ABSTRACT

Objective To investigate the changes of the difference between hematocrit (Hct) and plasma albumin (Alb) in the course of patients with systemic capillary leak syndrome (SCLS). Methods 281 case reports on human vascular leaking from the PubMed database from January 1st, 1996 to September 30th, 2015 were screened by systematic review method. Studies related to intracranial vascular leakage or intraocular vascular leakage were excluded. 213 articles related to SCLS were identified (164 in English, 16 in French, 8 in Japanese, 7 in German, 7 in Spanish, 4 in Italian, 2 in Chinese, 2 in Danish, 2 in Dutch, and 1 in Swedish). Due to the unavailable full text, 40 articles were excluded. A total of 173 articles related to SCLS were screened, of which 84 patients were enrolled. The data of Alb, Hct, age, gender, weight change, the length of hospital stay and 24-hour fluid infusion volume in SCLS patients were recorded, and the difference between Hct and plasma Alb (Hct-Alb) was calculated. According to the time when accurate Hct and Alb data were collected, they were divided into three groups: basic value group before onset, value group at onset and value group at recovery/discharge after onset. The levels of Hct and Alb and Hct-Alb at different time points in the course of the disease were compared. Pearson test was used to analyze the correlation between Hct-Alb and 24-hour fluid infusion volume. Results ① A total of 12 cases with both exact values of Alb and Hct [or hemoglobin (Hb)] at the time of onset and recovery after treatment were selected from 84 cases of SCLS. It was shown that the Hct-Alb at the time of onset was significantly higher than that after treatment (26.33±16.36 vs. 0.55±8.81, P < 0.001). ② A total of 17 cases with both the pre-onset baseline value and the exact values of Alb and Hct (or Hb) at the time of onset were selected from 84 cases of SCLS. It was shown that the Hct-Alb at the time of onset was significantly higher than that of the pre-onset basic value (15.83±11.37 vs. 1.82±7.97, P < 0.001). ③ A total of 14 cases with both exact values of Alb, Hct and 24-hour fluid infusion volume at the time of onset were selected from 84 cases of SCLS. It was shown that the Hct-Alb was 35.45±19.58 at the time of onset. The average 24-hour fluid infusion volume was (9.82±4.95) L, and the maximum volume of fluid infusion was 20 L. Pearson correlation analysis showed that the Hct-Alb at the time of onset was significantly positively correlated with 24-hour fluid infusion volume (r = 0.578, P < 0.05). Conclusions In the analysis of SCLS cases published with adequate data available from 1996 to 2015, it was revealed that: ① the difference in Hct-Alb levels at the onset of SCLS was 32.06±17.41. ② The greater the difference between Hct and plasma Alb, the more amount of fluid required to maintain normal blood pressure.

ABSTRACT

Objective To study and explore the relationship between neural function defect and plasma albumin (ALB) and fibrinogen (FIB) level in young patients with cerebral infarction .Methods 83 young patients with cere-bral infarction were selected as the observation group ,62 healthy people were randomly selected as the control group . The serum albumin,fibrinogen and neurological deficit scores were compared between the two groups .According to the degree of nervous function defect ,the patients of the observation group were divided into mild group ,moderate group and severe group,the plasma albumin in patients with different neurologic impairment and the level of fibrinogen were compared.Results Compared to the control group, the plasma albumin [(51.64 ±13.91 ) g/L], fibrinogen [(6.59 ±2.93)g/L] and neurological deficit scores [(22.95 ±14.32)points] in the observation group were signifi-cantly higher (t =8.132,10.764,10.888,all P <0.05).In the observation group,the plasma albumin and fibrinogen levels had statistically significant differences in patients with different degree of neural function defect (F =9.959, 7.192,all P <0.05),and plasma albumin,fibrinogen were positively correlated with neurological deficits (r =0.635, 0.726,all P <0.05).Conclusion The serum albumin and fibrinogen levels in young patients with cerebral infarc-tion are abnormally increased,their expression levels are closely related with neural function defect degree ,and can be used as important indicators of disease monitoring ,assessment of prognosis.

ABSTRACT

Objective To observe plasma albumin,total bilirubin and serum inflammatory factors,gonadal hormone levels in men with bipolar disorder,and to provide evidence for the diagnosis of the disease.Methods 48 men with bipolar disorder were selected as disease group,and 42 cases of male physical examination were selected as control group.Plasma albumin,total bilirubin and serum inflammatory factors,gonadal hormone levels of the two groups were detected.Results The levels of serum albumin and total bilirubin of the disease group were (43.4 ± 3.9)g/L and (9.4 ±1.9)μmol/L,which were lower than those of the control group[(49.6 ±3.0)g/L and (11.8 ± 2.1)μmol/L],the differences were statistically significant (t =8.361,5.692,all P 0.05).The serum level of E2 of the disease group was (23.2 ± 16.1)pg/mL,which was lower than (35.9 ±18.0)pg/mL of the control group,the serum T level of the disease group was (10.8 ±4.9)ng/mL,which was higher than (4.2 ±1.6)ng/mL of the control group,the differences were statistically significant(t =3.533,8.344,all P <0.05).Conclusion Plasma albumin,total bilirubin and serum IL -1β,IL -4 levels can be used as references for clinical diagnosis of bipolar disorder,and serum E2 and T levels can be used as risk factors for bipolar disorder.

ABSTRACT

Objective To explore the molecular basis of the characteristics of the serum auto-fluorescent spectrum in patients with ovarian cancer and the changes that might be induced by surgery. Methods Using fluorospectrophotometer and 300nm excitation light,the serum auto-fluorescent spectrum of 84 patients with ovarian cancer before and after the surgery and 30 healthy people were detected. Meanwhile, the serum tumor signs (CEA, CA199 and CA125), hemoglobin and plasma albumin level of all patients with ovarian cancer were detected. Their correlation with the fluorescence spectral characteristic parameters were analyzed. Results Compared with healthy people, the serum auto-fluorescent spectrum in patients with ovarian cancer exhibited purple-shifted position of λ2 peak and red shift in λ4 peak,had higher peak extent inλ1, λ2, λ4 and λ6 peak, and larger peak area of λ1, λ2, λ3, λ4 and λ6. Compared with those in ovarian cancer patients before surgery, the serum auto-fluorescent spectrum in these patients after operation had red shifts in λ1, λ2 and λ3 peak, lower peak extent in λ1, λ2, λ3 and λ4, and smaller peak area in λ1, λ2, λ3,λ4 and λ6 peak. In ovarian cancer patients, the serum level of CEA was positively correlated with the λ2 peak extent and the peak area of λ2 and λ3, while the serum level of CA125 was positively correlated with the peak extent of λ1-λ4 and λ6 and the peak area of λ1-λ3. The serum level of CA199 was negatively correlated with the λ2 position and positively correlated with the peak extent of λ1-λ6 and the peak area ofλ1-λ3 and λ6 in patents with ovarian cancer. Besides, the serum albumin was positively correlated with theλ2 peak position and negatively with the peak extent of λ1-λ6 and the λ1-λ3 peak area, while the level of hemoglobin was positively correlated with λ1 peak position. Conclusions The elevated serum tumor markers and lower albumin (plasma) level lead to the changes of the serum autofluorescence spectra characteristic parametersin in patients with ovarian cancer.These changes can be modestly corrected by surgery.

ABSTRACT

Objective To investigate the correlation between prognosis of patients with severe acute pancreatitis and changes of plasma albumin. Methods A total of 235 patients with severe pancreatitis who were admitted into hospital from September 2008 to October 2013 were selected as research object. According to the prognosis, they were divided into the survival group (143 cases who were recovered and still alive) and the death group (92 cases who died of the deterioration of diseases). Retrospectively analyzed the basic situation of the pa-tients and the changes of serum albumin levels in the two groups. And the correlation between prognosis of patients with severe acute pancrea-titis and changes of plasma albumin were evaluated by the logistic regression analysis. Results Compared with the death group,there were significant differences (P<0. 05) in terms of age,APCHEⅡ score,Ranson score,SOFA score,length of hospital stay,duration of mechani-cal ventilation,continuous renal replacement therapy,average serum albumin level,decrease of serum albumin within 24 hours,and variation of serum albumin. And ROC curves suggested the best cutoff value of decrease of serum albumin within 24 hours to predict death in patients within 24 hours was 4. 24 g/L,and the best cutoff value of variation of serum albumin to predict death was 13. 6%. Logistic regression analy-sis showed that the major risk factors of death in severe acute pancreatitis were decrease of serum albumin within 24 hours,variation of serum albumin within 24 hours,average serum albumin level,and the APCHEⅡscore. Conclusion Changes in plasma albumin can reflect the se-verity of severe acute pancreatitis,and the sharp decrease of serum albumin within 24 hours can be used as a risk factor for death.

ABSTRACT

Objective To investigate low plasma total cholesterol concentration in hospitalized patients,which is associated with malnourishment.Methods A total of 358 inpatients who were treated in our hospital from January 1 to February 1,2009,were enrolled.The relationships between low plasma total cholesterol (TC) level and other parameters were analyzed.Results Irrespective of women or men,the morbidities of low plasma TC,low plasma albumin,low plasma total proteins,low plasma triglyceride (TG) and low hemoglobin (Hb) levels were highly consistent with the result of subjective global assessment (SGA) (P <0.05).Plasma TC concentration was positively correlated with albumin,TG,Hb,lymph cells number,and body mass index (P<0.05).TC levels were different between men and women (P <0.0001).Meanwhile,low plasma TC level was not correlated with age,but it was correlated with total parenteral nutrition.Conclusion Low plasma TC concentration may relate to malnourished hospitalized patients.

ABSTRACT

Objective To observe the curative effect of Shenshuai Pellet on chronic renal failure patient and its influence on hemoglobin, blood plasma albumin and blood rheology. Methods 60 cases of chronic renal failure patients were randomly divided into the treatment group and control group. On the basis of high quality low-protein low-phosphorus diet, envies the vegetable protein, correction water and electrolyte balance, the acidosis and so on, the control group was treated with Oxyamyli tectus Aldehydum, the treatment group was given Shenshuai Pellet orally. Results Total effectiveness of treatment group and control group was 90% and 60% respectively. The clinical symptoms of treatment group were obviously improved.
