ABSTRACT

Idiopathic epiretinal membrane(IERM)is a kind of epiretinal membrane without any other known ocular conditions, which occurs mostly in the middle-aged and elderly people over the age of 50. As IERM worsens, the structure and function of the retina in the macular region are altered, leading to symptoms like reduced vision and metamorphopsia. The pathogenesis of IERM remains unclear, and surgery is the primary treatment option. However, there is no consensus on the best time to have surgery, and there are differences in how well patients recover their vision following surgery. Optical coherence tomography(OCT)and OCT angiography(OCTA), as non-invasive and rapid diagnostic tools to observe retinal microstructure and blood flow changes in the macula, have been extensively utilized in clinical settings. The use of OCT and OCTA parameters to predict postoperative visual acuity has emerged as a hot topic in IERM research. This article provides a comprehensive review of current research on the correlation between various OCT and OCTA parameters and the prediction of postoperative visual acuity in IERM, aiming to assist clinicians in determining the optimal timing for surgery and balancing the benefits and risks involved.

ABSTRACT

Purpose: To determine the factors affecting the visual outcome after surgical repair of limbal corneal lacerations at a tertiary eye care center in South India. Methods: A retrospective analysis of patients diagnosed with limbal tears between 2011 and 2021 was conducted. Demographic information such as age, gender, cause of injury, and size of the laceration was recorded. Comprehensive ocular examination was performed, including gentle B scan evaluation whenever not contraindicated for detailed posterior segment evaluation. Only those cases with a minimum follow?up of one year were included. Postoperative best?corrected visual acuity, intraocular pressure (IOP), cornea clarity, and integrity of the wound at last follow?up were noted. Results: Out of the 20 patients, 15 (75%) were males and 5 (25%) were females. The mean age was 42.6 � 22.4 years. All 20 patients had a penetrating injury, with four (20%) injured by a stick, two (10%) by an iron rod, three (15%) due to road traffic accident (RTA), three (15%) by glass, and eight (40%) with other nonspecific objects [two (10%) with needle, two (10%) with elastic rope, two (10%) with bangle, and two (10%) with metal]. The average time between the injury and the surgery was 48 hours (2 days). Four (20%) patients underwent a second surgery within a week of repair. After limbal tear repair, at final follow?up at 3 years, 7 (35%) had VA worse than 20/800, 3 (15%) had VA between 20/100 and 20/800, and 10 (50%) achieved VA better than 20/80. Conclusion: Preoperative visual acuity (VA), mode of injury, and size of wound affect the final visual outcome after surgical repair of limbal corneal laceration. Preoperative VA and mode of injury were statistically significant even in the multivariate analysis.

ABSTRACT

@#AIM: To observe the efficacy of the minimal scleral buckling in the treatment of long-standing retinal detachment.<p>METHODS: The clinical data of 54 eyes in 54 cases of long standing retinal detachment treated by the minimal scleral buckling in our hospital during January 2014 to September 2018 were analyzed retrospective. Patients were followed up for 6mo. The rate of postoperative retinal reattachment, the best corrected visual acuity(BCVA), intraocular pressure, eye axis and astigmatism were observed. According to the preoperative time of retinal detachment, retinal detachment range, macular involvement or not and presence of PSF or not, the improvement of postoperative BCVA were analyzed.<p>RESULTS: The retina reattached in 49 eyes(91%)after primary operation. Postoperative persistent subretinal fluid was observed in 29 eyes of them by optical coherence tomography. At the last follow up, BCVA increased in 12 eyes(22.2%), remained unchanged in 38 eyes(70.4%), and decreased in 4 eyes(7.4%).The preoperative time of retinal detachment, retinal detachment range, macular involvement or not and presence of PSF or not had an impact on the postoperative recovery of BCVA.<p>CONCLUSION: The minimal sclera buckling surgery is an effective method to treat long standing retinal detachment. But postoperative visual acuity recovery is related to many factors.

ABSTRACT

Abstract?AIM: To evaluate the visual acuity, visual function, q uality of lfi e, and cost/effectiveness of cataract surgery in patients with low soc ioeconomics tatus ( LSES ) who received government subsidies for free cataract surgery.?METHODS: Retrospectively, 334 eyes of 334 patient s were reviewed. Visual function and quality of l ife questionnaires we re available in 83 patie nts. The cost/effectiveness of cataract surgery was analyzed in 225 patients with LSES and 12 0 patients who rec eived standard cataract surgery. Mutl iple linear regression anal ysis and logistic regression analysis were used to assess the factors affecting the postoperative visual acuity.?RESULTS: At 1wk postoperatively, uncorrected visual acuity ( UCVA ) was improved significantly compared to these preoperait vely (P<0.50 ), 235 eyes (70.4%) had UCVA of 20/40 or better, and 16 eyes (4.8%) had UC VA wor se than 20/400. Multiple linear regresis on ana lysis revealed that other ocular diseases and cataract classification wer e factors contributing to po stop erative UC VA. Logistic regression analysis showed that low preoperative visual ac uti y and other ocular diseases were risk factors of postopera tive low vision, and the odds ratio (95%confidence int erval) values were 6.5 (2.8-14.8) and 1.3 (1.0-1.6), respe ctively.Visual fun ction and quality of life were significantly improved after the surgery. Th e cost/effetc iveness of c ataract su rgery in patients with LSES was lower than that in standard cataract patients.?CONCLUSION: Visual acuity and visual function were significantly better after the surgery in catara ct patients wti h LSES, and the operation cost was low.Quality of life in these patients was significantly improved.

ABSTRACT

PURPOSE: To evaluate factors influencing stereoacuity outcomes after pediatric cataract surgery. METHODS: We retrospectively reviewed 38 eyes of 22 patients who had undergone irrigation and aspiration of cataracts and primary posterior chamber intraocular lens implantation from February 1992 to July 2006. In all patients, stereoacuity was assessed with the Titmus stereo test. Patients were divided into two groups according to stereoacuity: group 1 (n=9) had a stereoacuity less than 100 sec/arc; group 2 (n=13) had a stereoacuity more than 100 sec/arc. Nine variables associated with stereoacuity were evaluated in each group. RESULTS: Postoperative strabismus was the strongest factor influencing the stereoacuity level. Good postoperative mean visual acuity was related with good stereoacuity. Though mean visual acuity in two children with exodeviation over 30PD was 0.7 and 0.8, their stereoacuity was 400 sec/arc. Showing little difference in the spherical equivalent between the eyes was also related to good stereoacuity. CONCLUSIONS: Postoperative strabismus onset, postoperative mean visual acuity between the two eyes, and differences in the spherical equivalent were important factors influencing stereoacuity in patients after pediatric cataract surgery.

ABSTRACT

PURPOSE: To investigate postoperative visual acuities of patients who underwent vitrectomy for their dense nondiabetic vitreous hemorrhage, and in addition to assess postoperative visual results of the group who underwent early vitrectomy. METHODS: Included in the study were 52 patients (52 eyes) who had undergone vitrectomy and been followed up for longer than 4 months after surgeries. Authors investigated the underlying diseases of vitreous hemorrhage, surgical outcomes, and the rate of complications. In addition, we evaluated the results of the group who had undergone vitrectomy between 4 weeks and 6 weeks after development of hemorrhage. RESULTS: The most common cause of vitreous hemorrhage was branch retinal vein occlusion followed by macular hole, ocular trauma, central retinal vein occlusion, age related macular degeneration, Terson`s syndrome, and unknown cases. Visual acuity before operation is less than light perception to 0.3 and improved in 44 eyes (84.6%) after operation. The rate of more than 5 lines' improvement is 63.4% (33 eyes) and 51.9% (27 eyes) obtained their visual acuity of 0.5 or better. In the group which had been operated 4 to 6 weeks after the development, visual acuity improved in 100% (16 eyes). Thirteen eyes (81.2%) showed more than 5 lines' improvement of their visual acuity and 12 eyes had 0.5 or better (75.5%). The most common complication after surgeries is cataract in 5 eyes (10.8%), others were macular hole in 3 eyes (5.7%), retinal detachment in 2 eyes (3.8%), macular hole in 1 eye (6.2%). CONCLUSIONS: Pars plana vitrectomy is a good procedure to improve visual acuity for the patients who have suffered persistent visual disturbances due to non-diabetic vitreous hemorrhage. Also, early vitrectomy should be considered for early visual rehabilitation.

ABSTRACT

PURPOSE: This study aimed to analyze the clinical characteristics, results of operational method and causes of operational failure in rhegmatogenous retinal detachment except macula hole induced retinal detachment, and to analyze several factors of vision. METHOD: We reviewed the records of 720 patients, 743 eyes with regmatogenous retinal detachment who had undergone operation from January 1990 to December 1999, and followed up for at least 3 months or longer. RESULT: Scleral buckling (728 eyes, 98.0%), gas injection (9 eyes, 1.2%) and vitrctomy (6 eyes, 0.8%) were done as a primary operation. Anatomical success rate was 684 eyes(34.9%). A good visual acuity ( V A >or=0.5) was achieved in 239 eyes (34.9%), reasonable vision (VA 0.4~0.15) in 192 eyes(28.1%), ambulatory vision (VA 0.1~0.02) in 95 eyes (13.8%), whereas 158 eyes (23.2%) became blind (VA
