ABSTRACT

Background: Caring for mental healthcare users (MHCUs) with a comorbid disorder of human immunodeficiency virus (HIV) and schizophrenia has always been challenging and requires expertise, skill, intuition and empathy. Objectives: The objective of this study was to explore and describe the experiences of psychiatric nurses caring for MHCUs with a comorbid disorder of HIV and schizophrenia. Method: A qualitative, exploratory, descriptive and contextual research design was used. Eight participants were selected through purposive sampling for individual in-depth interviews to collect data. Thematic analysis was used to analyse data. Results: Three themes emerged from this study. The first theme is that the psychiatric nurses experienced deep frustration because they were capable but unable to manage MHCUs with HIV and schizophrenia because of poor infrastructure and other contributing barriers. The second theme identified that the psychiatric nurses experienced discrimination against MHCUs compromising their holistic recovery. Lastly, the psychiatric nurses identified the need for health care workers in general hospitals and communities and families of MHCUs with a comorbid disorder to be educated in mental health issues to ensure continuous medical care. Conclusion: The results of this study showed that psychiatric nurses became exhausted when trying to cope with difficult nursing situations. The challenges they faced had negative consequences for the mental health of the psychiatric nurses and compromised patient care. Contribution: This study adds knowledge to nursing practice, nursing education and nursing research by implementing recommendations to mitigate the challenges of psychiatric nurses caring for MHCUs with HIV and schizophrenia.

ABSTRACT

Objective:To examine the relationship between conflict management model, perceived social support and psychological distress in psychiatric nurses, and to analyze the mediating effect of conflict management model in the relationship between perceived social support and psychological distress in psychiatric nurses.Methods:In November 2019, a total of 208 nurses in Shandong Mental Health Center were investigated with General Demography Questionnaire, Nurses' Interpersonal Conflict Handling Form Scale, Kessler Psychological Distress Scale and Perceived Social Support Scale.Results:The total score of psychological distress of psychiatric nurses was 22.74±7.71, and the level of psychological distress was negatively correlated with the integrated conflict management model and perceived social support ( r values were -0.329--0.266, P<0.001). The most commonly used conflict management model of psychiatric nurses was "integration". The integrated conflict management model had a partial mediating effect in perceived social support and psychological distress, and the mediating effect accounted for 20.2% of the total effect. Conclusions:Psychological distress of psychiatric nurses is at a moderate to severe level, and integrated conflict management model and perceived social support play an important role in reducing psychological distress of psychiatric nurses. Nursing administrators should carry out relevant intervention studies to improve nurse-patient conflict and guide nurses to adopt integrated conflict management model to reduce psychological distress and improve mental health level.

ABSTRACT

Objective To investigate the level and related factors of humanistic caring ability of psychiatric nurses in a tertiary hospital in Guangzhou. Methods One hundred and seventy-eight nurses were investigated with Caring Ability Inventory. Results The score of the caring ability of the investigated nurses was 196.72 ± 16.33. Female nurses scored higher than male nurses in caring ability. The humanistic caring ability of nurse with bachelor degree was higher than that from nursing school(P<0.01) . The score of caring ability of nurses with more than 15 years of work experience was significantly higher than others(P<0.01). The score of humanistic care ability of nurses who are not only children is significantly higher than that of only children(P<0.01). Conclusion The psychiatric nurses' caring ability needs to be further improved. Gender, only children or not, the professional title, education and working years are the influence factors of humanistic caring ability in psychiatric nursing staff.

Objective To investigate the status quo and influencing factors of work burnout of psychiatric nurses, and to provide theoretical basis for formulating intervention strategies for work burnout of psychiatric nurses. Methods A questionnaire was conducted on 812 nurses selected from 6 tertiary A psychiatric hospitals in Shandong province as the research object by stratified random sampling, and the questionnaire was conducted by using the Maslach Burnout Inventory (MBI), the Kessler Psychological Distress Scale (K10) and Emotion regulation questionnaire (ERQ). Results There were significant differences in job burnout among psychiatric nurses with different educational background, age, nurse-patient conflict and income level. There were significant differences in the dimensions of job burnout among psychiatric nurses with different titles (P< 0.05), There were significant differences in emotional exhaustion (F=8.417, P=0.000) and depersonalization level (F=7.918, P=0.000) between psychiatric nurses with different frequency of nurse-patient conflict .There were significant differences in affective exhaustion (F=6.426, P=0.002) and personal accomplishment (F=3.444, P=0.032) between psychiatric nurses with different monthly income levels. Psychiatric nurses'age, working years, psychological distress, cognitive reappraisal, inhibition of expression were associated with job burnout(r=0.075~0.570).Regression analysis revealed that the factors impacting emotion exhaustion (R2=0.374, F=81.627, P=0.000) and depersonalization (R2=0.240, F=65.014, P=0.000) were psychological distress, inhibition of expression, cognitive reappraisal, conflict between nurses and patients. The factors impacting diminished personal accomplishment (R2=0.199, F=68.332, P=0.000) were psychological distress, cognitive reappraisal and age. Conclusions The work fatigue of psychiatric nurses was at the moderate level.Emotional regulation and psychological distress were the influencing factors of job burnout .Nursing administrators in psychiatric department should pay attention to the improvement of nurses' sense of burnout, improve the ability of nurses' emotional management, cultivate positive mood and behavior pattern, reduce anxiety and depression and so on, so as to effectively improve the mental health level of nurses in psychiatric department.

ABSTRACT

Objective To investigate the prevalence of workplace violence and explore the relationship between workplace violence and emotional intelligence among psychiatric nurses Methods Totally 453 nurses were enrolled from five psychiatric hospitals via regional stratification and clustersampling in Shandong province. The workplace violence frequency scale, emotional intelligence scale were administered. Results The average workplace violence score among psychiatric nurses was (6.64 ± 3.56) points, while the emotional intelligence average score was (3.79 ± 0.37) points. The each dimensions of emotional intelligence were negatively correlated with workplace violence and types of workplace violence among psychiatric nurses(r=-0.496--0.214, P<0.01). Results of hierarchical liner regression showed that self-emotion management, type of ward, emotional awareness, hospital degree, gender could negatively predict workplace violence of psychiatric nurses (R2=0.333,F=44.636, P=0.000). Conclusions Workplace violence is prevalent among psychiatric nurses, and emotional intelligence might have influence on workplace violence.

ABSTRACT

Objective To explore the effect of group psychotherapy on mental health of psychiatric nurses after violence.Methods Group psychosocial interventions were conducted on psychiatric nurses who were exposed to violence.The measures included observation and understanding,learning and self-understanding,self-acceptance,self-adjustment and behavioral improvement for 8 weeks,once a week,60-120 min each time.Before and after intervention,the mental health status of psychiatric nurses was evaluated by self-reporting inventory (SCL-90).Results The scores by SCL-90 in the psychiatric nurses before and after intervention were higher than those in the national norm with statistically significant difference (P <0.05).The scores by SCL-90 as well as all of dimensions after the intervention were all significantly higher than those before intervention (P<0.05).Conclusion Group counseling intervention has a positive effect on improvement of mental health of psychiatric nurses after violent incidents.

Regression analysis revealed that the factors impacting psychological distress were positive coping style, perceived organizational support, expressive suppression, negative coping style and nurse-patient conflict (R2=0.224,F=46.611,P=0.000). Conclusions Psychiatric nursing managers should pay attention to the cultivation of emotional management ability of nurses and the ability to deal with conflict,encouraging them apply positive thinking cognition and positive coping style to deal with negative events in the work,creating a supportive work environment in order to improve the level of mental health.

ABSTRACT

PURPOSE: The purpose of this study was to identify the relationships between Moral Sensitivity, Empathy and Perceived Ethical Climate for nurses working in national mental hospitals and to provide basic data for the development of interventions that enhance Moral Sensitivity and help psychiatric hospital nurses in Ethical Decision-making. METHODS: This descriptive cross-sectional study was conducted with 182 psychiatric nurses working in the national mental hospitals. Data were collected through self-report questionnaires which included the Moral Sensitivity Questionnaire (MSQ), Interpersonal Reactivity Index (IRI), and Hospital Ethical Climate Survey (HECS). RESULTS: The mean score for MSQ was 5.19, for IRI, 2.49, and for HECS, 3.73. There was a significant positive correlation between moral sensitivity and empathy (r=.24, p=.001), between moral sensitivity and perceived ethical climate (r=.17, p= .025) and between empathy and perceived ethical climate (r=.17, p=.025). CONCLUSION: Findings indicate that moral sensitivity can be enhanced by improving empathy, a personal factor, and ethical climate, an organization factor. Further identification of the relationship between moral sensitivity and personal and organizational factors will be useful in the development of interventions to enhance moral sensitivity of nurses working in psychiatric wards.

ABSTRACT

Objective To investigate the influencing factors of the organizational commitment of psychiatric nurses in a city. Methods 412 psychiatric nurses from three mental health institutions in Jinan adopted the method of cluster sampling were investigated by organizational commitment scale. Results The sore of the organizational commitment of 395 psychiatric nurses was(60.51±10.50)points. There was statistical difference of the score of organizational commitment of psychiatric nurses on different position(t=3.194, P=0.002), work shift(t=4.614, P=0.000), parents or not(t=2.067, P=0.039), suffered verbal aggression(VA) or not(t=-2.950, P=0.003). There was a positive correlation between age and the score of psychiatric nurses′organizational commitment(r=0.113, P=0.025), and a negative correlation between the frequency of verbal aggression and the score of psychiatric nurses′organizational commitment (r=-0.109, P=0.030). The score of psychiatric nurses′normative commitment was negatively correlated with educational degree(r=-0.119, P=0.018) and the frequency of verbal aggression(r=-0.102, P=0.042). The score of psychiatric nurses′continuance commitment was positively correlated with age(r=0.188, P=0.000),working years(r=0.190, P=0.000), educational degree (r=0.119, P=0.018)and work income (r=0.119, P=0.018), negatively correlated with title (r=-0.189, P=0.000) ,employment forms (r=-0.181, P=0.000)and hospital level(r=-0.108, P=0.033). The score of psychiatric nurses′affective commitment was negatively correlated with educational degree(r=-0.107, P=0.034),the frequency of verbal aggression (r=-0.188, P=0.000)and physical assault(PA) (r=-0.101, P=0.045). Conclusions The level of psychiatric nurses′organizational commitment was correlated with personal factors, family factors, work factors, VA and PA.

ABSTRACT

PURPOSE: This study was done to verify the validity and reliability of a Psychiatric Nurses' Image Scale (PSYNIS) for Korean psychiatric and mental health nurses. METHODS: A methodological study design was used with an exploratory factor analysis, Pearson's correlation, and a fitness of the modified mode for validity. Cronbach's alpha coefficients and an alternative-form method for reliability were used. Psychiatric Nurses' Image Scale (PSYNIS) was tested with 345 psychiatric and mental health nurses, residing in G city, J city, and J province. Data were collected from Jun. 1 to Aug. 2, 2012. Responses were obtained from respondents through self reports method and each item had a possible score of 5. Collected data were analyzed using the SPSS 20.0 and LISREL 8.54 programs. RESULTS: The 28 items making up the instrument were classified into the following 4 factors: 'Professionalism', 'Activism', 'Coordination competence', and 'Personalism'. These factors explained 63.2% of the total variance. Fitness of the modified mode was good (chi2= 1052.30, RMSEA=.05, GFI=.90, AGFI=.86, NFI=.97, and CFI=.98). The reliability of the PSYNIS was .95 (Cronbach's alpha). CONCLUSION: Results of the present study suggest that the PSYNIS is useful for efficiently evaluation of the image of psychiatric nurses.

ABSTRACT

PURPOSE: The purpose of this study was to explore level of moral distress, moral sensitivity, and ethical climate of nurses working in psychiatric wards and identify factors that influence moral distress. Also this study was done to describe the content of moral distress. METHODS: Data were collected through self-report questionnaires and focus group interviews. A survey was conducted with 108 psychiatric nurses and 8 volunteers were interviewed. Instruments used in this study were the Moral Distress Scale for Psychiatric Nurses, Korean version of the Moral Sensitivity Questionnaire, and Ethical Climate Questionnaire. Data were analyzed using SPSS/WIN 20.0 program and content analysis. RESULTS: The mean score for moral distress was 3.74 and for moral sensitivity, 4.67. The significant factors influencing moral distress were clinical career in psychiatric wards, moral sensitivity, personal profit and friendship. As a result of content analysis, 3 domain of moral distress were found: moral distress situations, responses in moral distress situations, factors to overcome moral distress and resources that enable ethical nursing. CONCLUSION: The results suggest that in the development of interventions to decrease moral distress for nurses working in psychiatric wards, factors identified as influencing moral distress and content of moral distress should be considered.
