ABSTRACT

The geographical area and the total population of Manipur shows that the majority are rural areas with higher rural population. People residing in rural areas are more prone to different kind of illness and diseases as compared to urban residents. Therefore, providence of healthcare services are vital to rural peoples. The state Governments, Health Department or the concerned authorities need to survey and takes up certain steps for the easier availability of the healthcare services and also the quality of facilities and services provided in the rural areas

ABSTRACT

In order to further promote hierarchical medical system and enhance the capacity of primary healthcare services, China began to build compact county medical community. At present, the development of China′s compact county community still exists such problems as the construction of service system, the basic institutional mechanism, the quality of supply capacity and the core guarantee mechanism. In order to solve the existing problems, the authors took Shaxian District of Sanming City, Fujian Province, Dancheng County of Zhoukou City, Henan Province, and Yangqu County, Taiyuan City, Shanxi Province as examples to summarize the experience of the advanced pilot areas of the policy. In the future, the construction of compact county community in China should be based on the construction of " single-core multi-layer" system, starting from the five levels of collaborative management, practical operation, system construction, basic guarantee and multi-party supervision, to comprehensively enhance the healthcare services, and finally achieve the goal of the strategy of hierarchical medical and Healthy China.

ABSTRACT

Este ensaio aborda a importância e a dimensão da força de trabalho do complexo da saúde para o mercado de trabalho brasileiro. Explora a tendência de crescimento da ocupação em face das restrições para o aumento de sua produtividade, especialmente nas atividades do núcleo desse complexo. Em um contexto de recorrente restrição orçamentária do Estado, o desenvolvimento do Sistema Único de Saúde acaba por resultar em um aumento da cobertura, porém, com deterioração das condições e da remuneração da força de trabalho vinculadas às atividades nucleares do complexo da saúde. Esse movimento tem sido acompanhado por tensões na partilha dos recursos com os demais pilares da política social. O ensaio conclui que, para se alcançar qualidade de atendimento e condições adequadas de trabalho, a política pública de saúde demanda o equacionamento adequado de seu financiamento e uma maior articulação com a matriz institucional da política social mais geral. Mesmo admitindo-se a necessidade de maior eficiência do gasto, os recursos existentes são insuficientes. Além disso, é apontada a necessidade institucional de articulação da política de saúde com as demais políticas sociais visando dar uma atenção mais abrangente e de qualidade à população.


This essay discusses the importance and the dimension of the workforce in the healthcare grid in the Brazilian labor market. It examines the growing occupation trend in relation to restrictions to the rise in productivity, especially in the activities in the center of the healthcare complex. In a context of recurrent governmental budget constraints, the development of the Unified Health System ultimately results in increased coverage, but with deteriorating conditions and wages. This trend has also been accompanied by tensions in resource sharing with other pillars of social policies. The essay concludes that, in order to achieve high quality treatment and adequate working conditions, public health policy requires a better solution for its funding and greater linkages with the institutional matrix of general social policies. Even with the need for greater efficiency in spending, current resources are insufficient. Furthermore, it is important to highlight the need for institutional interaction of health policy with other social policies, seeking to ensure a more comprehensive and quality service for the population.

ABSTRACT

No processo de construção do SUS, como política universal e que busca assegurar a integralidade das ações, a assistência à demanda não agendada na atenção básica constitui desafio não superado. Objetivo: analisar a concepção de profissionais de saúde quanto ao papel das unidades básicas de saúde (UBS) na assistência a essa demanda. Método: estudo transversal considerou dados qualitativos obtidos por meio de questionários e entrevistas. Incluídos 106 médicos de 6 PS/AMA e 190 profissionais de 30 UBS. Estes opinaram sobre razões de a população procurar PS/AMA por ocorrências nosológicas próprias da atenção básica. Para análise, utilizou-se a técnica de análise do conteúdo, segundo categorias temáticas. Resultados: insuficiência de recursos e dificuldades no processo de trabalho das UBS (50,8%) foi a explicação mais frequentemente apontada por médicos de PS/AMA para justificar procura inadequada. Apenas 33,3% dos profissionais das UBS opinaram que essas ocorrências deveriam ser atendidas na atenção básica. Conclusões: A concepção limitada sobre o papel das UBS na atenção a essa demanda, sobretudo de profissionais da atenção básica, resulta, possivelmente, em práticas restritivas ao acesso da população.


In the process of creation of the Unified Health System (SUS) as a universal policy seeking to ensure comprehensive care, unscheduled assistance in primary healthcare units (UBS) is an unresolved challenge. The scope of this paper is to analyze the viewpoint of health professionals on the role of primary healthcare units in meeting this demand. It is a transversal study of qualitative data obtained through questionnaires and interviews with 106 medical practitioners from 6 emergency medical services and 190 professionals from 30 units. They explained why people seek emergency care for occurrences pertaining to primary care. The content analysis technique with thematic categories was used for data analysis. Lack of resources and problems with primary health unit work processes (50.8%) were the reasons most frequently cited by emergency care physicians to explain this inadequate demand. Only 33.3% of the health unit professionals agreed that these occurrences should be attended in the primary healthcare services. The limited viewpoint of the role of health services on the unscheduled care, particularly among primary care professionals, possibly leads to restrictive practices for access by the population.
