ABSTRACT

La neumonía por tuberculosis es un diagnóstico difícil de hacer, dado que no es considerado un germen común y su presentación clínica suele ser más subaguda o crónica, no obstante en el trabajo realizado por Vélez et al, en la ciudad de Medellín, se encontró que hasta 2.9% de las neumonías estudiadas fueron por tuberculosis, lo cual no es un porcentaje despreciable. A continuación presentamos dos casos de neumonía por tuberculosis de difícil diagnóstico, llamativos dada la evolución y presentación inusual y con los cuales queremos nuevamente llamar la atención como diagnóstico diferencial en pacientes que no responden al tratamiento de neumonía. Tuberculosis pneumonia is difficult to diagnose, since it is not considered a common germ and its clinical presentation is often more subacute or chronic; however, in the work of Vélez et al. in the city of Medellin, it was found that up to 2.9% of pneumonias studied were caused by tuberculosis which is not a negligible percentage. Two cases of tuberculosis pneumonia difficult to diagnose are presented, which are striking given the evolution and unusual presentation and with which we again call attention as a differential diagnosis in patients who do not respond to treatment of pneumonia. ABSTRACT

Tuberculosis is an infectious disease that has displayed increasing incidence in the last decades. In the ENT area, the most common onset is cervical lymphadenitis, which accounts for 95% of ENT cases. The posterior pharyngeal wall, tonsillar pillars, sidewalls, soft palate, and tongue are the least probable locations. A 40 year old male presented in the ENT OPD with a growth at base of tongue, involving the lateral sulcus, anterior tonsillar pillar and lower pole of tonsil left side. A biopsy was taken from the same area and sent for histopathological examination with a strong clinical suspicion of squamous cell carcinoma. The final histopathological diagnosis was tuberculosis of tongue. The patient showed good response on ATT.

ABSTRACT

La tuberculosis es una de las enfermedades más antiguas conocidas por el hombre y una de las principales causas de muerte alrededor del mundo. Se estima que aproximadamente un tercio de la población mundial ha sido infectada por esta bacteria del complejo Mycobacterium tuberculosis, cuyo órgano diana es el pulmón, sin embargo se describe que hasta en un tercio de los casos puede haber compromiso de otros órganos. Una de las formas especiales de presentación de esta patología es la llamada tuberculosis miliar, que puede ser tanto pulmonar como extrapulmonar. En este artículo se expone el caso de una privada de libertad cuya autopsia se realizó en la sección de Patología Forense del Departamento de Medicina Legal de Costa Rica.


The tuberculosis is one of the oldest diseases known by mankind and one of the main causes of death around the world. It is estimated that approximately one third of the world population has been infected by this bacteria of the Mycobacterium tuberculosis complex, whose organ target is the lung, however it is described that even in one third of the cases can be affectation of other organs. One of the special forms of presentation of this pathology is the so-called miliary tuberculosis, which can be pulmonary or extrapulmonary. In this article we describe the case of a woman prisoner whose autopsy was performed in the Forensic Pathology Section of the Legal Medicine Department of Costa Rica.

ABSTRACT

The protean manifestations of tuberculosis continue to defy ones imagination. For this case, possibilities on how the tubercule bacilli managed to reach the maxillary antrum, and whether the lesion was primary of secondary were discussed. It is emphasized that since tuberculosis can mimic any disease and occur anywhere in the body, it should always be considered in the differential diagnosis of all disease entities.(Conclusion)
