ABSTRACT

Objective@#To understand the status quo and differences of the rights and interests of primary medical staff in a community, and to provide targeted suggestions for improving the rights of such staff.@*Methods@#From October to November 2018, a questionnaire survey was conducted on all the medical staff of 8 community health service centers in a district of Shanghai, and 562 valid samples were obtained.@*Results@#The primary medical staff had the highest demands for " pay rise" (4.783±0.598), " paid vacation" (4.569±0.873)and " additional reserve fund" (4.553±0.963); the lowest demands were placed on " education-based promotion" (3.797±1.382), " refresher training" (3.801±1.314), and " opinion feedback mechanism" (4.018±1.223). At the same time, the rights and interests of these staff in different occupational categories were significantly different(P<0.05)in " daily reimbursement" , " housing subsidy" , " refresher training" and " opinion feedback mechanism" .@*Conclusions@#The salary demands of the primary medical staff are the most obvious, and there are differences in the demands of medical staff in different occupational categories. It is suggested to establish a more reasonable salary system and pay attention to the differentiated appeals of different medical personnel.

ABSTRACT

Objective@#To learn the knowledge,attitude and practice(KAP)of primary healthcare workers on influenza vaccine and 23-valent pneumonia vaccine in Xiuzhou District of Jiaxing,and to provide basis for vaccination of influenza vaccine and 23-valent pneumonia vaccine.@*Methods@#From November 2018 to January 2019,the primary healthcare workers from eight community health service centers in Xiuzhou District were selected by multi-staged random sampling method. A self-designed questionnaire was used to investigate the KAP of influenza vaccine and 23-valent pneumonia vaccine among primary healthcare workers. Logistic regression analysis was employed to select the influencing factors for vaccination and recommendation behaviors of primary healthcare workers. @*Results@# Totally 105 healthcare workers(95.45%)completed the survey. Among them,86(81.90%)and 76(72.38%)workers believed influenza vaccine and 23-valent pneumonia vaccine effective;87(82.85%)and 88(83.81%)workers believed the two vaccines safe;82(78.10%)and 58(55.24%)workers knew the vaccination procedures;58(55.24%)and 34(32.38%)workers knew the key recommended population of vaccines;77(73.33%)workers knew that the vaccine costs could be paid by medical insurance. The vaccination rate of influenza or(and)pneumococcal vaccine among the respondents was 27.62%,and the recommendation rate was 46.67%. The results of multivariate logistic regression analysis showed that healthcare workers who were female(OR=7.250,95%CI:1.388-37.883),believed the two vaccines effective(OR=4.390,95%CI:1.167-16.515),believed the two vaccines safe(OR=11.977,95%CI:2.417-59.343)and knew the payment method of vaccine cost(OR=11.945,95%CI:2.924-48.793)were more likely to vaccinate;healthcare workers who majored in public health(OR=2.641,95%CI:1.338-5.214),believed the two vaccines effective(OR=6.944,95%CI:1.655-29.134),knew the key recommended population of vaccines(OR=4.380,95%CI:1.125-17.053),had vaccinated the two vaccines(OR=7.902,95%CI:1.693-36.877)were more likely to recommend. @*Conclusion@#Primary healthcare workers in Xiuzhou District have less awareness of influenza vaccine and 23-valent pneumonia vaccine,then have lower vaccination rate(27.62%)and recommendation rate(46.67%).

ABSTRACT

Objective To explore the cognitive level and demand of chronic disease prevention and treatment integration in the County hospitals and primary health care institutions. Methods A stratified cluster sampling method was used to investigate the medical staff of five county-level hospitals and 39 community health service centers in Liandu District, Yunhe County and Jingning County, and qualitative interviews and on-site questionnaire survey were carried out among 573 medical staff from August to October in 2016. Results A total of 252 medical personnel at the county level or above, accounting for 43.98%, and 321 medical personnel in primary health care institutions, accounting for 56.04%. And 96.86% of the medical staff thought it is necessary to integrate medical treatment and prevention. Only 32.98% think that the local medical and anti-integration were the real ones and only 36.13% have contacted the"top five prevention and control offices" at the county level. Two-way referral of key chronic patients and promotion of grassroots promotion of appropriate technology were better. And 77.38% of medical staff at medical institutions above the county level and 75.70% of medical personnel of primary medical institutions participated in the two-way referral work, with 66.67% of county level medical staff of above medical institutions and 93.46% medical staffs of primary medical institutions participated in the promotion of grassroots workplaces for appropriate technologies. And 82.72% of the medical staff held or participated in appropriate technical training courses for chronic diseases within one year, but the proportion of holding or participating in ≥3 times was only 24.08%. Conclusion The work that county level five platform to promote chronic disease prevention and control of chronic disease prevention and treatment of medical integration still need to be strengthened. We should use the appropriate training mode to improve comprehensive prevention and treatment of chronic diseases among primary medical staff.

ABSTRACT

Objective:To seek the main incentive factors that influencing medical staff′s enthusiasm through the medical staff satisfaction survey among the grass-roots level medical staff in Shaanxi province.Methods:Choosing the north, middle and south parts of Shaanxi province and using the questionnaire survey, factor analysis method and Krukal-Wallis test were used.Results:Main drive common factor affect the enthusiasm of the staff with train-ing and development, compensation and protection level, management mode and team cooperation, working condi-tions and working environment, and different characteristics of medical staff on incentive satisfaction differences. Conclusion:In order to improve medical staff′s work enthusiasm, it is necessary to combine the Shaanxi regional specific circumstances, to establish a suitable incentive mechanism.
