@#Introduction: Diabetes is a chronic metabolic disease and noted to be incidence is intensifying globally and contemplated as epidemic. The study is aimed to assess the coronary artery disease risk profile associated diabetes mellitus patient and to identify the clinical pharmacist care services in the management and to control the risk burden in the clinical practice. Method: A prospective observational study was conducted among the consecutive patients of coronary artery disease associated diabetic patients in a tertiary care teaching hospital over 6 months period. A sample of 150 patients was recruited in the study. Data analysis was done with graph pad prism software 5.01. Results: The present study revealed that coronary artery disease in diabetes was more prevalent in age group between 41-50 years. About 54.66% patients with hyperlipidemia were at risk to develop the coronary artery disease complication. Glycated hemoglobin test was detected in 40% of the patient showing abnormal levels and around 43.33% of patient had an abnormal fasting blood sugar level. The study showed only 32% of patients was prescribed Insulin & oral hypoglycemic agents and 13 % were treated with statins. Conclusion: It could be concluded that the causative factors should be controlled and treated with an early need for amalgamation of clinical pharmacist care services with the health care team on life style modification counseling could ultimately improve the patient health outcomes and also lowers progression of coronary artery disease risk complications among diabetic patients.

ABSTRACT

Apresenta-se uma revisão das definições e metodologias de dispensação elaboradas pelos órgãos públicos reguladores, organizações profissionais e de saúde no Brasil e em outros países e alguns autores brasileiros, sob a perspectiva da contemplação de aspectos técnicos relativos aos medicamentos e a influência da filosofia da atenção farmacêutica. Vários modelos de dispensação consideram os aspectos técnicos relativos aos medicamentos e alguns acrescentam a necessidade de orientação para o uso correto dos medicamentos ou ainda elementos da filosofia da atenção farmacêutica. Mas é necessário rever a definição da dispensação no Brasil para que possam ser criados procedimentos possíveis de serem realizados na prática em uma farmácia comunitária e que cumpram com os objetivos desta atividade farmacêutica.


The present article presents a review of the definitions and methodologies of pharmaceutical dispensing elaborated by the regulating authorities, professional and health organizations in Brazil and abroad and also by some Brazilian authors from the perspective of the technical and philosophical aspects involved. Several dispensing models consider the technical aspects related to the medicines and some add the need for orientation with regard to correct use or even elements of the philosophy underlying the pharmacist care model. However, for creating procedures that can be put into practice in a community pharmacy and that meet the purpose of this pharmaceutical activity it is necessary to rethink the concept of pharmaceutical dispensing in Brazil.

ABSTRACT

Polifarmácia e falta de adesão à prescrição médica são problemas freqüentes na terapêutica farmacológica de idosos que podem prejudicar o resultado do seu tratamento. Realizou-se um estudo transversal exploratório e quantitativo em base populacional para levantar a freqüência de idosos aderentes e avaliar se a polifarmácia interfere na adesão. A pesquisa foi realizada com 466 idosos de Porto Alegre, RS. Os instrumentos foram aplicados por entrevista individual. A freqüência de idosos aderentes foi de 173 (37,1 por cento) e maior entre os que utilizavam menos fármacos. Os resultados encontrados sugerem a implementação de programas educacionais para auxiliar os idosos no seguimento à terapêutica farmacológica.


Polipharmacy and medication non-adherence are problems faced frequently in the treatment of elderly patients. An exploratory cross-sectional study and quantitative approach were conducted to assess the frequency of treatment-adherence in elderly and how polipharmacy can affect adherence. Four hundred and sixty six elderly answered a questionnaire in Porto Alegre, RS in individual interviews. The adherence frequency found was 173 (37.1 percent) and was higher among those, who use less medication. These results indicate the need for implementing educational programs for the elderly in order to help them to follow their drug therapy.
