ABSTRACT

Objective:To investigate the expression levels and diagnostic value of platelet parameters, fibrinogen and D-dimer in different Traditional Chinese Medicine (TCM) syndromes of deep venous thrombosis (DVT).Methods:From June 2015 to June 2019, a total of 500 DVT patients were enrolled and collected by two attending TCM doctors and classified according to syndromes differentiation. The differences of platelet count (PLT), mean platelet volume (MPV), platelet distribution width (PDW), platelet crit (PCT), platelet larger cell ratio (P-LCR), fibrinogen (FIB) and D-dimer were detected and compared.Results:According to TCM syndromes differentiation, 500 patients were divided into three groups, including 286 cases (57.2%) of downward flowing of damp-heat group, 132 cases (26.4%) of blood stasis and heavy damp group, and 82 cases (16.4%) of spleen-kidney-yang deficiency group. The levels of MPV, PDW, P-LCR, FIB and D-dimer among the groups were statistically significant difference ( P<0.05 or P<0.01). The MPV level was significantly higher and D-dimer level was significantly lower in blood stasis and heavy damp group than in the downward flowing of damp-heat group ( P<0.05). The levels of MPV, PDW and P-LCR in spleen-kidney-yang deficiency group were significantly higher than those in downward flowing of damp-heat group and blood stasis and heavy damp group ( P<0.05 or P<0.01). The levels of FIB and D-dimer were significantly lower than those in the downward flowing of damp-heat group and blood stasis and heavy damp group ( P<0.05 or P<0.01). The FIB (AUC=0.593) and D-Dimer (AUC=0.673) were statistically significant in the differential diagnosis of DVT between blood stasis and heavy damp group and spleen-kidney-yang deficiency group ( P<0.01). The MPV (AUC=0.601 5), PDW (AUC=0.615 4), P-LCR (AUC=0.606 1), FIB (AUC=0.616 4) and D-Dimer (AUC=0.721 8) were statistically significant in the differential diagnosis of DVT between downward flowing of damp-heat group and spleen-kidney-yang deficiency group ( P<0.01). Conclusion:The MPV, PDW, P-LCR, FIB and D-dimer have a certain correlation with DVT TCM syndrome types, and also have a certain reference value for its differential diagnosis, which can be used as an effective supplement to the objective indicators of Micro syndrome differentiation of traditional Chinese medicine.

ABSTRACT

Platelets play a pivotal role in the thromboembolic, inflammatory, and immunomodulatory process. The alteration of platelet quality often affects the treatment and prognosis of critically ill patients, and has a significant correlation with mortality. With the further research on the function of the platelet, it is found that the abnormal platelet quality can present during the early stage of the illness of the critically ill patients. In order to evaluate the alterations of the platelet quality more accurately, the further studies of platelet parameters, including platelet counts (PLT), platelet hematocrit (PCT), platelet large cell ratio (PLCR), mean platelet volume (MPV), platelet distribution width (PDW), immature platelet fraction (IPF) and so on, are still be the focus of current researches in the field of critical illness. At the same time, the application of thromboelastography/thrombelastography-platelet mapping (TEG/TEG MP) to the measurement of platelet function, especially the researches on the inhibitory rate of adenosine diphosphate (ADP) and the inhibitory rate of arachidonic acid (AA), is the hot spot of current researches. With regard to the diagnosis, prognosis and early goal-directed therapy (EGDT) of the critically ill patients, it is important to comprehensively apply the methods of platelet function evaluation.

ABSTRACT

Objective To explore the relationship between platelet activating factors and four platelet parameters in diabetic retinopathy (DR).Methods Eighty patients in our hospital were chosen from January 2015 to December 2015,regular ophthalmological examination and fundus fluorescence were performed on all cases.On the basis of international clinical diabetic retinopathy classification standard,the patients were divided into four groups:No DR group with 20 cases,non-proliferative diabetic retinopathy (NPDR) group with 20 cases,and proliferative diabetic retinopathy (PDR) group with 20 cases.20 healthy people who received physical examination during the same period were recruited as the control group.The flow cytometry instrument was used to determinate platelet activating factor CD62p and CD63,automatic blood analyzer was used to determinate platelet parameter which including blood platelet count (PLT),mean platelet volume (MPV),plateletcrit (PCT) and platelet distribution width (PDW),and the changes of platelet activation factor,parameters and its clinical significance in DR were discussed.Results With the development of DR,CD62p,CD63,MVP were numerical gradually increased,and PLT were numerical gradually reduced,the differences were statistically significant (all P < 0.05);Compared with control group,CD62p,CD63,MVP were increased and PLT were reduced in other group,the differences were statistically significant (all P < 0.05);Compared with pure diabetes group,CD62p,CD63,MVP were increased and PLT were reduced in NPDR group and PDR group,the differences were statistically significant (all P < 0.05);Compared with NPDR group,CD62p,CD63,MVP were increased and PLT were reduced in PDR group,the differences were statistically significant(all P < 0.05).PDW were numerical gradually increased in control group,pure diabetes group,NPDR group,PDR group,the differences were statistically significant(all P < 0.05);Control group compared with pure diabetes group,NPDR group compared with PDR group,the differences in PDW was not statistical significant (all P > 0.05);But compared with control group or pure diabetes group,PDW of NPDR group and PDR group increased,the differences were statistically significant(all P < 0.05).The differences in PCT among groups was not statistically significant(all P > 0.05).Conclusion The platelet activating factor CD62p,CD63 and parameters of platelet show differences in each diabetic retinopathy period,platelet activating factor in the occurrence and development of diabetic retinopathy have important significance.Monitor the platelet activating factor and parameters of platelet can help surveillance of the disease.

ABSTRACT

Objective To understand the relationship between the changes of coagulation function ,platelet parameters and retic‐ulocyte parameters with the Child‐Pugh grade in the patients with liver cirrhosis .Methods The coagulation function index (PT , APTT ,Fig ,TT) ,platelet parameters(PLT ,MPV ,PDW ,PCT) and reticulocyte parameters(RET# ,RET% ,IRF) were detected in 163 patients with liver cirrhosis and 50 healthy controls by using the automatic hematological analyzer and the automatic blood co‐agulation analyzer .Then the changes of coagulation function ,platelet parameters and reticulocyte parameters were compared be‐tween the liver cirrhotic group and the control groups ,and among different Child‐Pugh grade groups .Results Compared with the control group ,Fig in the liver cirrhosis group was declined ,PT ,APTT and TT were prolonged ,PLT and PCT was decreased ,while MPV and PDW were elevated ,RET count ,RET% and IRF were increased ,the difference between the two groups were statistically significant(P<0 .05);Fig was gradually declined with the Child‐Pugh grade elevation ,PT ,APTT and TT were prolonged ,PLT and PCT was decreased ,while MPV and PDW were increased with the Child‐Pugh grade elevation ,and RET count ,RET% and IRF were increased (P<0 .05) .Conclusion The blood coagulation disturbance and abnormality of platelet parameters and reticulocyte parameters exist in the patients with liver cirrhosis .The changes of coagulation function ,platelet parameters and reticulocyte param‐eters are closely related to the Child‐Pugh classificatione ,which are the important indicators for judging the liver damage ,bleeding tendency and bone marrow hematopoietic function in the patients with liver cirrhosis .

ABSTRACT

Objective To study the changes and clinical value of platelet(PLT)parameters and coagulation indicators in children with severe pneumonia.Methods 97 children were divided into severe pneumonia groupⅠand severe pneumonia groupⅡ according to whether children were associated with other diseases besides severe pneumonia,and 30 healthy chil-dren were in control group.The levels of PLT count,mean platelet volume(MPV),prothrombin time(PT),activated par-tial thromboplastin time(APTT),fibrinogen(FIB),antithrombin-Ⅲ(AT-Ⅲ),and D-dimer(DD)among three groups were compared.Results The differences of PLT,MPV,DD and AT-Ⅲ activities were all significant among three groups (all P < 0.05).PLT,MPV and DD levels in group Ⅰ were all significantly higher than those of control group ([454.00±157.00]×109/L vs [300.00±63.00]×109/L ;[9.66±1.24]fL vs [8.90±0.37]fL;[0.47±0.37] mg/L vs [0.27±0.06]mg/L,respectively);AT-Ⅲ activity in groupⅠ was lower than control group([79.91 ± 20.34]% vs[107.03±8.11]%)(both P <0.05).AT-Ⅲ activity and PLT level in group Ⅱwas (66.11±11.12)%and (279.00±185.00)×109/L respectively,which were both significantly lower than group Ⅰ,MPV and DD level was (10.37± 1.51)fL and (0.70±0.46)mg/L respectively,which were both higher than groupⅠ (both P <0.05).Conclusion There is obvious coagulation dysfunction in children with severe pneumonia.The changes in PLT,MPV,AT-Ⅲ and DD levels are associated with the severity of pneumonia.

ABSTRACT

BACKGROUND: Because the platelet parameters have recently been used as indicators for various clinical conditions, there is a need to accurately estimate the platelet parameters by using an automated hematology analyzer. The aim of this study was to investigate an effect of a platelet transfusion on such platelet parameters as the mean platelet volume (MPV), the platelet volume distribution width (PDW), the mean platelet component (MPC) and the platelet component distribution width (PCDW) in transfused patients. METHODS: The study subjects were 25 patients who were admitted to the Department of Hematology & Oncology and they had been transfused with platelets. CD62P labeling was performed in the remaining portions of single donor platelets products (SDP) and the pooled platelet concentrates (PC) just before the SDP and PC were released. The platelet parameters were determined using the ADVIA 120 and the whole blood samples from the patients before the platelet transfusions and within 10~16 hours after the transfusion. RESULTS: There were no significant difference of all the platelet parameters between the SDP (n=21) and the pooled PC (n=8). The MPC and PCDW of the SDP and the PC were significantly lower than those of the samples from the patients before transfusion. However, the PCDW of the samples from the patients after transfusion was significantly lower than that before transfusion. CONCLUSION: Because platelet transfusions lower the value of the PCDW in patients, the laboratory staff and clinicians should be aware of this when interpreting the PCDW.

ABSTRACT

Objective To explore the diagnostic and prognostic significances of platelet parameters changes in Kawasaki disease(KD).Methods The platelet total(PLT),mean platelet volume(MPV),platelet distribution width(PDW)and plateletcrit(PCT)were measured in 23 cases of KD on acute febrile phase and early recovery phase.It was compared with 33 cases suffered from other febrile diseases during the same period and in similar ages.Each KD child should accept the cardiology ultrasonic examination.Results The level of PLT,PCT increased in KD patients.It was more significant in early recover period than in acute febrile period(P0.05).PLT negatively correlated with MPV,PDW(r=0.358,0.268 Pa0.05).Conclusions The changes of platelet parameter play a very important role in diagnosis of KD.But it still can not be certain that whether the changes can help us estimate the variety of the patients condition and prognosis of KD children.It requires further study.

Objective To explore the changes of high-sensitivity C reactive protein(hs-CRP) and platelet parameter levels in newborns with hypoxic-ischemic encephalopathy(HIE)and its clinical significance.Methods Thirty-five infants with HIE and 16 healthy newborns were selected as study cases and controls respectively by automantc biochemistry analyzer.Serum hs-CRP content was measured by reaction rate method;Platelet parameter levels were detected by collecting blood samples from peripheral vascular of heel,and the activity of creatine kinase(CK),creatine phosphokinase isoenzyme(CK-MB),lactate dehydrogenase(LDH),alpha-hydroxybntyric dehydrogenase(?-HBDH),alanine transaminase(ALT),aspartate aminotransferase(AST) were assayed as well.Results 1.The hs-CRP levels in newborns with HIE increased obviously in acute stage,and significant difference were observed compared with controls(P0.05).2.PLT in newborns with HIE decreased significantly in acute stage compared with that of controls(P0.05).3.The values of CK,CK-MB,LDH,?-HBDH,AST,ALT in newborns with HIE were significantly higher than those in controls in acute stage(P
