macular plus peripapillary ILM peeling was significantly higher as compared to no peel category. The hazard of ReRD in patients undergoing macular plus peripapillary peel was significantly reduced as compared to no peel procedure.

ABSTRACT

Through the different image technologies, peripapillary atrophy can be classified into four types: α zone, β zone, γ zone and δ zone. Each type of them has different imaging and histological features. Peripapillary atrophy is a common structure in peoples' eyes and it is correlated with many factors and diseases such as age, myopia, glaucoma, non-arteritic anterior ischemic optic neuropathy and age-related macular degeneration which bring difficulties to our diagnosis and antidiastole. Classifying and qualifying peripapillary atrophy properly, which can evaluate the changes and degrees, can benefit our studies about their mechanism and offer the clinical biological indictors and research basis.
