ABSTRACT

Peripheral odontogenic tumors (POTs) are one of the rare neoplasms to occur on the gingiva. Peripheral odontogenic fibroma (POdF) is the most common POT constituting a prevalence of 1.2%. A plethora of lesions sharing similar clinical features makes the diagnosis difficult. Histopathological examination plays a key role in these types of cases. Here, we present a rare case of POdF in a 36-year-old female patient with a lobulated pebbled-like exophytic growth on the right gingiva. Histopathological investigation showed the presence of odontogenic epithelium and dystrophic calcification which are the pathognomonic features of POdF. After surgical removal, POdF is seen to recur, but the exact recurrence rate cannot be estimated due to the scarcity of reported cases. In this case, follow-up of the patient showed no recurrence at 6, 12, and 18 months post-surgery.

ABSTRACT

A variety of localised gingival overgrowths are encountered by a clinician on a regular basis, specially a dentist encounters such growths very frequently; although all the growths show similar clinical features, it is their histological features which are different for each type of growths. Though ignored in daily practice, but histological examination is the only method to form the correct diagnosis avoidance of which could sometimes lead to ignoring serious consequences like malignancies. This article reviews and presents three cases of such growths i.e. Peripheral odontogenic fioma which is a rare benign odontogenic neoplasm, Fioid and Pyogenic granuloma. It stresses upon the importance of histopathological examination of all such gingival conditions, and how it is ignored and should be mandatory for such cases.

ABSTRACT

El Fibroma Odontogénico Periférico fue definido por la OMS en el año 2005 como una neoplasia benigna rara constituida por tejido fibroso maduro y una cantidad variable de epitelio odontogénico inactivo. Dada su presentación clínica, localización y baja prevalencia suele ser mal diagnosticado como una lesión reaccional. Se presenta un caso clínico de fibroma odontogénico periférico tratado mediante remoción quirúrgica y se realiza una revisión de la bibliografía respecto a la patología con el propósito de esclarecer algunos aspectos de esta lesión, además de incluirla dentro de los posibles diagnósticos diferenciales de lesiones reaccionales gingivales. El objetivo del siguiente artículo es presentar un caso clínico de FOP tratado mediante remoción quirúrgica y aportar en el diagnóstico diferencial de las lesiones reaccionales gingivales.


Peripheral odontogenic fibroma was described by the World Health Organization (WHO) in 2005, as a rare benign tumor containing mature fibrous connective tissue with a varying amount of inactive odontogenic epithelium. Though its clinical presentation, localization and low prevalence, it tends to be misdiagnosed as a reactive lesion. We present a case report of a Peripheral Odontogenic Fibroma treated by surgical resection and a narrative review of the literature with the purpose of clarifying different aspects of this lesion besides considering it as a possible differential diagnosis of reactive gingival lesions. The purpose of this article is to present a case report of peripheral odontogenic fibroma treated by surgical resection. Also to contribute to the differential diagnosis of gingival reactive lesions.

ABSTRACT

Peripheral ossifying fibroma (POF) is rare pathology and its appearance in maxillary anterior is very rare. It occurs due to irritants such as trauma, microorganisms, plaque, calculus, restorations, and dental appliances. It is also called by other names like epulis, peripheral fibroma with calcification, POF, calcifying fibroblastic granuloma, peripheral cementifying fibroma, peripheral fibroma with cementogenesis, and peripheral cemento-ossifying fibroma. POF may present as a pedunculated swelling, or it may have a broad attachment base. The lesion is usually red to pink with areas of ulceration, and their surface may be smooth or irregular. We are presenting one case of POF in the maxillary anterior region.

ABSTRACT

Peripheral odontogenic fibroma (POF) is a rare benign mesenchymal odontogenic neoplasm with or without odontogenic epithelium, which is considered to be the mucosal analogue of central odontogenic fibroma. Though peripheral odontogenic tumours are rare, comprising of only 0.05% of all biopsy specimens, POF is the most common peripheral odontogenic tumour, more common than its central counterpart by a ratio of 1.4: 1. In this report, we describe a healthy 40-year-old female who was seen with a recurrent POF of the left mandible.

Objetivo: Relatar um caso de um tumor raro imitando uma lesão gengival inflamatória tendo em foco não apenas as suas características, mas realizando um diagnóstico diferencial aprofundado com as lesões inflamatórias; a fim de colaborar para um diagnóstico mais eficiente por parte dos clínicos. Descrição do caso: Um paciente do sexo feminino, de 20 anos de idade apresentava uma massa gengival séssil e assintomática - entre os dentes 32 e 33 - com duração aproximada de 1 ano. Após biópsia excisional, a análise histológica revelou um neoplasma odontogênico caracterizado pela presença de ilhas de epitélio odontogênico aparentemente inativo, localizado em um estroma fibro-mixóide. Características consistentes com o diagnóstico de fibroma odontogênico periférico. Conclusão: O fibroma odontogênico periférico é freqüentemente confundido com uma lesão reacional inflamatória. No entanto, a presença de particularidades de cada lesão pode auxiliar o clínico, a fim de se obter um diagnóstico mais preciso. A biópsia excisional é o tratamento mais indicado. Porém, o prognóstico do fibroma odontogênico periférico pode ser diferente, devido a sua raridade e pouca informação sobre seu acompanhamento de longo prazo.


Purpose: To report a rare tumor mimicking a gingival inflammatory lesion focusing not only on its characteristics, but making a deepened differential diagnosis with inflammatory lesions, in terms to collaborate for an accurate diagnosis as possible by the clinicians. Case description: A 20-year-old woman presented an asymptomatic sessile gingival mass - between teeth 32 and 33 - with 1 year of duration. An excisional biopsy was performed and the histological examination revealed an odontogenic neoplasm characterized by the presence of islands of apparently inactive odontogenic epithelium, in a fibromyxoid stroma; all features consistent with the diagnosis of peripheral odontogenic fibroma. Conclusion: Peripheral odontogenic fibroma is often misdiagnosed as an inflammatory lesion. However, the existence of particularities of each lesion can guide the clinical to a more efficient diagnosis. Excisional biopsy is their treatment of choice, whereas; the peripheral odontogenic fibroma prognosis may be different given its rarity and its few data about long-term follow-up.
