ABSTRACT

Objective To evaluate the clinical effect of thrombolysis combined with prostaglandin El (PGE1) in the treatment of patients with acute pulmonary thromboembolism (APTE).Methods According to the digital table,88 patients with APTE corresponding to thrombolytic therapy were randomly divided into the simply thrombolytic group(49 cases) and the joint PGE1 group(39 cases).The simply thrombolytic group were given conventional thrombolysis,and the joint PGE1 group were given thrombolysis combined PGE1 therapy.The levels of PaO2,PaCO2,pulmonary artery systolic pressure(PAsP),cardiac troponin I(cTNI),B-type natriuretic peptide(BNP) were monitored,and the clinical efficacy was evaluated.Results After the treatment,the levels of PAsP,cTNI,BNP of the joint PGE1 group were better than those of the simply thrombolytic group(t =2.173,2.349,2.495,all P <0.05).The clinical effective rate of the joint PGE1 group was 82.1%,which was significantly higher than that of the simply thrombolytic group (59.2 %) (χ2 =5.339,P < 0.05).The critical event occurrence of the joint PGE 1 group was 7.7 %,which was significant lower than that of the simply thrombolytic group (24.5 %) (χ2 =4.333,P < 0.05).Conclusion Thrombolysis combined PGEI therapy may be more effective in reducing pulmonary artery pressure,protecting myocardia,improving the right heart function than only thrombolysis,and thus improve clinical symptoms and short-term prognosis in the APTE patients.

ABSTRACT

Objective To investigate the influence of the liposome PGE1 on the ischemia-reperfusion injury of the expanded flap.Methods The minipigs were set up as an animal model.They were divided into 2 groups.The expanded flaps (8 cm × 2 cm ) was designed and elevated on the back of the minipigs.An image analysis technology was used to evaluate the survival rate of the expanded flap,while a biochemistry method was used to detect the concentrations of malondialdehyde (MDA),myeloperoxidase (MPO) and microvessel density (MVD) in the tissue.Skin tempreture was measured after the drug administration.Results The survival area of the expanded flap in the Lipo PGE1 group was significantly enlarged (P<0.05),compared with the control group.The MPO and MDA concen tration in the tissue was statistically different between two groups (P< 0.05),but there was no difference of the MVD concentration between two groups.The skin temperature was higher in the Lipo PGE1 group.Conclusions The Lipo PGE1 could promote survival rate in the expanded flap and protect it from ischemic injury.

ABSTRACT

PURPOSE: Intracavernosal self injection therapy is now being widely used to treat patients with erectile dysfunction. However, there is a large number of patients who give up the self injection program due to the fear of injection. Therefore, less invasive route of drug administration is highly recommended. We investigated the efficacy of intraurethral instillation of Prostaglandin El (PGE1 ) solution in the patients who showed full erection with intracavernosal injection of PGE1. MATERIALS AND METHODS: Twenty-nine impotent patients who showed full erection with intracavernosal injection of PGE1 were included in this study. We estimated the grades and durations of the penile erection after intraurethral instillation of PGE1 RESULTS: Sixteen out of 29 patients(55.2%) showed full erection with intraurethral instillation. Two patients(6.9%) showed urethral pain which disappeared within 24 hours. But there was no systemic side effect CONCLUSIONS: Intraurethral instillation of PGE1 appears to be safe, well tolerated, and less invasive treatment modality Thus it can be selected as an alternative treatment of impotence in selected cases.

ABSTRACT

PURPOSE: Nitric oxide(NO)-cyclic GMP pathway has a dominant role In penile erection over other neurotransmission pathway, which suggests that NO donor is a more physiologic and effective agent than others in the treatment of erectile dysfunction. However, the clinical efficacy of NO donor remains debatable. We investigated the effect of combination of NO donor with prostaglandin El(PGE1) on penile erection in the rat to predict its clinical utilization. MATERIALS AND METHODS: Intracavernous injection of PGE1 alone(10(-7)-10(-4)M) or NO donor alone(SIN-1, SNAP, SNP, 10(-7)-10(-3)M) induced penile erection in a dose dependent manner. RESULTS: Of those, SNP at high doses produced full erection, comparable to that induced by cavernous nerve stimulation. It also caused significant drop of systemic blood pressure. In combined injection of PGE1(5xl0-6M) with NO donor, every combination with SIN-1 , SNAP, or SNP enhanced penile erection all doses(10(-7)-10(-3)M). Of those, combined injection with low dose SNP was found to be most erective to produce penile erection without significant hypotension. CONCLUSIONS: The results suggest that intracavernous injection of combined NO donor, especially low dose SNP, with PGE1 will be effective in the treatment of erectile impotence, although it require clinical investigation.

ABSTRACT

? carotene could decrease significantly the ear index and the quantity of Evan' s blue exudation that were skin photosensitive side actions in mice treated by photodynimic therapy ( PDT ) . The PGE 1 content in skin and the histamine content in skin and blood of mice were remarkably raised by raised by PDT. For example, the PGE 1 content in hematoporphyrin derivative group was 6.4 fold greater than that of control group. The PGE1 and histamine cotents were obviously decreased to nearly normal level by pretreatment with ? carotene. The results indicated that ? carotene can antagonise the skin photosensitive side action induced by PDT and the action was related to the decrease of PGE1 and histamine content in skin of mice. The cyclo-oxidase inhibitor indomethacin and the H1 receptor blocker benadryl could also decrease ear index and the quantity of Evan' s blue exudation
