ABSTRACT

As pessoas diagnosticadas com transtorno bipolar vivenciam episódios de depressão, mania/hipomania e até mesmo episódios mistos. O transtorno tem causa multifatorial, apresenta uma intensa carga genética e está associado aos sintomas físicos e psicológicos. Há vários aspectos importantes no tratamento do transtorno bipolar, sendo a medicação e a psicoterapia cognitivo-comportamental dois pilares fundamentais. Utilizandose do artefato cultural lançado em março de 2015, dirigido e escrito por Paul Dalio, o filme Touched with Fire (Tocados pelo Fogo), objetiva-se relacionar o transtorno bipolar vivenciado por Carla, com uma das intervenções psicossociais que mais demonstram eficácia em prevenir a recorrência maníaca e depressiva: a Terapia Cognitivo Comportamental (TCC) e suas técnicas. Este trabalho baseia-se em um método de abordagem qualitativa de natureza descritiva, cuja fundamentação teórica foi elaborada através de uma revisão bibliográfica da literatura produzida sobre o tema. A psicologia baseada em evidências salienta que a TCC é uma das abordagens mais eficazes para o tratamento do transtorno. Destacou-se técnicas cognitivo-comportamentais para utilizar no tratamento de Carla, sendo elas: psicoeducação, gráfico do humor ­ afetivograma, reestruturação cognitiva, cenário da pior hipótese, intervenção familiar e prevenção à recaída. A partir dos achados, percebe-se que várias áreas da vida do sujeito diagnosticado com transtorno bipolar são afetadas, mas com acompanhamento psiquiátrico e medicamentoso, aliado à TCC, há bons prognósticos.(AU)


Diagnosed people with bipolar disorder experience episodes of depression, mania/hypomania and mixed episodes. The disorder has multifactorial causes, presents heavy genetic relation, and is associated with physical and psychological symptoms. Several aspects on bipolar disorder treatment are important, with medication and Cognitive Behavioral Therapy (CBT) being two fundamental pillars. Using the movie written and directed by Paul Dalio, Touched With Fire (2015), as a cultural artifact the present work aim to connect the bipolar disorder experienced by the character Carla with one of the most effective psychosocial interventions on prevent maniac and depressive recurrence: Cognitive Behavioral Therapy and its techniques. The method used on the present study was of qualitative and descriptive nature, with the theorical base produced through a review of literature on the theme. Evidence-based psychology points that CBT is one of the most effective for bipolar disorder treatment. We highlighted behavioral cognitive techniques to use on Carla's treatment: psychoeducation, mood chart, cognitive restructuring, worst case scenario hypothesis, family intervention and relapse prevention. Based on our findings we note that several areas of the subject's life diagnosed with bipolar disorder are affected, but there is good prognosis with psychiatric and medication follow-up allied to cognitive behavioral therapy.(AU)


Personas con diagnóstico de trastorno bipolar vivencian episodios de depresión, manía/hipomanía e incluso episodios mezclados. El trastorno tiene causa multifactorial, presenta una fuerte carga genética y se asocia a síntomas físicos y psicológicos. Hay varios aspectos importantes en el tratamiento del trastorno bipolar, siendo la medicación y la psicoterapia cognitivo-conductual dos pilares fundamentales. Utilizando el artefacto cultural lanzado en marzo de 2015, dirigido y escrito por Paul Dalio, la película Touched with Fire (Tocados por el Fuego), tiene como objetivo relacionar el trastorno bipolar vivenciado por Carla con una de las intervenciones psicosociales que más demuestran eficacia en la prevención de la recurrencia maníaca y depresiva: la Terapia Cognitivo-Conductual (TCC) y sus técnicas. Este trabajo se basa en una metodología cualitativa de carácter descriptivo. Para el desarrollo de la fundamentación teórica se elaboró una revisión bibliográfica de la literatura producida acerca del tema. La psicología basada en evidencias resalta que la TCC es uno de los abordajes más efectivos para el tratamiento del trastorno. Teniendo en cuenta el tratamiento de Carla, se destacaron técnicas cognitivo conductuales a utilizarse: psicoeducación, gráfico del estado de ánimo ­ afectivograma, reestructuración cognitiva, peor escenario, intervención familiar y prevención de recaídas. A partir de los hallazgos, se evidencia que varias áreas de la vida del sujeto diagnosticado con trastorno bipolar se ven afectadas, pero con acompañamiento psiquiátrico y farmacológico, combinado con la TCC, hay buenos pronósticos.(AU)

ABSTRACT

RESUMEN La situación actual generada por la pandemia de la COVID-19 afecta a todos los seres humanos, fundamentalmente a aquellos vinculados a personas enfermas o infectadas por el virus. Los profesionales de la Psicología en el contexto nacional e internacional aportan sus saberes y brindan ayuda a los pacientes y al personal de la salud. En el trabajo se pretende exponer las experiencias de la atención psicológica en un centro de aislamiento de personas contactos de pacientes con COVID-19.


ABSTRACT The current situation generated by the COVID-19 pandemic affects all human beings, mainly those linked to people who are sick or infected by the virus. Psychology professionals in the national and international context contribute their knowledge and provide help to patients and health personnel. The article aims to expose the experiences of psychological care in an isolation center for contact persons of patients with COVID-19.

Introdução: Este estudo caracterizou pacientes adultos que tiveram solicitação de psicodiagnóstico enquanto estavam internados em leitos do Sistema Único de Saúde (SUS) em uma unidade psiquiátrica de um hospital geral universitário de Porto Alegre no ano de 2017. Com o intuito de subsidiar o planejamento deste serviço, assim como a discussão com as equipes assistenciais a respeito das indicações para a solicitação, buscou-se conhecer as características tanto dos pacientes beneficiados por este exame como de suas internações. Traçou-se o perfil demográfico, identificou-se as especificidades das internações quanto ao motivo, tempo de permanência e diagnóstico psiquiátrico, e observou-se as características da própria avaliação psicológica. Método: Estudo exploratório, quantitativo e transversal, em que os dados foram resgatados dos prontuários de forma retrospectiva. A análise dos dados foi feita de forma descritiva e inferencial, com auxilio do SPSS versão 16.0. Resultados: Dos 247 pacientes internados, 17,8% tiveram solicitação de psicodiagnóstico, sendo que 93,2% (41) tiveram o processo concluído. A demanda principal foi para pacientes adultos jovens, possivelmente em primeira internação psiquiátrica e em processo de avaliação diagnóstica, um dos possíveis objetivos de uma internação psiquiátrica. Conclusão: O psicodiagnóstico auxilia no entendimento dinâmico do paciente; entretanto, este é um recurso técnico que precisa ser usado de forma estratégica durante a internação psiquiátrica por conta dos fatores que podem envolver desde o funcionamento da própria unidade até o estado mental e emocional do indivíduo em momento de crise. Introduction: This study characterized adult patients that had indication for psychodiagnosis while in a public psychiatric unit in a general university hospital in Porto Alegre, southern Brazil, in 2017. With the purposes of encouraging the discussion about psychodiagnosis indication among care teams and planning for this service, this study sought a better comprehension of patients' characteristics and their hospitalizations. Demographics, profile of the period of hospitalization, including reason for admission, length of stay, and psychiatric diagnosis, and characteristics of psychological assessment were described. Methods: This exploratory, quantitative, and cross-sectional study retrieved data from medical records retrospectively. Data were then analyzed descriptively and inferentially with SPSS, version 16.0. Results: Of 247 hospitalized patients, 17.8% had indication for psychodiagnosis, and 93.2% (41) had the procedure completed. The group that demanded most was young adults, possibly in their first psychiatric hospitalization and in the process of psychiatric assessment, one of the main objectives of a psychiatric admission. Conclusion: Psychodiagnosis helps to understand the dynamics of a patient; however, it should be used with caution in psychiatric hospitalization because of factors ranging from the operation of the unit itself to the mental and emotional state of a patient during an acute crisis. ABSTRACT

RESUMEN Fundamento: los proveedores de sistemas y servicios de salud necesitan brindar una atención integral a las personas en vigilancia epidemiológica por la COVID-19. Se requieren tecnologías para la intervención que faciliten y mejoren el acompañamiento, la conducción, el sostén y el alivio del malestar psicológico a ese grupo de pacientes. Objetivo: proponer una metodología para el abordaje psicológico de los prestadores de los servicios hacia las personas en vigilancia epidemiológica por COVID 19 en la provincia de Cienfuegos. Métodos: trabajo de investigación y desarrollo, realizado en el Departamento de Psicología de la Facultad de Ciencias Médicas de Cienfuegos. Se emplearon métodos del nivel teórico y empírico, entre estos últimos el de criterio de expertos, constituido por ocho profesores que cumplían los criterios de selección. A cada experto le fue entregada la propuesta y un cuestionario confeccionado al efecto que agrupaba ocho criterios a evaluar. Las respuestas se procesaron cuantitativamente, a través de conteo y por su representación porcentual, además se calcularon la media y la desviación estándar. Resultados: los expertos consideraron que existía coherencia entre las etapas, consistencia entre sus instrumentos de indagación y los de intervención oportuna, además de pertinente e idónea así como de gran utilidad para los proveedores. Conclusiones: la propuesta metodológica ostenta los requerimientos estructurales y funcionales que le imprimen el sello de calidad para los fines que se elaboró. Es un instrumento que puede ser perfeccionado a medida que su uso se extienda.


ABSTRACT Foundation: providers of health systems and services need to provide comprehensive care to people in epidemiological surveillance by COVID-19. Technologies are required for an intervention which facilitate and improve the accompaniment, driving, support and the relief of psychological discomfort to this group of patients. Objective: to propose a methodology for the psychological approach of service providers to people in epidemiological surveillance by COVID 19 in the Cienfuegos province. Methods: research and development work, conducted at the Department of Psychology of the Cienfuegos Faculty of Medical Sciences. Methods of the theoretical and empirical level were used, among the latter expert criteria, composed by eight professors who met the selection criteria. Each expert was given the proposal and a questionnaire prepared for this purpose which grouped eight criteria to be evaluated. The responses were processed quantitatively, through counting and by their percentage representation, and the mean and standard deviation were also calculated. Results: experts considered that there was coherence between stages, consistency between their instruments of inquiry and those of timely intervention, as well as pertinent and suitable as well as very useful for providers. Conclusion: this methodological proposal shows structural and functional requirements which print a quality seal for the purposes it was developed. It is an instrument that may be improved as its use expands.

RESUMEN Fundamento: el modelo de la Psicología de la Salud en Cuba tiene dentro de sus objetivos la intervención psicológica en tiempo de desastres. La COVID 19 es una de las mayores catástrofes naturales de la historia, que a su paso ha dejado y dejará fatales consecuencias de orden psicológico en los seres humanos. Objetivo: ofrecer a los psicólogos que asisten a personas en riesgo o que padecen de la COVID-19 y a los prestadores asistenciales, las pautas de actuación psicológicas. Métodos: revisión documental que fusiona el estado del arte de la temática, las mejores evidencias y la experiencia de los profesionales de la provincia Cienfuegos. Resultados: el Grupo de Psicología y el Capítulo de la Sociedad Cubana de Psicología de la Salud de Cienfuegos proponen las pautas de actuación para los psicólogos vinculados a la asistencia de los pacientes con riesgo y/o enfermos de la COVID-19 y los prestadores asistenciales. Conclusiones: el protocolo de actuación psicológica responde a las necesidades psicológicas que presentan personas con riesgo o enfermos de la COVID-19 durante el proceso asistencial y a las necesidades psicológicas de trabajadores que participan en este proceso.


ABSTRACT Foundation: the model of Health Psychology in Cuba has among its objectives psychological intervention in times of disasters. COVID 19 is one of the greatest natural catastrophes in history, which has left and will leave fatal psychological consequences for human beings. Objective: to offer psychological action guidelines to psychologists who assist people at risk or who have COVID-19 and to healthcare providers. Methods: documentary review that merges the subject state of art, the best evidence and the experience of professionals from the Cienfuegos province. Results: The Psychology Group and the Chapter of the Cuban Society of Health Psychology of Cienfuegos propose guidelines for psychologists in charge of providing care to patients at risk and / or patients with COVID-19 and healthcare providers. Conclusion: the protocol of psychological intervention responds to psychological needs of people at risk or patients with COVID-19 during the care process and to psychological needs of workers who participate in this process.

RESUMEN Fundamento: la línea de ayuda telefónica es una técnica y estrategia valiosa para enfrentar problemas de salud, servicio que requiere de una guía práctica psicológica con orientaciones que apoyen la actuación frente a una persona que solicita ayuda. Objetivo: diseñar una guía de prácticas psicológicas para conducir a los psicólogos que prestan ayuda en la línea CONVIDA. Métodos: investigación en Sistemas y Servicios de Salud, en modalidad de investigación aplicada, desarrollada por el Departamento de Psicología de la Facultad de Ciencias Médicas de Cienfuegos, durante la segunda quincena de abril a primera semana de mayo 2020. Se emplearon métodos del nivel teórico y empírico (análisis de contenido y la ponderación). En un primer momento se realizó revisión de los documentos emitidos por el MINSAP y en segundo momento se reunieron los especialistas (tres) que laboran en la línea confidencial, a la vez que se consultaron siete psicólogos del Departamento de Psicología, con experiencia en orientación psicológica. Resultados: se integraron varios componentes organizados en cuatro fases de la guía: comienzo, recolección de la información, intervención y cierre, de utilidad para los psicólogos que atienden la línea telefónica CONVIDA. Las contribuciones de mayor importancia fueron, la determinación de los componentes, la concepción sobre la estructura de la guía y la selección de las técnicas de influencia psicológica. Conclusiones: se presenta una guía de prácticas psicológicas para los profesionales que están prestando el servicio de ayuda psicológica telefónica a las personas que lo solicitan, en medio de la pandemia de la COVIC 19.


ABSTRACT Foundation: telephone helpline is a valuable technique and strategy to face health problems, a service that requires a practical psychological guide with guidelines that support action in front of a person requesting help. Objective: to design a psychological practice guide to orient psychologists who provide help on the CONVIDA line. Methods: research in Health Systems and Services, in applied research modality, developed by the Psychology Department of the Faculty of Medical Sciences of Cienfuegos, from the second half of April to the first week of May 2020. Methods of the theoretical and empirical levels were used (content analysis and weighting). At first, it was carried out a review of the documents issued by MINSAP, and second, specialists (three) working in the confidential line met, and seven psychologists from the Department of Psychology, with experience in psychological counseling, were consulted. Results: Several components organized into four phases of the guide were integrated: beginning, information collection, intervention and closure, useful for psychologists who attend the CONVIDA telephone line. The most important contributions were: components determination, conception on the guide structure and selection of psychological influence. Conclusions: a guide for psychological practice is presented for professionals who are providing telephone psychological help to people who request it, in the midst of the COVIC pandemic 19.

ABSTRACT

Medical resilience is a dynamic capacity, which has the potential to improve the well-being of physicians and to enhance the quality of the clinical relationship. Strategies to promote resilience are important to achieve a sustainable medical practice and improve patient care. Mindfulness training has demonstrated to be an effective tool to promote resilience in physicians. This paper contextualizes the place of mindfulness in medical practice and describes the ways through which it can contribute to resilience in medicine. The concept of mindfulness, its relationship with health practice is reviewed and the benefits of the practice of mindfulness in the clinical relationship are described. We suggest that the benefits achieved through a mindfulness-based medical practice are mediated by two axes. One is the nonspecific and specific effect of mindfulness-based practices and the other is the integration of explicit and implicit knowledge of clinical practice. We conclude that medical practice that integrates mindfulness can contribute to the challenge of achieving greater levels of individual, staff and institutional resilience. There is a need to have continuing mindfulness training programs for health professionals and to integrate this concept in the curriculum of health care professionals.

ABSTRACT

Objective To investigate the effect of group psychological counseling and peer education on depressive and anxiety symptoms of pregnant females with selective multiple pregnancy reduction in second trimester. Methods Sixty-two pregnant females with selective multiple pregnancy reduction in second trimester were randomly divided into intervention group(n=30)and control group (n=32), two groups of pregnant women all received routine nursing. The pregnant women of intervention group received group psychological counseling and peer education for one month before multiple pregnancy reduction. Results Before the intervention, the anxiety levels of the two groups of pregnant women were not significantly different. After the intervention, the intervention group had less anxiety symptoms than before the intervention[ before(54.2 ± 3.2)points,after(43.4 ± 3.1)points,t=13.250,P=0.000].After the intervention, the intervention group had less anxiety symptoms than the control group[the intervention group(43.4±3.1)points, the control group(50.0±4.4)points,t=6.712,P=0.000].Before the intervention, the depression levels of the two groups of pregnant women were not significantly different. After the intervention, the depression levels of the control group was not significantly different than those before the intervention. After the intervention, there was significantly lower depression score of the intervention group than that of the control group(P < 0.05) [before(56.9 ± 3.5)points, after(45.3 ± 2.5)points, t=14.773,P=0.000].After the intervention, the intervention group had lower depression score than the control group[the intervention group (45.3 ± 2.5) points, the control group (54.6 ± 3.0) points, t=12.802,P=0.000]. Conclusions Group psychological counseling and peer education can effectively improve depressive and anxiety symptoms of pregnant females with selective multiple pregnancy reduction in second trimester.

ABSTRACT

La investigación se dirigió a la validación concurrente del Autorreporte vivencial en una muestra de sujetos supuestamente sanos. Fue realizada en el período comprendido entre septiembre de 2010 y junio de 2011. Se asumió un paradigma cuantitativo y se realizó un estudio correlacional. La muestra de trabajo fue no probabilística intencional, incluyó a 138 sujetos dentro del rango de edades estudiadas (18-60 años). Fueron utilizadas un grupo de técnicas: Autorreporte vivencial, Inventario de Situaciones y Respuestas de Ansiedad (ISRA-B), Inventario de Depresión de Beck y el Inventario de expresión de la ira estado-rasgo (STAXI-2). Se establecieron asociaciones significativas entre los diferentes factores del Autorreporte vivencial e instrumentos previamente validados, relacionadas con dichos factores. Las pruebas psicológicas aplicadas explicaron la mayor parte de la varianza de los diferentes factores del Autorreporte vivencial. Se evidenciaron asociaciones entre la tendencia general al registro de vivencias negativas y estas pruebas psicológicas.


This investigation was aimed at concurrent validation of Experience Self-report in a sample of supposedly healthy subjects. It was made during September, 2010 and June, 2011. A quantitative paradigm was followed and a correlation study was carried out. The work sample was non probabilistic intentional, it included 138 subjects between 18 and 60 years of age. A group of techniques were used: Experience Self-report, Situational Inventory and Answers of Anxiety (ISRA-B), Beck Depression Inventory and Inventory of the showing of anger state-trait (STAXI-2). Significant associations among different factors of Experience Self-report and previous validated instruments were established. Psychological applied tests explained major part of variance analysis of the different factors of Experience Self-report. Associations among general register tendency of negative experience and these psychological tests were evidenced.

Background: Disrupted eating behaviors and psychological issues can have a role in the development of childhood obesity. Aim: To assess the effects of psychological support as an adjunct to the treatment of obesity among children. Material and Methods: Retrospective analysis of a group of 152 obese children aged 3 to 16 years, who received medical, Nutritional and physical training treatment. Seventy three (Group I) were invited to participate in a protocol of psychological support and 79 formed the control group (Group 2). Body mass index, body fat mass, waist circumference, insulin sensitivity, lipid profile, eating and physical activity habits were evaluated. Family structure, history of food rejection, psychological, physical and social development, and adherence to the program were also analyzed. To analyze responses to treatment, Group I was further divided into those who attended the psychology sections (intervened group) and those who did not attend (non intervened group). Results: At baseline, there were no differences in anthropometric, metabolic, familial and psychological profile among Groups I and 2. Family structure, eating behaviors and physical and social development were normal. Those associated with lack of control appeared as negative characteristics. There was a significant improvement in eating habits and physical activity among intervened children of Group 1 and Group 2. No significant change in weight was recorded in all three groups. Compliance with the program was significantly better among intervened children in Group 1 (62.1 percent at sixth months), compared with non intervened children of Group 1 (15.9 percent) and Group 2 (30.3 percent). Conclusions: Obese children who attended psychological support therapy as an adjunct to a weight reducing program, had a better compliance with treatment but did not achieve a higher weight loss.

ABSTRACT

Se realizó un ensayo clínico controlado y abierto de fase II en niños de siete a once años de edad de ambos sexos, matriculados en escuelas primarias del distrito Joaquín de Agüero de la ciudad de Camagüey durante el curso 1998-1999, con el propósito de determinar la efectividad del tratamiento con sugestión directa, dinámica de grupo o sugestión en hipnosis para suprimir el hábito de succión digital complicada. Se escogieron 60 succionadores por edad y sexo de forma aleatoria estratificada y se conformaron tres grupos de 20 niños para cada tratamiento, que se evolucionaron al mes y a los seis meses. Se hallaron los mejores resultados con el método de sugestión en hipnosis.


A clinical, controlled, open phase II study in seven to eleven years old children of both sexes, listed in elementary schools of Joaquín Agüero district in Camagüey city from 1998-1999 was carried out with the aim of determining the effectiveness of treatment with direct suggestion, group dynamic or suggestion in hypnosis to suppress complicated digital fingersucking habit. Sixty suckers were selected as to age and sex in stratified, ramdomized way; three groups of 20 children were formed for each treatment, and evolved in one and six months. Better results were found with the suggestion and hypnosis method.

Los datos de los artículos revisados se insertaron de acuerdo a los diferentes acápites tratados, tales como: Dinámica Grupal, Liderazgo en el grupo, Técnicas dinámicas. La síntesis de datos permitió profundizar en técnicas educativas que a pesar de constituir el intento más difundido de mayor influencia en estos momentos en la educación moderna, no siempre cumplen con los requisitos de calidad que garanticen la modificación de actitudes y conductas de la población en relación con la salud bucal. El conocimiento y uso correcto de cada una de estas técnicas, guardan relación con la promoción de salud, si somos capaces de motivar padres de familia, profesionales de la salud y pedagogos.


The purpose of the study was to review participation technics of education for Health and the different elements necessary for its functioning in Stomatology. Data were selected from The National Meeting of Medical Students at the Convention Palace in l984, How to study Pedagogical Experiences of Progress, l989; Technology of the Alternative Communication and Interpretative Journalism, l989; Creativity plus Group Dynamics=Eureka l992; Recomendacoes, cuidados especiais em odontología l993. Data of the reviewed articles were inserted according to the different items delt with, such as: Group Dynamics, Leadership in the Group, Creativity + Group, dynamic technics. The results of the data synthesis made us to deepen in educative technics, which sometimes are not effective in the requisites of quality that guarantee modifications of attitudes and behaviours of the people in relation to the oral health, although it is the widely known intent of greater influence in modern education at these moments. Concluding, the knowledge and right use of each of these technics, are related with the health promotion, if we are able to motivate family parents, health professionals and pedagogues.
