ABSTRACT

ABSTRACT BACKGROUND: Ankle taping (AT) is effective in preventing ankle sprain injuries in most common sports and is employed in rehabilitation and prevention sports. OBJECTIVE: This study aimed to investigate the effectiveness of AT to restricting excessive frontal plane ankle movements in semi-professional basketball players throughout the training session. DESIGN AND SETTING: A cross-sectional study was performed at the Universidad Europea de Madrid. METHODS: Forty male and female semi-professional basketball players were divided into two groups. The ankle dorsiflexion range of motion (ROM) and interlimb asymmetries in a weight-bearing lunge position were evaluated at four time points: 1) with no tape, 2) before practice, at 30 min of practice, and 3) immediately after practice. RESULTS: In male basketball players, no differences were observed in the right and left ankles between the baseline and 30 min and between baseline and 90 min of assessment. In female athletes, significant differences were reported between baseline and pre-training assessments for the right ankle and also significant differences between baseline and 90 min in both ankles. CONCLUSIONS: Ankle taping effectively decreased the ankle dorsiflexion ROM in male and female basketball players immediately after application. However, ROM restriction was very low after 30 and 90 min, as assessed in a single basketball practice. Therefore, the classic taping method should be revised to develop new prophylactic approaches, such as the implementation of semi-rigid bracing techniques or the addition of active stripes during training or game pauses.

In the past two decades, the survival of HER2-positive early-stage breast cancer patients has significantly improved with the development of HER2-targeted therapies. The focus has been placed on maximizing the clinical benefit of HER2-positive early-stage breast cancer by optimizing the treatment frameworks and therapeutic strategies in this field. In this paper, several important clinical studies of HER2-positive early-stage breast cancer in the neoadjuvant or adjuvant settings will be summarized and analyzed to provide clues for the development of personalized treatment strategies in the future.

ABSTRACT

Disease-modifying therapy (DMT), the dominating treatment for the remission stage of multiple sclerosis (MS), has reshaped the treatment situation of MS in China due to the increasing number of DMT drugs introduced since 2018. However, the current treatment status is still not satisfied because of the long-time lack of DMT drugs in the past and the latest advances in the diagnosis and treatment of MS. The treatment strategies and goals of MS advanced continuously attributed to the more clarified pathophysiology, more optional DMT drugs and more surveillance methods in disease progression and therapeutic efficacy combined with MRI and biomarkers,etc. Personalized treatment and follow-up protocol established on evidence based prognostication, drug and patient-related factors, probably will be one of the most optimal treatment algorithm in the future. Here is a comprehensive review on how to perform this personalized treatment based on national conditions.

ABSTRACT

Emerging evidence has demonstrated the vital role of metabolism in various diseases or disorders. Metabolomics provides a comprehensive understanding of metabolism in biological systems. With advanced analytical techniques, metabolomics exhibits unprecedented significant value in basic drug research, including understanding disease mechanisms, identifying drug targets, and elucidating the mode of action of drugs. More importantly, metabolomics greatly accelerates the drug development process by predicting pharmacokinetics, pharmacodynamics, and drug response. In addition, metabolomics facilitates the exploration of drug repurposing and drug-drug interactions, as well as the development of personalized treatment strategies. Here, we briefly review the recent advances in technologies in metabolomics and update our knowledge of the applications of metabolomics in drug research and development.

RESUMEN El asma severa incluye un pequeño porcentaje de pacientes que varía de acuerdo a los diferentes países, aun así, los pacientes con asma grave no controlada tienen una morbilidad desproporcionadamente alta junto a una utilización de la asistencia sanitaria. Objetivos: Evaluar la respuesta al tratamiento con biológicos en pacientes asmáticos severos y la importancia de realizar la clasificación de fenotipo y endotipo, en una muestra de pacientes asmáticos severos entre 18 y 60 años de ambos sexos que concurrieron al servicio de Alergia e Inmunología del Hospital Nuevo San Roque, Córdoba, Argentina en el periodo marzo 2016 y marzo 2019. Material y métodos: Se realizó análisis de historia clínica, determinación de IgE, eosinófilos, clasificación de asma según GINA, espirometría, pruebas cutáneas, Test de control de Asma (Asthma control Test) (ACT) y respuesta según Evaluación Global Efectividad Tratamiento por el médico (Global evaluation of treatment effectiveness) (GETE). Resultados: Un total de 12 pacientes asmáticos severos, con test cutáneos positivos (Prick test), valores de IgE y eosinófilos elevados. En los valores espirometricos de pos BD, se observaron diferencias en las tres etapas de seguimiento, los valores iniciales fueron de 50,1 ± 4,4, aumentando a 83,5 ± 4,4 a los 6 meses, y con resultados medios de 88,5 ± 4,15 a los 12 meses. Los valores de ACT fueron al inicio 10,17 ± 1,44, a los 6 meses 22,33 ± 0,68 y al año de 24,67 ± 0,14. Puntaje GETE a los 6 meses, la mayoría de los pacientes refirió tener GETE control completo del asma (42%) o mejor control del asma (33%), mientras que el profesional que los asistió no refirió haber observado ningún paciente con nivel excelente mientras que el 75% fueron clasificados como mejor control del asma. Conclusión: La evaluación inicial correcta del paciente asmático y el tratamiento de precisión adecuado son las herramientas que deben aplicarse para obtener no solamente la remisión de las manifestaciones clínicas si no también una amplia mejoría en su calidad de vida.


ABSTRACT BACKGROUND Severe asthma includes a small percentage of patients that varies across different countries, yet patients with uncontrolled severe asthma have disproportionately high morbidity and healthcare utilization. Objectives: To assess the response to treatment with biologics in severe asthmatic patients and the importance of performing the phenotype and endotype classification in a sample of severe asthmatic patients between 18 and 60 years of age of both sexes who attended the Allergy and Immunology service of the Nuevo Hospital San Roque, Córdoba, Argentina from March 2016 to March 2019. Material and methods :Analysis of: medical history, measurement of laboratory parameters: IgE, eosinophils, asthma classification according to GINA, spirometry, skin tests, Asthma control Test (ACT) and response according to Global evaluation of treatment effectiveness (Global Evaluation Effectiveness Treatment by the doctor). Results: A total of 12 severe asthmatic patients, with positive Prick tests, elevated IgE and eosinophil values. Regarding spirometry: in the post BD values, differences were observed in the three stages of follow-up: the initial spirometry values were 50.1 ± 4.4, increasing to 83.5 ± 4.4 at 6 months, and with mean results of 88.5 ± 4.15 at 12 months. The ACT values were at baseline 10.17 ± 1.44, at 6 months 22.33 ± 0.68 and at one year: 24.67 ± 0.14. GETE score At 6 months, most of the GETE patients reported having complete asthma control (42%) or better asthma control (33%), while the professional who assisted them did not report having observed any patient with an excellent level; and 75% were classified as better asthma control. Conclusion: The correct initial evaluation of the asthmatic patient and the appropriate precision treatment are the tools that must be applied to obtain not only the remission of the clinical manifestations but also a broad improvement in their quality of life.

ABSTRACT

As an effective additive manufacturing technology, 3D printing technology has been widely used in aerospace, construction, machinery manufacturing and other fields, but it is rarely used in the pharmaceutical field, especially in the field of traditional Chinese medicine (TCM). In 2015, Spritam instant tablets produced by 3D printing technology greatly promoted the development and research of 3D printing technology in the pharmaceutical industry. In this paper, we reviewed three types of 3D printers (binder jet, material extrusion and inkjet printing) with high compatibility in the pharmaceutical industry, including the working principles, printing materials, product characteristics and others. In addition, the application prospects and existing problems of 3D printing technology in the production of innovative preparations of TCM, the development of innovative medical devices and the development of innovative functional foods were analyzed. The paper aimed to provide a reference for the development and preparation of innovative TCM by 3D printing technology.

BACKGROUND: Application of three-dimensional printing technology provides an effective supplementary means for minimally invasive and individualized surgery of posterior column fracture of tibial plateau. OBJECTIVE: To summarize the present situation of diagnosis and treatment of posterior column fracture of tibial plateau, and to look forward to the effect of three-dimensional printing technology in the diagnosis and treatment of posterior column fracture of tibial plateau. METHODS: A computer-based retrieval of CNKI, WanFang, and PubMed databases was conducted using the keywords of “the fracture of tibial plateau, posterior column, 3D printing technology” in Chinese and English, respectively. Totally 210 articles were retrieved, and 56 eligible articles were finally included for review based on inclusion and exclusion criteria. The detailed contents included classification of posterior column fracture of tibial plateau, surgical treatment, application status of three-dimensional printing technology and postoperative rehabilitation. RESULTS AND CONCLUSION: Posterior column fracture of tibial plateau is a special type of plateau fracture, so the traditional method may lead to misdiagnosis. However, the solid model based on three-dimensional printing can directly make diagnosis, and analyze the injury mechanism and classification. In the treatment of posterior column fracture of tibial plateau, there are various traditional surgical approaches to choose: The posterior approach (postmedial approach, posterolateral approach), backward medial inverted “L” into the approach, anterolateral/anterolateral approach, anterolateral posterolateral combined approach, arthroscopy minimally invasive treatment and so on, but have not been unified yet. The application of three-dimensional printing technology has further improved the diagnosis and classification of posterior column fracture of tibial plateau, and provides a reference for preoperative guidance of personalized surgical plan, surgical approach selection, estimation of intraoperative bone implant volume and postoperative rehabilitation.

ABSTRACT

@#A biomarker is defined as a biological molecule found in the blood, other body fluids, or tissues that is a sign of normal or abnormal processes or a condition or disease. In cancer research, biomarkers are classified as diagnostic, prognostic, or predictive. The identification and application of biomarkers in clinical practice are important for evaluating their usefulness for clinical diagnosis, treatment and prognostic warning and for determining the biological effects of anti-cancer drugs, and they are currently one of the hottest topics in oncological translational research. Currently, translational research on biomarkers mostly focus on oncological diagnosis and molecular typing, targeted therapy, treatment protocol selection and optimization, prognostic prediction, etc. Here, we review the progress of translational research on treatments based on biomarkers in oral squamous cell carcinoma as well as the clinical application of inhibitors targeting EGFR, PD1, PI3K, WEE1, the Wnt/β-catenin pathway, the SHH pathway, and the ERK pathway. The prospect of research strategies for personalized treatments based on biomarkers in oral squamous cell carcinoma is also discussed.

ABSTRACT

Determining effective traditional Chinese medicine (TCM) treatments for specific disease conditions or particular patient groups is a difficult issue that necessitates investigation because of the complicated personalized manifestations in real-world patients and the individualized combination therapies prescribed in clinical settings. In this study, a multistage analysis method that integrates propensity case matching, complex network analysis, and herb set enrichment analysis was proposed to identify effective herb prescriptions for particular diseases (e.g., insomnia). First, propensity case matching was applied to match clinical cases. Then, core network extraction and herb set enrichment were combined to detect core effective herb prescriptions. Effectiveness-based mutual information was used to detect strong herb-symptom relationships. This method was applied on a TCM clinical data set with 955 patients collected from well-designed observational studies. Results revealed that groups of herb prescriptions with higher effectiveness rates (76.9% vs. 42.8% for matched samples; 94.2% vs. 84.9% for all samples) compared with the original prescriptions were found. Particular patient groups with symptom manifestations were also identified to help investigate the indications of the effective herb prescriptions.

"Seeking truth through narration" means that the doctor listens to the story which the patient tells about the disease, then classifies and integrates the story to obtain the patient's comprehensive and multi-dimensional information about "body, psychology and spirit", "the whole life" and "the whole environment". These help the doctor to analyze the subjective and objective causes for the occurrence and development of the disease, and to find the sufferings on the patient's body, mind, spirit and social adapta-tion so as to provide personalized treatment and care programs for the patient."The whole life"includes the patient's past, present condition and recovering expectation about the diseases. "The whole environment"mostly indicates the patient's circumstance, such residence, working/studying environment, as well as family, education, cultural and economic background. "Body, psychology and soul" are about physical, psychologi-cal and spiritual sufferings. The doctor exams all the above to have a whole picture about the patient and effectively to provide personalized treatment programs.

As one of special groups in social groups,vagrant psychiatric patients have uniqueness.There are many ethical issues in their diagnosis and treatment,such as voluntary principles and involuntary medical treatment,protective constraints,patient's privacy protection,personalized treatment,fairness and so on.Therefore,to protect the fights and interests of psychiatric patients,it should strictly follow the standard of involuntary hospitalization and four principles of medical ethics when they are admitted to hospital;it should give protective constraints and make the diagnosis of mental disorder when they exist impulses and negative risks involved themselves or others.Meanwhile,it should give psychological counseling,take minimum limit to patient as principle,develop personalized treatment plan,provide humanity service,pay attention to the protection of privacy rights and promote early recovery of patient.

ABSTRACT

Human leukocyte antigen G ( HLA-G) is a member of the non-classical HLA classⅠb family. It is considered to play a crucial role in immune tolerance. A unique feature of HLA-G is the struc-tural diversity as surface expressed and as secreted molecules, which is mainly attributed to alternative spli-cing of the primary transcript. HLA-G can promote the invasion and metastasis of tumor cells through various ways. In addition, HLA-G has been described included in exosomes. Exosomes released by most cell types are nano-sized vesicular bodies that contain lipid bilayer and rich contents. As a new marker for diseases, exosomes are extensively involved in the occurrence and development of diseases. Recent studies have found that exosomes can express soluble HLA-G, which reveal a new way by which HLA-G regulates tumor micro-environment. In this review, we focus on the expression of HLA-G on exosomes to provide new thoughts for the early detection and treatment of tumors.

METHODS:Innovative strategies for chronic disease man-agement in internet big data era were explored from respective of pharmacists,based on national condition,referring to internation-al chronic disease management. RESULTS:The development of internet big data era brought thinking of pharmacists about new type chronic disease management mode. Pharmacists established chronic disease patients medication safety management center based on big data platform,and provided full-range and multi-level pharmaceutical care,including real-time intervention,personal-ized pharmaceutical care,human concern and timeless and spaceless patient education,clinical evidence-based medicine. CONCLU-SIONS:Big data-based chronic disease management mode could reduce the risk of drug use,promote rational drug use,and open a new era of“patient-centered”pharmaceutical care conception and pharmacist’s function transformation.

ABSTRACT

Maturity-onset diabetes of the young (MODY) is a monogenic form of diabetes that is characterized by an early onset, autosomal dominant mode of inheritance and a primary defect in pancreatic beta-cell function. MODY represents less than 2% of all diabetes cases and is commonly misdiagnosed as type 1 or type 2 diabetes mellitus. At least 13 MODY subtypes with distinct genetic etiologies have been identified to date. A correct genetic diagnosis is important as it often leads to personalized treatment for those with diabetes and enables predictive genetic testing for their asymptomatic relatives. Next-generation sequencing may provide an efficient method for screening mutations in this form of diabetes as well as identifying new MODY genes. In this review, I discuss a current update on MODY in the literatures and cover the studies that have been performed in Korea.

The outcome of diffuse.Large B-cell lymphoma (DLBCL) has been improved in the last decades and will be raised further with the development of novel agents in the future.Personalized treatment for DLBCL is based on its molecular heterogeneity.Except for concurrent translocation of myc and bcl-2,neither clinical nor biological factor is sufficiently available to be used to guide the deviation of R-CHOP therapy in DLBCL.Gene expression profiling and whole genomic analytic techniques are expected to be important approaches at present to determine personalized treatment for DLBCL.The advance in the field being reported by Laurie H.Sehn from University of British Columbia in the 54 ASH meeting was summarized in the paper.

Rapid development of personalized medicine in cancer treatments needs urgent support of proper tumor biomarkers.Comparing to tissue tests,serum/plasma tumor biomarkers show advantages in easy sample collection and flexibility of detecting methods.Nowadays,large scales of investigations have discovered sufficient blood tumor biomarkers for preclinical diagnosis and evaluation of clinical treatment or prognosis,with the development of screening techniques and strategies.Moreover,tremendous promotion would appear in cancer diagnosis and treatment with future profound investigation and rigorous verification from clinical practice.
