ABSTRACT

Objective:To investigate the value of red cell distribution width (RDW), procalcitonin (PCT), C-reactive protein (CRP), mean platelet volume (MPV)/platelet count (PLT) in predicting the prognosis of patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD).Methods:The clinical data of 137 patients with AECOPD admitted to the Department of Laboratory Medicine of Haiyang People's Hospital from March 2020 to March 2021 were retrospectively analyzed. According to their prognosis, these patients were divided into the survival group ( n = 99) and the death group ( n = 38). RDW, MPV, and PLT were measured using a blood cell analyzer (mindray ABC5390) in all patients. PCT and CRP levels were measured using the ETHealthcare instrument in all patients. RDW, PCT, CRP, and MPV/PLT were compared between the two groups. The value of RDW, PCT, CRP and MPV/PLT in predicting the prognosis of patients with AECOPD was analyzed using receiver operating characteristic curves. Results:The length of hospital stay and hospitalization expenses in the survival group were (10.75 ± 2.51) days and (1.49 ± 0.46) ten thousand yuan, respectively, which were significantly shorter and lower than (12.81 ± 3.36) days and (2.18 ± 0.57) ten thousand yuan in the death group ( t = 6.11, 14.45, both P < 0.05). The level of PLT in the survival group was significantly higher than that in the death group [(214.01± 63.97) × 10 9/L vs. (189.04 ± 61.75) × 10 9/L, t = 2.07, P < 0.05]. RDW, PCT, CRP, MPV, and MPV/PLT in the survival group were (13.18 ± 2.30)%, (4.30 ± 1.82) ng/L, (31.06 ± 10.38) mg/L, (11.39 ± 2.16) fL, and (0.05 ± 0.01), respectively, which were significantly lower than (16.65 ± 1.78)%, (9.55 ± 2.11) ng/L, (68.21 ± 20.94) mg/L, (12.28 ± 2.09) fL, (0.06 ± 0.02) in the death group ( t = 8.38, 14.45, 13.82, 2.18, 3.88, all P < 0.05). The receiver operating characteristic curve analysis results showed that the area under the curves depicting the value of RDW, PCT, CRP, MPV/PLT and their combination in predicting AECOPD was 0.831, 0.978, 0.966, 0.713, 0.988, with the predictive sensitivity of 62.6%, 89.9%, 91.9%, 59.6%, 98.0%, respectively, and the predictive specificity of 97.4%, 97.4%, 100.0%, 65.8%, 92.1%, respectively. Conclusion:Combined detection of RDW, PCT, CRP and MPV/PLT has a high value for the prediction of AECOPD. Corresponding indicators should be selected according to the actual situation of patients to guide clinical diagnosis and treatment.

ABSTRACT

Objective:To investigate the values of combined detection of serum C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), platelet count (PC), platelet count/mean platelet volume (PC/MPV) and platelet plateletcrit (PCT) in the diagnosis of chronic periprosthetic joint infection (PJI).Methods:From January 2013 to December 2019, 441 patients underwent hip or knee joint revision at Department of Articular Surgery, The First Hospital Affiliated to Xinjiang Medical University. The patients were divided into a chronic PJI group and an aseptic prosthetic loosening group. In the chronic PJI group of 147 cases (86 hip ones and 61 knee ones), there were 64 males and 83 females, with a mean age of 66 (54, 72) years. In the aseptic prosthetic loosening group of 294 cases (210 hip ones and 84 knee ones), there were 98 males and 196 females, with a mean age of 63 (49, 72) years. The preoperative levels of CRP, ESR, PC, PC/MPV and PCT were compared between the 2 groups. The best cut-off value, sensitivity and specificity of the above indicators for the diagnosis of chronic PJI were recorded. The diagnostic efficacy of the 5 indicators in combination for chronic PJI was evaluated by comparing the area under the curve (AUC) among the indicators and analyzing the results of combined diagnostic detections.Results:Except for gender and joint revision site, there was no significant difference in the other general data between the 2 groups, showing comparability ( P>0.05). The levels of CRP, ESR, PC, PC/MPV and PCT in the PJI group were significantly higher than those in the aseptic prosthetic loosening group ( P<0.05). For CRP, ESR, PC, PC/MPV and PCT, respectively, the best cut-off values were 9.05 mg/L, 38.5 mm/h, 288×10 9/L, 29.34 and 0.33%, the sensitivities 83%, 71%, 44%, 44% and 33%, the specificities 85%, 86%, 84%, 84% and 90%, and the AUCs 0.868, 0.822, 0.688, 0.696 and 0.659. For CRP+ESR+PC+PC/MPV+PCT and CRP+PC+PC/MPV+PCT, respectively, the AUCs were 0.871 and 0.882, the sensitivities 80% and 84%, and the specificities 86% and 84%, showing significant differences in the diagnosis of chronic PJI compared with ESR, PC, PC/MPV and PCT alone ( P<0.05). Conclusion:In the diagnosis of chronic PJI, serum CRP and ESR combined with PC, PC/MPV and PCT have a reference value, but PC, PC/MPV or PCT alone only has a limited value.

ntroduction: Platelet-rich fibrin (PRF) is used in many regenerative treatments. Preparing PRF in glass tubes requires quick handling and generates biohazard concerns about silica contamination and glass breakage. Using plastic tubes may be an alternative to glass tubes. Objectives: This study investigated the formation of PRF prepared in polypropylene (PP) and polystyrene (PS) tubes compared with glass tubes. Methodology: PRF was prepared from human blood (n=20) in PP, PS, and glass tubes. The time required for PRF clot formation and retraction from the tube wall were observed. The PDGF-AB, TGF-β1 levels, and the gross/SEM appearance of the PRF clots were also evaluated. Results: The PRF clots in PP and PS tubes formed and retracted significantly slower compared with those in glass tubes. The PDGF-AB levels in the PRF from PP, PS, and glass tubes were not significantly different. Although the TGF-β1 levels in the PRF from PP and glass tubes were not different, that from the PRF from PS tubes was significantly higher. The gross structure and SEM appearance of PRF from the three tube types were similar. Conclusion: The slow clot formation in PP and PS tubes can extend PRF handling time while retaining PDGF-AB level and fibrin appearance. Owing to this delay in clot formation, we could obtain the plasma without adding any anticoagulant chemical agents. The plasma can be used for accelerating bone regeneration as platelet-rich plasma.

ABSTRACT

BACKGROUND: The purpose of this study was to independently evaluate the beneficial effects of a high dose of transamine administrated prior to CPB on the postoperative hematologic aspect and bleeding. MATERIALS AND METHODS: This study included randomly selected groups of 40 adult patients undergoing OHS with CPB. All patients were divided into 2 groups: transamine group (T-group, n=20) and placebo group (P-group, n=20). The T-group received a high-dose of transamine (10 g) before and during CPB. The P-group received normal saline at the same times and served as a control group. RESULTS: The results of comparative studies between the 2 groups in the same hematologic variables were summarized as follows. (1) During CPB, the fibrinogen concentrations and platelet counts were significantly lower in the P-group than in the T-group (p<0.01). (2) During CPB, production of D-dimer occurred in 18 patients (90%) in the P-group and did not occur in the T-group (0%) (p<0.0001). (3) At CPB-off, the % concentration of fibrinogen (70.2+/-3.9%) and the % platelet counts (72.4+/-4.5%) of the T-group were significantly higher than those (54.5+/-3.8%, 64.3+/-2.9%) of the P-group (p<0.01). (4) Postoperative values of PT (14.0+/-0.03 sec.) and aPTT (27.6+/-0.1 sec.) of the T-group were significantly lower than those (16.0+/-0.02sec., 30.1+/-0.1sec.) of the P-group (p<0.05). (5) Postoperative bleeding and requirement of whole blood and other blood products were significantly less in the T-group than in the P-group (p <0.05). (6) There were no significant hypercoagulability signs such as cerebral em bolism, myocardial infarction, pulmonary embolism, or any other neurological prob lems in either group. CONCLUSIONS: We concluded that a high dose of transamine administered prior to CPB prevents the activation of fibri nolytic system and has beneficial effects of reducing the postoperative bleeding t endency without apparent hypercoagulability signs.
