ABSTRACT

This study aims to evaluate the evolution of the degree of physical disability and the eye, hand and foot score, from diagnosis to medication discharge, according to sociodemographic and clinical variables, in patients diagnosed with leprosy. This is a cross-sectional study, based on the analysis of 71 patients diagnosed with leprosy, followed by Hospital Eduardo de Menezes, a reference center in Belo Horizonte, Minas Gerais, Brazil, between January 2017 and December 2020. There was a predominance of females (53.5%), average age of 46 years old, the majority coming from other municipalities in the interior of Minas Gerais (54.9%). The most frequent clinical form was borderline (64.8%), multibacillary operational classification (84.5%). Degree of physical disability 0 was the most prevalent at diagnosis (56.5%) and at discharge (53.1%); in relation to the eye, hand and foot score, there was variation between 0 and 10 at diagnosis and between 0 and 8 at discharge. By the McNemar test, it was observed that of the patients who had degree of physical disability at the time of diagnosis, 30.8% had no disability at discharge. When comparing the evolution of the GIF in paucibacillary and multibacillary patients at the time of diagnosis and discharge, there was maintenance in 59.3% and improvement in 17.2% of the patients. The presented data indicates that many patients were treated late, already presenting severe forms and with sequelae, since the study was carried out in a reference center, which treats cases with complications already installed and more complex cases. The data suggest that assisted patients undergoing treatment with multidrug therapy may experience improvement in disabilities already established. The same occurred with eye, hand and foot score, at the end of the established treatment there was an improvement compared to admission.(AU)

ABSTRACT

Introduction: The use of computers, tablets, and cell phones with the Internet by people with quadriplegia from spinal cord injuries is much lower when compared to the general population. The quality of life of these people can be substantially improved through access to these technologies, which would allow a quadriplegic to take advantage of the rapid evolution of information and communication. Many of these individuals have the function of preserved neck and mouth muscles, and it is possible to manipulate intraoral devices as an adaptation tool. Case Report: We report an intraoral device installation in a patient with quadriplegia, a victim of a firearm accident, who had a great desire to reuse the tablet for internet access. A device made of acrylic resin, similar to a myorelaxant plate, was designed to allow the use of a computer that was comfortable and at the same time, the patient could talk and not harm the dental structures. The person responsible agreed to participate in the research and signed the consent form. In addition, the work was submitted to an ethics committee. Considerations: The ideal intra-oral device for patient rehabilitation should be inexpensive, easy to adapt and promote muscle relaxation besides allowing the patient to expand their abilities and digitally increase their autonomy for society.

ABSTRACT

Introduction: Disability is considered a health problem worldwide. People with physical disabilities are susceptible to different health problems. Despite being a prioritized group, access to health services is lower than that of the general population, conditioned by the presence of different types of barriers. Objective: To describe the barriers and facilitators of access to primary health care in people with physical disabilities. Methods: A bibliographic review was carried out whose methodology was based on the search, selection, review, interpretation and synthesis of the scientific evidence related to the research problem. It was used as a source of primary scientific literature published in the databases Scielo, Redalyc, Latindex and PudMed in the last 5 years, with the MeSH search terms in Spanish, English and Portuguese. Results: A total of 67 documents were identified, of which 46 were used in the research carried out. The remaining 21 manuscripts were excluded due to methodological deficiencies that limited their use. Conclusions: The access of people with physical disabilities to health services is conditioned by situations that are interpreted as barriers or facilitators. In this sense, the factors related to the national health system and the characteristics and environment of the person with a disability can act as a barrier or as a facilitator, depending on their expression(AU)

ABSTRACT

There exist many psychosocial sequelae associated with mobility impairment, especially in low-resource settings where access to mobility assistive devices is limited. Objectives: This study aims to (1) define the burden and presenting aetiologies of mobility impairment in the rural Northern Region of Malawi and (2) assess the relationship between physical disability, life satisfaction and access to mobility aids. Methods: At mobility device donation clinics throughout the Northern Region of Malawi, adults living with mobility impairment were surveyed with a demographic questionnaire and a series of validated surveys to assess their physical activity levels (Global Physical Activity Questionnaire [GPAQ]), degree of mobility impairment (Washington Group Extended Set Questions on Disability) and life satisfaction (patient-reported outcomes measurement information systems satisfaction with participation in social roles and general life satisfaction). Results: There were 251 participants who qualified for inclusion, of which 193 completed all surveys. Higher physical activity scores were positively correlated with increased life satisfaction: (1) satisfaction with participation in social roles (0.481, p < 0.0001) and (2) general life satisfaction (0.230, p < 0.001). Respondents who had previously used a formal mobility device reported 235.5% higher physical activity levels ([139.0%, 333.0%], p = 0.006), significantly higher satisfaction with participation in social roles ([0.21, 6.67], p = 0.037) and equivocally higher general life satisfaction ([−1.77, 3.84], p = 0.470). Conclusion: Disability and mental health do not exist in isolation from one another. Given the positive correlations between formal mobility device usage and both physical activity and life satisfaction, interventions that increase access to mobility-assistive devices in undertreated populations are imperative. Contribution: This study contributes to the understanding of the complex relationship between physical disability, access to mobility aids, and life satisfaction. Results from this study suggest the potential benefit that increasing access to mobility aids may have in improving the quality of life of mobility impaired persons in resource-limited settings, such as the Northern Region of Malawi

ABSTRACT

Abstract Aim: Through reports of the athletes with disabilities interviewed, this study aimed to understand how the experiences lived in mainstream sports occur among people without disabilities. Methods: Husserl's classic phenomenology was the methodological framework adopted. The phenomenological interview was carried out with an intentional sample that included six athletes with some type of physical disability and later its recording was transcribed for the procedures of phenomenological reduction and intentional crossing to explain the meaning of what is experienced by these athletes in mainstream sport. Results: Five categories essentially describe how these experiences occur: operational body barred in the world; shaping the movement; the invisibility of disability; determination stimulus; and normalization of social relations in mainstream sports. These experiences correspond to a dynamic process in which each part does not necessarily follow the other. Conclusion: The experience in mainstream sports allowed the interviewees to improve their experience of capacity, self-efficacy, and recognition through the mutuality between self-perception and the expectation of acceptance by society.

ABSTRACT

The objective of this study was to stratify risk factors for cardiovascular diseases (CVD) in ten years in wheelchair basketball players. The percentage of cardiovascular risk was stratified by the Framingham Score (FS) and Global Risk Score (GRS). Of the thirteen players evaluated, 38.46% were overweight and obese and 77%, presented changes in body fat percentage and waist circumference. The GRS identified 15.38% of the players with intermediate risk for developing CVD and according to GRS, 15.4% of men presented intermediate risk, 7.7% high risk for diabetes and 7.7% very high risk for atherosclerotic diseases. The results of body composition demonstrate a predisposition for the development of CVD and the ERG identified individuals with high risk and very high risk. It is concluded that the identification of risk factors is useful for a greater prevention of CVD and appropriate advice on the adoption of healthy lifestyle habits by these individuals.

ABSTRACT

ABSTRACT Introduction Sitting volleyball is an adapted sport played by players in the seated position, due to impaired mobility or amputations. Court dimensions are adjusted to the players' average height, and during the games, the actions performed are similar to those of traditional volleyball. Objective To analyze the tactical performance of lower limb amputees and physically disabled athletes during seated volleyball games. Methods Data were collected from video footage, by a single observer, of twelve games of the Brazilian north-northeast championship 2017. The analysis investigated: i) average length of rallies; ii) frequency of attack actions; iii) game actions by amputation type; iv) outcomes of the actions by amputation type; v) direction and frequency of movements by disability. Results The average duration was 70 rallies per game, with the minimum disabled athletes performing the highest number of attack actions (43). Transfemoral amputees performed the highest number of blocking actions per game (10.5, in average). Conclusion Sitting volleyball has characteristics that are determined mainly by the type of disability. Players with minimal disabilities perform more attack actions and have higher performance per action, while amputees perform actions closer to the net, such as blocking and blocking points. The results also suggest that the type of amputation influences the direction of movement. Level of evidence II; Study Prospective.

ABSTRACT

The social model has considered the concept of emancipation in the field of social participation of people with disabilities that is less frequently investigated. This study aimed to explore the facilitators and barriers of emancipation process in persons with physical disability.METHODSThis grounded theory study was conducted in the welfare originations of Tehran and Zanjan from October 2017 to September 2018. The study was conducted using semi‑structured interviews of 12 participants with physical disabilities, two family members, one nurse, one physician, and two employees of the Welfare Organization. Purposeful sampling was performed with maximum variation. All interviews were transcribed verbatim and data were analysed using the Strauss and Corbin 2008. The study continued until data saturation occurred.RESULTSThe analysis of the data led to the extraction of 2067 codes, 25 subcategories and 8 main categories. The study findings appeared in 4 main categories for barriers and 4 main categories for facilitators. Barrier categories include non-supportive community, unprotected interactions between family, person and self-disability. Facilitating categories includes flexible environment, supportive programs, emancipated family and individual competencies.CONCLUSIONSThere are barriers and facilitators to the process of emancipation, the most important of which are individual factors. So individual motivational interventions with physical disabilities can be effective in achieving the emancipation process.

ABSTRACT

Objective:The Nagasaki Prefectural Centre of Medicine and Welfare for Children was established in 1954 as a center for children with physical disabilities in Nagasaki prefecture. Medical education consultation in the local area of Nagasaki prefecture was started in the following year. Methods:We examined the number of patients and the area selected for this project over the recent 36 years. Documents from the center's library were examined, and orthopedic physicians who had previously participated in the project were interviewed. Results:The number of patients and visiting areas in the project continuously decreased. This patient tendency was caused by the declining birth rate in Japan and the satisfaction from human services in local areas. The decrease in the number of local areas visited was affected by the financial administration of our center and the development of a traffic network in the mainland of Nagasaki prefecture. Health visitors now working in the island continue to require visits to pediatric specialists from the mainland twice a year. The number of visits to four selected isolated islands has been maintained till date. Conclusion:Medical support from the mainland is necessary for isolated islands because of the absence of physicians, especially those for children with physical disabilities, and visiting the hospital in the mainland by airplane is difficult for these children.
