ABSTRACT
RESUMO A Reforma Sanitária Brasileira tem sido estudada com apoio de diferentes referenciais teórico--metodológicos, e essa variedade de abordagens motivou esta pesquisa. Portanto, o objetivo deste artigo foi analisar estudos sobre o movimento sanitário, publicados entre 1987 e 2019, a partir de uma revisão sistemática de 28 artigos selecionados, entre 426 capturados, no 'Portal de periódicos da Capes', na 'SciELO' e na 'Biblioteca Virtual em Saúde'. Os resultados apontam que a maioria dos autores são sujeitos engajados ética e politicamente no movimento e utilizam, em grande medida, a abordagem marxista. Em relação às fontes de informação, foram priorizadas a pesquisa bibliográfica e a consulta a documentos institucionais ou publicizados pelas entidades da Reforma Sanitária Brasileira. Poucos estudos, entretanto, explicitaram claramente as estratégias metodológicas e o embasamento teórico utilizado para realizar a análise textual. Conclui-se que a escolha dos referenciais teórico-metodológicos está relacionada com a trajetória profissional e política dos autores, impactando diretamente na delimitação dos objetos de estudo e, consequentemente, no alcance e nos resultados das análises sobre o movimento sanitário.
ABSTRACT The Brazilian Health Sector Reform has been approached from different theoretical and methodological perspectives, and that variety of approaches is what motivated this research. Thus, the aim of this article is to analyze studies about the Health Reform Movement, published between 1987 and 2019, from a systematic review of 28 selected articles, among the 426 collected on the 'Capes Periodicals Portal', 'SciELO', and 'Virtual Health Library'. The results indicate that the majority of authors are subjects ethically and politically engaged in the movement and largely use the Marxist approach. Concerning information sources, bibliographic research and consultation of institutional documents or documents published by Health Sector Reform entities were prioritized. However, few studies clearly explain the methodological strategies and the theoretical basis used to perform a textual analysis. It is concluded that the choice of the theoretical and methodological references is related to the professional and political trajectory of the authors, directly impacting on the delimitation of the objects of study and, consequently, on the scope and results of the analyses on the Health Reform Movement.

Objetivou-se analisar a atuação do Conselho Nacional de Saúde no núcleo do processo decisório da política de saúde entre 2014-2017. A análise foi baseada nos projetos políticos em disputa na arena política da saúde e nos projetos dos governos de Dilma e de Temer. Com a mudança no projeto de governo, o Conselho passou a atuar pela via judicial, além de pelas vias social e parlamentar, passando do alinhamento (crítico) à franca oposição, até sua neutralização com o boicote da participação do Conselho nas discussões de pautas da política de saúde nos anos 2016-2017. Embora não tenha tido poder suficiente para mudar a correlação de forças em torno do desmonte do Sistema Único de Saúde (SUS), o Conselho figurou-se em um bloco político de resistência e de enfrentamento em defesa do SUS constitucional. El objetivo fue el análisis de la actuación del Consejo Nacional de Salud en el núcleo del proceso de decisión de la política de salud entre 2014-2017. El análisis tuvo como base los proyectos políticos en disputa en la arena política de la salud y en los proyectos de los gobiernos Dilma y Temer. Con el cambio en el proyecto de gobierno, el Consejo pasó a actuar, además de en las vías social y parlamentaria, en la vía judicial, pasando del alineamiento (crítico) a la franca oposición, hasta su neutralización, con el boicot de la participación del Consejo en las discusiones de pautas de la política de salud en los años 2016-2017. Aunque no haya tenido poder suficiente para cambiar la correlación de fuerzas alrededor del desmontaje del Sistema Brasileño de Salud (SUS), el Consejo se configuró como un bloque político de resistencia y de enfrentamiento, en defensa del SUS constitucional. This study aimed to analyze the role of the National Health Council in the core of the decision-making process of health policy in the period 2014-2017. The analysis was based on the political disputed projects in the health policy sphere and on the government projects of former Presidents Dilma Rousseff and Michel Temer. Due to the government's project change, in addition to the social and parliamentary channels, the Council started to act through the judicial channel as well, moving from critical alignment to outright opposition until its neutralization, with the Council's participation boycott in the health policy agenda discussions in the period 2016-2017. Although it did not have enough power to change the balance of forces surrounding the deconstruction of the Brazilian National Health System (SUS), the Council was part of a political bloc of resistance and confrontation in defense of the constitutional SUS. RESUMO Diversas abordagens desenvolvidas no âmbito das ciências humanas e sociais têm sido utilizadas nas análises políticas em saúde, uma delas é a análise de conjuntura. Com objetivo de sistematizar os principais conceitos, referenciais e estratégias metodológicas desse tipo de estudo e problematizar sua aplicação na área da saúde, foi realizada uma revisão sistemática sobre o tema. Não foi identificada uma única vertente teórica de referência nem um percurso metodológico padrão, mas merece destaque o fato de a maioria dos artigos não se preocupar em explicitar, de maneira clara, teorias e métodos. Os artigos que deram enfoque à dimensão política incluíram, entre seus objetivos, analisar fatos e atores envolvidos. Sobre os artigos que tratam de questões de saúde, foi possível notar maior preocupação com aspectos teóricos e metodológicos, sendo que todos comentaram sobre a relação entre o setorial e outras dimensões de caráter político e/ou estrutural. Ao considerar que a análise de conjuntura tem o propósito de subsidiar a ação, constata-se que, quando limitada a um setor, restringe a visão acerca das determinações que incidem sobre os fatos e atores, inclusive em saúde, condição que pode prejudicar a tomada de decisão.
ABSTRACT Several approaches developed in the humanities and social sciences have been used in the political analysis in health, one of them is the conjuncture analysis. In order to systematize the main concepts, references and methodological strategies of this type of study and to problematize its application in the health area, a systematic review was carried out on the subject. It was not identified a single theoretical reference or a standard methodology, but it is worth highlighting the fact that most articles do not bother to clearly explain theories and methods. The articles that focused on the political dimension included, among their objectives, analyzing facts and social actors involved. Regarding the articles that deal with health issues, it was possible to note a greater concern with theoretical and methodological aspects, and all commented about the relationship between the sectorial and other dimensions of a political and structural nature. Considering that the conjuncture analysis has the purpose of subsidizing the action, it is observed that when it is limited to a sector, it restricts the vision related to determinations that affect the facts and social actors, including in health, a condition that may harm the decision making.
