ABSTRACT

@#[Objective] To construct a Nomogram model for the prediction of essential hypertension (EH) risks with the use of traditional Chinese medicine (TCM) syndrome elements principles in conjunction with cutting-edge biochemical detection technologies. [Methods] A case-control study was conducted, involving 301 patients with essential hypertension in the hypertensive group and 314 without in the control group. Comprehensive data, including the information on the four TCM diagnoses, general data, and blood biochemical indicators of participants in both groups, were collected separately for analysis. The differentiation principles of syndrome elements were used to discern the location and nature of hypertension. One-way analysis was carried out to screen for potential risk factors of the disease. Least absolute shrinkage and selection operator (LASSO) regression was used to identify factors that contribute significantly to the model, and eliminate possible collinearity problems. At last, multivariate logistic regression analysis was used to both screen and quantify independent risk factors essential for the prediction model. The “rms” package in the R Studio was used to construct the Nomogram model, creating line segments of varying lengths based on the contribution of each risk factor to aid in the prediction of risks of hypertension. For internal model validation, the Bootstrap program package was utilized to perform 1000 repetitions of sampling and generate calibration curves. [Results] The results of the multivariate logistic regression analysis revealed that the risk factors of EH included age, heart rate (HR), waist-to-hip ratio (WHR), uric acid (UA) levels, family medical history, sleep patterns (early awakening and light sleep), water intake, and psychological traits (depression and anger). Additionally, TCM syndrome elements such as phlegm, Yin deficiency, and Yang hyperactivity contributed to the risk of EH onset as well. TCM syndrome elements liver, spleen, and kidney were also considered the risk factors of EH. Next, the Nomogram model was constructed using the aforementioned 14 risk predictors, with an area under the curve (AUC) of 0.868 and a 95% confidence interval (CI) ranging from 0.840 to 0.895. The diagnostic sensitivity and specificity were found to be 80.7% and 85.0%, respectively. Internal validation confirmed the model’s robust predictive performance, with aconsistency index (C-index) of 0.879, underscoring the model’s strong predictive ability. [Conclusion] By integrating TCM syndrome elements, the Nomogram model has realized the objective, qualitative, and quantitative selection of early warning factors for developing EH, resulting in the creation of a more comprehensive and precise prediction model for EH risks.

ABSTRACT

Based on the traditional Chinese medicine (TCM) theories of holism, syndrome differentiation and treatment, and preventive treatment of disease, the concept of whole course management of tumors throughout the treatment process is advocated. The strategy includes preventive treatment before tumor therapy, focusing on protecting the body's vital qi and strengthening the spleen and Qi. TCM formulas such as Sijunzi Decoction (四君子汤), Shenling Baizhu San (参苓白术散) and Xiangsha Liujunzi Decoction(香砂六君子汤) are used to enhance platelet reserves. The main therapeutic methods during tumor conventional therapy include preventing disease transformation and strengthening vital qi to eliminate pathogenic factor, by invigorating spleen and soothing liver, nourishing liver and kidney, purging fire for removing toxin, and removing blood stasis for promoting tissue regeneration. TCM formulas such as Xiangsha Liujunzi Decoction (香砂六君子汤), Shenling Baizhu Powder (参苓白术散), Fuzi Lizhong Decoction (附子理中汤), Zhibai Dihuang Decoction (知柏地黄汤), and Qiangen Powder (茜根散)are used to reduce platelet destruction and depletion during tumor therapy and reverse the worsening trend. After tumor conventional therapy, the focus is on preventing relapse, adjusting the balance of Yin and Yang, by nourishing the kidneys, spleen, and lungs. TCM formulas such as Dingkun Pill (定坤丹)and Zuogui Pill (左归丸) are used with modifications to prevent thrombocytopenia. The staged prevention and treatment of tumor treatment-induced thrombocytopenia using TCM can maximize the benefits for patients.

ABSTRACT

Health-preservation thoughts of the four major schools (Confucianism, Buddhism, Taoism and Medicine) have important reference values for the health preservation and health care industry. The core of health-preservation thought of the Taoism is taking mind training as the focus and physical exercise as the supplementary means, Taoism is nature-admiring, emphasizes the idea of natural laws and inaction and humanity follows natural laws, as well as advocates following nature's courses, realizing the passive inaction, and keeping calmness, also, it advocates the way of keeping good health through preserving vital essence, energy and mentality, in order to achieve the purpose of training vital essence into energy, training energy into mentality and maintaining complete vital essence, energy and vitality. Through long-term development, Medicine has formed a relatively complete set of theoretical and systematic health-preservation methods. The holistic concept of Medicine emphasizes the uniformity among human, nature and society, considers human body as an organic whole, and advocates the health-preservation thoughts of living in harmony with nature, accommodating the four seasons, keeping good circulation of energy and blood, dredging collaterals, nourishing essence and replenishing energy, accumulating healthy energy and repelling pathogenic factors, maintaining the inseparability of the body and spirits, as well as taking proper exercises and keeping mental calmness. Based on the health-preservation thoughts of the four major schools, this paper summarizes the modern health-preservation guiding thoughts of dredging collaterals, nourishing essence, taking exercises and keeping mental calmness, in order to provide the theoretical guidance for traditional Chinese medicine health-preservation services.

ABSTRACT

Based on the systematic summary of the results of the fourth general survey of traditional Chinese medicine resources, the cultivation of large varieties of Chinese material medica and the latest research on health industrial development, the novel concepts and scientific connotations of generalized science of Chinese material medica are put forward, and the basic ideas and methods of a new Chinese medicine academic system, the cultivation system of large varieties of Chinese medicinal materials and the application system of the large health industry are constructed. This kind of generalized science of Chinese material medica, rooted in the traditional Chinese culture and the theory of "preventive treatment of disease", can avoid the narrow prospect induced by the increasing specialization and refinement of knowledge of science of Chinese material medica. It will play an important role in the modernization, industrialization, internationalization of traditional Chinese medicine.

ABSTRACT

Objective To investigate the effect of heat-sensitive point moxibustion on high-normal blood pressure in persons with yin or yang deficiency.Methods Eighty-six persons with high-normal blood pressure were randomized to a treatment group (42 cases, 20 with yin deficiency and 22 with yang deficiency) and a control group (44 cases, 19 with yin deficiency and 25 with yang deficiency). The control group received health education and diet guidance and the treatment group, heat-sensitive point moxibustion in addition. Both groups were followed up twice a week with blood pressure measured. The clinical therapeutic effects, hypertension diagnosis rates and neutral constitution transformation percentages were compared between the two groups of persons with different constitutions after one month of treatment.Results The total efficacy rate was 90.0% in persons with yin deficiency and 100% in persons with yang deficiency of the treatment group and correspondingly 78.9% and 92.0% of the control group; there was a statistically significant difference between the two groups (P<0.05). Post-treatment hypertension diagnosis rate 15.0% in persons with yin deficiency and 9.1% in persons with yang deficiency of the treatment group and correspondingly 36.8% and 16.0% of the control group; there was a statistically significant difference between the two groups (P<0.05). Post-treatment neutral constitution transformation percentage was 23.3% in persons with yin or yang deficiency of the treatment group and 13.6% in those of the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusions Heat-sensitive point moxibustion can reduce high-normal blood pressure and hypertension incidence and effectively rectify biased constitution in persons with yin or yang deficiency.

ABSTRACT

Zhangjiagang Hospital of Traditional Chinese Medicine, as a collaborative unit of the project, participated in the clinical consistency evaluation in project of TCM Practice guidelines for Prevention and Treatment of Biqiu (allergic rhinitis) organized by the State Administration of Traditional Chinese Medicine. The evaluation results showed that the guideline met the clinical practice requirements. This article summarized the clinical evaluation experience from the three aspects: The diagnosis in TCM and Western medicine is clear and definite, but the English translation needs to be considered; The definition of remission phase of Biqiu (allergic rhinitis) is difficult, to interfere by identifying the constitution is innovative; The advantages of "preventive treatment of diseases" in TCM should be fully played in role, improving the satisfaction of patients.

ABSTRACT

Objective To observe the effects of different TCM exercises for preventive treatment of diseases on sleep quality and multidimensional mental health level of college students.Methods According to the willing of the research objects, 132 college students were divided into Yijinjing group, Baduanjin group, Wuqinxi group, Tai Chi group, and jogging group. They exercised 45 min each time, 5–6 times per week, received central guidance every two weeks, and exercise intensity was timely adjusted based on master of physical activity and reaction to the situation of college students, for 2 months. PSQI, BFS, SDS and SAS before and after intervention in each group were observed. Results After intervention, five kinds of exercises could improve sleep situation of college students, and had their own focus in sleep quality, falling-asleep time, sleep time, sleep efficiency, sleep disorders, sleep medication, daytime dysfunction. There was statistical significance in BFS activity score, thought score, anger score, excitation score, and depression score after intervention in Yijinjing group compared with before intervention (P<0.05); There was statistical significance in activity score, total joy score, calm total score after intervention in Wuqinxi group compared with before intervention (P<0.05). There was statistical significance in SAS total scores of college students in Yijinjing group, Baduanjin group, and jogging group after intervention compared with before intervention (P<0.05). There was statistical significance in SAS standard scores of college students in Yijinjing group, Baduanjin group, and jogging group after intervention compared with before intervention (P<0.05). There was no statistical difference in SDS scores in the five groups before and after intervention (P>0.05).Conclusion 5 kinds of exercises can improve the sleep quality from different aspects. Yijinjing group and Wuqinxi group can improve the mental health of college students, of which Yijinjing group can bi-directionally regulate the positive and negative psychological indicators of college students.

ABSTRACT

Hypertension is a chronic medical condition which is characterized by persistent elevation of arterial blood pressure.Hypertension is the most recognized risk factor for cardiovascular diseases.It has become one of the most important medical conditions threatening the public health worldwide.To achieve early prevention and reduce the incidence of hypertension and other related cardiovascular diseases,prehypertension had been proposed in 2003 as an important component of first-line preventive strategies targeting cardiovascular diseases.Progress on the clinical significance and mechanism associated with prehypertension has been made since the notion was proposed.Results from in-depth mechanistic studies of prehypertension have shed new light on the early prevention and treatment of hypertension and hypertension-caused target organ damage.It is worth noting that the concept of "treat before the onset of the disease" stated in traditional Chinese medicine (TCM) has been applied to the clinical research of prehypertension with increasingly recognized results.The current review thus summarized recent progress on the clinical research of prehypertension and TCM-guided prevention and treatment of prehypertension.

Objective · To understand the awareness of preventive treatment of disease in community residents, analyze the influencing factors, and explore ways to improve the awareness and service utilization. Methods · Community residents over 18 years of age who were from 26 community health service centers which were randomly selected in Shanghai were surveyed with a self-designed questionnaire. General descriptive analysis was used for basic information and chi-square test for influencing factors. OR values were calculated and Logistic regression analysis was performed. Results · The awareness rate in community residents was low (57.1%). They got to know the preventive treatment of disease mainly through radio or television (48.4%), newspapers or magazines (36.9%), community propaganda (35.5%), and so on. The influencing factors were age, occupation, education, income, and ways to get to know the preventive treatment of disease. In short, the awareness rate and cognitive level in vulnerable groups were lower. Conclusion · The effects of current propaganda of the preventive treatment of disease is limited. The propaganda should be more scientific and pertinent, so as to deepen the cognition of community residents and promote the service utilization.

ABSTRACT

Objective To understand the influence factors and ratio of TCM“preventive treatment of disease”service selection of elderly people in communities of Changsha city. Methods Stratified random sampling method was used. Elderly people in two communities in Changsha city participated in the survey and were invited to fill in the questionnaires. The questionnaire was divided into three parts: social demographic characteristics of community residents;self-acknowledgement of community residents’ health (self-rated health and health management);community residents’ attitudes towards “preventive treatment of disease” service. Results Totally 360 questionnaires were distributed. Except for unqualified questionnaires, a total of 320 questionnaires were returned, with effective rate of 88.8%. Selection ratio of“preventive treatment of disease” service of elderly people in community was 86.2%. Different educational levels, professions, insurances, family incomes, self-rated health and health management had statistical significance with elderly people's selection of“preventive treatment of disease”service (P<0.05). Health management, self-rated health and the family income were important factors influencing elderly people's selection of TCM “preventive treatment of disease” services. Conclusion The carrying out of “preventive treatment of disease” service in community requires improvement of community residents’ awareness of self health management, and cheap route among people.

ABSTRACT

[Objective] To discuss the effect of the community chronic disease management mode which combines physical identification and“preventive treatment”in control of GLU and TG of diabetes. [Method] 300 patients with T2 DM were randomly divided into the control group and intervention group with 150 patients in each group. The intervention group were physical y identified, and the management mode based on theories of“preventive treatment”including health education, diet control, exercise and medication used in the intervention group according to the result of physical identification, while the general management was used in the control group. 1 year later, changes of patients ’GLU and TG were observed. [Result] The community chronic disease management mode which combines physical identification and “preventive treatment”can improve T2 DM patients’GLU and TG, and reduce the GLU and TG of patients with QiXu, YangXu, and ShiRe types of physique. [Conclusion] The community chronic disease management mode which combines physical identification and“preventive treatment”is proved to be effective, and it improves the management of diabetes in communities.

With the improvement of people's living standards, the degree of aging is increasing, so the health demands is increasing greatly. In China, General Medicine attracts more and more attention, and the training of general practitioner is important and urgent. First of all ,“providing good service for the patients” should be the central concept in the GP training. Secondly, based on the national conditions, we should learn the modern medicine theories and absorb the essence of“Preventive Treatment of Dis-ease”in traditional Chinese medicine. In the actual implementation process we should pay attention to the integration of western philosophy and modern technology. Finally , we shoulduse the example of Xuanwu Hospital's general practitioner standardization training to conduct empirical research tothe above prelimi-nary understanding of the situation.

ABSTRACT

This article was aimed to analyze literatures on preventive treatment of disease in acupuncture and moxi-bustion in order to understand its status and development trend. Bibliometrics and statistics method in NoteExpress were used in the analysis of related literatures about preventive treatment of disease in acupuncture and moxibustion from 1981 to 2013, which were included in CNKI. The analysis and research were from aspects such as year distri-bution, keyword, journal distribution, author distribution, and fund distribution. The results showed that there were 537 articles in total from 1981 to 2013. The number of literatures was in an overall growing trend.“Zusanli”,“Inner Canon of Huangdi”,“sub-health”, and“clinical research”were high frequency keywords. The journal distribu-tion conformed to the Brad Ford’s law. The author distribution conformed to the Lotka’s law. The number of fund papers was also increased. It was concluded that research on preventive treatment of disease in acupuncture and moxibustion was still in the period of rapid development and had not reached maturity. The preventive treatment of disease in acupuncture and moxibustion had broad application prospects. However, the research still required to be strengthened.

[Objective] To emphasize the importance of early prevention and treatment, this article discusses on the theory and application of preventive treatment of disease from traditional Chinese medicine in prevention and treatment of carotid atherosclerosis in patients with diabetes. [Methods] With de-tailed research into related study of risk factors affecting carotid atherosclerosis in type 2 diabetic patients from recent years, comprehensive summary of tra-ditional risk factors and risk factors of the latest findings were demonstrated. The prevention and treatment of carotid atherosclerosis in type 2 diabetes mel itus was discussed in two aspects by theory of“preventive treatment of disease. [Result] In diabetic patients without carotid atherosclerosis, early and timely intervention can prevent the occurrence and development of the diabetes vascular lesions; In diabetic patients with carotid atherosclerosis, active treatment can reduce the incidence of stroke, myocardial infarction and other critical care. [Conclusion] In patients with type 2 diabetes, risk factors affect-ing carotid atherosclerosis should be control ed and intervened early, which can effectively reduce the occurrence and development of the diabetes vascular lesions to improve the quality of life, but early diagnosis technology and indicators need to be further researched.
