ABSTRACT

Objetivo: estudiar y comparar la inclinación de la cabeza entre niños con dentición primaria completa y dentición mixta primera fase. Materiales y métodos: el estudio incluyó a 80 voluntarios entre 4 y 9 años, a quienes se les tomó una fotografía facial de perfil derecho estandarizada, en Posición Natural de Cabeza. Éstas fueron analizadas mediante software de edición de imágenes para determinar la inclinación de la cabeza de cada voluntario. Para ello, se definió un plano (plano t-ex) formado por el punto Intertrágico (t) y Exocanto (ex) y se comparó respecto a la horizontal verdadera. Los datos obtenidos fueron tabulados, clasificados en dos grupos según el tipo de dentición y analizados estadísticamente. Resultados: el promedio de inclinación del plano t-ex fue de 22,7º en el grupo de niños con dentición primaria completa y de 25,8º en el grupo de niños con dentición mixta primera fase. La diferencia entre ambos grupos fue de 3,1º, con significancia estadística (p = 0,006). Conclusión: la inclinación de la cabeza de los niños con dentición mixta primera fase es mayor en sentido antihorario, respecto a la de los niños con dentición primaria completa.


Objective: to study and compare head tilt among children with complete primary dentition and first transitional period of the mixed dentition stage. Materials and methods: the study included 80 volunteers between 4 and 9 years of age, who were taken a standardized right profile facial photograph in a Natural Head Position. These photographs were analyzed using image editing software to determine the head tilt of each volunteer. For this, a plane (plane t-ex) formed by the Intertragic point (t) and Exocanth (ex) was defined and compared with respect to the true horizontal. The data obtained were tabulated, classified into two groups according to the type of dentition and statistically analyzed. Results: the average t-ex plane inclination was 22.7º in the group of children with complete primary dentition and 25.8º in the group of children with first transitional period of the mixed dentition stage. The difference between both groups was 3.1º, with statistical significance (p = 0.006). Conclusion: the head tilt of children with first transitional period of the mixed dentition stage is greater in an anti-clockwise direction, compared to that of children with complete primary dentition.

ABSTRACT

Development of the primary and permanent dentition is a complex process wherein there is series of interactions between the ectoderm and ectomesenchyme. A cascade of signaling pathways occur in a spatio-temporal manner resulting in the development and eruption of the human dentition. Any developmental aberrations in shape, size, number, and position can lead to deviations from normal development of teeth. In this book, dental anomalies including gemination, fusion, concrescence, dilaceration, dens invaginatus, DE, taurodontism, enamel pearls, fluorosis, peg?shaped laterals, dentinal dysplasia, regional odontodysplasia and hypodontia etc. are discussed. Diagnosing dental abnormality needs a thorough evaluation of the patient and careful clinical and radiographical examination is required. Furthermore, more complex cases need multidisciplinary planning and treatment.

Resumo Introdução Crianças com defeitos de desenvolvimento de esmalte (DDE) são mais suscetíveis à hipersensibilidade dentinária, maloclusões e dificuldades relativas à adesão de materiais restauradores. O conhecimento sobre a prevalência de DDE e seus fatores associados permite aos profissionais obter maior compreensão desse problema de saúde bucal, minimizando, dessa forma, as sequelas e melhorando a saúde bucal e a qualidade de vida dos indivíduos afetados. Objetivo Determinar a prevalência de DDE e seus fatores associados na dentição decídua. Método Trata-se de estudo transversal com amostra probabilística de 566 pré-escolares de 5 anos de idade, matriculados em creches públicas e privadas de Teresina, Piauí, Brasil. Foi aplicado questionário socioeconômico e de história médica da criança aos responsáveis. Os exames clínicos foram conduzidos por um único examinador (kappa = 0,93 para DDE e 0,86 para cárie) que utilizou os índices DDE modificado e ceo-d. Foi realizada a análise descritiva dos dados, além dos testes qui-quadrado e exato de Fisher e a regressão de Poisson, considerando como significativo p ≤ 0,05. Resultados A prevalência de DDE foi de 33,7%. A média de dentes com DDE por criança foi de 3,12 ± 2,23. O tipo mais prevalente de DDE foi opacidade demarcada (9,5%). Os segundos molares superiores foram os dentes mais afetados (11,9%). Houve associação entre DDE e estudar em creche pública (p = 0,026) e cárie (p = 0,012). Crianças com experiência de cárie apresentaram maior prevalência de DDE (RP: 1,29; IC95%: 1,01-1,64). Conclusão A prevalência de DDE na dentição decídua foi de 33,7% e se mostrou associada à experiência de cárie.


Abstract Background Children with Developmental Defects of Enamel (DDE) are more susceptible to dentin hypersensitivity, malocclusions, and difficulties related to the adhesion of restorative materials. Knowledge about the prevalence and factors associated with DDE allows professionals to gain a greater understanding of this oral health problem, minimizing sequelae, improving oral health and quality of life of affected individuals. Objective To determine the prevalence and factors associated with DDE in primary dentition. Method This is a cross-sectional study assessing a random sample of 566 preschool five-year-old children, enrolled in public and private preschools in Teresina, Piauí, Brazil. It was applied the socioeconomic and medical history of the child questionnaire for the parents or guardians. Clinical examinations were conducted by a single examiner (kappa = 0.93 for DDE and 0.86 for caries) that used the modified DDE index and dmft. Descriptive analysis, chi-square and Fisher's exact tests, and Poisson regression were performed, considering significant p ≤ 0.05. Results the prevalence of DDE was 33.7%. The mean DDE teeth per child was 3.12 ± 2.23. The most prevalent type of DDE was demarcated opacity (9.5%). The second molars were the most affected teeth (11.9%). There was an association between DDE and study at public preschool (p = 0.026) and dental caries (p = 0.012). Children with experience of caries were more likely to DDE (PR: 1.29; 95% CI: 1.01 to 1.64). Conclusion prevalence of DDE in the primary dentition was 33.7% and was associated with caries experience.

RESUMEN: Introducción: Con el fin de prevenir la progresión de la caries interproximal no cavitada en dientes temporales, se ha generalizado el uso de estrategias mínimamente invasivas como la aplicación de sellantes, barniz de flúor o la resina infiltrante, ya sea combinadas o como monoterapia. Pese a lo anterior, hay incertidumbre con relación al efecto de la infiltración de resina en combinación con el barniz de flúor en dientes temporales. Métodos: Realizamos una búsqueda en Epistemonikos, la mayor base de datos de revisiones sistemáticas en salud, la cual es mantenida mediante el cribado de múltiples fuentes de información, incluyendo MEDLINE, EMBASE, Cochrane, entre otras. Extrajimos los datos desde las revisiones identificadas, analizamos los datos de los estudios primarios, realizamos un metaanálisis y preparamos una tabla de resumen de los resultados utilizando el método GRADE. Resultados y conclusiones: Se identificaron nueve revisiones sistemáticas que en conjunto incluyeron tres estudios primarios que corresponden a ensayos clínicos aleatorizados. Se concluye que la infiltración de resina probablemente reduce el riesgo de la progresión de la caries interproximal no cavitada en dientes temporales. No se encontraron estudios que evaluaran los eventos adversos. ABSTRACT: Introduction: Minimally invasive techniques are widely used in non-cavitated interproximal caries treatment in primary dentition. Sealants, fluoride varnish, or resin infiltration can be applied in conjunction or as monotherapy. There is uncertainty regarding the effect of resin infiltration in conjunction with fluoride varnish in primary dentition. Methods: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conducted a meta-analysis and generated a summary of findings table using the GRADE approach. Results and conclusions: We identified 9 systematic reviews including three studies overall, of which all were randomized trials. We conclude that resin infiltration plus fluoride varnish probably decreases the risk of progression of non-cavitated interproximal caries in primary dentition. No studies were found that looked at adverse effects.

ABSTRACT

Context: Preventive orthodontic treatments are performed in the primary or mixed dentition period and provide a proper development of occlusion to avoid dentofacial anomalies. Aims: This study aims to evaluate the Baby-Risk of Malocclusion Assessment Index (ROMA) index regarding ease of use, reproducibility, and the epidemiological assessment of major orthodontic problems in children aged 4–6-year-old in Kerman (Iran) kindergartens. Setting and Design: This cross-sectional study was conducted on 1000 children aged 4–6 in Kerman (Iran) kindergartens, selected by cluster sampling, and data were collected using the Baby-ROMA index. Methods: After recording a demographic data collection form, the clinical examination of the teeth was carried out by a dental student. The data were analyzed with SPSS 18 using t-test, analysis of variance, Chi-squared test, and Mann–Whitney test at a significance level of P < 0.05. Results: In this study, the most common cause of malocclusion was dental caries (7.5%), followed by crossbite (6.1%). Furthermore, 31.6% of the subjects had one type of malocclusion, and 20.5% needed monitoring the occlusion before the growth spurt, while 11.1% needed immediate orthodontic treatment (mostly 6-year-old children). Moreover, 15.3% of the subjects had systemic problems; 3.4% had craniofacial, 22% had dental, and 2% had functional problems. The prevalence of class I canine relationship in the right canines was 82.32%, with 82.5% on the left side. The prevalence of open bite was higher in females with thumb-sucking habit. Conclusion: The Baby-ROMA index presented good reliability and ease of use for evaluating early orthodontic treatment needs in primary and mixed dentition periods.

OBJECTIVES@#The study aimed at identifying salivary microbiota in caries-free Chinese preschool children using high-throughput sequencing.@*METHODS@#Saliva samples were obtained from 35 caries-free preschool children (18 boys and 17 girls) with primary dentition, and 16S ribosomal DNA (rDNA) V3-V4 hypervariable regions of the microorganisms were analyzed using Illumina MiSeq.@*RESULTS@#At 97% similarity level, all of these reads were clustered into 334 operational taxonomic units (OTUs). Among these, five phyla (Firmicutes, Proteobacteria, Actinobacteria, Bacteroidetes, and Candidate division TM7) and 13 genera (@*CONCLUSIONS@#Our results revealed the diversity and composition of salivary microbiota in caries-free preschool children, with little difference between male and female subjects. Identity of the core microbiome, coupled with prediction of gene function, deepens our understanding of oral microbiota in caries-free populations and provides basic information for associating salivary microecology and oral health.

ABSTRACT

Abstract Hallermann-Streiff syndrome (HSS) is a rare oculomandibulofacial discephaly with hypotrichosis that occurs as a sporadic mutation. It is characterized by abnormal findings especially in head and face. Dental anomalies occur in 50-80% of the patients. In this case report, facial-oral findings and the existing dental anomalies of the syndrome in a 6-year-old male patient diagnosed with HSS were identified. Dental rehabilitation and a 12-month follow-up of the patient were reported. Each case presented with this rare syndrome may contribute to the literature to determine the prognosis of the disease and to take protective and preventive measures.


Resumen El síndrome de Hallermann-Streiff (HSS) es una discefalia oculomandibulofacial rara con hipotricosis que ocurre como una mutación esporádica. Se caracteriza por hallazgos anormales, especialmente en cabeza y cara. Las anomalías dentales ocurren en 50-80% de los pacientes. En este reporte de caso, se identificaron los hallazgos faciales-orales y las anomalías dentales existentes del síndrome en un paciente masculino de 6 años diagnosticado con HSS. Se informó la rehabilitación dental y un seguimiento de 12 meses del paciente. Cada caso presentado con este síndrome raro puede contribuir a la literatura para determinar el pronóstico de la enfermedad y tomar medidas de protección y prevención.

Abstract Objectives: to investigate the prevalence of dental anomalies in complete deciduous dentition of children with NSCL/P. Methods: this study included 75 children with NSCL/P and 286 healthy control. In both groups the children had deciduous dentition with ages varying from 4 to 6 years. Clinical examination, panoramic and periapical radiographies were performed and dental anomalies of number and shape were considered. Results: there was a higher prevalence of dental anomalies in the case group, compared to the control group. In all, 42 dental anomalies were identified, 25.33% in the case group and 8.04% in control group (p<0.001). Therewas a higher frequency of dental anomalies in NSCL/P (47.36%), followed by non-syndromic cleft lip (31.57%) and non-syndromic cleft palate (21.05%). The occurrence of agenesis (p= 0.005) and twinning (p = 0.029) were higher in the case group. Conclusions: the occurrence of agenesis and dental twinning was more frequent in the case group and may contribute to the definition of oral cleft subphenotype.


Resumo Objetivos: investigar a prevalência de anomalias dentárias na dentição decídua completa de crianças com FL/PNS. Métodos: este estudo incluiu 75 crianças com FL/PNS e 286 controles saudáveis. Em ambos os grupos as crianças tinham dentição decídua com idade variando de 4 a 6 anos. Exame clínico, radiografias panorâmicas e periapicais foram realizadas e anomalias dentárias de número e forma foram consideradas. Resultados: houve maior prevalência de anomalias dentárias no grupo caso, comparado ao grupo controle. Ao todo 42 anomalias dentárias foram identificadas, 25,33% no grupo caso e 8,04% no grupo controle (p<0,001). Houve maior frequência de anomalias dentárias na FL/PNS (47,36%), seguida da fissura labial não sindrômica (31,57%) e da fissura palatina não sindrômica (21,05%). A ocorrência de agenesia (p= 0,005) e geminação (p=0,029) foram maiores no grupo caso. Conclusão: a ocorrência de agenesia e geminação dentária foram mais frequentes no grupo caso e pode contribuir para a definição de subfenótipos de fissuras orais.

Essa pesquisa trata-se de uma análise transversal do perfil do aleitamento de crianças que frequentam a Bebê Clínica, um Programa Educativo/Preventivo em Saúde Bucal, e a ocorrência de oclusopatias. O objetivo foi analisar a influência do perfil do aleitamento sobre a condição oclusal de crianças que participam de um programa odontológico educativo-preventivo. Participaram da pesquisa 400 crianças entre 27 e 48 meses, sendo 200 que frequentam a Bebê-Clínica e 200 de escolas do município de Araçatuba, não participantes da Bebê-Clínica, Esta pesquisa constou da coleta de informações retrospectivas sobre o tempo, o tipo e a forma de aleitamento, além dos hábitos de sucção não nutritiva, por meio de um questionário realizado com as mães. Foi realizada também uma avaliação clínica da oclusão dentária dessas crianças, utilizando o índice de má oclusão preconizado pela OMS. Os dados foram analisados pelo teste do qui-quadrado. Os resultados indicaram em ambos os grupos uma elevada porcentagem de mães que amamentaram seus filhos (acima de 84%); elevada porcentagem de crianças com hábitos de sucção não nutritiva (acima de 57%) e um elevado índice de má oclusão dentária, sendo a principal, a mordida aberta anterior. Considerando os aspectos estudados nesta pesquisa, verificamos que ambos os grupos apresentaram resultados semelhantes, portanto não havendo influência da participação no programa da Bebê-Clínica(AU)


This research deals with a cross-sectional analysis of the breastfeeding profile that is frequent in the Baby Clinic, in an Educational / Preventive Oral Health Program, and an occurrence of occlusions. The objective was to analyze the influence of the breastfeeding profile on the occlusal condition of children who participate in an educational-preventive dental program. Participated in the research 400 children between 27 and 48 months, 200 of them attending the Baby Clinic and 200 from schools in the city of Araçatuba, not participating in the Baby Clinic. This research consisted of collecting retrospective information about time, type and form of breastfeeding, and non-nutritive sucking habits, through a questionnaire conducted with mothers. A clinical evaluation of the dental occlusion of these children was also performed, using the malocclusion index recommended by WHO. Data were analyzed by chi-square test.The results indicated in both groups a high percentage of mothers who breastfed their children (over 84%); high percentage of children with non-nutritive sucking habits (above 57%) and a high rate of dental malocclusion, the main one being the anterior open bite.Considering the aspects studied in this research, we found that both groups presented similar results, therefore there was no influence of participation in the program of the Baby Clinic(AU)

ABSTRACT Introduction: early childhood caries (ECC) is a public health problem. Recognizing caries patterns in affected children can help improve oral health programs focused on the preschool population. The aim of this study was to identify caries patterns in 3- to 5-year-old children in a low to middle-low socioeconomic area. Methods: a calibrated dentist recorded caries lesions using ICDAS criteria in 548 children attending four day-care centers in a low to middle-low socioeconomic area. ECC and S-ECC prevalence and the proportion of affected teeth/surfaces by lesion type for homologous teeth were calculated. Results: a total of 419 (76.5%) and 238 (43.4%) children had ECC and S-ECC, respectively. Average dmft was 3.7±3.7 and average affected surfaces were 5.8±7.7. Occlusal surfaces showed the highest caries experience, varying from 17.7% to 36.1%, showing statistical significance when compared to other molar surfaces. The upper smooth anterior surfaces were affected from 0.2% to 17.2%, while lower smooth anterior surfaces showed values between 0.0% and 6.8%. The percentage of dental caries experience in second molars varies from 37.1% to 42%, while in lower central and lateral incisors the values range from 1.8% to 4.6%. Conclusion: the specific caries pattern in preschool children with high prevalent ECC from a middle-low-income area indicate the need to design programs aimed at detecting early sings of dental caries in specific locations, as well as disease control strategies.


RESUMEN Introducción: la caries de la infancia temprana (CIT) es un problema de salud pública. Identificar el patrón de caries en los niños afectados puede ser útil para mejorar los programas de salud bucal centrados en la población preescolar. El objetivo del presente estudio consistió en identificar patrones de caries en niños de 3 a 5 años de edad en un área socioeconómica baja y media-baja. Métodos: un dentista calibrado registró lesiones de caries utilizando criterios ICDAS en 548 niños que asisten a cuatro guarderías de un área socioeconómica baja y media-baja. Se calculó la prevalencia de CIT y CIT-S (severa) y la proporción de dientes/superficies afectados por tipo de lesión para dientes homólogos. Resultados: hubo un total de 419 (76,5%) CIT y 238 (43,4%) CIT-S. El cpod promedio fue de 3,7 ± 3,7 y el promedio de superficies afectadas fue de 5,8 ± 7,7. Las superficies oclusales presentaron la mayor experiencia de caries, en un rango de entre 17,7% y 36,1%, lo que indica una significación estadística en comparación con otras superficies molares. Las superficies lisas anteriores superiores se vieron afectadas entre el 0,2% y el 17,2%, mientras que las superficies lisas anteriores inferiores mostraron valores entre 0,0% y 6,8%. El porcentaje de experiencia de caries dental en los segundos molares varía del 37,1% al 42%, mientras que en los incisivos centrales y laterales inferiores oscila entre el 1,8% y el 4,6%. Conclusión: el patrón específico de caries en niños en edad preescolar con alta prevalencia de CIT en un área de ingresos medios-bajos indica la necesidad de diseñar programas que incluyan acciones destinadas a detectar caries dentales en lugares específicos, así como estrategias para el control de la enfermedad.

Pulpectomies in primary molars are often hindered by several factors, including anatomical and physiological characteristics of posterior primary teeth and young patients' lack of cooperation with laborious treatments. This study was undertaken in search of easier but equally effective therapies that could eliminate infection, preserve the teeth and avoid extractions. The aim of the study was to estimate and compare clinical and radiographic success between pulp treatment with 3MixMP and pulpectomy with MaistoCapurro paste in primary necrotic molars. A longitudinal prospective study was conducted at the Department of Comprehensive Pediatric Dentistry of the Faculty of Dentistry of the University of Buenos Aires (20152017). The study included 46 primary molars with necrotic pulp of children without immune or metabolic compromise. Children and their legal guardians provided assent and informed consent. Selected molars were randomly divided into 2 groups: G1: Pulpectomy treatment with MaistoCapurro paste; and G2: Treatment with 3MixMP paste. Treatments were evaluated at 1, 3, 6, 12 and 18 months (intra and interrater agreement 0.92 and 0.84). Clinical success was considered to be the absence of any of the following: pain, sensitivity to percussion or palpation, swelling, fistula and nonphysiological mobility, while radiographic success was considered to be: absence of internal or external nonphysiological resorption, no progression or reduction of radiolucent periapical/interradicular lesion and evidence of bone regeneration. Percentages, 95% C.I., and CHI2 were calculated for the comparison between groups. Overall clinical success was 91.5% and 87.5% (p=0.48) and overall radiographic success was 88.3% and 82.3% (p=0.31) for G1 and G2 respectively. No significant clinical or radiographic difference was found between groups. Both treatments showed similar clinical and radiographic behavior during the study period (AU)


Las pulpectomías en molares primarios se ven dificultadas frecuentemente por las características anatómicas y fisiológicas de éstos y por la escasa colaboración que brindan los pacientes de corta edad ante tratamientos tan laboriosos. En búsqueda de terapéuticas más sencillas, pero igualmente eficaces, que consigan eliminar la infección para conservar las piezas y evitar las exodoncias, se ha planteado como objetivo de este estudio: estimar y comparar la proporción de éxito clínico y radiográfico entre el tratamiento pulpar con 3MixMP y la pulpectomía con pasta de MaistoCapurro en molares primarios con necrosis. Se realizó un estudio longitudinal y prospectivo en la Cátedra de Odontología Integral Niños de la Facultad de Odontología de la Universidad de Buenos Aires (2015 2017). Formaron parte del estudio 46 molares primarios con diagnóstico de necrosis pulpar, de niños sin compromiso inmunológico ni metabólico y que junto con sus responsables legales brindaron el asentimiento y el consentimiento informado. Los molares seleccionados fueron divididos aleatoriamente en 2 grupos: G1: Tratamiento de pulpectomía con pasta de MaistoCapurro y G2: Tratamiento con pasta 3MixMP. Los tratamientos fueron evaluados al mes, 3, 6, 12 y 18 meses (concordancia intraexaminador 0,92 e interexaminador 0,84), considerando como éxito clínico la ausencia de dolor, sensibilidad a la percusión y palpación, edema, fístula y movilidad no fisiológica; y como éxito radiográfico, ausencia de reabsorción interna o externa no fisiológica, no progresión o reducción de la lesión radiolúcida interradicular/periapical y evidencia de regeneración ósea. Se calcularon porcentajes, I.C 95% y CHI2 para la comparación. El éxito clínico global fue de 91,5% y 87,5% (p=0.48) y el éxito radiográfico global de 88,3% y 82,3% (p=0.31) para G1 y G2 respectivamente, sin diferencias significativas entre ambos grupos. En los periodos estudiados ambos tratamientos mostraron comportamientos clínico y radiográfico semejantes (AU)

ABSTRACT

La leche materna es el alimento que provee al niño desde su nacimiento de todos los nutrientes que necesita para su crecimiento y desarrollo físico y psicológico. Su efecto sobre el tejido dental ha sido objeto de controversias, ya que se ha visto que actúa sobre la mineralización del esmalte y se ha asociado a caries de la infancia temprana. La leche humana tiene diversos componentes protectores para el esmalte dental pero al mismo tiempo contiene factores que pueden generar la pérdida de minerales. El propósito de esta revisión es exponer los elementos que producen la desmineralización y la remineralización del esmalte y proponer algunas medidas que apoyen la integridad de este tejido, así como recomendar la lactancia materna como la alimentación principal que el infante debe recibir durante el primer año de vida. La leche humana tiene diversas proteínas y elementos que ayudan en el proceso de desmineralización y remineralización del esmalte dental de la primera dentición. Se puede concluir que, la leche materna es el alimento ideal para los niños en los primeros años de vida. Por lo que hay que promover la lactancia materna como el método preferido para alimentar a los bebés. Se debe implementar la higiene desde que el primer diente erupciona en boca. Esta medida evitará que el posible efecto desmineralizante de la leche ejerza un efecto irreversible sobre el esmalte dental. Es conveniente orientar a las madres sobre el almacenamiento de la leche materna de tal manera que al ser consumida por el niño, no se encuentre acidificada, y no produzca un efecto dañino sobre el esmalte.


Breast milk provides the child of all the nutrients he needs for his growth and physical and psychological development beginning at birth. Its effect on dental tissue has been the subject of controversy, since it has been seen to act on the mineralization of enamel and has been associated with early childhood caries. Human milk has various protective components for tooth enamel but at the same time it contains factors that can cause the loss of minerals. The purpose of this review is to review the elements that produce the demineralization and remineralization of enamel and propose some measures that support the integrity of this tissue, as well as to recommend breastfeeding as the main feeding source that the infant should receive during the first year of life. Human milk has various proteins and elements that help in the process of demineralization and remineralization of the tooth enamel of first dentition. It can be concluded that, breast milk is the ideal food for children during the first years of life. Therefore, breastfeeding should be promoted as the preferred method of feeding babies. Oral hygiene should be implemented as soon as the first tooth erupts. This measure will prevent the possible demineralizing effect of milk from exerting an irreversible effect on tooth enamel. It is advisable to guide mothers on the storage of breast milk so that when consumed by the child, it is not acidified, and does not produce harmful effects on the enamel.

Abstract Skeletal Class III is a malocclusion characterized by anterior crossbite as a result of an abnormal skeletal maxillary and mandibular base discrepancy, which involves disharmony of craneofacial skeleton and profile. The preferred management for children having skeletal Class III malocclusion with retruded maxilla and/or prognathic mandible is the use of devices that encourage the growth and anterior movement of the maxilla bone and/or restrict the exessive mandible growth. The orthopedic treatment consisting of a face mask with rapid maxillary expansion (RME) produces the most dramatic results in the shortest period of time. The purpose of this article is to review a quick correction of skeletal class III maloclussion in the primary dentition through a case example with use of a face mask plus rapid maxillary expansion therapy in a 5 year-old male patient due to a combination of retruded maxilla and protruded mandible, in primary dentition, who was treated with a Petit face mask in conjunction with a bonded RME intraoral appliance added with bite blocks. The first evident occlusal outcomes were a clockwise rotation of the mandible, a positive overjet of 3 mm, a correct overbite, a canine Class I relationship, and a bilateral flush terminal plane. After discussing the present clinical case report and the related published literature, we concluded that skeletal class III malocclusions should be treated as soon as the first clinical signs of abnormal craniofacial growth are recognized, during the first years of life.


Resumen La clase III esquelética es una maloclusión caracterizada por mordida cruzada anterior como resultado de una discrepancia esquelética de la base maxilar y mandibular, que implica desarmonía craneofacial y del perfil. El tratamiento de elección para niños con maloclusión clase III esquelética con mandíbula maxilar y/o prognática es el uso de dispositivos que estimulan el crecimiento y el movimiento anterior del hueso maxilar y/o restringen el crecimiento excesivo de la mandíbula. El tratamiento ortopédico que consiste en una máscara facial con expansión maxilar rápida (EMR) produce resultados en un período de tiempo más corto. El propósito de este artículo es revisar una corrección rápida de la maloclusión de clase III esquelética en la dentición primaria mediante el uso de una máscara facial más terapia de expansión maxilar rápida en un paciente masculino de 5 años debido a una combinación de maxilar retruido y mandíbula prominente, en dentición primaria, que fue tratada con una mascarilla Petit junto con un dispositivo intraoral unido con bloques de mordida. Los primeros resultados evidentes fueron una rotación en sentido horario de la mandíbula, un overjet positivo de 3 mm, una sobremordida correcta, una relación canina de Clase I y un plano terminal de descarga bilateral. Después de discutir el presente informe de caso clínico y la literatura publicada relacionada, concluimos que las maloclusiones de clase III esqueléticas deberían tratarse tan pronto como se reconozcan los primeros signos clínicos de crecimiento craneofacial anormal, durante los primeros años de vida.

ABSTRACT

A mordida cruzada funcional, caracterizada pelo desvio funcional durante o fechamento mandibular, acomete, em sua maioria, crianças em idade precoce. Uma das terapias preconizadas para a remoção de interferências oclusais e correção de mordida cruzada nas fases de dentadura decídua e mista é a terapia de Planas, baseada nos conceitos de reabilitação neuro-oclusal. As pistas Planas, quando comparadas com os aparelhos ortopédicos e/ou ortodônticos convencionais, apresentam baixo custo, além de muitas outras vantagens, como não depender da colaboração do paciente, ser simples, eficaz e estável ao longo do tempo. A técnica consiste, basicamente, de ajuste oclusal, seja por remoção (desgastes seletivos) ou por acréscimos de resina composta. O artigo tem como objetivo descrever o caso clínico de um paciente portador de mordida cruzada posterior funcional, na fase da dentadura mista, tratada de forma interceptiva com Pistas Indiretas Planas. The functional crossbite, characterized by functional deviation during jaw closing, affects mostly children at an early age. One of the recommended therapies for removal of occlusal interferences and correction of crossbite during the deciduous and mixed dentition is Planas therapy, based on the concepts of neuro-occlusal rehabilitation. The Planas Tracks, when compared to conventional orthopedic and/or orthodontics appliances, are low cost, besides many other advantages, as to be independent on patient cooperation, to be simple, eficiente, and stable over time. The technique consists, basically, of occlusal adjustment, by removal or addition of composite resin. The article aims to describe the case of a patient with posterior crossbite, at the stage of mixed dentition treated on an interceptive way with Planas Indirect Tracks. It is also described the preparation method, advantages, and technical instructions.(AU)

Talon cusps are very common in permanent dentition, while rarely reported in primary dentition. Unilateral is more common than bilateral occurrence of talon cusps and more common in males than females. The occurrence of bilateral occurrence of talon cusps in primary dentition has not been reported very frequently. The purpose of the present case report was to describe a rare case of bilateral talons in 4-year old child.

Objective @#To investigate the prevalence of malocclusion in primary dentition among preschool children in Kunming. To analysze the relationship between malocclusion and oral bad habits, genetic factors and the caries prevalence in preschool children. @*Methods@#According to economic conditions and distribution area of Kunming school, a total of 2 116 preschool children from nine kindergarten in Kunming city were surveyed. Oral examniation of 2 116 preschool children were conducted by trained dentists to investigate the prevalence of malocclusion and the prevalence of dental caries in Kunming. A self-designed questionnaire was used to investigate the oral habits and genetic factors of the children in the study and to analyze the correlation between malocclusion and oral bad habits, genetic factors and caries rate of deciduous teeth.@*Results@# Among the 2 116 preschool children examined, 1 516 were malocclusion, the prevalence of malocclusion was 71.64%, and the rate of dental caries was 63.50%. Bad habits and genetic factors were the risk factors of malocclusion, and there was no statistical difference in the prevalence of malocclusion between the tested children and those with or without caries. @*Conclusion @#The prevalence of malocclusion in the preschool children in Kunming city is high. The dentist is required to do the work of preventing bad habits. Parents need to improve the awareness of oral health care, together to do the early diagnosis of malocclusion, early diagnosis, early treatment.

ABSTRACT

O objetivo do presente estudo foi avaliar a associação entre má oclusão e o impacto na qualidade de vida relacionada à saúde bucal em crianças pré-escolares e suas famílias. Inicialmente, foi criado um instrumento específico para a avaliação. O instrumento foi desenvolvido e teve suas propriedades psicométricas avaliadas, bem como passou pelo processo de validação em uma amostra não probabilística. O instrumento apresentou validade e resultados psicométricos satisfatórios. No entanto, para que os resultados pudessem ser confirmados e a validade avaliada de maneira mais criteriosa, um novo estudo foi realizado em uma amostra representativa da mesma população. Assim, uma amostra randomizada de 721 crianças brasileiras pré-escolares com idade entre 3 a 5 anos de idade foram submetidas a exame clínico bucal usando os critérios de Foster e Hamilton. Os pais/cuidadores preencheram o questionário de qualidade de vida e outro com informações sociodemográficas e socioeconômicas da criança e sua família. A análise estatística envolveu análise descritiva, teste de Mann-Whitney e Regressão de Poisson hierarquizada ajustada. Os resultados mostraram que a prevalência de má oclusão foi de 38,0%. Os tipos mais prevalentes foram: overjet aumentado (19,4%), mordida cruzada anterior (17,2%) e mordida aberta anterior (16,4%). Impacto negativo na qualidade de vida foi observado em 66,4% das crianças que apresentaram má oclusão. A prevalência de ter algum impacto na qualidade de vida foi 2,64 vezes maior em crianças com má oclusão (95% IC 1,82-3,84). Mordida aberta anterior e overjet aumentado também foram relacionados com impacto na qualidade de vida (p < 0,001). A presença da má oclusão foi associada com impacto negativo na qualidade de vida. A análise por tipos de má oclusão mostrou que mordida aberta anterior e sobressaliência anterior superior aumentada apresentaram impacto negativo na qualidade de vida de crianças pré-escolares e suas famílias.


The aim of this study was to assess the impact of malocclusion on Oral HealthRelated Quality of Life (OHRQoL) in a sample of Brazilian preschool children and their families. First of all a specific instrument was develped for the evaluation and had its psychometric skills assessed, as well as passed through the validation process in a non-probabilistic sample. The instrument was valid and showed satisfactory psychometric results. However, in order for the results to be confirmed and an assessed validity of more criteria, a new study was performed on a representative sample of the same population. Thus, a cross-sectional study was carried out involving 721 children 3-5 years of age. A clinical exam was performed to evaluate the malocclusions according to criteria proposed by Foster and Hamilton. This examination was conducted by a calibrated dentist. Parents/caregivers answered the Malocclusion Impact Scale for Early Childhood (MIS-EC) and the questionnaire on socioeconomic and demographic characteristics. Data analysis involves descriptive statistics, chi-square, Mann-Whitney and hierarchically adjusted Poisson regression. The results showed that the prevalence of malocclusion was 38.0%. The most frequent conditions were increased overjet (19.4%), anterior crossbite (17.2%) and anterior open bite (16.4%). Negative impact on OHRQoL was found in 66.4% of children with malocclusion. The multivariate Poisson regression analysis showed that prevalence of having any impact on OHRQoL was 2.64 times higher for children with malocclusion (95% CI 1.82-3.84) compared to those without. A significant association was found between increased overjet, anterior open bite and OHRQoL (p < 0.001). The findings showed that malocclusion impact on OHRQoL.

Introducción: Las alteraciones oclusales son un problema frecuente en la dentición primaria, su identificación desde edades tempranas permitirá reconocer variaciones en las características normales, que pueden influir en el desarrollo de la oclusión permanente. Objetivo: Establecer las características oclusales en dentición primaria, de los niños y niñas de 5 años de la zona urbana y rural del municipio de Andes (Colombia). Materiales y métodos: Se realizó una valoración clínica de 125 niños mediante inspección visual de la relación entre los componentes dentarios. Se calculó la prevalencia para cada una de las alteraciones oclusales y se estimaron además la frecuencia de alteraciones combinadas. Resultados: Se encontró relación canina clase I en el 84% y clase II con un 12%. La sobremordida horizontal y la vertical fueron normales en el 60,8% y en el 58,4% respectivamente y el 6,4% tenían mordida cruzada posterior. No se encontró ninguna alteración oclusal en el 48,8% de los niños; mientras que en el 3,2% las 4 características oclusales evaluadas estaban alteradas. No hubo diferencias estadísticamente significativas en las características oclusales según sexo. Conclusión: La presencia de alteraciones en la oclusión, detectadas en mayor proporción que el promedio nacional, requiere implementar en el municipio acciones para su detección y tratamiento oportuno que contribuyan a un adecuado crecimiento y desarrollo craneofacial


Introduction: The occlusal alterations are a frequent problem in the primary dentition, their identification from early age, allow to recognize variations in the normal characteristics, which can influence the development of permanent occlusion. Objective: To establish the occlusal characteristics in primary dentition, in children 5-year-old of Andes municipality, Colombia. Materials and methods: One hundred twenty five children were evaluated by a clinical visual inspection of the relationship between the dental components. Prevalence for each occlusal characteristic was calculated and the combined frequencies of alterations were also estimated. Results: Class I canine relationship in 84% and Class II in 12% were found. The horizontal and vertical overbite were normal in 60.8% and 58.4% respectively, and 6.4% had posterior cross bite. None occlusal alteration was found in 48.8% of children; while 3.2% had alteration in the four occlusal characteristics evaluated. There were no statistically significant differences in occlusal characteristics by sex. Conclusion: The presence of occlusal alterations, in a greater proportion than the national average, requires implementing in the municipality actions for its detection and treatment that contribute to adequate craniofacial growth and development.

ABSTRACT. Introduction: in assessing children's smile, a gingival smile is usually accepted as normal, but there is no scientific evidence to support this statement. The goal of this research project was to describe the structural characteristics of posed unforced smile in children with normal occlusion in deciduous and mixed dentition. Methods: a cross-sectional study in 122 children aged 3 to 12 years with normal occlusion. Each kid was recorded a video clip and taken a photogram in unforced posed smile, which was used to quantify and describe smile variables. These variables were compared by gender and dentition type: the quantitative variables were analyzed with one-way ANOVA test and Student's t test, and the qualitative variables with Pearson's chi-squared test, using a significance level of 0.05. Results: the soft tissues evaluated in smile showed significant differences between deciduous dentition / early mixed dentition and silent mixed dentition / late dentition. These values were lower in females. There was a prevalence of high smile in deciduous dentition and early mixed dentition, and medium smile in silent mixed dentition and late mixed dentition. 89.3% of children had a matching smile arch. The smile rate was proportional among the dentition stages evaluated and between males and females. Conclusions: the high smile is predominant among child patients in early deciduous dentition and mixed dentition, and gingival exposure reduces as kids grow.

El objetivo de esta investigación fue describir las características estructurales de la sonrisa posada no forzada en niños con normoclusión, en dentición decidua y mixta. Métodos: estudio transversal realizado en 122 niños entre tres y 12 años con normoclusión. A cada uno se le realizó un video clip y se le extrajo un fotograma en sonrisa posada no forzada, sobre el cual se cuantificaron y describieron las variables de la sonrisa. Se compararon estas variables por género y tipo de dentición: las cuantitativas con la prueba ANOVA de una vía y t de Student y las cualitativas con el x2 de Pearson. Se asumió un nivel de significancia del 0,05. Resultados: los tejidos blandos evaluados en sonrisa presentaron diferencias significativas entre las denticiones decidua y mixta inicial, y las denticiones mixta silente y tardía; estas medidas fueron menores en mujeres. OBJECTIVE: To assess facial morphology (Pattern) and sagittal relationship between dental arches (Class), and establish a potential association between them and the variables sex, age and ethnicity, among schoolchildren aged between 4 and 9 years old (mean age of 6.7 years) in primary and mixed dentitions.METHODS: The sample comprised 875 children (457 males and 418 females) attending schools in Descalvado, São Paulo, Brazil. An attempt was made with a view to establish a potential association between children's morphological features with sex, age and ethnicity.RESULTS: Descriptive analysis revealed a predominance of facial Pattern I (69.9 %) and Class I (67.4 %). Statistical tests (p < 0.001) showed that Class I was more frequent among Pattern I children, whereas Class II prevailed among Pattern II, and Class III was frequent among Pattern I and III children. Ethnicity was the only variable associated with facial pattern.CONCLUSIONS: Results suggest that facial pattern and sagittal relationship between dental arches tend to be correlated. Ethnicity was associated with facial pattern, with Pattern I being the most recurrent among Caucasians and facial Pattern II being recurrent among Afro-descendant subjects.


OBJETIVO: avaliar a morfologia facial (Padrão) e a relação sagital entre as arcadas dentárias (Classe) e determinar a associação entre sexo, idade e etnia, em escolares com 4 a 9 anos de idade (média de 6,7 anos), nas fases de dentadura decídua e mista.MÉTODOS: a amostra constou de 875 crianças (457 do sexo masculino e 418 do sexo feminino) de escolas do município de Descalvado/SP, que tiveram suas características morfológicas relacionadas ao sexo, idade e etnia.RESULTADOS: com base na análise descritiva dos dados, constatou-se que há predominância do Padrão I (69,9%) e da Classe I (67,4%). Nos testes estatísticos (p < 0,001), ficou evidente que a Classe I foi mais frequente no Padrão I; a Classe II, no Padrão II; e a Classe III, igualmente frequente nos Padrões I e III; apenas a raça teve associação com o padrão facial.CONCLUSÃO: o padrão facial e a relação sagital entre as arcadas dentárias tendem a estar relacionados, e a etnia tem associação com o padrão facial, sendo mais recorrente o Padrão I nos leucodermas e o Padrão II nos melanodermas.
