ABSTRACT

Objetivo: Analisar a percepção do enfermeiro acerca das condições de trabalho no contexto da Atenção Primária à Saúde (APS) durante a pandemia de SARS-CoV-2. Métodos: Trata-se de um estudo descritivo, transversal, de abordagem qualitativa, recorte de um estudo multicêntrico e de abrangência nacional. Os dados apresentados neste estudo correspondem aos obtidos no estado do Rio Grande do Norte, onde foram entrevistados 45 enfermeiros da APS no período de dezembro de 2020 a abril de 2021. Após as entrevistas, as narrativas gravadas foram transcritas e analisadas através da Análise de Conteúdo de Bardin. Resultados: A partir da fala dos enfermeiros, evidenciou-se que a utilização de Equipamentos de Proteção Individual, a organização e desenvolvimento de novos protocolos e fluxos assistenciais trouxeram sofrimento psíquico aos profissionais, tanto pelas condições de trabalho inadequadas, quanto pela proximidade com o sofrimento de pacientes e o medo de contágio. Conclusão: Mesmo com tantas dificuldades, os enfermeiros lotados na atenção primária à saúde tiveram que reconstruir suas práticas na perspectiva de garantir, dentro dos limites impostos pela situação adversa, a melhor assistência possível, mostrando a força e resiliência das equipes da APS nos mais diversos contextos. Objective: To analyze the perception of nurses about working conditions in the context of PHC during the COVID-19 pandemic. Methods: This is a descriptive, cross-sectional study with a qualitative approach, part of a multicenter study with a national scope. The data presented in this study correspond to those obtained in the state of Rio Grande do Norte, in which 45 nurses from primary health care were interviewed from December 2020 to April 2021. After the interviews, the recorded narratives were transcribed and analyzed through the Bardin's Content Analysis. Results: The collective discourse showed that the use of Personal Protective Equipment, the organization and development of new protocols and care flows brought psychic suffering to professionals, both due to inadequate working conditions and the proximity to the suffering of patients and the fear of Contagion. Conclusion: Even with so many difficulties, nurses working in primary health care had to rebuild their practices in order to guarantee, within the limits imposed by the adverse situation, the best possible assistance, showing the strength and resilience of PHC teams in the most diverse contexts. Objetivo: Analizar la percepción de los enfermeros sobre las condiciones de trabajo en el contexto de la APS durante la pandemia de COVID-19. Métodos: Se trata de un estudio descriptivo transversal con enfoque cualitativo, parte de un estudio multicéntrico de alcance nacional. Los datos presentados en este estudio corresponden a los obtenidos en el estado de Rio Grande do Norte, en el que fueron entrevistados 45 enfermeros de atención primaria de salud entre diciembre de 2020 y abril de 2021. Después de las entrevistas, las narraciones grabadas fueron transcritas y analizadas a través de la escala de Bardin. Análisis de contenido. Resultados: El discurso colectivo mostró que el uso de Equipos de Protección Individual, la organización y desarrollo de nuevos protocolos y flujos de atención trajeron sufrimiento psíquico a los profesionales, tanto por las inadecuadas condiciones de trabajo como por la proximidad al sufrimiento de los pacientes y el miedo al Contagio. Conclusión: Incluso con tantas dificultades, los enfermeros que actúan en la atención primaria de salud tuvieron que reconstruir sus prácticas para garantizar, dentro de los límites impuestos por la situación adversa, la mejor asistencia posible, mostrando la fortaleza y resiliencia de los equipos de APS en los más diversos contextos. Objetivo: Analisar a percepção de enfermeiras sobre a autonomia no exercício de suas práticas no contexto da atenção primária à saúde. Métodos: Estudo descritivo de abordagem qualitativa que analisou 108 entrevistas realizadas com enfermeiras(os) das quatro capitais da região sudeste do Brasil. As entrevistas, guiadas por roteiro semiestruturado, foram gravadas e transcritas. Os dados produzidos foram tratados e explorados com auxílio do software NVIVO®. Resultados: A maior parte das participantes eram mulheres, brancas, residiam na mesma cidade onde trabalham, graduaramse em instituições privadas. Foram organizadas duas categorias: repercussões da autonomia e seus desdobramentos para a resolutividade das necessidades em saúde dos usuários; e (des)conhecimento sobre a regulamentação das práticas da enfermeira: desafios para a autonomia. Conclusão: As enfermeiras compreendem a importância da autonomia para suas práticas, mas enfrentam interferências no cotidiano do trabalho, tanto por parte da gestão, da estrutura dos serviços ou da necessidade do uso de protocolos que garantam o exercício de sua atividade profissional de modo autônomo. Objective: To analyze nurses' perception of autonomy to exercise their practices in the context of primary health care. Methods: A descriptive study with a qualitative approach that analyzed 108 interviews carried out with nurses from the four capitals of the southeastern region of Brazil. The interviews, guided by a semi-structured script, were recorded and transcribed. The data produced were processed and explored with the help of the NVIVO® software. Results: Most of the participants were women, white, lived in the same city where they work, and graduated from private institutions. Two categories were organized: repercussions of autonomy and its consequences for solving users' health needs; and (lack of) knowledge about the regulation of nursing practices: challenges for autonomy. Conclusion: Nurses understand the importance of autonomy for their practices, but they face interference in their daily work, either by management, the structure of services or the need for protocols that guarantee the exercise of their professional activity. Métodos: Estudio descriptivo con enfoque cualitativo que analizó 108 entrevistas realizadas con enfermeros de las cuatro capitales de la región sureste de Brasil. Las entrevistas, guiadas por un guión semiestructurado, fueron grabadas y transcritas. Los datos producidos fueron procesados y explorados con la ayuda del software NVIVO®. Resultados: La mayoría de los participantes eran mujeres, de raza blanca, vivían en la misma ciudad donde trabajan y egresaron de instituciones privadas. Se organizaron dos categorías: repercusiones de la autonomía y sus consecuencias para la solución de las necesidades de salud de los usuarios; y (falta de) conocimiento sobre la regulación de las prácticas de enfermería: desafíos para la autonomía. Conclusión: Los enfermeros comprenden la importancia de la autonomía para sus prácticas, pero enfrentan interferencias en su trabajo diario, ya sea por parte de la dirección, la estructura de los servicios o la necesidad de protocolos que garanticen el ejercicio de su actividad profesional. RESUMO Objetivo: Avaliar a acurácia de médicos generalistas em reconhecer a retinopatia diabética por meio da retinografia colorida, com um curso de capacitação com duração de 2 horas, comparando a capacidade de rastrear e classificar a retinopatia diabética em relação ao exame presencial com oftalmologista. Métodos: No primeiro braço do estudo, de 142 pacientes diabéticos incluídos, avaliaram-se 274 olhos, em que esses pacientes foram examinados com oftalmoscópio binocular indireto e classificados quanto ao grau da retinopatia diabética. No segundo braço do estudo, 14 médicos não especialistas em oftalmologia receberam um treinamento de 2 horas para o diagnóstico de retinopatia diabética com a retinografia colorida e se aferiu a acurácia desses profissionais em rastrear a retinopatia diabética antes e depois do curso de capacitação, utilizando as retinografias obtidas na primeira frente do estudo. Resultados: Verificou-se aumento significativo da sensibilidade (82% para 99%) e da especificidade (44% para 83%) na detecção da retinopatia diabética pelos médicos generalistas, com o curso de capacitação. Conclusão: O médico generalista capacitado pode avaliar a retinopatia diabética por meio da retinografia colorida, sendo o programa de rastreamento dessa complicação do diabetes uma proposta viável e benéfica ao país.


ABSTRACT Objective: To assess the accuracy of general practitioners in recognizing diabetic retinopathy through color retinal retinography, with a two-hour training course, by comparing the capacity of screening and classifying diabetic retinopathy. Methods: In the first arm of the study, of 142 diabetic patients included, 274 eyes were evaluated, and these patients were examined with an indirect binocular ophthalmoscope and classified according to the degree of diabetic retinopathy. In the second arm of the study, 14 physicians who are not specialists in ophthalmology received two-hour training to diagnose diabetic retinopathy with color retinography, and the accuracy of these professionals in screening for diabetic retinopathy before and after the training course was measured using the photographic images obtained on the first front of the study. Results: There was a significant increase in sensitivity (82% to 99%) and specificity (44% to 83%) in detecting diabetic retinopathy by general practitioners, after attending the training. Conclusion: Qualified general practitioners can assess diabetic retinopathy through color retinography, and the screening program for this diabetes complication is a viable and beneficial proposal for the country.

ABSTRACT

Abstract Objective: to analyze the factors related to sleep disorders reported by Nursing professionals during the COVID-19 pandemic. Method: this is a cross-sectional and analytical study conducted with Nursing professionals from all Brazilian regions. Sociodemographic data, working conditions and questions about sleep disorders were collected. The Poisson regression model with repeated measures was used to estimate the Relative Risk. Results: 572 answers were analyzed, which revealed that non-ideal sleep duration, poor sleep quality and dreams about the work environment were predominant during the pandemic, with 75.2%, 67.1% and 66.8% respectively; as well as complaints of difficulty sleeping, daytime sleepiness and non-restorative sleep during the pandemic were reported by 523 (91.4%), 440 (76.9%) and 419 (73.2%) of the Nursing professionals, respectively. The relative risk of having such sleep disorders during the pandemic was significant for all variables and categories studied. Conclusion: non-ideal sleep duration, poor sleep quality, dreams about the work environment, complaints regarding difficulty sleeping, daytime sleepiness and non-restorative sleep were the predominant sleep disorders among Nursing professionals during the pandemic. Such findings point to possible consequences on health, as well as on the quality of the work performed.


Resumo Objetivo: analisar os fatores relacionados às alterações no sono relatadas pelos profissionais de enfermagem durante a pandemia de COVID-19. Método: trata-se de um estudo transversal e analítico, realizado com profissionais de enfermagem de todas as regiões do Brasil. Foram coletados dados de caracterização sociodemográfica, condições de trabalho e questões sobre alterações de sono. Para estimar o Risco Relativo foi utilizado o modelo de regressão de Poisson com medidas repetidas. Resultados: foram analisadas 572 respostas, as quais revelaram que a duração não ideal do sono, a má qualidade do sono e os sonhos com o ambiente de trabalho foram predominantes durante a pandemia, com 75,2%, 67,1% e 66,8% respectivamente, assim como as queixas de dificuldade ao dormir, sonolência diurna e sono não restaurador durante a pandemia foram relatadas por 523 (91,4%), 440 (76,9%) e 419 (73,2%) dos profissionais de enfermagem, respectivamente. O risco relativo de apresentar tais alterações de sono, durante a pandemia foi significativo para todas as variáveis e as categorias estudadas. Conclusão: duração não ideal do sono, má qualidade do sono, sonhos com o ambiente de trabalho, queixas de dificuldade ao dormir, sonolência diurna e sono não restaurador foram as alterações do sono predominantes entre os profissionais de enfermagem durante a pandemia. Estes achados apontam para possíveis consequências na saúde, bem como na qualidade do trabalho realizado.


Resumen Objetivo: analizar los factores relacionados con los trastornos del sueño que informaron los profesionales de enfermería durante la pandemia de COVID-19. Método: se trata de un estudio transversal y analítico realizado con profesionales de enfermería de todas las regiones de Brasil. Se recolectaron datos sobre caracterización sociodemográfica, condiciones de trabajo y preguntas sobre trastornos del sueño. Para estimar el Riesgo Relativo se utilizó el modelo de regresión de Poisson con medidas repetidas. Resultados: se analizaron 572 respuestas, que revelaron que durante la pandemia predominaron la duración del sueño no ideal, la mala calidad del sueño y los sueños sobre el ambiente laboral, con 75,2%, 67,1% y 66,8% respectivamente, además 523 (91,4%), 440 (76,9%) y 419 (73,2%) profesionales de enfermería manifestaron quejas de dificultad para conciliar el sueño, somnolencia diurna y sueño no reparador durante la pandemia, respectivamente. El riesgo relativo de padecer trastornos del sueño durante la pandemia fue significativo para todas las variables y categorías estudiadas. Conclusión: la duración del sueño no ideal, la mala calidad del sueño, los sueños sobre el ambiente laboral, las quejas de dificultad para conciliar el sueño, la somnolencia diurna y el sueño no reparador fueron los trastornos del sueño predominantes en los profesionales de enfermería durante la pandemia. Estos hallazgos indican posibles consecuencias para la salud, así como para la calidad del trabajo realizado.

ABSTRACT

Resumo A pesquisa tem por objetivo descrever o perfil sociodemográfico e de saúde dos profissionais de enfermagem no contexto da pandemia da COVID-19 e propõe uma análise reflexiva sobre a essencialidade da categoria frente às demandas intrínsecas dos pacientes e do sistema de saúde brasileiro, especialmente, no contexto da emergência de saúde pública deflagrada pelo avanço exponencial do vírus SARS-CoV-2. O estudo revela a relação entre as injustiças históricas e os diferentes tipos de desigualdade que afetam e causam a vulnerabilidade da profissão, com fulcro na apresentação de potenciais perspectivas decorrentes desse processo histórico e dos acontecimentos recentes.


Abstract The present study aims to describe the sociodemographic and health profile of nursing professionals in the context of the COVID-19 pandemic and propose a reflective analysis on the essentiality of the category facing the intrinsic demands of patients and the Brazilian health system, especially in the context of the public health emergency triggered by the exponential advance of the SARS-CoV-2 virus. This study reveals the relationship between historical injustices and the different types of inequality that impacted and caused the vulnerability of the profession, with an emphasis on the presentation of potential perspectives arising from this historical process and recent events.

ABSTRACT

Resumo O artigo analisa a associação entre níveis de fadiga por compaixão e engajamento no trabalho com a COVID-19 em profissionais de enfermagem. Estudo longitudinal, tipo antes e depois, realizado com profissionais de enfermagem da linha de frente, nos períodos pré-pandêmico e pandêmico. Utilizou-se as versões brasileiras da Professional Quality of Life Scale e da Utrecht Work Engagement Scale. Observou-se altos níveis de satisfação por compaixão (≥43,0), baixos níveis de burnout (<23,0) e estresse traumático secundário (<23,0); e níveis altos de vigor (≥4,0 e ≤4,99), absorção (≥4,0 e ≤4,99) e escore geral (≥4,0 e ≤4,99). Houve correlação moderada, negativa e significativa do burnout com vigor (r: -0,505; p-valor: <0,001), no pré-pandemia; e com o escore geral, no pré-pandemia (r: -0,543; p-valor: <0,001) e período pandêmico (r: -0,458; p-valor: <0,001). Não houve alterações nos níveis de engajamento no trabalho. Profissionais com fadiga por compaixão tiveram redução do vigor, absorção e escore geral, classificados como médios no período pandêmico (≥2,0 e ≤3,99); e aumento da dedicação, que era baixa (≥1,0 e ≤1,99) no pré-pandemia. Concluiu-se não haver associação danosa entre fadiga por compaixão e engajamento no trabalho com a COVID-19 nos profissionais de enfermagem.


Abstract This article examines the association between levels of compassion fatigue and work engagement with COVID-19 in nursing professionals. A longitudinal, before-and-after study was conducted with nursing professionals working in the frontline in the pre-pandemic and pandemic periods. Our study applied the Brazilian versions of the Professional Quality of Life Scale and the Utrecht Work Engagement Scale. High levels of compassion satisfaction (≥43.0), low levels of burnout (<23.0) and secondary traumatic stress (<23.0), as well as high levels of vigor (≥4.0 and ≤4.99), absorption (≥4.0 and ≤4.99), and overall score (≥4.0 and ≤4.99) were observed. Moderate, negative, and significant correlations of burnout with vigor (r: -0.505; p-value: <0.001), in the pre-pandemic period; and with overall score, in the pre-pandemic (r: -0.543; p-value: <0.001) and pandemic periods (r: -0.458; p-value: <0.001), were also observed. No changes in levels of work engagement were found. Professionals with compassion fatigue showed decreased vigor, absorption, and overall score, rated as medium in the pandemic period (≥2.0 and ≤3.99), and an increased dedication, which was low (≥1.0 and ≤1.99) in the pre-pandemic period. It was concluded that there is no harmful association between compassion fatigue and work engagement with COVID-19 in nursing professionals.

ABSTRACT

Introdução: Ao desempenhar e promover práticas que visam a melhoria da saúde, a equipe de enfermagem se assumiu como protagonista e linha de frente em tempos de pandemia de COVID-19. Logo, fica evidente a exposição pessoal-profissional-familiar; a carência de pessoal e a insegurança laboral-emocional, além das consequências que vão recair sobre os contextos de saúde e de trabalho destes profissionais. Objetivo: Analisar os contextos de saúde e trabalho de profissionais de enfermagem durante a pandemia de COVID-19. Metodologia: Estudo de método misto, combinando as abordagens por meio da estratégia de incorporação concomitante - QUAN(qual), realizado em Minas Gerais, Brasil, entre agosto e dezembro de 2020. Aplicou-se, via ligação telefônica, um roteiro de perguntas a 58 profissionais enfermeiros, técnicos e auxiliares de enfermagem. O roteiro continha perguntas abertas e fechadas ligadas a questões sociodemográficas e aos contextos e condições de saúde, trabalho e COVID-19. Realizou-se análise quantitativa descritiva e Análise de Conteúdo Temática. Resultados: A maioria mulheres (93,1%) técnicas de enfermagem (69%). 39,7% trabalhavam em plantão noturno; 58,6% relataram terem sofrido violências no trabalho e 48,3% relataram diagnósticos de transtorno mental. Os contextos de trabalho na pandemia evidenciaram necessidade de estímulos e valorização profissional e relações com fatores institucionais, a dinâmica e organização do trabalho, condições adequadas de trabalho e ao favorável relacionamento interpessoal, enquanto que contextos de saúde evidenciaram relação com interesses, oportunidades, rotina, normalidade, esperança e tenacidade que seriam trazidos pelo fim da COVID-19. Conclusão: A pandemia de COVID-19 fortaleceu um contexto de saúde e de trabalho crítico, amedrontado e inseguro da equipe de Enfermagem brasileira, exacerbando a necessidade de providências, ações e políticas que considerem a Saúde do Trabalhador como estruturantes para a organização dos serviços.


Introducción: Al realizar y promover prácticas que tienen como objetivo la mejora de la salud, el equipo de enfermería asumió el protagonismo en los tiempos de la pandemia por COVID-19. Por tanto, es evidente la exposición personal-profesional-familiar, la falta de personal y la inseguridad laboral-emocional, además de las consecuencias que recaerán sobre la salud y los contextos laborales de este grupo profesional. Objetivo: Analizar el contexto sanitario y laboral de profesionales de enfermería durante la pandemia por COVID-19. Metodología: Estudio de método mixto, combinando los abordajes a través de la estrategia de incorporación concomitante - QUAN(qual), realizado en Minas Gerais, Brasil, entre agosto y diciembre de 2020. Se aplicó un guion de preguntas, mediante llamada telefónica, a 58 profesionales, personal técnico y auxiliares de enfermería. El guion contenía preguntas abiertas y cerradas relacionadas con cuestiones sociodemográficas y los contextos y condiciones de salud, trabajo y COVID-19. Se realizó un análisis cuantitativo descriptivo y un análisis de contenido temático. Resultados: La mayor parte de la muestra estuvo compuesta por mujeres (93.1 %) y personal técnico de enfermería (69 %). El 39.7 % trabajaba en turno de noche, el 58.6 % declaró haber sufrido violencia en el trabajo y el 48.3 % informó de diagnósticos de trastorno mental. Los contextos de trabajo en la pandemia mostraron la necesidad de estímulo y valoración profesional y las relaciones con los factores institucionales, la dinámica y la organización del trabajo, las condiciones adecuadas de trabajo y la relación interpersonal favorable. Los contextos de salud, mientras tanto, mostraron una relación con los intereses, las oportunidades, la rutina, la normalidad, la esperanza y la tenacidad que se produciría al final de la COVID-19. Conclusión: La pandemia de la COVID-19 fortaleció un contexto de salud y trabajo crítico, atemorizado e inseguro del equipo de enfermería brasileño, exacerbando la necesidad de providencias, acciones y políticas que consideren la salud de la persona trabajadora como estructurante para la organización de los servicios.


Introduction: By performing and promoting practices that aim to improve health, the nursing team has assumed itself as a protagonist during the COVID-19 pandemic. Therefore, the personal, professional, and family exposure is evident; the lack of personnel and occupational security, in addition to the consequences on the health and work contexts of these professionals. Objective: To analyze the health and work contexts of nursing professionals during the pandemic of COVID-19. Methodology: This is a mixed-method study that combines approaches through the concurrent incorporation strategy - QUAN(qual), conducted in Minas Gerais, Brazil, between August and December 2020. A script of questions was applied over the phone to 58 professional nurses, nursing technicians, and nursing assistants. The script included open and closed questions related to sociodemographic issues and health, work, and COVID-19 contexts and conditions. A descriptive quantitative analysis and thematic content analysis were performed. Results: The majority were women (93.1%) nursing technicians (69%). The 39.7% of the participants worked the night shift; the 58.6% reported experiencing violence at work and the 48.3% was diagnosed with mental disorders. The work contexts during the pandemic showed that there was a need for encouragement and professional evaluation; they also portrayed the relationships with the institutional factors, the work dynamics and the organization, the appropriate working conditions and the favorable interpersonal relationships. On the other hand, health contexts showed the relationship between interests, opportunities, routine, normality, hope and tenacity that would occur at the end of COVID-19. Conclusion: The pandemic of COVID-19 strengthened a critical, fearful, and insecure health and work context of the Brazilian nursing team, exacerbating the need for provisions, actions and policies that consider worker's health crucial for the organization of services.

Conclusão: Acolhimento, escuta ativa, reconhecimento social, oferta de serviços de saúde mental, melhorias na organização do trabalho e estímulo à adoção de hábitos de vida saudáveis são ações que podem ser adotadas por gestores no ambiente laboral com os profissionais de enfermagem diante da pandemia da covid-19


Conclusion:Welcoming, active listening, social recognition, offering mental health services, improvements in work organization, and encouraging the adoption of healthy lifestyle habits are actions that can be adopted by managers in the work environment with nursing professionals in the face of the COVID-19 pandemic


Conclusión: acogida, escucha activa, reconocimiento social, oferta de servicios de salud mental, mejora en la organización del trabajo y fomento de la adopción de hábitos de vida saludables son acciones que pueden adoptar los gestores en el trabajo con los profesionales de enfermería ante la pandemia de covid -19

Objetivo: conhecer as perspectivas de enfermeiras sobre a utilização da telenfermagem durante a pandemia da COVID-19. Métodos: estudo exploratório e qualitativo, realizado entre os meses de janeiro e fevereiro de 2021, em um município de Mato Grosso, Brasil. Para a coleta de dados, utilizou-se entrevistas individuais, analisadas por meio da técnica de análise de conteúdo sob a luz da teoria das transições de Afaf Ibrahim Meleis. Resultados: as participantes mostraram-se receptivas à telenfermagem, porém a baixa acessibilidade e o desconhecimento acerca deste recurso pela população podem influenciar na aplicabilidade e no alcance. Em contrapartida, houveram participantes que destacaram a importância do cuidado presencial associado à telenfermagem. Na condição de pacientes, as enfermeiras não foram unânimes em relação à sua aceitação. Conclusão: as participantes reconhecem a otimização que a tecnologia pode oferecer, porém, dada a incipiência da inserção da telenfermagem no contexto brasileiro e a modos particularidades de compreender a necessidade de avanço e aprimoramento de práticas, há uma parcela que apresenta ainda dificuldades nesse processo de transição quer seja para a concretização dessa forma de assistência como na utilização para seus cuidados pessoais. Objective: to know the perspectives of nurses on the use of telenursing during the COVID-19 pandemic. Methods: exploratory and qualitative approach, carried out between January and February 2021, in a municipality of Mato Grosso, Brazil. For data collection, individual interviews were used, analyzed using the content analysis technique under the light of Afaf Ibrahim Meleis' theory of transitions. Results: the participants were receptive to telenursing, but the low accessibility and lack of knowledge about this resource by the population can influence its applicability and reach. On the other hand, there were participants who highlighted the importance of face-to-face care associated with telenursing. As patients, nurses were not unanimous about their acceptance. Conclusion:: the participants recognize the optimization that technology can offer, however, given the incipience of the insertion of telenursing in the Brazilian context and the particular ways of understanding the need to advance and improve practices, there is a portion that still presents difficulties in this process transition, whether for the realization of this form of assistance or in the use for their personal care. Objetivo: Caracterizar os estudos de intervenção cadastrados por enfermeiros no Registro Brasileiro de Ensaios Clínicos. Métodos: Estudo descritivo, com abordagem quantitativa, realizado mediante acesso virtual ao Registro Brasileiro de Ensaios Clínicos, para busca dos estudos cadastrados na plataforma. Após leitura dos 3350 resumos disponíveis, os 320 cadastrados por enfermeiros foram selecionados para compor a amostra. Foi realizada a análise descritiva, no software R. Resultados: Foram encontrados estudos registrados entre 2010 e 2019, dos quais 49,7% localizavam-se na região Sudeste, 49,3% cadastrados por doutores, 46,3% filiados a universidades federais. A área da enfermagem mais estudada (18,8%) tratou-se de saúde do adulto, 72,9% possuíam dois braços, 53,1% com recrutamento concluído, 50,3% realizados com cegamento e o tipo de intervenções mais estudado (21,9%) foi a educativa. Conclusão: A maioria dos estudos de intervenção cadastrados por enfermeiros no Registro Brasileiro de Ensaios Clínicos testaram intervenções educativas, em adultos, possuíram cegamento e foram randomizados. ABSTRACT

Objetivo: Analisar as decisões exaradas pelo Poder Judiciário de Rondônia, referente aos pedidos de provisão de profissionais de enfermagem para os hospitais públicos, com vistas a identificar qual tem sido a fundamentação predominante na litigância. Métodos: O procedimento adotado foi a análise documental indireta. Procurou-se explorar a análise sob uma abordagem quali-quantitativa. O recorte documental deteve-se na atuação do TRF1 Rondônia, quanto à 26 decisões, referentes a 23 processos judiciais. Resultados: Evidenciou-se que os pedidos de provisão de profissionais de enfermagem foram todos denegados na Seção Judiciária da capital do estado, enquanto foram deferidos nas Subseções do interior. Contudo, para o suprimento de um numerário específico de enfermeiros e/ou técnicos de enfermagem, foi indeferido em todos os juízos do TRF1. Prevaleceu o provimento do pedido de provisão de enfermeiros, fundamentado no princípio da legalidade. Conclusão: Não há um perfil decisório predominante no TRF1, haja vista ser todas as decisões de primeira instância, inexistindo decisões de mérito prolatadas pelo Tribunal em segunda instância. Dessa forma, cada juízo mantém seu próprio tracejado cognitivo jurisdicional, inexistindo uma uniformização para decisões sobre a mesma matéria. Prevaleceu o provimento do pedido de provisão de enfermeiros, fundamentado no princípio da legalidade. (AU)


Objective: To analyze the decisions issued by the Judiciary of Rondônia regarding requests for the provision of nursing professionals to public hospitals, in order to identify the predominant grounds in litigation. Methods: The procedure adopted was indirect documentary analysis. We sought to explore the analysis from a qualitativequantitative approach. The documentary cut focused on the performance of TRF1 Rondônia, regarding 26 decisions related to 23 judicial processes. Results: It was evidenced that requests for the provision of nursing professionals were all denied in the Judicial Section of the state capital, while they were granted in the Subsections of the interior. However, for the supply of a specific number of nurses and/or nursing technicians, it was denied in all the courts of TRF1. The provision of the request for nurses prevailed, based on the principle of legality. Conclusion: There is no predominant decision-making profile in TRF1, since all decisions are at the first instance, with no merit decisions made by the Court at the second instance. Thus, each judge maintains their own jurisdictional cognitive tracing, without there being a uniformity for decisions on the same subject. The provision of the request for nurses prevailed, based on the principle of legality. (AU)

El recorte documental se centró en la actuación del TRF1 Rondônia, en relación con 26 decisiones relativas a 23 procesos judiciales. Resultados: Se evidenció que todas las solicitudes de provisión de profesionales de enfermería fueron denegadas en la Sección Judicial de la capital del estado, mientras que fueron concedidas en las Subsecciones del interior. Sin embargo, para el suministro de un número específico de enfermeros y/o técnicos de enfermería, se denegó en todos los juzgados del TRF1. Prevaleció la provisión de la solicitud de enfermeros, fundamentada en el principio de legalidad. Conclusión: No hay un perfil decisivo predominante en el TRF1, ya que todas las decisiones son de primera instancia, no existiendo decisiones de mérito pronunciadas por el Tribunal en segunda instancia. De esta manera, cada juez mantiene su propio trazado cognitivo jurisdiccional, sin haber una uniformidad para las decisiones sobre la misma materia. ABSTRACT

Following the revision of the structure and content of the Model Core Curriculum for Medical Education to be more outcome-based and the legal status of the medical practice performed by medical students in the clinical clerkship, we have revised the Guideline for Participatory Clinical Clerkship. The following items were revised or newly described : significance of enhancing the participatory clinical clerkship, scope of medical practice, confidentiality, patient consent, patient consultation and support service, objectives of the clinical clerkship, simulation education, departments where the clinical clerkship is conducted, assessment in the clinical practice setting, CC-EPOC, and entrustable professional activities. A foundation has been established to promote seamless undergraduate and postgraduate medical education. However, future work is needed to examine the specific level of performance expected at the end of the clinical clerkship and department-specific clinical practice goals and educational strategies.

Objective To put forward suggestions for improving the scheme of general practice for functional communities from the perspective of supply and demand,guide the efficient use of the resources of general practice by the communities,and incorporate the general practice of communities into hierarchical diagnosis and treatment management. Methods In July 2021,stratified random sampling was employed to conduct the questionnaire surveys of the young and middle-aged (demand side) and the general practitioners (supply side),respectively.SPSS 20.0 was used for data analysis. Results The two sides had the same cognition about the main reasons for not signing a contract with a family doctor,which were the lack of knowledge about general practitioners and the lack of face-to-face communication opportunities.They had the same response about the form of services,with high acceptance of medical services via WeChat,outpatient consultation,and the internet.There were differences in service content between the two sides.The top three demands of the young and middle-aged were appointment registration and referral in superior hospitals,medication guidance,and massage,acupuncture,and moxibustion.The top service self-rated by general practitioners was personalized guidance and report interpretation of physical examination,and the bottom was massage,acupuncture,and moxibustion. Conclusions The general practice varies between the demand and supply sides.General practitioners should be encouraged to enter and learn functional communities and provide personalized services,thus improving the general medical service in functional communities.

ABSTRACT

ObjectiveTo systematically review the clinical experience of four sessions of Masters of Traditional Chinese Medicine and two sessions of National Famous Chinese Medicine Practitioners in treating ulcerative colitis (UC). Data mining and analysis were conducted to clarify the diagnosis and treatment ideas and characteristics of prescription used by these famous doctors in treating UC. MethodsRelevant literature on the treatment of UC by renowned doctors was retrieved from the establishment of the database until March 31, 2023. The literature was collected from databases such as China National Knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform, Chinese Science and Technology Periodicals Database, and China Biomedical Literature Database. The data mining techniques including frequency analysis, association rules, and cluster analysis were conducted using the Ancient and Modern Medical Case Cloud Platform V2.3.5. ResultsA total of 157 literatures were included in this study, including 115 clinical case data. The study found that UC can be categorized into 14 types of syndrome patterns for treatment, including large intestine dampness-heat syndrome (75,65.22%), syndrome of dampness stagnancy due to spleen deficiency (23, 20.00%), spleen-kidney yang deficiency syndrome (21, 18.26%). The main affected organs were the spleen (85, 73.91%) and large intestine (75, 65.22%), and they were closely related to liver (24, 20.87%) and the kidney (21, 18.26%). The predominant pathogenic factors were dampness (83, 72.17%) , heat (80, 69.57%) and qi deficiency (65, 56.52%). The treatment involved 30 kinds of treatment methods, including heat-clearing and dampness-draining method (75, 65.22%), pleen-tonifying and qi-boosting method (25,21.74%) and spleen-invigorating and dampness-transforming method (23, 20.00%). The medication involved 187 ingredients, with the most commonly used being heat-clearing herbs (37, 19.79%) and tonifying herbs (27, 14.44%). The tastes of the herbs were mostly sweet (85, 45.45%) , bitter (80, 42.78%) , and pungent (71, 37.97%). The association rules revealed 16 high-frequency combinations mainly composed of Huanglian (黄连), Baishao (白芍) and Gancao (甘草) along with Baizhu (白术), Fuling (茯苓), Muxiang (木香) and Danggui (当归). ConclusionFamous doctors are skilled in diagnosing and treating UC based on the differentiation of the zang-fu organs and qi-blood. The key pathological mechanism is “spleen deficiency as the root, and large intestine damp-heat as the manifestation”. The core treatment approach is “heat-clearing, spleen-tonifying, and dampness-draining”, with the inclusion of “regulating qi and blood, and balancing cold and heat”.

@#Introduction: The Coronavirus disease pandemic of 2019 (COVID-19) has had psychological implications and altered the public’s well-being. In response to the pandemic, mental health practitioners play an essential role in mitigating the adverse psychological effects among the public. However, as the demand for mental health services increases, the responsibilities of mental health professionals are far stretched from what they can handle. Therefore, this research is aimed to explore the psychological impact of the pandemic on the mental health practitioners in Selangor and their coping strategies. Method: A total of 10 mental health practitioners who are clinical psychologists and counselors participated in the one-to-one interview. The collected data were analysed using thematic analysis. Result: The higher client loads, the transition to virtual therapy, burnout, and anxiety were found to have an impact on the mental health practitioners’ well-being. Despite the challenging psychological impact, the mental health practitioners expressed a high level of satisfaction with their jobs, which they attributed to the supportive organisational culture and their intrinsic drive for selecting this profession. The mental health practitioners have reported that perceived organisational support, social support, and self-care approaches are effective coping mechanisms. Mindfulness skills such as breathing techniques and meditation are often practiced by the participants to calm the mind and body from a long day of work. Conclusion: The implication of the study contributes to breaking the stigmatisation of help seeking behaviour among the helping professions, as well as urging the policy makers, governments, and foundations to play a part in providing accessible and quality mental healthcare to the society.

ABSTRACT

Objective:To survey on the training needs of general practitioners (GPs) in the context of public health emergency.Methods:A questionnaire was developed through literature review and interviews with health department leaders, administrators and GPs in grassroots health institutions as well as experts in the field. The contents of questionnaire included the basic information, perception of epidemic impact, knowledge and skills to learn, and the preferred training contents related to public health emergency. The questionnaire survey was conducted among 430 GPs from 44 grass-roots institutions of 6 cities/districts in Suzhou selected by multi-stage convenient sampling method from April to May 2022. Kano model was used to analyze the needs and preferences of general practitioners for training contents related to the prevention and control of COVID-19 epidemic.Results:A total of 391 valid questionnaires were collected, with an effective response rate of 90.93%. More than half of respondents hoped to learn about the general diagnosis and treatment, first aid, and emergency management of common cardiovascular and cerebrovascular diseases, as well as common community emergencies through training. In the training contents domains, special training for COVID-19 prevention and control was necessary; knowledge and practical skills were expected as the basic training needs; research and teaching ability and self-regulation ability were training needs of charm attributes; and professional quality, doctor-patient communication, and management ability were all training needs of no difference attributes.Conclusion:In the context of public health emergency, the specialized training of epidemic prevention and control is prioritized for GPs, the training of knowledge and practical skills, research and teaching abilities and self-regulation abilities are also needed.

ABSTRACT

Objective:To develop an assessment scale for measuring knowledge, attitude and practice of general practitioners in primary care towards diabetic microangiopathy.Methods:The knowledge-attitude-practice (KAP) theory was used as a framework to draw up the initial item pool based on related literature and guidelines during March to October 2022. Two rounds of Delphi consultation were conducted among 15 experts from general medicine and related fields. The positive coefficient, authority coefficient and coordination coefficient of experts were calculated, and the threshold table of indexes was screened, and the final assessment scale of KAP was developed after the two rounds of consultation.Results:The developed primary care general practitioners′ KAP assessment scale for diabetic microangiopathy consists of 3 primary indexes and 52 secondary indexes(25 of knowledge scale, 13 of attitude scale, 14 of practice scale). The positive coefficient of experts was 100.0% and the authority coefficient of experts was 0.89 in both rounds of consultation; and the coordination coefficient of experts was 0.319 and 0.322 for the first and second consultations, respectively (both P<0.05). Conclusion:A KAP assessment scale of diabetic microangiopathy for primary care general practitioners has been developed in this study. The expert positive coefficient, authority coefficient and expert coordination coefficient meet the requirements, which provides reference for evaluating the management ability of diabetic microangiopathy of primary care general practitioners.

ABSTRACT

Objective:To survey the knowledge levels about diagnosis and management of asthma among general practitioners in community health centers in Shanghai and to analyze influencing factors.Methods:A questionnaire survey was conducted from May to June 2022 among general practitioners from 80 community health care centers in Shanghai. The questionnaire contained the basic information of the responders; knowledge about the diagnosis of asthma (symptoms, diagnostic methods, interpretation of bronchodilation and fractional exhaled nitric oxide, FeNO tests); and treatment of asthma (choosing and usage of therapeutic drugs). Logistic regression was used to analyze.Results:A total 324 general practitioners completed the questionnaire survey. Among them 221 (68.21%) had bachelor degree; 200 (61.73%) were attending doctors; the median working duration was 11 years; 174 (53.70%) had training on asthma knowledge in last 3 years. The survey results showed that 55 (16.98%) responders misinterpreted results of bronchodilation test, and 90(5.86%)misinterpreted results of FeNO test for diagnosis of asthma; 244 responders (75.31%) suspected implication of long-term cough for diagnosis of asthma; 277(85.49%)would order bronchodilation text to suspected patients; 273(84.26%)prescribed inhaled corticosteroid plus long-acting beta-2 agonist to asthma patients. For mild asthma patients, 144 responders(44.44%)suggested to use budesonide formoterol as needed; for moderate to severe asthma, 174(53.70%)suggested to use budesonide formoterol regularly and as needed, 100(30.86%)suggested to use salmeterol fluticasone regularly and salbutamol as needed. Logistic analysis showed that longer working duration was correlated with higher misinterpretation rate of bronchodilation and FeNO tests( OR=0.798,95% CI:0.694-0.918, P=0.002; OR=0.859,95% CI:0.739-0.998, P=0.047). The accuracy rate of all the questions was significantly higher in general practitioners who had training on asthma knowledge than those who didn′t receive training in last 3 years(all P<0.05). Conclusions:Most general practitioners in community health service centers in Shanghai have relatively high levels of knowledge about diagnosis and treatment of asthma. Years of work experience and training experience can affect the levels of asthma knowledge among community general practitioners.

ABSTRACT

Objective:To investigate the current status of psychosomatic medical service competence and training needs among general practice resident physicians.Methods:An online questionnaire survey was conducted from August to September 2021, among general practice resident physicians selected by cluster sampling from 4 affiliated hospitals of Tongji University. The questionnaire contained the basic information, the self-assessment and training needs of psychosomatic medical service competence of participants, and psychosomatic medical service requirements of patients in the community.Results:A total of 159 questionnaires were distributed, and 140 were collected (88.1%). The survey showed that 25.7% (36/140) of participants believed that 50% or more patients had physical symptoms accompanied by emotional problems; 29.3% (41/140) thought that 50% or more patients had sleep problems, but 60.0% (84/140) of them failed to provide appropriate treatment. The rate of "core knowledge and concepts of psychosomatic medicine" was 47.5% (665/1 400) and the correct rate of "common psychosomatic diseases" was (64.6±23.4)%. Merely 21.4% (30/140) participants reported satisfaction with the psychosomatic medical service competence; 62.8% (88/140) respondents believed that the coverage rate of the above-mentioned competence training in the current general practice residential training program was less than 50%, and 66.4% (93/140) supported that more psychosomatic training was "necessary". The acceptance rate of online training, workshops, MDT, and clinical practice ranged from 56.4% (79/140) to 63.4% (89/140).Conclusions:The survey indicates that there are large needs for psychosomatic medical services among patients in community, but the competence and confidence of general practice resident physicians to provide the service are significantly insufficient. Therefore, it is suggested to strengthen the training of psychosomatic medical service competence in the general practice residency training program.
