ABSTRACT

This paper summarized the clinical experience of CHEN Tongyun in the treatment of postinflammatory dyspigmentation with the method of unblocking and nourishing qi and blood. It is believed that the core pathogenesis of this disease is poor qi movement and skin blood stasis, for which the method of unblocking and nourishing qi and blood should be used. Postinflammatory pigmentation on the face is mainly caused by qi stagnation and blood stasis, and it is suggested to regulate liver and spleen, move qi and invigorate blood usually with modified Tonghua Decoction (通化汤). Postinflammatory hypopigmentation is mainly due to qi and blood depletion, for which the treatment should be fortifying the spleen and strengthening kidney, replenishing qi and generating blood, and modified Yangfu Decoction (养复汤) is commonly used. Simultaneously, medicinals of ascending and descending functions, moving qi and blood, warming yang and nourishing yin should be combined, and the results from modern pharmacological research should be considered.

ABSTRACT

The “conception vessel, governor vessel and thorough vessel sharing the same origin” theory refers to the concept that the conception vessel, governor vessel and thorough vessel all originate from the same source within the uterus and emerge from the perineum. Based on the modern research of McNeal's prostate zonal anatomy theory, it is believed that the conception vessel, governor vessel and thorough vessel have the closest relationship with the prostate in terms of their origin and running course. It is proposed that the essential meaning of the ‘conception vessel, governor vessel and thorough vessel sharing the same origin’ theory in relation to the prostate is that the conception vessel, governor vessel and thorough vessel respectively connect to the transition zone, peripheral zone, and central zone of the prostate. Moreover, the differences in the yin-yang attributes of the conception vessel, governor vessel and thorough vessel exhibit distinct functional characteristics in different zones of the prostate, which serve as an important basis for the physiological and pathological differences in various prostate zones. Based on this premise, a prostate meridian, qi and blood syndrome differentiation method is further proposed, wherein the differences in the meridian characteristics of the conception vessel, governor vessel and thorough vessel determine the differences in the yin-yang aspects of qi and blood in different prostate zones. When clinical diagnosis is conducted, the inherent physiological differences of the different prostate zones should be fully taken into account, as well as the pathological characteristics of the disease, in order to guide the diagnosis and treatment of related clinical conditions.

ABSTRACT

This parer summarized the clinical experience of Professor ZHANG Zhiyuan in treating stroke sequelae from the perspective of regulating qi and blood. It is believed that the basic pathogenesis of stroke sequelae is disharmony of qi and blood, and it is advocated that the basic treatment method is to regulate qi and blood. For the three major symptoms of stroke sequelae, including hemiplegia, cognitive dysfunction, and sluggish speech, the effective formulas have been summarized. Hemiplegia is often treated with Chongsu Qiju Decoction (重塑起居汤) to tonify the center and replenish qi, activate blood and dredge collaterals. Cognitive dysfunction is often treated with modified San Hua Decoction (三化汤) to resolve phlegm and purge heat, regulate qi and tonify blood. And sluggish speech is often treated with Zishou Jieyu Decoction (资寿解语汤) to warm and tonify yang qi, expel wind and remove phlegm. Meanwhile, the self-made Zhinao Huayu Decoction (治脑化瘀汤) is used to prevent the occurrence of stroke sequelae, and to regulate the blood and qi by using wind medicinals and the method of relaxing and purging bowels to cure the disease.

ABSTRACT

ObjectiveBased on the medical cases of Qi and blood co-treatment of traditional Chinese medicine(TCM) masters, to discover the syndrome and treatment rules and medication experience of Qi and blood co-treatment through data mining. MethodFrom December 1999 to November 2020, the Qi and blood treatment cases of TCM masters were retrieved from China National Knowledge Infrastructure (CNKI). Frequency statistics, association rules, cluster analysis and other methods were used for data mining. ResultThe analysis of 591 medical cases of 57 national medical master found that blood stasis, Qi deficiency, Qi stagnation, blood deficiency and phlegm were the most common syndromes. The tongue was reddish, pale or dark, the moss was white or thin, and the pulse was thin, stringy, heavy and slippery. In the treatment of Qi and blood, the disease in the early stage is mostly in Qi and blood itself. At this time, the emphasis should be on regulating Qi and blood, or tonic or attack or both. At the same time, attention should be paid to invigorating the spleen, soothing the liver and tonifying the kidney. Core drugs include Danggui Buxuetang, Buyang Huanwutang, Huangqi Guizhi Wuwutang, Taohong Siwutang, Si Junzitang, Linggui Zhugantang, Xiaoyaosan, Danggui Shaoyaosan and other chemical cut. ConclusionWhen treating Qi and blood together, Chinese medical masters attach great importance to the relationship between Qi and blood and the development stage of diseases, and emphasize the precision and dynamic differentiation of treatment. Their theories and experience of diagnosis and treatment are worthy of clinical application and promotion.

ABSTRACT

Objective:To explore the application value of modified Shiquan Dabutang in the treatment of elderly patients with osteoporotic intertrochanteric fractures (OIFs) due to Qi and blood deficiency by observing its impacts on inflammatory and bone metabolism indexes. Method:Ninety-eight elderly patients admitted to our hospital for OIFs of Qi and blood deficiency syndrome from April 2018 to April 2020 were randomized into an observation group (<italic>n</italic>=49) and a control group (<italic>n</italic>=49). Following the proximal femoral nail antirotation (PFNA) fixation, patients in the control group were treated with Guipiwan, while those in the observation group received the modified Shiquan Dabutang. The clinical efficacy, inflammatory and bone metabolism indexes, and complications were compared between the two groups after four weeks of treatment. Result:The levels of such serum indexes as fibroblast growth factor-2 (FGF-2), osteoprotegerin (OPG), transforming growth factor-<italic>β</italic><sub>1</sub> (TGF-<italic>β</italic><sub>1</sub>), <italic>β</italic>-endorphin (<italic>β</italic>-EP), bone-specific alkaline phosphatase (BALP), and osteocalcin (BGP) in the observation group after treatment were significantly elevated as compared with those in the control group (<italic>P</italic><0.05), whereas the serum tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) and <italic>D</italic>-dimer (<italic>D</italic>-D) declined (<italic>P</italic><0.05). The TCM symptom score in the observation group after treatment was obviously lower than that in the control group, while the Harris Hip Score (HHS) was higher (<italic>P</italic><0.05). The overall response rate of the observation group was 93.88% (46/49), higher than 75.51% (37/49) of the control group (<italic>χ<sup>2</sup></italic>=6.376, <italic>P</italic><0.05). The total incidence of incision infection, deep vein thrombosis of lower limbs, and pulmonary infection in the control group was 24.49% (12/49), significantly higher than 6.12% (3/49) in the observation group (<italic>χ<sup>2</sup></italic>=6.607, <italic>P</italic><0.05). Conclusion:The modified Shiquan Dabutang is able to alleviate inflammation, regulate bone metabolism, promote bone repair, and reduce the incidence of complications in elderly patients with OIFs due to Qi and blood deficiency.

ABSTRACT

Objective:To observe the effect of modified Bazhentang on the nutritional status and immune function of patients with Qi and blood deficiency syndrome in neoadjuvant chemotherapy (NAC) for gastric cancer. Method:One hundred and ten patients were randomly divided into observation group and control group with 55 cases each. Both groups accepted FOLFOX6 protocol. Patients in control group took Jianpi Shengxue tablets orally, 3 tablets/time, 3 times/day. Patients in observation group received modified Bazhentang, 1 dose/day. The course of treatment was six weeks in both groups. Before and after treatment, scores were graded according to patient generated-subjective global assessment (PG-SGA), Qi and blood deficiency syndrome, and the Revised Piper Fatigue Scale (PFS-R). Levels of serum total protein (TB), albumin (ALB), prealbumin (PAB), CD4<sup>+</sup>, CD8<sup>+</sup>, helper T lymphocyte 17 (Th17), regulatory T cell (Treg), immunoglobulin G (IgG), IgM, and IgA were detected before and after therapy. Body mass index (BMI) and fat free mass index (FFMI) were measured before and after treatment. Weight loss was recorded, and the acute or subacute toxicity of anticancer drugs was evaluated. Result:The degree of malnutrition in the observation group was lower than that in the control group (<italic>Z</italic>=2.401,<italic>P</italic><0.01). The levels of TB, ALB and PAB in the observation group were higher than those in the control group (<italic>P</italic><0.01). The CD4<sup>+</sup>, Treg and CD4<sup>+</sup>/CD8<sup>+</sup> levels in the observation group were higher than those in the control group (<italic>P</italic><0.01). The CD8<sup>+</sup>, Th17 and Th17/Treg levels were lower than those in the control group (<italic>P</italic><0.01). Besides, the levels of IgM and IgA in the observation group were higher than those in the control group (<italic>P</italic><0.01). The PG-SGA score and weight loss in the observation group were lower than those in the control group (<italic>P</italic><0.01). The BMI and FFMI data of the observation group were higher than those of the control group (<italic>P</italic><0.05). The scores of PFS-R and Qi-blood deficiency syndrome were lower than those of the control group (<italic>P</italic><0.01). The incidence of nausea and vomiting in the observation group was 45.45% (25/55), lower than 65.45% (36/55) in the control group (<italic>χ</italic><sup>2</sup>=4.452,<italic>P</italic><0.05). Conclusion:Modified Bazhentang can be used to assist gastric cancer patients with NAC, which can improve nutritional status and immune function, promote immune balance, reduce clinical symptoms and fatigue, and reduce chemotherapy toxicity and side effects, so it is worthy of clinical use.

ABSTRACT

Objective:To observe the effect of Dingkundan in adjuvant treatment of clinical symptoms, quality of life, immune function and prognosis of patients with radiotherapy and chemotherapy after endometrial carcinoma (EC) operation. Method:Patients were divided into control group (82 cases) and observation group (86 cases) according to random number table. A total of 75 patients in control group completed the study (4 patients fell off or lose visit, and 3 patients were eliminated), while 77 patients in observation group completed the study (5 patients fell off or lose visit, and 4 patients were deleted). After operation, patients got brachytherapy, external pelvic irradiation and chemotherapy. Patients in control group got Bazhenwan, 1 pill/time, 2 times/day, and those in observation group got Dingkundan, 7 g/time, 2 times/day. The course of treatment lasted for 4 months, and long-time follow-up data was recorded. Before treatment, and at the second and fourth month after treatment, deficiency of Qi and blood was scored. Toxic reactions after radiotherapy and chemotherapy were recorded, and incidence rate of acute and advanced radiation injury of bladder and rectum and toxicity of chemotherapeutic drugs at grade 3 or above grade 3 level were compared. And levels of T lymphocyte subsets (CD3<sup>+</sup>, CD4<sup>+</sup>, CD8<sup>+</sup> and CD4<sup>+</sup>/CD8<sup>+</sup>) were detected, European collaborative quality of life Cancer Core Scale (EORTC QLQ-C30) was evaluated, and expressions of pce125 (CA125), epididymis protein 4 (HE4), Dickkopf-related protein-1 (DKK1), vascular endothelial growth factor (VEGF), matrix metalloproteinase-9 (MMP-9) and transforming growth factor-<italic>β</italic><sub>1</sub> (TGF-<italic>β</italic><sub>1</sub>) were tested before and after treatment. The follow-up was made for every three months, and the progression (recurrence/metastasis) of patients was recorded. Result:Scores of deficiency of Qi and blood in observation group were lower than those in control group at the second and fourth week after treatment (<italic>P</italic><0.01). Incidence rates of acute and advanced radiation injury of bladder and rectum and toxicity of chemotherapeutic drugs at grade 3 or above grade 3 level and incidence rates of bone marrow suppression, gastrointestinal toxicity, neurotoxicity were lower than those in control group (<italic>P</italic><0.05). Five functional dimensions and overall quality of life score based on EORTC and QLQ-C30 in observation group were higher than those in control group (<italic>P</italic><0.01), and scores of three symptom dimensions were lower than those in control group (<italic>P</italic><0.01). Levels of CD3<sup>+</sup>, CD4<sup>+</sup> and CD4<sup>+</sup>/CD8<sup>+</sup> were higher than those in the control group (<italic>P</italic><0.01), and CD8<sup>+</sup> was lower than those in the control group (<italic>P</italic><0.01). Levels of CA125, HE4, DKK1, VEGF, MMP-9 and TGF-<italic>β</italic><sub>1</sub> were lower than those in the control group (<italic>P</italic><0.01). The disease progression rate in observation group was 18.18% (14/77), which was lower than 33.33% (25/75) in control group (<italic>χ</italic><sup>2</sup>=4.572, <italic>P</italic><0.05). Conclusion:In adjuvant treatment of patients with radiotherapy and chemotherapy after EC operation, Dingkundan can reduce the symptoms of Qi and blood deficiency syndrome and side effects caused by radiotherapy and chemotherapy, improve the quality of life and immune function, inhibit the expression of tumor markers and tumor growth factor, delay the progression of tumor and improve the prognosis.

ABSTRACT

Objective:To explore the regulatory effect of modified Liu Junzitang on the immune function, nutritional status and intestinal microecology in advanced gastric cancer patients with syndrome of deficiency of Qi and blood. Method:The 86 advanced gastric cancer patients with syndrome of deficiency of Qi and blood were randomly divided into control group and observation group according to their admission numbers, with 43 cases in each group. The control group was given Yiqi Yangxue oral liquid on the basis of basic treatment while the observation group was given modified Liu Junzitang. After 4 weeks, compare the clinical efficacy of two groups of patients, traditional Chinese medicine (TCM) syndrome score, gastrointestinal function recovery, adverse reaction and quality of life, immune function, T cell subsets CD3<sup>+</sup>, CD4<sup>+</sup>, CD8<sup>+</sup>, C<sub>3</sub> and C<sub>4</sub> levels, immunoglobulin A (IgA), immunoglobulin G (IgG), immunoglobulin M (IgM), nutritional status: albumin (propagated), prealbumin (PA), serum total protein (TP) and hemoglobin (Hb) content changes, the intestinal micro ecology: <italic>Bifidobacterium</italic>, <italic>Lactobacillus</italic>, <italic>Enterococcus aureus</italic>, <italic>Escherichia coli</italic> content changes. Result:The total effective rate of the observation group was 95.35% (41/43), which was significantly higher than 79.07% (34/43) of the control group (<italic>χ<sup>2</sup></italic>=5.108,<italic>P</italic><0.05), after treatment, the TCM syndromes such as dizziness, pale complexion, palpitation, shortness of breath and fatigue in the observation group were significantly lower than those in the control group (<italic>P</italic><0.05), the bowel sound recovery, exhaust and defecation time of the observation group were significantly lower than those of the control group (<italic>P</italic><0.05), the quality of life scores in the observation group, such as the nature-to-human correspondence, form and spirit integration, specific modules, functional areas, and overall health score, were significantly higher than those in control group (<italic>P</italic><0.05), the CD3<sup>+</sup>, CD4<sup>+</sup>, CD4<sup>+</sup>/CD8<sup>+</sup>, C<sub>3</sub>, C<sub>4</sub>, IgA, immune function indexes such as IgG and IgM were significantly higher than those of the control group, and the CD8<sup>+</sup> level was lower than which of control group (<italic>P</italic><0.05), the nutritional status levels such as Alb, PA, TP and Hb in the observation group were significantly higher than those of the control group (<italic>P</italic><0.05), <italic>Bifidobacterium</italic>, <italic>Lactobacillus</italic>, and <italic>E. faecalis </italic>in the observation group were higher than those in the control group, and <italic>E. coli</italic> was lower than the control group (<italic>P</italic><0.05), the adverse reaction rate of the observation group was 11.63% (5/43) and the control group was 16.28% (7/43) , and there was no statistical difference between two groups. Conclusion:Modified Liu Junzitang has a good clinical effect on advanced gastric cancer patients with syndrome of deficiency of Qi and blood. It can improve TCM syndromes and gastrointestinal function, improve quality of life, and its mechanism is related to improving immune function, enhancing nutritional status, and improving intestinal microecology, and it has good safety.

ABSTRACT

Objective:To study the clinical efficacy of modified Shengyutang on patients with active stage psoriasis vulgaris due to Qi and blood deficiency. Method:The 134 cases were randomly divided into control group and observation group, with 67 cases in each group. The control group was given avic a capsule + Danggui Buxuewan, while the observation group was given avic a capsule + modified Shengyutang for 4 weeks, respectively. The psoriasis area and severity index (PASI), dermatological life quality index (DLQI) and psoriasis vulgaris due to Qi and blood deficiency syndrome were observed before and after treatment. The serum growth factor [endothelial cell specific molecule-1 (ESM-1), transforming growth factor-<italic>β</italic><sub>1</sub>(TGF-<italic>β</italic><sub>1</sub>), vascular endothelial cell growth factor (VEGF)], hemorheological indicators [high cut blood viscosity (HBV), low cut blood viscosity (LBV), erythrocyte sedimentation rate (ESR)], CC cphenotype receptor(CCR)6, CC cphenotype ligand 20 (CCL20), monocyte chemotactic protein-4 (MCP-4) in serum and tissue fluid of lesions were detected. Clinical efficacy and recurrence follow-up for 12 months were compared. The safety was evaluated between two groups. Result:Three cases in control group and one case in observation group fell off during the study period. The total effective rate was 96.97% (64/66) in observation group, which was higher than 81.25% (52/64) in control group (<italic>χ</italic><sup>2</sup>=5.064, <italic>P</italic><0.05). During the 12-month follow-up, the recurrence rate was 20.31% (13/64) in observation group, which was lower than 51.92% (27/52) in control group (<italic>χ</italic><sup>2</sup>=6.038, <italic>P</italic><0.05). Compared with control group after treatment, PASI, DLQI, TCM syndromes, ESM-1, TGF-<italic>β</italic><sub>1</sub>, VEGF, HBV, LBV, ESR, CCR6, CCL20 and MCP-4 in observation group were significantly reduced (<italic>P</italic><0.05). No obvious blood and urine routine, or heart, liver and renal dysfunction was observed in the two groups. The incidence of adverse reactions was 3.03% (2/66) in observation group, which was lower than 26.56% (17/64) in control group (<italic>χ</italic><sup>2</sup>=5.764, <italic>P</italic><0.05). Conclusion:Modified Shengyutang can significantly improve the clinical symptoms of patients with active stage psoriasis vulgaris due to Qi and blood deficiency, with a low recurrence rate and the incidence of adverse reactions.

ABSTRACT

Objective:To observe the regulatory effect of modified Shengmaiyin adjuvant chemotherapy on the nutritional status and immune function of children with acute lymphocyte (ALL). Method:One hundred children with ALL chemotherapy were randomly divided into observation group and control group. Both groups were given VDLD regimen as the induction therapy and CAM regimen as the early therapy. Control group was given Shengmaiyin orally, 10 mL/time, 3 times/day. Observation group was given modified Shengmaiyin orally, 1 does/day. The course of treatment for both groups was 3 months. Micro-nutrition assessment (MNA) was carried out before and after treatment, and serum total protein (TB), albumin (ALB), prealbumin (PAB), T cell CD4<sup>+</sup>, CD8<sup>+</sup>, immunoglobulin G (IgG), IgM, IgA levels were detected before and after treatment. The ratio of CD4<sup>+</sup>/CD8<sup>+</sup> was calculated. The traditional Chinese medicine (TCM) syndrome before and after treatment and the piper fatigue scale (PFS-R) and universal core scale of children's quality of life (PedsQL) were scored. The changes of white blood cells (WBC), red blood cells (RBC), hemoglobin (Hb) and platelets (PLT) were evaluated before and after treatment. Result:The nutritional status of the observation group was better than that of the control group after treatment (<italic>Z</italic>=2.018, <italic>P</italic><0.05). The observed fatigue was lighter than that of the control group (<italic>Z</italic>=2.029, <italic>P</italic><0.05). The MNA score of the observation group was higher than that of the control group (<italic>P</italic><0.01). The scores of PFS-R and deficiency of both Qi and blood were lower than those of the control group (<italic>P</italic><0.01). The CD4<sup>+ </sup>levels and CD4<sup>+</sup>/CD8<sup>+</sup> levels of TB, ALB and PAB in the observation group were higher than those in the control group (<italic>P</italic><0.01), while the CD8<sup>+</sup> was lower than the control group (<italic>P</italic><0.01). The IgM and IgA levels in the observation group were higher than those in the control group (<italic>P</italic><0.01). The RBC, Hb and PLT levels of the observation group were higher than those of the control group (<italic>P</italic><0.01). Conclusion:Modified Shengmaiyin can be used as adjuvant therapy for children with ALL chemotherapy by improving nutritional status, immune function and immune balance, reducing clinical symptoms, promoting the recovery of hematopoietic system and improving the quality of life.

ABSTRACT

Objective:To discuss the clinical efficacy of Xuesusheng granule for cancer related fatigue (CRF) deficiency of both Qi and blood in clinical practice, and to investigate its effect on immune function and quality of life. Method:According to random number table method, 118 patients were divided into observation group (59 cases) and control group (59 cases). Patients in control group got Ejiao Huangqi oral liquid, 10-20 mL/time, 2 times/day. Patients in observation group got Xuesusheng granule, 10 g/time, 3 times/day. The treatment was continued for 8 weeks in both groups. Before and after treatment, scores of cancer fatigue scale (CFS), functional assessment of cancer therapy (FACT) and Qi and blood deficiency syndrome were graded, and degree of fatigue was discussed with brief fatigue inventory scale-Chinese (BFI-C). Levels of white blood cell count (WBC), neutrophil count (NEU), platelet count (PLT), red blood cell count (RBC), and hemoglobin (Hb) were compared before and after treatment.T lymphocyte subsets of (CD3<sup>+</sup>, CD4<sup>+</sup>, CD8<sup>+</sup>) levels, natural killer (NK) cells, interleukin-1(IL-1), IL-6, IL-8 and tumor necrosis factor -<italic>α</italic> (TNF -<italic>α</italic>) were also detected, and the safety was evaluated. Result:After treatment the scores of each factor in CFS scale and the total score of CFS in the observation group were lower than those in the control group (<italic>P</italic><0.01), and the degree of fatigue was lower than that in control group (<italic>Z</italic>=2.101, <italic>P</italic><0.05). The total score of FACT and the scores of four dimensions of physiology, society/family, emotion and function in the observation group were lower than those in the control group (<italic>P</italic><0.01). Levels of WBC, NEU, RBC and Hb in the observation group were higher than those in the control group (<italic>P</italic><0.01). Scores of BFI-C, deficiency of Qi and blood<italic> </italic>were lower than those in control group (<italic>P</italic><0.01). Levels of NK, CD3<sup>+</sup>, CD4<sup>+</sup> and CD4<sup>+</sup>/CD8<sup>+</sup> in the observation group were all higher than those in the control group (<italic>P</italic><0.01), while the IL-1, CD8<sup>+</sup>, IL-6, IL-8 and TNF-<italic>α</italic> levels in observation group were lower than those in the control group (<italic>P</italic><0.01). Not observed, there was no adverse reaction related to Xususheng granule. Conclusion:Xuesusheng granule can improve the symptoms of fatigue, reduce the degree of fatigue, improve the quality of life, reduce the degree of bone marrow suppression, improve immune function, and regulate immune inflammatory factors with high clinical efficacy in the use for patients with CRF and deficiency of Qi and blood.

ABSTRACT

Objective:To observe the rehabilitation effect of modified Guipitang administration combined with traditional Chinese medicine (TCM) hot pressing in patients with deficiency of Qi and blood syndrome breast cancer postoperative, and investigate its effect on immune function and tumor markers. Method:One hundred and fifty-four patients were divided into observation group (77 cases) and control group (77 cases) by random number table. Two groups were given comprehensive treatment measures after operation. Patients in control group additionally took Bazhen granules orally, 8 g/time, 2 times/day, for eight weeks. Patients in observation group additionally took Guipitang orally for syndrome differentiation, 1 dose/day for eight weeks. The chest, shoulders and upper limbs of the affected side were hot-pressed with TCM, 20 min/time, 2 times/ day, 5 days a week, the first 4 weeks. The occurrence of lymphedema, subcutaneous fluid, poor skin flap growth, sleep disturbance, shoulder joint dysfunction, etc. Were recorded in both groups. Before and after treatment, the scores of European organization for research and treatment of cancer quality of life questionnaire core-30(EORTC QLQ-30), and scores of cancer-induced fatigue and Qi and blood deficiency were graded. T lymphocyte subsets (CD3<sup>+</sup>, CD4<sup>+</sup>, CD8<sup>+</sup> levels and CD4<sup>+</sup>/CD8<sup>+</sup>), regulatory T cells (Treg), inhibitory T cells (Ts), cytotoxic T cells (Tc), human growth differentiation factor 3 (GDF3), serum carbohydrate antigen 153 (CA153), carcinoembryonic antigen (CEA) and human epidermal growth factor -2 (HER-2) levels were detected before and after treatment. Result:After treatment, the observation group incidence of lymphedema, subcutaneous effusion, poor skin flap growth, sleep disturbance and shoulder joint dysfunction was 8(10.39%), 9(11.69%), 11(14.29%), 25(32.47%) and 8 (10.39%) respectively in the observation group, all lower than 18(23.38%), 20(25.97%), 23(29.87%), 46(59.74%) and 19(24.68%) in the control group(<italic>P</italic><0.05, <italic>P</italic><0.01). The scores of overall quality of life and function scores in the observation group were higher than those in the control group (<italic>P</italic><0.01), hile symptom score was lower than that in the control group (<italic>P</italic><0.01). The scores of cancer-induced fatigue deficiency of Qi and blood syndrome in the observation group were lower than those in the control group (<italic>P</italic><0.01). The Tc, CD3<sup>+</sup>, CD4<sup>+</sup>, CD4<sup>+</sup>/CD8<sup>+</sup> levels in the observation group were higher than those of the control group (<italic>P</italic><0.01), while the Treg, Ts, CD8<sup>+</sup> levels were lower than those in the control group (<italic>P</italic><0.01). The GDF3, CA153, CEA, HER-2 levels in the observation group were lower than those in the control group (<italic>P</italic><0.01). Conclusion:On the basis of conventional comprehensive interventions of western medicine, Guipitang combined with TCM hot pressing for breast cancer patients after surgery can reduce postoperative complications, reduce fatigue, postoperative symptoms and TCM syndromes. Besides, it can enhance the immune function of the body, improve the quality of life, promote postoperative recovery, and inhibit the expression of tumor markers, thus improving the prognosis of patients.

ABSTRACT

Objective:To investigate the clinical efficacy of acupuncture combined with modified Shengyutang in treatment of cervical spondylotic radiculopathy with Qi-blood deficiency and its effect on T-lymphocyte subsets. Method:Totally 201 cases of cervical spondylotic radiculopathy with blood deficiency were randomly divided into 3 groups, with 67 cases in each group. Group A was treated with modified Shengyutang. Group B was treated with acupuncture. Group C was treated with modified Shengyutang combined with acupuncture. The effective rate of each group and the therapeutic effect of deficiency of Qi and blood were observed. Neck disabilitv index(NDI), visual analogue score(VAS), SF-36 mental component summary(SF-36MCS)and SF-36 physical component summary(SF-36PCS) were compared before and after treatment. Serum levels of interleukin-1β (IL-1β), interleukin-6 (IL-6) and calcitonin related peptide (CGRP) were measured before and after treatment. The levels of CD3+, CD4+, CD8+ and the ratio of CD4+/CD8+ were compared before and after treatment. The therapeutic safety indexes of each group were observed. Result:After treatment, the total clinical effective rate of group C was significantly higher than that of group A and B (P<0.05). NDI and VAS scores were significantly reduced (P<0.05). The scores of SF-36MCS and SF-36PCS were significantly increased (P<0.05). The score of Qi-blood deficiency syndrome was significantly lower (P<0.05). Serum levels of IL-1β, IL-6 and CGRP were significantly lower (P<0.05). The levels of CD3+, CD4+ and CD4+/CD8+ were significantly increased (P<0.05, P<0.01), whereas CD8+ was significantly decreased (P<0.05, P<0.01). No significant adverse reactions occurred in each group during the treatment. Conclusion:acupuncture combined with Shengyutang has a significant clinical effect on cervical spondylosis with Qi-blood deficiency, and is worth popularizing.

ABSTRACT

Notoginseng Radix et Rhizoma, one of precious and important traditional Chinese medicinal herb, has the functions of dispersing blood stasis and stopping bleeding, detaching swelling and alleviating pain, and invigorating Qi and blood. It is a traditional Chinese medicine for promoting blood circulation and removing blood stasis. After processing, the efficacy of Notoginseng Radix et Rhizoma was obviously different, the raw products mainly dispersed blood stasis and hemostasis, while the main effect of processed products was to replenish blood and Qi. In recent years, more attention has been paid to Notoginseng Radix et Rhizoma research, mainly focusing on the chemical constituent, pharmacological action and clinical application of the raw products. Although the research on processed Notoginseng Radix et Rhizoma has been increasing in these years, the mechanism of processing and the changes of bioactive constituents before and after processing are still unclear. This paper systematically summarized the modern processing methods of Notoginseng Radix et Rhizoma and compared the changes in chemical constituent and pharmacological action before and after processing through literature search. It is proposed that modern technologies should be put forward to study the correlation between the chemical constituent transformation and enriching blood as the breakthrough point, in order to explain the processing mechanism of Notoginseng Radix et Rhizoma, improve the quality evaluation system of this decoction pieces, and provide guarantee for the safety and effectiveness of its clinical medication.

ABSTRACT

OBJECTIVE@#To observe the clinical effects on cervical spondylosis of neck type treated with the combined therapy of needling and needling techniques at the positive reaction points as compared with the conventional acupuncture.@*METHODS@#A total of 90 patients of cervical spondylosis of neck type were randomized into an observation group and a control group, 45 cases in each one. In the observation group, the combined therapy of needling and needling techniques at the positive reaction points was adopted. The positive reaction points were selected in the neck, shoulder and back regions. In the control group, the conventional acupuncture therapy was provided. The main acupoints were Tianzhu (BL 10), Fengchi (GB 20), Quchi (LI 11), Waiguan (TE 5), Neck-Jiaji (EX-B 2) of C to C and points. The supplementary acupoints were Yinxi (HT 6) and Geshu (BL 17). The treatment was given once every two days, 10 treatments as one course in the two groups. Separately, before treatment, after the 1st treatment and at the end of one course of treatment, the scores of symptoms and physical signs and the scores of visual analogous scale (VAS) were observed in the two groups. The comprehensive clinical effects were compared between the two groups at the end of treatment.@*RESULTS@#After the 1st treatment and at the end of treatment, the scores of symptoms and physical signs and VAS scores were reduced obviously as compared with those before treatment in the two groups (all 0.05). At the end of treatment, the scores of symptoms and physical signs and VAS scores in the observation group were reduced more obviously as compared with the control group (both <0.05). The curative and remarkably effective rate was 84.1% (37/44) in the observation group, better than 65.1% (28/43) in the control group (<0.05).@*CONCLUSION@#Either the combined therapy of needling and needling techniques at the positive reaction points or the conventional acupuncture is effective in the treatment of cervical spondylosis of neck type. The combined therapy of needling and needling techniques at the positive reaction points achieves the superior effects as compared with the conventional acupuncture.

ABSTRACT

In female patients with migraine, the triggering or exacerbating factors for migraine including stress, menstruation and fatigue, should be treated intensively in addition to treatment for headache. Patients with coldness and weak constitution become a state of qi and blood deficiency in terms of Kampo medicine under the circumstances of severe fatigue, lack of sleep and menopausal symptoms. They tend to suffer from migraine at around the 4th or 5th day of menstrual period after rather heavy menstrual bleeding on the 2nd or 3rd day. We experienced 4 patients with headache during the late period of menstruation whose symptoms were successfully treated by juzentaihoto known to improve qi and blood. In Case 1, daily prescription was changed from unkeito to juzentaihoto. In Case 2, during the 7 days of menstrual period, juzentaihoto was added to tokishigyakukagoshuyushokyoto, while juzentaihoto was substituted for tokishakuyakusan in Case 3 and Case 4. In 9 cases of headache, including these 4 cases, juzentaihoto was efficacious against migraine headache during the late period of menstruation with coldness (9/9 cases), fatigability (9/9 cases) and dryness symptoms (7/ 9 cases). These results indicate that juzentaihoto is effective for migraine during the late period of menstruation, especially in patients with fatigue after menstrual bleeding. The administration period of juzentaihoto should be individualized for each patient depending on the severity of qi and blood deficiency.

ABSTRACT

Objective To observe the clinical efficacy of heat-clearing, dampness-removing, and Qi-blood-tonifying Chinese herbal medicine compound for the treatment of recurrent genital herpes(RGH), and to investigate the expression levels of Th17 cells and Treg cells in peripheral blood of patients with RGH as well as their role in the pathogenesis of RGH. Methods Sixty RGH patients were randomly divided into experimental group and control group, 30 cases in each group. The experimental group was treated with Chinese medicine decoction, and the control group was treated with Aciclovir tablets orally. The treatment course covered 6 months. The percentages of Th17 cells and Treg cells in the peripheral blood of the RGH patients were detected with flow cytometry. Results (1) After treatment for 3, 6 months, the total effective rate of the experimental group was 53.33%, 80.00%, and that of the control group was 23.33%, 46.67% respectively. There were significant difference between the two groups(P 0.05). The inter-group comparison showed the decrease of Th17 cell percentage and Th17/Treg cell ratio as well as the increase of Treg cell percentage in the experimental group was superior to that in the control group, the differences being significant (P<0.05). Conclusion The curative effect of Chinese herbal compound for RGH is better than that of the control group. Chinese herbal medicine compound can decrease the level of Th17 cells, increase the level of Treg cells, thus to decrease the ratio of Th17/Treg cells, indicating that heat-clearing, dampness-removing, and Qi-blood-tonifying Chinese herbal medicine compound can correct the imbalance of Th17/Treg cells, and then plays a role in anti-inflammation and immune regulation.

[Objective] To summarize the experience of Professor ZHANG Qin, a famous Chinese doctor in Zhejiang Province, in the treatment of dysmenorrhea by clinical syndrome differentiation. [Methods] Through the examples of teacher ZHANG's clinical treatment of primary dysmenorrhea and secondary dysmenorrhea in three cases, combined with the usual clinical experience of Professor ZHANG Qin to follow, analyze and summarize teacher ZHANG's clinical experience of the treatment of dysmenorrhea. [Results] Three cases show that, whether it is the primary dysmenorrhea or secondary dysmenorrhea, ZHANG believes that the total incidence of cold stagnation of Qi, blood circulation is not smooth, the main reason for the lack of collateral. A sense of cold dampness to menstrual wading, passengers in the cytoplasm, with warm and cold, blood, Tiaochong;with blood stasis, expelling, warming channels to dispel cold, removing blood stasis and relieving pain; also, liver blood deficiency and blood stasis of uterus, cure properly via removing film, streteving liver and removing stasis to relieve pain. Along with symptoms, with mild blood for treatment, in order to reconcile Qi and blood, Chongren unblocked. [Conclusion] Professor ZHANG Qin's experience in the treatment of dysmenorrhea by clinical syndrome differentiation has distinctive features and positive effects, so it is worth in-depth study and application.
