ABSTRACT

Objective:To analyze the application of path quality inspection combined with "zero defect" service management in nursing quality management of orthopedic ward.Methods:From January 2018 to December 2019,196 patients admitted to the Department of Orthopaedics of Panzhihua Central Hospital, Sichuan Province. Ninety-eight patients from January to December 2018 were as control group, 98 patients from January to September 2019 were as observation group. The control group adopted the routine nursing management mode, while the observation group adopted the path type quality inspection combined with the "zero defect" service management mode. The differences in the incidence of nursing adverse events, quality of life scores and nursing satisfaction were compared between the two groups.Results:The incidence of nursing adverse events during hospitalization in observation group and control group was 6.12% (6/98) and 15.31% (15/98), the difference was statistically significant ( χ 2 value was 4.320, P<0.05). Mental score, sleep score, diet score, mental health score, total score were (94.38±8.36), (93.28±8.33), (95.37±8.43), (91.89±10.48), (377.12±20.08) points in observation group, significantly higher than (85.65±10.74), (81.01±9.48), (87.69±7.97), (85.47±12.02), (331.09±13.39) points in the control group, the difference between the two groups was statistically significant ( t value was 3.118-18.880, P<0.01). The nursing satisfaction of observation group and control group was 92.86% (91/98), 82.65% (81/98), the difference was statistically significant ( χ 2 value was 4.749, P<0.05). Conclusion:The application of path quality inspection combined with "zero defect" service management in the nursing quality management of orthopedic ward is conducive to reducing the incidence of nursing adverse events, improving the quality of life of patients after surgery, and improving patients' satisfaction with nursing services.

ABSTRACT

Background: Lithium is considered first line drug effective in treating manic and mixed episodes of bipolar affective disorders throughout the globe. But the chronic and heterogenous nature of disease, along with toxicity of lithium often make patients non-adherent to medication as well as diminished health related quality of life. Present study was done to find out the prospect of regular supervision and follow up with therapeutic drug monitoring in optimization of lithium therapy based on health-related quality of life outcomes.Methods: It was a prospective, non-randomized, observational study of a cohort of subjects who are suffering from bipolar affective disorders and on lithium therapy. Patients were regularly followed up with therapeutic drug monitoring and personalized interview with questionnaires like WHO Quality of Life Score (QOL-Bref), Montgomery-Asberg Depression Scale (MADRS) and Medication Adherence Rating Scale (MARS).Results: Results revealed there was significant improvement in health-related quality of life of patients who were monitored with therapeutic drug monitoring and prescribed lithium therapy.Conclusions: Hence to maintain patients’ quality of life improved throughout the cycle of bipolar disorder spectrum, regular follow-up visits with monitoring of serum levels of lithium is needed, so that adverse effects would be minimal and adherence to medication become optimal. These optimal dosing resulting in optimal benefit to patients can be achieved with the involvement of clinical pharmacological consultation.

ABSTRACT

OBJECTIVE@#To correlate male lower urinary tract symptoms between age, quality of life scores,parameters of uroflowmetry and prostate size.@*PATIENTS AND METHODS@#Two hundred eight males were included in this study. Uroflowmetry parameters,age, International Prostate Symptom Score (IPSS), Quality of Life (QoL) scores and prostate sizewere gathered. For correlation, distribution of age, uroflowmetry parameters and prostate size werefirst compared to IPSS. Analysis of variance was used to compare age of patients, while Kruskall-wallis test was used to compare the QoL, uroflowmetry parameters, and prostate size on each IPSSgroups. Ordinal logistic regression analysis was used to correlate IPSS to age, quality of life,uroflowmetry parameters, and prostate size both for multivariate and univariate analysis.@*RESULTS@#There was no significant correlation between age and IPSS. However, on profile distribution,the age distribution between symptom scores were statistically similar. Qol scores were directlyproportional to IPSS. Thus, patients with a worse QoL score were more likely to have higher IPSS.Qmax scores decreased as symptom severity increased. Patients with higher Qmax scores are lesslikely to have higher IPSS scores. Voided volume was observed to decrease as IPSS severity increased,but this was not statistically significant. Patients with higher post void residual scores were morelikely to have higher IPSS. There was also no significant correlation between prostate size and IPSS.@*CONCLUSION@#There were no significant correlation between IPSS and age, voided volume and prostatesize. On the other hand, patients with a worse QoL score and a high post void residual had higherIPSS. Patients with a high Qmax, are less likely to have an elevated IPSS.

ABSTRACT

Objective To observe the effect and quality of life score of laparoscopic choledochotomy and T tube drainage for the patients of extrahepatic bile duct stones.Methods A total of 80 patients with calculus of extrahepatic bile duct in Shandong Provincial Third Hospital from February 2016 to April 2017 were retrospectively analyzed,including 41 males and 39 females,among which,40 patients were treated with laparoscopic choledocholithotomy and T tube drainage(the study group),and the other 40 cases were operated on laparotomy (the control group) by hierarchical randomization grouping.After operation,the time of operation,the amount of bleeding during the operation,the time of postoperative exhaust,the time of postoperative hospitalization and the difference of adverse reactions were observed in the two groups.Follow-up at the end of 1 st and 3th month,quality of life score was compared between the two groups.The measurement data with normal distribution were expressed as mean ± standard deviation (Mean ± SD),comparison between groups were performed using t test.The count data were expressed as rate(%),comparison between groups were performed using chi-square test.Results Comparison of data between the study group and the control group:operation time respectively were (97.23 ± 10.21) min,(117.52 ± 10.01) min,the amount of bleeding respectively were(87.73 ± 10.54) ml,(185.13 ± 11.56) ml,postoperative exhaust time respectively were (17.57± 2.96) h,(38.44 ± 3.06) h and the postoperative hospitalization time were (7.75 ± 1.21) d,(12.03 ± 3.85) d.The data of each group in the study group were lower than those in the control group,and the difference was statistically significant(P < 0.05).The quality of life scores of the 1 months and 3 months after treatment in the study group were (45.82 ± 3.22) scores and (47.29 ± 3.09) scores,the control group were (32.56 ± 3.29) scores and (36.19 ± 3.06) scores.The study group was significantly superior to the control group,and the difference was statistically significant(P < 0.05).The complications of the patients in the study group were not statistically significant between the control group and the control group(P > 0.05).Conclusions Compared with open surgery,laparoscopic choledocholithotomy with T tube drainage has significant clinical effect in the treatment of extrahepatic bile duct stones.It can improve the quality of life and safety,and it is suitable for clinical application.

ABSTRACT

Objective To investigate the clinical efficacy of liver-soothing and spleen-reinforcing acupuncture in treating functional constipation.Methods Sixty patients with functional constipation were randomly allocated to treatment and control groups, 30 cases each. The treatment group received liver-soothing and spleen-reinforcing acupuncture and the control group, oral administration of prucalopride succinate tablets. The number of complete spontaneous bowel movements (CSBM), the number of spontaneous bowel movements (SBM), constipation symptoms (defecation time, defecatory difficulty and fecal characters) scores and The Patient Assessment of Constipation Quality of Life (PAC-QOL) score were recorded in the two groups before and after two and four weeks of treatment. The clinical therapeutic effects were compared.Results After two weeks of treatment, there were statistically significant pre-/post-treatment differences in CSBM and SBM in the treatment group (P0.05). After two and four weeks of treatment, there were statistically significant pre-/post-treatment differences in constipation symptoms scores in thetwo groups (P0.05). The total efficacy rate was 80.0% in the treatment group and 73.3% in the control group; there was no statistically significant difference between the two groups (P>0.05).Conclusions Liver-soothing and spleen-reinforcing acupuncture has a marked therapeutic effect on functional constipation. It produces a quick effect with high safety.

ABSTRACT

Objective To investigate the clinical efficacy of acupuncture plus pelvic floor biofeedback treatment for the neurogenic bladder following spinal cord injury.Method Ninety patients with neurogenic bladder following spinal cord injury were randomly allocated to acupuncture, biofeedback and combination groups, 30 cases each. Every group of patients received acupuncture, pelvic floor biofeedback treatment or a combination of two treatments in addition to conventional bladder rehabilitation training. The LUTS score and the quality of life score were recorded and the bladder diary was kept before and after treatment. A statistical analysis was carried out.Result The LUTS score, the quality of life score, the residual urine volume and the mean voiding frequency decreased significantly in the three groups of patients after four weeks of treatment compared with before treatment (P<0.05) and decreased more significantly in the combination group than in the acupuncture and biofeedback groups (P<0.05). Maximum voided volume increased significantly in the three groups of patients after treatment compared with before (P<0.05) and increased more significantly in the combination group than in the other two groups (P<0.05).Conclusion Acupuncture plus pelvic floor biofeedback can significantly improve bladder function and the quality of life in patients with neurogenic bladder following spinal cord injury. Its clinical therapeutic effect is superior to that of acupuncture or biofeedback.

ABSTRACT

Objective To investigate and compare the clinical efficacies of acupuncture at myofascial trigger points and extracorporeal shock wave and their effects on the quality of life in patients with myofascial pain syndrome. Method Seventy-eight patients with myofascial pain syndrome were allocated, using a random number table, to acupuncture and shock wave groups, 39 cases each. The two groups received acupuncture at myofascial trigger points and extracorporeal shock wave, respectively. The clinical therapeutic effects were compared between the two groups of patients after 20 consecutive days of treatment. The quality of life score was recorded in the two groups of patients before and after one month of treatment. Result The VAS score (1.57±0.83), the PRI score (1.87±1.06) and the PPI score (0.94±0.72) in the acupuncture group were lower than the VAS score (2.16±0.95),the PRI score (2.68±1.14) and the PPI score (1.53±0.81) in the shock wave group. All had a statistically significant difference (P<0.05). The VAS score, the PRI score and the PPI score decreased significantly in the two groups of patients after treatment compared with before. Eight scaled scores: physical functioning, physical role functioning, bodily pain, social role functioning, vitality, general health perceptions, emotional role functioning and mental health increased significantly in the two groups of patients after treatment compared with before (P<0.05) and increased more in the acupuncture group (P<0.05). The marked efficacy rate was 84.6% in the acupuncture group, which was significantly higher than 61.5% in the shock wave group (P<0.05). Conclusion Both acupuncture and extracorporeal shock wave therapy have a marked clinical effect on myofascial pain syndrome. It can further improve the quality of life in the patients.

ABSTRACT

Objective To investigate the efficacy of rehabilitation in treatment of patients with hypertensive cerebral hemorrhage. Methods 64 patients with hypertensive intracerebral hemorrhage according to whether the implementation of the rehabilitation treatment of patients were randomly divided into two groups, the observation group and the control group,and each group(32 cases) was observed at two of the limb motor function,activities of daily living and quality of life scores. Results There were high blood pressure in patients with cerebral hemorrhage FuglMeyer score, Barthel index score, QQL quality of life score after 4 weeks after treatment, scores were significantiy improved by the test showed significant difference( all P <0. 05). Conclusion Active rehabilitation in patients with hypertensive intracerebral hemorrhage could improve the patients' limb motor function and daily life, improve quality of life of patients.

ABSTRACT

Objective To estimate the feasibility of QOL score in functional assessment of or-thotopic neobladder after radical cystectomy. Methods Questionnaire survey,QOL score sheet and urodynamic examination were done in 22 patients with orthotopic neobladder.Investigations were fo-CUS on the micturition interval during daytime and nighttime.Micturition time,degree of incontinence and their relationship with QOI.scores and urodynamic parameters wete also analyzed. ResuIts There were 3,6,7,1,2,3 and 0 cases which QOI.score were 0 to 6 accordingly.In the dav time,2 patients complained a light incontinence while 1 patient of moderate incontinence.In the nighttime.3 patients had the complaint of moderate incontinence while 4 patients of light incontinence.The average daytime mlcturltlon interval was 136 min(45-210 rain).The average maximum urinary flow rate,maximal urethral closure pressure and postvoid residual were 12.9 ml/s(2.7-22.1 ml/s),69.3 cm H2O(33-114 em H20)and 91.8 ml(5-300 m1)respectively.QOL scores had D.sitive correlation with the degree of incontinence during daytime and nighttime(daytime:r-0.510,P=0.015,night time:r=0.911,P<0.001).The QOL scores had negative correlation with daytime micturition inter-val(r=-0.749,P<0.001).The factors which influenced the QOL scores included the maximaI ure-thral closure pressure,postvoid residual and the maximum urinary flow rate. Conclusions QOL score reflects the patients"continent ability,subjective feelings on quality of life.It is correlated with the common urodynamic parameters.QOI.score might be used as a new index to estimate the function of orthotopic neobladder.
