ABSTRACT

Early internal fixation for intertrochanteric fracture of the femur can improve the quality of patients′ life and reduce disability and mortality. In the past, treatment of intertrochanteric fracture of the femur had some problems, such as head and neck screw cut-out, hip varus deformity, nail withdrawal, femoral neck shortening, and internal fixator fracture, etc., for which no reasonable explanation could be provided. The "lateral wall" theory has been recognized by most scholars and used to guide the development and clinical application of internal fixation, which has not entirely avoided the occurrence of the complications mentioned above. A correct understanding of the mechanics of the lateral wall of the proximal femur helps choose a reasonable internal fixation method for the treatment of intertrochanteric fracture of the femur to reduce the incidence of complications. The author firstly discusses the lateral wall measuring and fixation strengthening methods, reasonably explore the reasons for postoperative complications according to the theory of lever-pivot balance, and proposes that the proximal femoral bionic nail (PFBN) is an ideal choice for the treatment of intertrochanteric fracture of the femur.

Resumen: Introducción: El ligamento cruzado anterior es una estructura anatómica que tiende a dar problemas a los especialistas al formular un diagnóstico certero. Existen herramientas de medición que apoyan el diagnóstico como el KT1000; sin embargo, aún presenta algunas limitaciones, pues sólo logra medir un solo eje de movimiento, mientras que la maniobra de pivot-shift logra capturar más información sobre el estado del ligamento, ya que permite evaluar los tres ejes de movimiento de la articulación. La maniobra de pivot-shift carece de una ejecución estándar, por ello puede producir discordancia entre evaluadores; así se crea la necesidad de una herramienta capaz de estandarizar la maniobra al tiempo que arroje una medición interpretable. Material y métodos: La aplicación «pivot-shift meter¼ es un software para móviles capaz de capturar información de los movimientos realizados al ejecutar una maniobra de pivote. Se utilizó el índice de correlación intraclase para medir la correlación intra- e interobservador de la información capturada por la aplicación. Los datos fueron obtenidos de 66 participantes aleatorios, mediante el giroscopio integrado del teléfono móvil. Resultados: Con los 66 ensayos realizados se observó que los evaluadores tienden a alcanzar una alta reproducibilidad al ejecutar las maniobras dentro del periodo de tiempo establecido por la aplicación, que alcanzó un índice de correlación intraclase por encima de 90%. Conclusión: La correlación intra- e interobservador es lo suficientemente alta como para poder realizar un análisis cuantitativo de la maniobra de pivot-shift a través de un teléfono móvil.


Abstract: Introduction: The anterior cruciate ligament is an anatomical structure that tends to cause problems for specialists when formulating an accurate diagnosis. There are measurement tools that support the diagnosis such as the KT1000; however, it still has some limitations, since it only manages to measure a single axis of movement, while the «pivot-shift¼ maneuver manages to capture more information about the state of the ligament, as it allows to evaluate the three axes of movement of the joint. The «pivot-shift¼ maneuver lacks a standard execution, therefore it can produce disagreement between evaluators; this creates the need for a tool capable of standardizing the maneuver, while providing an interpretable measurement. Material and methods: Pivot-shift meter application, a mobile software capable of capturing information on the movements made when executing a pivot maneuver. Intraclass correlation coefficient to measure the intra- and inter-observer correlation of the information captured by the application. The data was obtained from 66 random participants, using the mobile phone's built-in gyroscopes. Results: With the 66 tests carried out, it was observed that the evaluators tend to achieve high reproducibility when executing the maneuvers within the time period established by the application, which reached an intraclass correlation coefficient above 90%. Conclusion: The intra- and inter-observer correlation is high enough to be able to perform a quantitative analysis of the pivot-shift maneuver, through a mobile phone.

ABSTRACT

BACKGROUND: Personalized positioning guide plate based on three-dimensional CT data can improve the accuracy of posterior-stabilized prosthesis joint replacement, but bone structure reconstruction based on three-dimensional CT scan data ignores the thickness of distal femoral articular cartilage, which will affect the use of personalized osteotomy guide plate and the clinical effect of total knee arthroplasty. OBJECTIVE: To investigate the safety, accuracy and early clinical efficacy of three-dimensional printed screw positioning guide based on MRI and three-dimensional CT assisted medial pivot-total knee arthroplasty in the treatment of osteoarthritis. METHODS: A total of 110 patients with knee osteoarthritis admitted to Department of Orthopedics, Nanjing Hospital Affiliated to Nanjing Medical University from February 2018 to April 2019 were selected. All patients received medial pivot-total knee arthroplasty, and were randomly assigned to two groups. The 58 patients in the experimental group completed knee osteotomy with the assistance of personalized osteotomy guide plate based on MRI and three-dimensional CT. The 52 patients in the control group completed knee osteotomy with the traditional intramedullary and extramedullary positioning methods. The operation time and intraoperative blood loss of the two groups were recorded. Postoperative follow-up was conducted to review the X-ray films. KSS score of knee function and the visual analogue scale score were evaluated. RESULTS AND CONCLUSION: (1) 110 patients were followed up for 6-10 months without serious postoperative complications. (2) The operation time and intraoperative blood loss in the experimental group were less than those in the control group (P 0. 05). (5) The results showed that the personalized osteotomy guide plate assisted internal axial knee prosthesis replacement based on MRI and three-dimensional CT could effectively improve the accuracy of prosthesis placement and shorten the operation time. The method was safe and feasible, and the early clinical effect was satisfactory.

ABSTRACT

Introducción La artroplastia total de rodilla es un procedimiento exitoso que mejora significativamente la calidad de vida de los pacientes disminuyendo el dolor e incrementando su capacidad funcional. Sin embargo, la literatura reporta hasta 25% de pacientes insatisfechos. El objetivo de este estudio es presentar los resultados clínicos y la satisfacción de un grupo de pacientes operados con implantes de tipo pivote medial y ultra-congruente. Materiales y métodos Se realizó un estudio observacional retrospectivo. Se revisaron las historias clínicas de todos los pacientes operados con los sistemas mencionados con mínimo 12 meses post-operatorios. Se analizaron resultados de 100 rodillas (96 pacientes) que completaron los cuestionarios, con una media de seguimiento de 33.5 meses. Se aplicaron los cuestionarios KSS, KOOS y WOMAC. Se estudiaron diferencias en dolor y función entre la etapa pre-operatoria y el último seguimiento. Se analizó la satisfacción de los pacientes con respecto al dolor y la capacidad funcional. Se evaluaron complicaciones presentadas. Resultados Todos los criterios presentaron una mejora estadísticamente significativa entre la etapa pre-operatoria y el último seguimiento. 97% de los pacientes reportaron estar satisfechos con los resultados. Las medias de los cuestionarios fueron entre 83.97 y 95.3. Se presentó 1 caso de revisión por infección peri-protésica. Discusión Las prótesis de pivote medial y ultra-congruente constituyen opciones eficaces y seguras para el tratamiento de artrosis de rodilla. Es necesario realizar estudios prospectivos y comparativos que generen evidencia de más alto nivel para confirmar los beneficios de estos diseños.


Background Total knee arthroplasty is a successful procedure that improves patient quality of life by reducing pain and increasing their functional capacity. However, literature reports up to 25% dissatisfaction in patients. The objective of the study is to present the clinical results and satisfaction of a group of patients operated on using ultra-congruent medial pivot-type implants. Methods A retrospective observational study was carried out. The medical records of all the patients who underwent knee arthroplasty surgery with a specific system were reviewed, including patients with a minimum of 12 months follow up. An analysis was performed on the results of 100 knees (96 patients) that had completed questionnaires. There was a mean follow-up of 33.5 months. The KSS, KOOS, and WOMAC questionnaires were used. Differences in pain and function between the pre-surgical stage and the last follow-up were studied. An analysis was made on patient satisfaction with regard to pain and functional capacity. Specific complications were evaluated. Results All evaluation criteria showed a statistically significant improvement between the pre-surgical stage and the last follow-up. Almost all (97%) of patients reported being satisfied with the results. The means of the questionnaires were between 83.97 and 95.3. One case of revision due to peri-prosthetic infection was presented. Discussion Medial pivot and ultra-congruent prostheses designs are effective and safe options for the treatment of osteoarthritis of the knee. Prospective and comparative studies that generate higher level evidence are necessary to confirm the benefits of these designs.

ABSTRACT

Irregular rainfall can restrict crop development, so irrigation of pastures serves as a strategy to intensify grass-fed animal production systems. Due to the direct relationship between the availability of soil water and nutrients, the objective was to evaluate the nutrient content in forages irrigated by a central pivot system. This work was carried out in the experimental area of irrigated agriculture at the State University of Mato Grosso do Sul, in Aquidauana-MS, Brazil. The experiment was setup in a completely randomized design, with five replications, in subdivided plots. The treatments of the plots were irrigated and non-irrigated, and the treatments of the subplots were the foragesUrochloa brizantha cv. Xaraes, Pennisetum purpureum cv. Napier, Panicum maximum cv. Mombasa and Urochloa ruziziensis cv. Ruziziensis. Foliar concentrations of the nutrients Nitrogen (N), Phosphorus (P), Potassium (K), Calcium (Ca), Magnesium (Mg), Sulfur (S), Boron (B), Copper (Cu), Iron (Fe), Manganese (Mn) and Zinc (Zn) were evaluated. Statistical analyses performed included analysis of variance, Tukey test at p <0.05 and linear correlation matrix at p <0.05 and p <0.01 between nutrients and productivity. Irrigation results in higher levels of the macronutrients P, K and Mg, and Fe and Mn micronutrients in forages. The nutrients that correlated with productivity were P and Fe, which both presented negative correlation. The nutrients correlated better with each other than with forage productivity


A irregularidade do regime pluviométrico pode tornar-se uma restrição ao desenvolvimento das culturas, de maneira que a irrigação de pastagens serve como estratégia para a intensificação dos sistemas de produção animal a pasto. Devido a relação direta entre a disponibilidade de água no solo e os nutrientes, objetivou-se avaliar os teores dos nutrientes em forrageiras irrigadas por pivô central. O trabalho foi realizado na área experimental de agricultura irrigada da Universidade Estadual de Mato Grosso do Sul, em Aquidauana-MS. Adotou-se o delineamento inteiramente casualizado, com cinco repetições, em parcelas subdivididas. Os tratamentos das parcelas foram, irrigado e não irrigado, e os tratamentos das subparcelas, foram as forrageiras Urochloa brizantha cv. Xaraés, Pennisetum purpureum cv. Napier, Panicum maximum cv. Mombaça e Urochloa ruziziensis cv. Ruziziensis. Foram avaliadas as concentrações foliares dos nutrientes Nitrogênio (N), Fósforo (P), Potássio (K), Cálcio (Ca), Magnésio (Mg), Enxofre (S), Cobre (Cu), Ferro (Fe), Manganês (Mn) e Zinco (Zn). Foi realizada a análise de variância, teste Tukey p<0,05 e matriz de correlação linear p<0,05 e p<0,01 entre os nutrientes e a produtividade. A irrigação propicia maiores teores dos macronutrientes P, K e Mg e dos micronutrientes Fe e Mn nas forrageiras. Os teores de nutrientes que correlacionaram com a produtividade foram o P e o Fe, ambos apresentaram correlação negativa. Os teores de nutrientes correlacionaram melhor entre si do que com a produtividade das forrageiras

ABSTRACT

Depression is related to stagnation in the body. At present, the treatment with acupuncture for depression focuses on the theories of the governor vessel, liver, spleen, stomach and the entity of five organs, especially for the adult group. However, the attention to adolescent depression is insufficient. It is recorded in that meridian is taken as the pivot of three meridians, dominates the regulating of the ascending and dispersing of and plays the key role in treatment. The authors believe that starts growing at the period of youth, to which s meridian is corresponded. It is viewed that adolescent depression is closely related to the pivot function of . In this paper, based on the theory of "taking as the pivot", the mechanism of adolescent depression and the acupoint selection in acupuncture treatment are explored so as to utilize this theory in the treatment of adolescent depression with acupuncture.

ABSTRACT

ABSTRACT: The aim of the present study is to evaluate the energy balance and energy efficiency of the silage maize crop in the Center for Research, Development and Technology Transfer of the Universidade Federal de Lavras (CDTT-UFLA). The crop was irrigated by center pivot and the stages of maize cultivation and energy inputs were monitored for the 1st and 2nd crops of the 2014/2015 harvest. Results from the energy analysis showed the crop had a total energy input of 45,643.85 MJ ha-1 and 47,303.60 MJ ha-1 for the 1st and 2nd crops and a significant predominance of direct energy type (about 92% of the matrix). Regarding direct energy inputs, the diesel oil was the most representative, contributing with approximately 38% of the total energy demand. Conversely, the irrigation system contribute with 3.92% e 5.97% in the 1st and 2nd crops, representing the largest indirect energy input. Nevertheless, irrigation and crop management allowed the system achieving high levels of productivity, resulting in an energy efficiency of 25.1 and 28.1 for the first and second crops respectively.


RESUMO: Este trabalho tem como objetivo avaliar o balanço energético e a eficiência energética da cultura milho para silagem implantada no Centro de Desenvolvimento e Transferência de Tecnologia da Universidade Federal de Lavras (CDTT-UFLA). A cultura foi irrigada por pivô central sendo que as etapas de cultivo do milho e entradas de energia foram acompanhadas para 1ª e 2ª safras do ano agrícola de 2014/2015. Os resultados da análise energética mostraram que a cultura teve uma entrada total de energia de 45.643,85MJ ha-1 e 47.303,60MJ ha-1 para a 1ª e 2ª safra, com uma predominância significativa de energia do tipo direta (cerca de 92% da matriz). O óleo diesel foi a fonte de energia direta que mais impactou na matriz, com uma participação de aproximadamente 38%. Já a irrigação teve uma participação de 3,92% e 5,97% na 1ª e 2ª safra respectivamente, representando a maior fonte de energia indireta. Apesar disso, a irrigação e o manejo da cultura permitiram que o sistema alcançasse altos níveis de produtividade, resultando em uma eficiência energética de 25,1 e 28,1 para a 1ª e 2ª safra.

ABSTRACT

In , had used Qimen (LR 14) for the most times compared with other acupoints among the limited numbers of acupuncture prescriptions. Qimen (LR 14) is the front- point of the liver meridian, acting on smoothing liver and regulating activity. It is commonly used to treat the disorders caused by retarded circulation of and blood, such as distension and pain of the hypochondriac region and masses. Based on the opening-closing-pivot theory of the six meridians, the specificity of the running course of the meridian and the property of Qimen (LR 14), the mechanisms of Qimen (LR 14) were analyzed on the treatment of febrile diseases, aiming to enhance the considerations and application of this points in clinical practice and contribute to the further understanding of ' s academic thoughts on the syndrome differentiation in the treatment of diseases.

Objective To evaluate the effectiveness of anterior cruciate ligament (ACL) reconstruction combined with anterolateral ligament(ALL)reconstruction in ACL injury patients with high-grade pivot shift.Methods From May 2015 to April 2016,156 patients underwent ACL reconstruction by the same surgeon,and 22 of them with grade 2/3 pivot shift were included in this study.Anteroposterior knee stability was evaluated using KT1000 measurement,and the rotatory stability was assessed using the pivot-shift test.The Lysholm score was used to monitor the clinical function.Results There were 14 male subjects and 8 females,with an average age of 29.3 years.The mean follow-up period was 8.6 months.The mean side-to-side difference of anteroposterior knee laxity was 2.1 ± 0.6 mm,significantly improved compared with the preoperative 8.9 ± 3.1 mm.The preoperative pivot-shift indicated 2+ in 20 patients and 3+ in 2 patients,while at the final follow-up,21 patients had negative pivot shift with 1 of 1+ pivot shift.The difference was significant.The average Lysholm score improved significantly from 60.5 ± 12.3 preoperatively to 79.2 ± 7.8 at the final follow-up.Conclusion The rotatory instability can be effectively restored through the ACL reconstruction combined with ALL reconstruction in patients with high-grade pivot shift.The early-stage knee stability and functional outcomes indicate significant improvement postoperatively.

ABSTRACT

Objective To evaluate the efficacy of pivot-regulating manipulations applied in traditional Chinese medicine (TCM) clinical nursing for infantile anorexia based on the meridian and acupoint theory. Methods One hundred and twenty children with anorexia were randomly divided into trial group and control group , 60 cases in each group. The control group was given spine-pinching therapy based on the conventional nursing care. The trial group was given pivot-regulating manipulations based on the conventional nursing care , and the pivot-regulating manipulations covered manipulating spleen -stomach meridian , regulating Neibagua, kneading Banmen, kneading abdomen, manipulating acupoints of Zhongwan, Tianshu and Zusanli, and pinching spine according to the deficiency-excess differentiation, once every other day and 3 times constituting one course of treatment. After treatment for 2 courses, the clinical efficacy of the two groups was evaluated. The changes in TCM symptom scores and physical examination indexes of body height, body mass, head circumference, chest circumference, height-for-age (HT/A), weight-for-age (WT/A) in the two groups were observed before and after treatment. Results(1) Five cases(3 differentiated into excess syndrome and 2 into deficiency syndrome) of the trial group dropped out and quitted from the trial, and 6 cases of the control group dropped out and quitted from the trial. (2) After two courses of treatment, the total effective rate of the trial group was 94.55%, and that of the control group was 87.04%, the difference being significant (P < 0.05). (3) After treatment, the scores of appetite and food-intake amount, complexion and lip color, water intake, restlessness, and urination as well as TCM symptom scores in both groups were improved; the WT/A of the trial group was also improved (P < 0.05 compared with those before treatment). The trial group had better effect on improving the scores of appetite and food-intake amount, complexion and lip color, and restlessness as well as TCM symptom scores than the control group (P < 0.05 or P < 0.001). The trial group also showed better effect on improving body mass, body height, head circumference, and HT/A than the control group(P<0.05). Conclusion As a nursing method based on the meridian and acupoint theory as well as syndrome differentiation and treatment, pivot-regulating manipulations are effective for the treatment of infantile anorexia by quickly starting an effect and obviously relieving the symptoms.

ABSTRACT

Objective To evaluate the early clinical outcomes of anterior cruciate ligament(ACL)reconstruction combined with anterolateral tenodesis procedure in patient with high grade pivot-shift after ACL injuries.Methods Among the 140 patients diagnosed as ACL injury and undergoing operations in our hospital between June and December 2015,18 presenting high grade pivot-shift 2+ or 3+ under anesthesia were included into this study.During their surgeries,the anterolateral tenodesis procedure using iliotibial tract ITB was applied together with ACL reconstruction.The anteroposterior stability of the knee was examined and pivot-shift test was conducted right after the operation as well as 6 and 12 months after that.The Lysholm score was used to evaluate the clinical function of their knees.Results The mean follow-up period of the 18 patients was 12.7 ± 2.4 months.There were 14 males and 4 females,with an average of 28.3 ± 7.1 years.No patients complained about unstable symptoms during the follow-up.The mean side-to-side difference of anteroposterior knee laxity assessed using KT1000 at 30 degree knee flexion was 2.3 ± 0.7 mm,which was significantly improved compared with preoperative assessment(9.2 ± 2.6 mm)(P<0.01).For the preoperative examination,the pivot-shift examination indicated 2+ in 13 patients and 3+ in 5 patients.However,all were negative right after the operation.Seventeen patients presented negative pivot-shift at the final follow-up,while 1 had 1+ rotational laxity.Significant differences were found in the Pivot-shift test before and after the operation.The average Lysholm score improved significantly from 67.7 ± 5.9(ranging from 55 to 78)before the operation to 89.0-± 4.6(ranging from 75 to 95)after that.Conclusion The high grade pivot-shift rotational instability could be effectively restored using the anterolateral tenodesis procedure,in addition to ACL reconstruction.The early stage clinical outcomes indicate significant improvement of the knee stability postoperatively.

In this paper, the opinions of previous researches were summarized; based on this, the viewpoints of each opinion were reviewed, and the authors' opinion on method to differentiate "" diseases and "" disease was proposed. It was concluded the differences of "" diseases and "" disease inwas only based on style of the composing, not involving medical principles. The role of two parts on diagnosis and treatment was similar.

ABSTRACT

Foi avaliada a viabilidade econômica de três sistemas de produção por meio da mensuração dos custos de produção e da análise dos indicadores financeiros. Os três sistemas (S1, S2 e S3), situados na fronteira oeste do Rio Grande do Sul, foram avaliados no período de outubro de 2010 a outubro de 2011. A pastagem estabelecida no S1 foi de azevém (Lolium multiflorumLam)e trevo branco (Trifolium repensLam); no S2, trevo branco, trevo vermelho (Trifolium pratenseLam), cornichão (Lotus corniculatusLam), azevém e aveia preta (Avena strigosaSchreb); e no S3, azevém, cornichão, trevo branco e trevo vermelho. Os animais foram pesados na entrada e na saída das pastagens. A coleta de dados para mensuração do custo foi realizada mensalmente utilizando-se planilhas apropriadas. As produtividades foram de 369, 772 e 637kg/ha para os sistemas S1, S2 e S3, respectivamente. O S2 apresentou o maior custo total (R$ 2.101,59/ha), enquanto o menor foi obtido pelo S1 (R$ 1.626,93/ha). Na análise do custo operacional médio dos três sistemas, os itens que apresentaram maiores valores foram a implantação de pastagem, a energia elétrica, a manutenção da pastagem e a depreciação. Quanto às margens, os sistemas S1, S2 e S3 apresentaram, respectivamente, R$ 33,46, 1.162,55 e 701,01/ha para margem bruta; R$ -63,51, 955,40 e 541,05/ha para margem operacional; e R$ -583,18, 291,61 e -28,99/ha para lucro total. A rentabilidade foi de -0,47, 4,37 e 3,01% e a lucratividade foi de -5,94, 39,92 e 27,38% para o S1, S2 e S3, respectivamente. A irrigação de pastagens para terminação de bovinos é viável economicamente, no entanto a sua adoção deve sempre visar a altas produtividades zootécnicas devido ao seu elevado custo de produção.


The economic viability of three systems of production was evaluated through the measurement of costs and analysis of financial indicators. The three systems (S1, S2 and S3), located on the Western Border of Rio Grande do Sul, were evaluated from October 2010 to October 2011. The pasture established is S1 was ryegrass (Lolium multiflorum Lam) and white clover (Trifolium repens Lam), in S2, white clover, red clover (Trifolium pratense Lam), Bird's-foot trefoil (Lotus corniculatus Lam), ryegrass and oats (Avena strigosa Schreb), and in S3, ryegrass, birdsfoot trefoil, white clover and red clover. The animals were weighed before and after the pastures. Data collection was conducted monthly, using spreadsheets suitable for the measurement of production cost. Productivities were 369, 772 and 637 kg/ha for systems S1, S2 and S3, respectively. S2 had the highest total cost (R$ 2101.59/ha), while the lower was obtained by S1 (R$ 1626.93/ha). In the analysis of the average operating costs, items that showed the highest values were the establishment of pasture, electricity, maintenance of pasture and depreciation. As for margins, the systems S1, S2, and S3 were, respectively, R$ 33.46, 1162.55 and 701.01/ha for gross margin; R$ -63.51, 955.40 and 541.05 ha-1 for operating margin; and R$ -583.18, -28.99 and 291.61 ha-1 for total profit. The return was -0.47, 4.37 and 3.01% and profitability was -5.94, 39.92 and 27.38% for the S1, S2 and S3 respectively. The positive result found in the financial evaluation of the three systems studied indicates the economic viability of irrigated pasture in finishing cattle. Irrigation of pasture for finishing cattle is economically feasible, however, its application should always aim high yields husbandry due to their high cost of production.

OBJECTIVE:To establish a fast,accurate,programmed and automatic mode for the statistical analysis of drug in-formation data so as to achieve the compatibility and the switching among multiple forms and the sharing of data information. METHODS:Common Excel commands and functions as well as their use methods were introduced and analyzed combined with drug information statistics cases. RESULTS & CONCLUSIONS:By taking steps such as exporting raw data,designing drug infor-mation sheets and summarizing drug raw data with the pivot table,the data of the drugs purchased by the hospital in a manner of centralized bidding and national essential drugs purchased can be calculated on the basis of different periods of time to fast and accu-rately search for drug data,facilitate data induction and processing and shorten statistics cycle. The above-mentioned steps are well applicable,easy to master,efficient and fast,reliable in the check of results and data,and suitable for the statistics for hospital drug information and other fields.

ABSTRACT

Objective To observe the effect of Jianpi Tiaozhong Xiaopi Recipe ( JTXP) for the treatment of functional dyspepsia ( FD) with spleen deficiency and Qi stagnation. Methods A total of qualified 110 cases were evenly randomized into Chinese medicine group and western medicine group. Chinese medicine group was given modified JTXP orally, and western medicine group was given Mosapride and Lansoprazole orally. After treatment for one continuous month, therapeutic effect was evaluated and the improvement of clinical symptoms before and after treatment was observed. After follow-up for half a year, the recurrence of FD was observed. Results ( 1) After treatment for one month, the scores of abdominal distension and fullness, gastric vague pain, poor ap petite, belching and hiccup were obviously decreased in both groups compared with those before treatment ( P<0.05) , and the relief of the symptoms in Chinese medicine group was superior to that in western medicine group (P<0.05). (2) The total effective rate was 94.5% in Chinese medicine group and was 81.8% in western medicine group, the difference being significant ( P<0.05). ( 3) The results of follow up showed that recurrence of FD was shown in 3 cases (5.8%) of Chinese medicine group and in 8 cases (17.8%) of western medicine group, the difference being significant ( P<0.05). ( 4) During the treatment, no obvious adverse reaction was shown in both groups. Conclusion JTXP exerts certain therapeutic effect for the treatment of FD with spleen deficiency and Qi stagnation.

ABSTRACT

PURPOSE: To evaluate minimum 5-year follow-up clinical and radiological results of total knee arthroplasty (TKA) using a posterior cruciate ligament sacrificing (PS), non-substituting Advance Medial Pivot Knee. MATERIALS AND METHODS: One hundred and twenty knees in 80 patients who could be followed up for more than 5 years after TKA using the PS Advance Medial Pivot Knee were evaluated retrospectively. The evaluations included the preoperative and postoperative range of motion (ROM), tibiofemoral angle, Knee Society (KS) knee and function scores, and Western Ontario and McMaster Universities Arthritis Index (WOMAC) score. The Kaplan-Meier method was used for survival analysis. RESULTS: The ROM increased from a preoperative mean flexion contracture of 7.6degrees and further flexion of 115.1degrees to a postoperative mean flexion contracture of 1.5degrees and further flexion of 120.5degrees. The tibiofemoral angle was changed from 4.6degrees varus preoperatively to 5.8degrees valgus postoperatively. The KS knee and function scores as well as WOMAC score significantly improved after surgery (p<0.05). Complications developed in 4 cases (3.3%): 2 cases of periprosthetic patellar fracture (1.7%) and 2 cases of aseptic loosening (1.7%). The seven-year survival rate was 98.1% in the Kaplan-Meier survival analysis. CONCLUSIONS: The minimum 5-year follow-up results of TKA using the PS Medial Pivot Knee were satisfactory.
