ABSTRACT

A patient who went through maxillectomy can have soft palate defects including oronasal fistulas and suffer from dysphagia and dysarthria due to velopharyngeal insufficiency. This defect causes the food to enter nasal cavity and creates hypernasal sound which debilitates a quality of life. An obturator can rehabilitate the substantial oral tissue defects. The maxillary obturator separates the nasopharynx from the oropharynx during speech and deglutition by closing of the defect. For edentulous obturator patient, it is difficult to obtain proper retention due to reduced peripheral sealing. Therefore, the contours of the defects must be used to maximize the retention, stability, and support. Hollow type obturator can improve physiologic function by reducing weight than the traditional obturator. This case report describes a patient with hemi-maxillectomy who recovers mastication, speech, deglutition, and appearance with a maxillary obturator using physiological border molding of the velopharyngeal area and double-processing method.

ABSTRACT

Objective@#To perform a comparative study of different fat-processing techniques to explore which technique could obtain higher fat content.@*Methods@#Lipoaspirates was purified with three groups of fat-processing techniques (sedimentation, centrifugation, and cotton pad filtering). Isometric purified fat tissue was centrifuged at 5 000 g for 5 minutes to quantify the oil, aqueous, and fat content from them. HE staining was applied to evaluate the effect of different fat-processing techniques on the integrity of adipocyte.@*Results@#The fat content, aqueous content, and oil content of three groups was respectively 86.0%, 9.6% and 4.4% for the cotton pad filtering group; 74.4%, 14.8% and 10.8% for the centrifugation group; 62.0%, 28.4% and 9.6% for the sedimentation group. In the HE histologic findings, no significant difference was observed in the integrity of adipocyte of three groups (P>0.05).@*Conclusions@#In comparison with the sedimentation group and the centrifugation group, more fat content, less oil content and aqueous content was observed in the cotton pad filtering group. The cotton pad filtering technique was helpful in postoperative volume retention.

ABSTRACT

Objective To investigate the diagnostic value of multislice spiral CT (MSCT)in ureteral polyps by multi-phase enhanced scan. Methods Seven cases of ureteral polyps confirmed by operation and pathology were retrospectively analyzed, including 5 fibro-epithelial polyps and 2 inflammatory polyps. All of the patients underwent MSCT multi-phase enhanced scan.Thin slice reconstruction,three-dimensional post processing including maximum intensity projection (MIP),multi planar reconstruction (MPR)and volume reconstruction (VR) were performed for each phase data. Results The fibro-epithelial polyps were more common in the young men.Their longitudinal diameters were significantly greater than the transverse diameters. The shapes were cordal, papillary,or columnar. They were not enhanced obviously in the arterial phase, however, enhanced obviously in the delayed phase.Inflammatory polyps were more common in adults and all of them were accompanied with stones.Their longitudinal diameters were slightly greater than the transverse diameters, and the shape was spindle. The margins were unclear with cord changes. The upper and lower ureters showed extensive thickening of the wall. These polyps were significantly enhanced in the arterial phase and further strengthened in the delayed phase. Conclusion The morphology and dynamic enhancement characteristics of ureteral polyps can be displayed and clearly diagnosed by MSCT.

ABSTRACT

Maxillary obturator prosthesis is the most frequent treatment option for management of partial or total maxillectomy. Heavy weight of the obturators is often a dislocating factor. Hollowing the prosthesis to reduce its weight is the well established fact. The alternate technique to hollow-out the prosthesis has been described in this article which is a variation of previously described processing techniques. A pre-shaped wax-bolus was incorporated inside the flasks during packing of the heat-polymerized acrylic resin to automatically create the hollow space. The processing technique described is a single step flasking procedure to construct a closed-hollow-obturator prosthesis as a single unit. To best understand the technique, this article describes management of a patient who had undergone partial maxillectomy secondary to squamous cell carcinoma rehabilitated with a hollow-obturator prosthesis.

ABSTRACT

The purpose of this study is to devlop the basic algorithm for the finite slsment method modeling of indivdual malocclusions. Usully, a great deal of time js spent in preprocessiong. To reduce the time required, we developed a stsndardized proccrdure for measuring the position of each tooth and a program to automatically preprocess. The following procedures were carride to complete this study. 1. Twenty-eigth teeth mofphologies were constructed three- dimensionally for the finite element analysis and saved as separate files. 2. Standard brackets were attached so that the FA points conincide with the center of the brackets. 3. The study model of a patient was made. 4. Using the study model, the crown inclination, and the vertical distance from the tip of a tooth was measured by using specially designed tools. 5. The arch form was determined from a picture of the model with an image processing technique. 6. The meadured data were input as a rotational matrix. 7. The program provides an output file containing the necessary information about the three-dimensional position of teeth, which is applicable to several finite element programs commonly used. The program for basic algorithm was made with Turbo-C and the subsequent outfile was applied to ANSYS. This standatdized model measuring procedure and the program reduce the time required, espccially for preprocessing and can be applied to other malocclusions easily.
