ABSTRACT

OBJECTIVE@#To investigate the clinical effect of aconite-isolated moxibustion at Yongquan (KI 1) combined with rivaroxaban for lower extremity venous thrombosis after total knee arthroplasty and the influence on hypercoagulation.@*METHODS@#Seventy-three patients of knee osteoarthritis with lower extremity venous thrombosis after total knee arthroplasty (KOA) were randomly divided into an observation group (37 cases, 2 cases dropped off) and a control group (36 cases, 1 case dropped off). The patients in the control group took orally rivaroxaban tablets, 10 mg a time, once a day. On the basis of the treatment as the control group, the aconite-isolated moxibustion was applied to Yongquan (KI 1) for the patients of the observation group, once daily and 3 moxa cones were used in each treatment. The duration of treatment was 14 days in both groups. Before treatment and 14 days into treatment, the ultrasonic B test was adopted to determine the conditions of lower extremity venous thrombosis in the two groups. Before treatment, 7 and 14 days into treatment, the coagulation indexes (platelet [PLT], prothrombin time [PT], activated partial prothrombin time [APTT], fibrinogen [Fib] and D-dimer[D-D]), the blood flow velocity of the deep femoral vein and the circumference of the affected side were compared between the two groups separately, and the clinical effect was evaluated.@*RESULTS@#Fourteen days into treatment, the venous thrombosis of the lower extremity was relieved in both groups (P<0.05), and that of the observation group was better than the control group (P<0.05). Seven days into treatment, the blood flow velocity of the deep femoral vein was increased compared with that before treatment in the observation group (P<0.05), and the blood flow rate in the observation group was higher than that in the control group (P<0.05). Fourteen days into treatment, PT, APTT and the blood flow velocity of the deep femoral vein were increased in the two groups compared with those before treatment (P<0.05); and PLT, Fib, D-D and the circumference of the limb (knee joint, 10 cm above the patella and 10 cm below the patella) were all reduced in the two groups (P<0.05). Compared with the control group 14 days into treatment, the blood flow velocity of the deep femoral vein was higher (P<0.05), PLT, Fib, D-D and the circumference of the limb (knee joint, 10 cm above the patella and 10 cm below the patella) were all lower in the observation group (P<0.05). The total effective rate was 97.1% (34/35) in the observation group, higher than 85.7% (30/35) in the control group (P<0.05).@*CONCLUSION@#Aconite-isolated moxibustion at Yongquan (KI 1) combined with rivaroxaban can effectively treat lower extremity venous thrombosis after total knee arthroplasty, relieve hypercoagulation, accelerate the blood flow velocity and alleviate swelling of the lower extremity in the patients with knee osteoarthritis.

ABSTRACT

OBJECTIVE@#To observe the effects of moxibustion at Yongquan(KI 1) on the cognitive function and lower limb motor function in patients with post-stroke cognitive impairment of kidney essence deficiency.@*METHODS@#Eighty-four patients with post-stroke cognitive impairment of kidney essence deficiency were randomly divided into an observation group(42 cases,1 case dropped off)and a control group(42 cases,1 case dropped off). The control group was treated with medication, electroacupuncture, rehabilitation training and repetitive transcranial magnetic stimulation(rTMS);on the basis of the treatment as the control group, moxibustion at bilateral Yongquan(KI 1)was adopted in the observation group. Both groups were treated once a day,5 days a week with 2-day interval,4 weeks were required. The Montreal cognitive assessment (MoCA) score, mini-mental state examination (MMSE) score, Fugl-Meyer assessment-lower extremity (FMA-LE) score, Berg balance scale (BBS) score, functional independence measure (FIM) score, modified fall efficacy scale (MFES) score and scale for the differentiation of syndromes of vascular dementia (SDSVD) score before and after treatment were observed in the two groups.@*RESULTS@#After treatment, the MoCA, MMSE, FMA-LE, BBS, FIM and MFES scores were higher than those before treatment in both groups (P<0.05), and the scores in the observation group were higher than those in the control group (P<0.05). After treatment, the SDSVD scores were lower than those before treatment in both groups (P< 0.05), and the SDSVD score in the observation group was lower than that in the control group (P< 0.05).@*CONCLUSION@#Moxibustion at Yongquan(KI 1) can improve the cognitive function and motor and balance function of lower limbs in patients with post-stroke cognitive impairment of kidney essence deficiency, reduce the risk of fall and improve the quality of life.

ABSTRACT

OBJECTIVE@#To compare the clinical effect between warming acupuncture combined with moxibustion at Yongquan (KI 1) and simple warming acupuncture for knee osteoarthritis with kidney-marrow deficiency.@*METHODS@#A total of 66 patients of knee osteoarthritis with kidney-marrow deficiency were randomized into an observation group and a control group, 33 cases in each one. Warming acupuncture was applied at Neixiyan (EX-LE 4), Dubi (ST 35), Zusanli (ST 36) and Xuanzhong (GB 39) on the affected side in both of the groups. In the observation group, mild moxibustion at bilateral Yongquan (KI 1) was adopted additionally. Each treatment lasted for 30 min, 3 times a week (once every other day), and the consecutive 6 weeks of treatment were required. The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score (such as joint pain, stiffness and physical function), the amount of joint effusion and the serum contents of interleukin-1β(IL-1β), tumor necrosis factor-α (TNF-α) and high-sensitivity C-reactive protein (hs-CRP) were observed before and after treatment in the two groups.@*RESULTS@#The total effective rate in the observation group was 93.3% (28/30), which was superior to 87.1% (27/30) in the control group (<0.05). Compared before treatment, the pain scores, stiffness scores, physical function scores, the amount of joint effusion and the contents of IL-1β, TNF-α and hs-CRP after treatment were significantly reduced in the two groups (<0.05), and the improvements of these indices in the observation group were superior to the control group (<0.05).@*CONCLUSION@#Warming acupuncture combined with moxibustion at Yongquan (KI 1) can improve joint function, reduce the amount of joint effusion and the contents of inflammatory response indices for knee osteoarthritis with kidney-marrow deficiency. The therapeutic effect of warming acupuncture combined with moxibustion at Yongquan (KI 1) is better than simple warming acupuncture.

ABSTRACT

<p><b>OBJECTIVE</b>To summarize and analyze the clinical literature regarding moxibustion at Yongquan (KI 1) for insomnia, and to provide clinical evidence of moxibustion at Yongquan (KI 1) for insomnia.</p><p><b>METHODS</b>With "moxibustion" "acupuncture-moxibustion" "Yongquan (KI 1)" "insomnia" and "sleep disorder", etc. as key terms, the clinical literature regarding moxibustion at Yongquan (KI 1) for insomnia was retrieved in CNKI, and VIP databases, and reviewed, summarized and analyzed.</p><p><b>RESULTS</b>A total of 27 clinical papers were retrieved, including 14 randomized controlled trials. In recent years, the number of clinical papers had increased. Few papers selected moxibustion at Yongquan (KI 1) as independent treatment for insomnia; most papers combined moxibustion with acupuncture, massage, etc. Moxibustion was commonly manipulated by patients or family members, and few papers applied moxibustion instruments to make the manipulation easier and safer.</p><p><b>CONCLUSION</b>Moxibustion at Yongquan (KI 1) for insomnia has attracted more and more attention. It is suggested to apply convenient and safe moxibustion instruments in clinical treatment, which is benefit to clinical generalization, but also provides convenient manipulation for further study regarding its clinical effect and mechanism.</p>

ABSTRACT

OBJECTIVE@#To observe the clinical efficacy of " needles therapy" combined with moxibustion at yongquan (KI 1) on perimenopausal insomnia (PMI) with heart-kidney imbalance.@*METHODS@#One hundred patients of PMI with heart-kidney imbalance were randomly divided into a medication group and an acupuncture group, 50 cases in each group. In the medication group, diazepam and oryzanol were given. " needles therapy" combined with moxibustion at yongquan (KI 1) were applied in the acupuncture group. The treatment was given for 2 courses, 10 days as a course. Before and after treatment, the Pittsburgh sleep quality index (PSQI) scale was used to evaluate the improvement of sleep quality in the two groups, and the clinical efficacy was compared.@*RESULTS@#After 2 courses of treatment, the total PSQI score was (4.88±1.80) in the acupuncture group, which was significantly lower than (8.00±1.93) in the medication group, and the difference was statistically significant (<0.05). Except for the sleep efficiency, the scores of other components in the acupuncture group were lower than those in the medication group, and the differences were statistically significant (all <0.05). The total effective rate was 90.0% (45/50) in the acupuncture group and 68.0% (34/50) in the medication group. The difference was statistically significant (<0.05). There was no obvious adverse reactions in entire course of the treatment in the acupuncture group and the safety was satisfactory.@*CONCLUSION@#" needles therapy" combined with moxibustion at Yongquan (KI 1) on PMI with heart-kidney imbalance has good clinical effect.
